Yeah I got the cutscene - I have no mods, so just seems to be an unfortunate bug

no, you don't need any item. you're meant to run up to the dude and it plays a cut-scene. you might have mods on which stops steam giving out the achievement.

I beat the game and didn’t get the achievement - is there anything else required? Picking up a specific item or something I may have missed?
