The CARCAS operatives are about to raid a semi-abandoned factory owned by the Nutrire.Co and their cultists. But suddenly, a mysterious sexy guy in a latex (?) balaklava arrives and tells the soldiers to hold on and wait for him outside. This handsome, gorgeous man is 100% not Miami Mutilator. Kill all the cultists as the Mutilator and save the world blah-blah.

The level features some custom enemies, weapons, objects and tiles.

Cameos - ToiletMaster, Seymy, Winterfrost, CsyecoktheSolly, Bizzard, Mace(Oryx) and a mysterious uhh, you better forget about him, he is mentally disabled.
