Bosses

・Cemetry of Ash

ludex Gundyr
Defeat ludex Gundyr

He is the first boss that players will encounter. Most of his attacks are parryable, and you could defeat him easily if you pick the right timing for parry.

・High Wall of Lothric

Vordt of the Boreal Valley
Defeat Vordt of the Boreal Valley.

He is the second boss that players will encounter. He is located at the bottom of High wall of Lothric, guarding the transiting zone to the next location - Undead Settlement.

Dancer of the Boreal Valley
Defeat Dancer of the Boreal Valley.

The Dancer is a tall armored knight with a spectral cape. Her boss fight could be triggered by two options:

You killed Emma at Cathedral in High Wall of Lothric
You killed three Lords of Cinders: Abyss Watchers, Aldrich and Yhorm

・Road of Sacrifices

Crystal Sage
Defeat Crystal Sage.

Crystal Sage is one of the main bosses, he should be killed to gain access to the Cathedral of the Deep.

・Cathedral of the Deep

Deacons of the Deep
Defeat Deacons of the Deep.

The Deacons of the Deep are a group of clerics that wield candlesticks in combat. They should be killed to gain access to the Irithyll of the Boreal Valley.

・Farron Keep: Lord of Cinder

Lords of Cinder: Abyss Watchers
Defeat the Abyss Watchers, Lords of Cinder.

Abyss Watchers "Heirs of the Wolf's Blood", one of the first Lords that you will meet. You could summon 4 NPC to aid you in the fight:

Sirris of the Sunless Realms - after speaking to her for the second time in the Firelink Shrine. Her summon sign appears to the left of the entrance to the boss room.
Londor Pale Shade - if you have 5 dark sigils from Yoel of Londor and are on good terms with Yuria of Londor.
Black Hand Gotthard.
Yellowfinger Heysel - if you have defeated her invasion and given 1 Pale Tongue to Rosaria.

・Catacombs of Carthus

High Lord Wolnir
Defeat High Lord Wolnir

High Lord Wolnir is a massive skeleton covered in jewelry, of which the bracelets act as the hitbox of the boss. It is recommended to destroy the bracelets as quickly as possible to reduce the chances of the boss summoning his sword or army of skeletons. There are no NPC summons. Wolnir isn't optional and must be defeated in order to traverse further into Irithyll of the Boreal Valley.

・Irythyll of the Boreal Valley

Pontiff Salyvahn
Defeat Pontiff Salyvahn.

Pontiff Sulyvahn is a tall humanoid dressed in bejeweled sorcerer's robes. This boss is not optional and must be killed for access to part of Irithyll. Three NPCs can be summoned to help you in the fight against Pointiff Sulyvahn:

Black Hand Gotthard - in front of Sulyvahn's gate, summoning him will grant the "By My Sword" gesture.
Londor Pale Shade - can be summoned in front of a statue to the right when facing the boss fog, as long as the player has been following Yuria's and Yoel's questline, and gives the "Duel Bow Gesture" when summoned.
Anri of Astora - Their sign will be to the right (facing away from the fog door) in front of a statue. A seriously powerful summon for this fight, Anri is capable of nearly soloing Sulyvahn, as they’re capable of parrying him.

・Anor Londo: Lord of Cinder

Lord of Cinder: Aldrich, Devourer of Gods
Defeat Aldrich, Devourer of Gods, Lord of Cinder.

Aldrich appears to the player as a dark mass of bones and flesh with the upper body of and garments similar to those worn by Gwyndolin in Dark Souls. Aldrich boss fight can be challenging to players of all skill levels. When avoiding Aldrich's attacks, it is recommended to roll away as opposed to blocking due to the erratic nature of his attacks. Anri of Astora can summon the player to their world for assistance with this boss fight if their questline has been fulfilled. I could recommend to fulfill her quest to practice Aldrich's moveset.

・Profaned Capital: Lord of Cinder

Lord of Cinder: Yhorm the Giant
Defeat Yhorm the Giant, Lord of Cinder.

Known as the "reclusive lord of the Profaned Capital", Yhorm is the descendant of an ancient conqueror, but was asked by the very people once subjugated to lead them. In the past, he was a skilled Giant combatant, powerful enough to become a lord of a city and a Lord of Cinder. During combat, attacks players massive machete that glows like embers after being brought down to a certain health threshold.

Due to the presence of a Storm Ruler that is absurdly powerful against Yhorm, he can be considered either the easiest or one of the hardest bosses in the game, depending on if one chooses on using this weapon. Seigward of Catarina could help you in this fight if you fulfilled his questline. After the fight you'll get armour set of Siegward, If you've not killed him earlier.

・Lothric Castle

Dragonslayer Armour
Defeat Dragonslayer Armour.

The Dragonslayer Armor is an autonomous suit of armor guarding the entrance to the Grand Archives, and is assisted by Pilgrim Butterflies, which are actually the ones animating the suit. Two NPCs can be summoned to help you in the fight against Dragonslayer Armour:

Eygon of Carim - His sign can be found in the building to the right of the stairway leading to the boss fog on the right side just beyond the doorway.
Sirris of the Sunless Realms - Her sign can be found at the left side of the stairs leading up to boss.

・Grand Archives: Lord of Cinder

Lord of Cinder: Lothric, Younger Prince
Defeat Lothric, Younger Prince, Lord of Cinder.

Lothric is a purely magical boss and is a force to be reckoned with while he's accompanied by his brother and partner boss Lorian, Elder Prince. He has two phases. In the first phase you fight against Lorian, after you've killed him Lothric revives his brother and now on you're fighting against two of them. In the second phase your main goal to kill Lothric on the back of Lorian.

Two NPCs can be summoned to help you in the fight against Lothric and Lorian:

Sirris of the Sunless Realms - Summon can be found just in front of the fog door on the left. Take care not to let her take the brunt of the aggression as she can only heal herself three to five times during the fight and can easily be killed before the second phase actually begins, leaving you to solo the hardest part of the fight.
Orbeck of Vinheim - You must have given him all 4 Scrolls and have bought all the sorceries that he sells. Summon sign can be found to the left, at the top of the stairs before the double doors to the fog door

Optional Bosses

・Undead Settlement

Curse-rotted Greatwood
Defeat the Curse-rotted Greatwood.

The Curse-Rotted Greatwood is a giant tree with human-like limbs. It is covered in sacs that act as the hitbox of the boss. When the player encounters it, they will find a couple of undead in the arena that will fight alongside it during combat. After being brought down to a certain health threshold, instead of going through a transformation, the battle arena changes. This massive tree is filled with curses, and is the object of worship for the Undead Settlement's inhabitants. This boss is optional, but you can kill him to unlock Transposing kiln for Ludleth to be able transform souls of bosses into weapons.

You can find this Tree near the end of the Undead Settlement. Cliff Underside is the nearest bonfire after opening the shortcut door in front of the boss area.

・Smouldering Lake

Old Demon King
Defeat Old Demon King.

The Old Demon King is a large demon covered in flames and wields a giant hammer in combat. This boss is optional and not required to complete the game. Players with long-ranged builds will have an easier time dealing with this boss, however with patience, melee builds will be able to best the boss as well. Two NPCs can be summoned to help you in the fight against The Old Demon King:

Knight Slayer Tsorig - His summon sign becomes available by killing him after he invades. He will invade in the hallway before the collapsing rope bridge. Summon can be found next to the bonfire to the right of the fog gate.
A White Phantom Great Swamp Cuculus - The sign can be found at the bottom of the stairs directly in front of the fog gate. If you summon her and defeat the boss with her surviving, the Spotted Whip and Cornyx's set will appear where you first rescued Cornyx of the Great Swamp.

・Consumed King's Garden

Oceiros, the Consumed King
Defeat Oceiros, the Consumed King.

Oceiros is found in the garden below Lothric Castle, guarding a shrine. It can be assumed that he is blind (like Seath from DS1), as his eye sockets are empty, and he makes references to this throughout the fight.

His death is required to unlock secret locations and ending "The End of Fire".

You can summon Hawkwood, the Deserter to help you. His summon sign is located in the center of the platform before fighting the two Church Knights in front of the boss room. If you do not see his sign then you must first defeat the Abyss Watchers and afterwards talk to him in Firelink Shrine to receive the Farron Ring, you must also defeat the Crystal Sage and make sure that Hawkwood has left the shrine.

・Untended Graves

Untended Graves
Reach the Untended Graves.

Untended Graves is a hidden, optional area in Dark Souls 3. The area can be accessed via an illusory wall behind a chest after Oceiros, the Consumed King's boss room. The area is a replica of Cemetery of Ash, but shrouded in total darkness and occupied by different enemies.

The area where was ludex Gundyr now belongs to Champion Gundyr.

Champion Gundyr
Defeat Champion Gundyr.

Champion Gundyr is a different version of Iudex Gundyr, however he utilizes the same moveset as Iudex Gundyr with the addition of new attacks. He is much more aggressive than he was in the Cemetery of Ash. After being brought down to a certain health threshold, rather than undergoing a physical transformation, his eyes glow red and begins attacking the player much more aggressively.
Still most of his attacks are parryable, which makes fight against him, much easier.

・Archdragon Peak

Archdragon Peak
Reach Archdragon Peak.

To reach Archdragon Peak, you must obtain the Path of the Dragon Gesture from Consumed King's Garden, and make your way to Irithyll Dungeon, beside the location of the Dragon Stone Torso, where the petrified dragon acolyte is seated in a meditation like position. Stand on the rug to the left of the acolyte and perform the Path of the Dragon gesture. A cutscene will play and you will be transported to the cliffside.

Ancient Wyvern
Defeat Ancient Wyvern.

The Ancient Wyvern is a large grey wyvern, and it's boss fight is unique in that there is an additional way of killing it besides the regular hit-and-run tactic required for most bosses. Instead of fighting the wyvern head on, players can opt to run up the ruins, fighting regular enemies instead, to get high enough to deliver a plunging attack onto the dragon's head.

The Nameless King
Defeat Nameless King.

The Nameless King is an armored humanoid (which is probably a son of Gwyn, known before as a god of war) riding atop a blue wyvern and attacks players with his spear. During the first phase of the boss fight, his name is shown as "King of the Storm," until the wyvern he is riding is killed. This begins the second phase where his name changes to "Nameless King."

Players may find the fight against the Nameless King to be difficult, however, being familiar his moveset definitely makes it easier. There's no NPC summons.

Covenants

Covenant: Way of Blue
Discover Way of Blue covenant.

The covenant can be gained by equipping the item Way of the Blue which is obtained from Emma in the Cathedral of High Wall of Lothric.

Covenant: Warrior of Sunlight
Discover Warrior of Sunlight covenant.

The Warriors of Sunlight Covenant is found early in the game and unlocked by equipping an item.

To Find the item to Join: From the Undead Settlement Bonfire, head left, through the corpse-house towards the burning tree with undead worshipping around it. From here, head straight ahead until you reach another bunch of houses, at the end of the stairs going down. Keep going straight through the stone archway, and into the door of the first house ahead. The room has many hanging corpses. The item can be found by falling through the gap in the floor.

Covenant: Mound-makers
Discover Mound-makers covenant.

The covenant can be gained by equipping the item Mound-Makers which is gotten from Holy Knight Hodrick. You must obtain the item: Mound-Makers and equip it.

Before the Curse-Rotted Greatwood breaks the ground, start at the Undead Settlement bonfire and continue through the house until reaching the camp with an Evangelist near a burning pile (where you find Loretta's Bone), from here take a right and cross the wooden bridge. At the other side of the bridge there's a bunch of crates and barrels, break those and walk behind the stables insted of entering. Behind a small house with a ladder is a cage spider NPC, it will give you a hint that there's something fishy going on with a cage. Climb up the ladder of the small house, then drop off the roof on the right side. You will see a large guy with a cage on his back, approach his cage and use the action button to examine it. A cutscene will appear and you will be teleported to the area where an Holy Knight Hodrick gives you the covenant. (Does not work with summons or while being invaded).

If you kill the Curse-Rotted Greatwood before acquiring the covenant, go to the entrance of the boss room after having defeated Aldrich. There you can activate the summon sign of Sirris of the Sunless Realms and join in cooperation. If successful, you will acquire the Covenant. You must not have turned Sirris hostile towards you.
If the Curse-Rotted Greatwood breaks the ground before acquiring the covenant AND failed the questline for Sirris of the Sunless Realms for whatever reason, it will be impossible to acquire the convenant's insignia until your NG+

Covenant: Blue Sentinels
Discover Blue Sentinels covenant.

The covenant can be gained by equipping the item Blue Sentinels which is obtained from Anri of Astora and Horace the Hushed on the Road of Sacrifices.

Covenant: Watchdogs of Farron
Discover Watchdogs of Farron covenant.

The covenant can be gained by equipping the item Watchdogs of Farron which is obtained by praying to the Old Wolf of Farron to obtain the item.
The Old Wolf is located in the poison swamp area, at the top of a tall ladder, the base of which is near the Boneshard Fragment.

Covenant: Rosaria's Fingers
Discover Rosaria's Fingers covenant.

The covenant can be gained by equipping the item Rosaria's Fingers which is located in Cathedral of the Deep.
To gain this item you need to find Rosaria. Go to Cathedral of the Deep and make your way up the shortcut elevator on the left side of the Cleansing Chapel, head outside onto the ledge, turn a corner and climb up the ladder. Head to the hole in the wall at the opposite side of the room and drop down onto the tiled-roof of the cathedral support below. Head up to the top of the support. Enter the cathedral through the first passageway to the right of the support. Traverse the precarious stone arches and reach the opposite side of the room from where you entered. Jump down to a stone platform below, then jump down a second time, to the area with guardrails. From here, simply hug the right wall until you reach Rosaria and her bonfire, past a broken fence and large iron doors.

Be sure to fulfill Sirris questline or to kill the Tree boss to get Mound-maker covenant.

Covenant: Aldrich Faithful
Discover Aldrich Faithful covenant.

The covenant can be gained by equipping the item Aldrich Faithful which is obtained from the Irithyll of the Boreal Valley. In Irithyll of the Boreal Valley, from the Pontiff Sulyvahn bonfire, proceed straight through the door, left up the stairs and right through the valley of dead giants and into the door at the far end. Immediately on your left is a wall that can be struck to reveal a ladder. Descend the ladder and fight the two Sulyvahn's Beasts that guard Archdeacon McDonnell, who will be located in the north-west corner of the room.

Covenant: Blade of the Darkmoon
Discover Blade of the Darkmoon covenant.

The covenant can be gained by equipping the item Blades of the Darkmoon which is obtained from Anor Londo. It is located in Anor Londo, at a tower accessed by walking over an invisible path from the rotating platform. The rotating stairs can be in any position. If it is at the top (allowing access to Anor Londo) go to the bottom of the stairs. If it's at the bottom, go to the top. There's a bonfire on a tower directly to the left of you, (if you're facing the Anor Londo bonfire,) and a broken platform on the rotating platform facing towards it. Just walk through the air straight ahead off of the broken platform. Do not deviate from the path until you are perpendicular to the bonfire, then turn to your right, and you will land safely next to the bonfire.

Upgrades

Supreme Weapon Reinforcement
Reinforce any weapon to the highest level.

You'll get this achievement nearly to the end of the game.

Ultimate Bonfire
Reinforce a bonfire to the highest level.

To get this achievement you need to upgrade your Bonfire to the maximum. All the locations of undead bone shard you could see there:
Undead Settlement - Starting at the Dilapidated Bridge bonfire, travel near the white birch tree where the giant fires arrows. To the right of this tree is a platform you can jump to with the Undead Bone Shard on a dead body.
Farron Keep - From the Keep Ruins bonfire, head down the walkway towards the door that opens once you light the three bonfires. Before reaching the door head to the right and look for a tower full of slugs. It is in the center of that tower. Video location (fextralife video starting at 1h 22m 27s)
Cathedral of the Deep - From the Cleansing Chapel bonfire, head out and through the graveyard. Head across the bridge and up some stairs past a Grave Warden and to the right. The shard is on a tombstone that is laying out over a gap, near a white birch tree.
Catacombs of Carthus - From the Abyss Watchers bonfire go down the stairs and to the right side of the ledge overlooking the level. Drop down on the series of platforms below, be careful of the two archer skeletons. Head through the doorway and kill the skeleton with a hat immediately to the right. This will cause the first Skeleton Ball, to break on impact with the wall and reveal a shard.
Smouldering Lake - Kill the Carthus Sandworm.
Smouldering Lake - From the Demon Ruins bonfire, head down the stairs into the long room filled with floating fireballs and demons. Head left towards the end of the room and the boneshard is located with bodies of dead demons. Beware a Fire Slime that will attempt to fall on you. (Video)
Irithyll of the Boreal Valley - Starting at the Church of Yorshka bonfire, go down the hallway stairs, and go right along the cliff. The Undead Bone shard is located behind the large tombstone at the end of this path. (Video)
Profaned Capital - After you fight the first Gargoyle of the area, it will be on the corpse of Laddersmith Gilligan next to the bonfire.
Lothric Castle - Starting at the Dragon Barracks bonfire, go up the stairs and to the bridge where the two dragons are. Run almost all the way across the bridge and drop off at the end. The Undead Bone Shard is underneath the bridge.
Grand Archives - Once you reach the rooftop area, head down the ladder and straight ahead until you can turn left to head up the rooftops. Head up and take a right and remain along the top until you can jump back into the archives. Defeat the Lothric knight and Thralls, walk past the wax pool, and find it on a corpse sitting in a chair. There is a ladder you can kick down here that will go back into the archive area where you will find the corpse of Orbeck (if you buy all his spells/defeat the Princes), and contains a number of Priests. It is also near the doorway that leads to the 3 NPC Hunters.

Ultimate Estus
Reinforce the Estus Flask to the highest level.

To get this achievement you need to upgrade Estus flask to the maximum. All the locations of estus shard you could see there:
High Wall of Lothric: On an anvil in the room where you get the Cell Key
Undead Settlement: By the burning tree with worshipping undead. This is to the left of the Undead Settlement bonfire, past the house
Road of Sacrifices: From the Crucifixion Woods bonfire head towards the Crystal Sage. Pass under the small bridge. The shard will be on the left side of the ruins near a fire that has two Lycanthrope.
Farron Keep: Go straight out from the starting bonfire. The shard is near a fallen tower guarded by slugs.
Cathedral of the Deep: Right outside the Cleansing Chapel bonfire where enemies are praying
Smouldering Lake: On a body near a hole from the Old King's Antechamber bonfire. Look for an illusory wall in the bonfire room.
Alternatively there is a separate path that leads you to the opposite side of the hole and can be reached if you go down the hall to the right of the one containing a black knight behind an illusionary wall
Irithyll Dungeon: From the bonfire, walk forward into the dungeon and turn right. Now go straight, head through the locked gate that requires the Jailbreaker's Key, then drop down. Go right into the room and there should be a chest which is a mimic that contains the shard.
Alternatively, as you play through the level you will eventually reach an open area with 3 enemies, one who will shoot a crossbow at you from beside an archway, one with an axe, and one who will ambush you from behind. Kill these enemies, proceed through the archway and there will be a mimic on your right. This mimic will drop the Estus Shard when killed.
Anor Londo: In a chest on the first floor of the large dark room with the Deacons (before the Aldrich fog wall). Enter from the double doors and hug the left wall to find it.
High Wall of Lothric/Consumed King's Garden: From Dancer of the Boreal Valley bonfire, climb up the ladder and go first left. Proceed down the stairs and kill the knight standing in front of the elevator. Use the elevator and jump out halfway, onto the wooden ledge. The Estus Shard is down the stairs to the right.
Lothric Castle: From Grand Archives bonfire, on the top of Lothric Castle near the Winged Knights. From the ladder that takes you to the Hunter's Ring, you will see an archway that leads to a lower roof/balcony. Estus Shard will be found all the way out near the railing.
Firelink Shrine: Purchase the Tower Key and get to the bridge that overlooks the Shrine. Drop down and find a window that allows you to get inside and get the shard on the rafters.
Alternatively, go upstairs and outside and find the tree near the raised area with loot on it, then run up the tree and jump onto the raised area - no key needed!

In Dark Souls 3 order to get this achievement you need to find all coals and make all infusions.

COALS

Farron Coal: Allows Heavy, Sharp & Poison Infusions

Sage's Coal: Allows Crystal, Blessed & Deep Infusions

Profaned Coal: Allows Dark, Blood & Hollow Infusions

Giant's Coal: Allows Lightning, Simple & Chaos Infusions

Road of Sacrifices: At the Halfway Fortress bonfire where you first meet Anri and Horace go down and drop down from the ledges on the right side, keep going straight along the right wall and enter a doorway in the end. Turn left and you will see a Black Knight guarding a few bodies, the body at the end of the hall holds the coal.

Farron Keep, from the first bonfire keep to the left side until you reach a small structure guarded by a darkwraith. The coal is in a body inside this structure.

Irithyll Dungeon, in a room adjacent to the one with the 6 jailers, right before the shortcut

On a Giant's corpse in Anor Londo. From the bonfire, go up the stairs and turn left inside the building.

GEMS

x1 guaranteed drop per NG from the Red Eyed Blue Knight to the left of the entrance to Emma at her church in High Wall of Lothric.

Drops from a Crystal Lizard, on the roof with the praying hollows, near the second bonfire in High Wall of Lothric.

Can begin the game with one if you choose it as a Burial Gift
Drops from the Fire Demon that Siegward of Catarina helps to defeat in Undead Settlement.

Gift from Hawkwood after defeating the Curse-Rotted Greatwood or Crystal Sage. Dropped by Hawkwood if killed.
1x from a Crystal Lizard, down in the ravine near the open sewer entrance in Undead Settlement.
1x from a Crystal Lizard, near the dead dragon at the top of the Old Wolf of Farron bonfire in Farron Keep. It is found behind the large gate on the opposite side of the Asylum Demon, and is the Crystal Lizard (1 of 3, the other two drop 1 Large Titanite Shard) found by going up the stairs to the left once on the other side of the bridge. You can access this part of the bridge by a small set of platforms just to the right of the gate nearest to you after going up the elevator.

From the Irithyll Dungeon bonfire, head straight into the large room and then turn right, proceed until the end of the corridor and open the gate with the Jailbreaker's Key. The ring is on the rocks below.

Starting equipment for the Pyromancer. Dropped by the Great Crab, up the ladder near the Farron Keep bonfire in the Road of Sacrifices.

Catacombs of Carthus: just after the stray demon, walking towards the 'Abandoned Tomb' Bonfire, there will be a corpse holding the ring at the end of a hallway.

Under the bridge between the 1st and 2nd bonfires of Road of Sacrifices. There is an area to the right of the bridge's beginning where players may drop down into the cave.

In Irithyll of the Boreal Valley. From Pontiff Sulyvahn's room, head back out towards the railing and fall straight onto the altar below.

Farron Keep, Keep Ruins bonfire. Drops from the Great Crab patrolling a white birch tree.

+1 Version (NG+): Found next to the giant crab just below the Crucifixion Woods bonfire in the Road of Sacrifices.

+2 Version (NG++): Located in Lothric Castle. Before the Lothric Prince Boss fight, there is a dome shaped roof with three golden Winged Knights. Go up the spiral staircase at the centre of the dome shaped roof. You'll encounter a ladder, don't go up the ladder, instead go around the ledge to find this ring on a corpse hidden in an alcove.

Found to the left of the swamp area of Road of Sacrifices inside a room with an enemy.

+1 Version (NG+): found in the rafters above the Archives. Drop down from the roof where the golden winged knights are located.

+2 Version (NG++): Located in the Consumed King's garden. When initially taking the elevator down, get off halfway to a ledge. Turn left and drop down onto some broken stairs. Pick up the item here and look to the left to find the ring on another set of broken stairs. You can get over there with a well timed roll.

・Master of Rings pt.3

Located in the Irithyll Dungeon. The ring is in a cell beside the open area with six patrolling Jailers.

Found in a chest on the second floor of the room with Gwynevere's painting in Irithyll of the Boreal Valley. Just beyond where either Siegward of Catarina is located during his questline or, more commonly notable, the Estus Soup is placed.

+2 Version (NG++): Located in the High Wall of Lothric. From the second bonfire, head down to the roof with the first crystal lizard and the enemy that morphs. Climb the ladder down. Run past the spear wielding Lothric knight. Head down into the room with the estus shard on the mantle. The ring will be on the wall to the left, hidden behind a barrel.

Found in the Consumed King's Garden. When taking the second elevator in the area which grants a shortcut, get off early to reach a ledge with a Lothric Priest. The ring will be beside him.

Ahead up more stairs and to the right of the Fire witch and 2 Pontiff Knights is an illusionary wall. Behind which you'll find the ring.

・Master of Rings pt.4

Found in the Darkmoon Tomb in Anor Londo to the right of the altar. For reference, the Darkmoon Tomb is hidden behind an illusory wall in the circular room before the lever which controls the platform leading to the Anor Londo bonfire.

Dropped from Ludleth of Courland when killed or can be obtained by visiting his throne before beginning the next Journey.

Archdragon Peak. Use the 'Path of the Dragon' gesture at the altar near the Dragon-Kin Mausoleum bonfire. The Drakeblood Knight spawns just next to this altar.

High Wall of Lothric: On an awning reachable by a minor jump in the courtyard. This area is patrolled by a Winged Knight and the stairs that lead to the platform which players must jump from is guarded by a Hollow Soldier.

Yuria of Londor (or Shrine Handmaiden if given Yuria's ashes): Three of them can be bought from her, for 5000 souls each.

Road of Sacrifices: One can be found on a corpse hanging on a ledge overlooking the Crucifixion Woods.

Consumed King's Garden: One can be found on a corpse in the toxic swamp to the left exiting the initial elevator.

"Nestling" Crow: Give Loretta's Bone to the Crow to receive a Ring of Sacrifice.

Endings

To Link the First Flame
Reach "To Link the First Flame" ending.

To reach this ending you just need to beat the game and link the flame.

The End of Fire
Reach "The End of Fire" ending.

To reach this ending you need to find the Eyes of the Firekeeper in the Untended Graves. They're located in the dark version of Firelink Shrine in place where's Irina, behind illusory wall. After this you need to give them to the Firekeeper and exhaust her dialogue. Make sure to select "yes" when prompted. After killing Soul of Cinder instead of linking fire, you need to summon the Firekeeper.

The Usurpation of Fire
Reach "The Usurpation of Fire" ending.

To reach this ending you need to fulfill Yoel's questline. You can find Yoel at the beginning of location "Undead Settlement" on the ruined bridge among the others of his kind. To reach ending follow this steps:
Request his "strength" by allowing him to level you up. Do this all five times. One level per two deaths. Must be done before killing the Abyss Watchers. Doing this will cause the door to the catacombs to open, and upon entering, kills off Yoel. This will cause a premature ending to the questline. Also NEVER cure your Hollowing by talking with the Firekeeper after obtaining Soul of the Firekeeper. The Purging stone and Velka Statue are safe to use, however. You should receive 5 dark sigils from him. Note: You can "cure" your hollowing via Purging Stone. This gives you your human appearance whilst keeping your dark sigils. This doesn't impact this path. So long as you DO NOT cure the dark sigil via Firekeeper.
Find Anri of Astora at the Halfway Fortress bonfire. Exhaust all dialogue.
Speak to them again at the Firelink Shrine after killing the Deacons of the Deep.
Find Anri in the Catacombs of Carthus in a hallway next to the first rolling skeleton ball. Exhaust all dialogue.
Choose dialogue option "no".
Never tell Anri of Horace's whereabouts.
Enter the Smouldering Lake and kill Horace in the cave.
To find the cave, hug the right wall when entering the lake.
Find Anri on the path near the wooden bridge before the High Lord Wolnir boss. Exhaust all dialogue.
If you killed Horace beforehand, choose dialogue option "Tell Anri of Smouldering Lake", which will have Anri set up a gravestone for Horace.
Otherwise, choose "say nothing". If Horace is still alive and you inform Anri of his whereabouts in Smouldering Lake, Anri will later be found dead.
Find Anri at the Church of Yorksha fire. Exhaust all dialogue. DO NOT kill the pilgrim/assassin in the corner of the room disguised as a statue. (NOTE: Even after Anri leaves, the pilgrim/assassin may still be there disguised as one of the statues. Don't attack anything in that area until you have progressed past step 8 (wedding)).
After killing Pontiff Sulyvahn, speak to Yuria at the Firelink Shrine. Exhaust all dialogue options using talk.
If done correctly she will tell you your spouse awaits you at the Temple of the Darkmoon. (If she won't mention the wedding, try warping to the pontiff's bonfire and using that to warp back to Firelink Shrine).
Perform the "wedding" ceremony in the Darkmoon tomb. The tomb is located behind a false statue in the room before the rotating tower to Anor Londo.
Talk to the Pilgrim NPC and accept the sword. Use the sword on your spouse and receive 3 dark sigils. You should have 8 Dark Sigils by now.
After talk to Yuria one last time at the Firelink Shrine, She will congratulate you on your ceremony and tell you to link the fire. (She will tell you to link the fire if you've put all four remains of Lords of Cinder and unlocked the final area even if you haven't performed the wedding rites with Anri; this does not mean you can skip the wedding rites).
Kill the Soul of Cinder and touch the fire. Enjoy. (Do not summon the Firekeeper if you gave her the Firekeeper Eyes as it will activate the "End of the Fire" ending.)

@Doc idk about unlooted versions of rings, but I guess if you'll start NG+ you would be able to loot only NG+ version. About scaling I dont remember much, but i guess they dont scale that much.
