Introduction
If you're Struggling to get the Professional Struggle Achievement, here's an easy way to do just that. We're going to be modding the save file to get this Achievement.
Getting Started

(Optional) Start a new game, and play until you break out of the glass test tube. This can be on any save slot: top, middle or bottom.
Once you have a save, go to this directory:

C:\Users\<YourName>\AppData\LocalLow\ChasingRatsGames\Struggling\76561198003727375

This is where your saves for Struggling are stored, and will contain all your saves. If you don't want to risk losing any of the saves you already have, either zip them in this directory, or just move them to somewhere else.

Open Notepad, and drop the save (.json) you want to use into Notepad. The top save is 0, middle is 1, bottom is 2.

Step One
After importing your save, look for two specific things in the save. First, make absolutely sure that Professional Struggle is not already marked as achieved, because otherwise the Achievement will not pop. To do this, find Professional Struggle, then immediately after, there will be a list of true/false; find the one that corresponds to "Professional Struggle". These are all in alphabetical order, so Professional Struggle is nine back from the end; mark that one as “false”, if it isn't.

Step Two
Second, find and replace the coordinates to set you just after the Anatidaephobia boss fight, where the Duck eats you. This means you'll only have to defeat the Duck's central nervous system, which is relatively easy, and should be a breeze for anyone, single- or multi-player. There's also basically no risk of dying in this room, unless you're actually trying. The coordinates you want are at the bottom of the save, and are [-1,-1,-1,-1,19,0,0,-1].

*Step Three
(Optional) Third, if you aren't starting a new game, the death count is located on the bottom of the save, but that's only for what it shows on the save when hovering over it; the actual death count is about two-thirds down, and is right next to the total time played. Change both death counts to 0, if you're not starting a new game. Setting both to 0 just ensures no cross-talk, which I don't think there is, but better to be safe.

Final Notes
With these things changed on a save, you should have a breeze actually beating the game without dying once. With a new save, you'll start in Troy's Lair, with absolutely nothing, other than some mannequin heads and boxes, but no hats. Leave the lair, and start from the final checkpoint. Beat the game, and "earn" that final achievement.
