I found it here: http://gmod.gamebanana.com/maps/180453, where it was redistributed by Exho (also not the creator).
If anyone knows who made this map I'd appreciate it if I could contact them about uploading it.

This map has a very medieval feel but doesn't look worn or damaged. It makes for some interesting game play in a strange, foreign environment.
