Hi! this is my first time writing a guide for a game i love, so i hope it looks decent. please feel free to .ask questions in the comments or correct things i messed up. thank you! - Marmu.
Beginning
When you first start the game, it would seem like the rest of SJSM, but after awhile spook will float down from the ceiling stating she 'didnt know there were rooms down here' before leaving

While you continue walking, you'll find yourself in the hospital itself. The water fountain will be your save spot, located on the right side of the lobby. You will want to continue to the two doors on the left, you do not need to check any of the rooms in that hallway unless you want to. You should find a door leading downstairs to the basement at the end of the hallway, go down and walk to the break room. There should be a key on one of the chairs to the right.

Afterwards, you should head back upstairs and go to the main lobby. Behind the front desk is a door leading to a hallway. The last door on the left should be unlocked, and to your left should be a key on a table leading to the maintenance room.
Maintenance room
After you collect the key, you should head back downstairs to the Maintenance room, located at the end of the hallway to your left. Interact with the generator and the power should cut out. Go back upstairs from there.

Once you're back upstairs, look to your left and there should be another two doors straight ahead leading to another part of the hospital. Go through them, and you should be in a long hallway.
In this hallway you will meet your first specimen of Karamari hospital, it should be blocking the door to the stairs, or already active depending on your save. Its quite slow, so dont worry about it too much.

At the right turn of the hallway, there should be a stairwell leading to the basement again. Go down, and you should be on the other side of the basement. There will be four doors leading to a morgue and one at the end of the hallway leading to a isolation room.


The morgue
One of the doors to the morgue will be unlocked, and on one of the gurneys (metal tables along the walls) should be a metal emblem. once you pick up that emblem, you've probably noticed the screen glitched.

You're about to meet specimen two, after you leave the room. Down the hallway should be body bags lined up against the wall, and at the end one of them should begin rising. turn around and run the other way, your sprint won't run out during this. Look left and there should be another hallway, this should be a sort-of maze, and head left when the hallway splits and keep going until you find a door, leading you outside the morgue.
Basement two, electric boogaloo
After you leave the basement, you should go back to the lobby of the hospital. spook will be there and talk for a bit, before leaving through the floor. After that, go through the door behind the front desk and then the second door on the left. interact with the plaque on the wall, and the wall should open to a stairwell. After you make it down, go through the door on the right, then go left down the hallway. Keep straight, and you might encounter specimen 3, mainly harmless and very slow.

you will eventually reach a room with a hanging person in the left corner, and a key on a table to the Isolation room. After you pick up the key, specimen 4 will begin chasing you. It is fast, like really fast. head back the way you came and go upstairs and back to the isolation room in the basement.
Ending
After you make it back to the basement, proceed through the hallway and you'll find yourself in a maze. in said maze is specimen 6. I cannot help you with the maze, but good luck. Afterwards you will be in a smaller room. At the end of that room is a key on a table to the exit, and a person in a locked room you can talk to through the window.

Proceed back through the basement afterwards, and up to the exit, where you can go through the door now and finish the DLC.
Secrets
After you finish the DLC, you can now replay it with small secrets. In the room with the emblem should be a painting to your left. you can pick up the sword in it by interacting with it.

spooky will now be in a nurses outfit, and its quite cute. :)
