This guide explains how to get every achievement in the game. It contains spoilers so read at your own risk.

Introduction
This is a complete guide to getting all achievements in the game. It assumes that you have at least a base understanding of the game but if you don't then I highly recommend reading these two guides first:
- Sheesh's "Getting started." guide
- Nerava's "In Celebration of Guidance" guide

The achievements use the following template:

Achievement name Achievement image Hint: this lists the initial hint given to help you unlock the achievement. It is found on the achievement board in the game. Effect: this lists the second hint ambiguously telling you what benefit you gained from unlocking the achievement. It is also found on the achievement board in the game. Requirement: this details the unlock requirement in less cryptic terms. Benefit: this details the effect in more concrete and mechanical terms. Notes: this is a more detailed addendum to the Requirement heading along with my own personal notes.

For reference, this is what the in-game "achievement board" is (credit to Sheesh for the image):


Unless otherwise stated, all achievements on the achievement board must be attained in a single playthrough. Anything you do in the sanctuary also doesn't count as progress towards these achievements. All other achievements (the non-achievement board ones) do not provide any benefits and are attained cumulatively over all of your playthroughs.

If there are any errors with my information, or you have new information to add (a number of the more obscure benefits don't have concrete numbers), then leave a comment with proof of your claim (a screenshot or video) and I will eventually look into it.
Achievement Board - Row 1



The Lord of the Flies



Hint: Is there even still a man amongst the cloud of flies?
Effect: The insects follow me everywhere and even simple fruit flies will join their brethren.
Requirement: have many "fly"-type followers of any kind at once (Bladefly, Firefly, Icefly, or Magifly).
Benefit: fruit flies are friendly to you (the black ones) and will become followers when you approach them.
Notes: the exact amount of flies required is currently unknown but the flies would be hard to count anyways. Casting one of the various fly-summoning spells until it breaks should give you enough flies.





At Veil's Door



Hint: I will die if I cannot stop the bleeding.
Effect: I am tougher for surviving the experience and built with greater fortitude.
Requirement: have your current Health lowered to the point where you begin bleeding out, then heal yourself to stop bleeding out.
Benefit: you start with +10% Fortitude.





Efficient Execution



Hint: It would have taken far too long to do each prisoner independently, so I lined three of them up and killed them all with one swing.
Effect: All executioners need their axe and now I may find one of my own.
Requirement: kill three enemies with one physical attack.
Benefit: you deal more cleave damage.
Notes: the easiest way to do this is to purchase a Greataxe, Great Cleaver, Battle Axe, or Scythe, go to the Town or Village, corner three Civilians, weaken them until they are bleeding out, then attack them all at once. Having the Nail of a Giant and/or Half a Shield mementos helps. The exact amount of extra cleave damage that you get is currently unknown.





One Solution



Hint: Pray to Thousandfold that you may continue to incite violence, but don't pray too hard.
Effect: Death will not stop me from committing violence.
Requirement: pray to Thousandfold until you die from HP loss.
Benefit: the Violence rating cap is increased from 200% to 10,000%.
Notes: in order to unlock Thousandfold, all five of the main gods must have been killed at least once and you must have completed at least three consecutive playthroughs (i.e. interacting with the pit after killing one of the five main gods, not dying anywhere else). Once you have done so, an altar to Thousandfold will have appeared in the usually-empty hut directly to the north of Song's chapel. You can actually see the empty hut change a little bit after each consecutive playthrough as the altar is slowly built.





Should Have Been There



Hint: There is nothing I can do... The fight is over but he is still bleeding badly and there is nothing I can do to stop it. I just kept walking.
Effect: Memory of a Coward.
Requirement: have a follower die from bleeding.
Benefit: unlocks the Coward class.
Notes: elementals and plants can't bleed out so you will need a humanoid follower (or possibly critter?). This is easiest to do after having already unlocked the Popular achievement.





Popular



Hint: I will need at least ten loyal followers to take up my cause.
Effect: I seem to have a small cult following.
Requirement: have 10 followers (excluding elementals and flies).
Benefit: you start with a Blood Mage follower who is equipped with a random short-range melee weapon.
Notes: the easiest way to get enough followers is to use the Animate Plant spell in a shrub-dense section of the Forest area or during the Daughter of Sow boss fight.
Achievement Board - Row 2



Humble Godslayer



Hint: I do not need any useless memories to distract me from my task.
Effect: I am stronger in every way now that I do not use a crutch.
Requirement: kill one of the five main gods without picking up any mementos.
Benefit: you start with +10% Dexterity, Aggression, Strength, Speed, Mobility, Perception, Fortitude, and Knowledge.
Notes: this also includes mementos that a character starts with so the achievement is not possible with the Historian, Cultist, Explorer, Chemist, Doctor, and Martyr characters (as well as the Misanthrope character if you already have the Ghost achievement).





A Mutually Assured Death



Hint: The half dozen heathens thought me dead. My wounds may be fatal but my resolve is stronger than ever. None of us will leave this room alive.
Effect: Memory of a Martyr.
Requirement: kill six enemies while bleeding out.
Benefit: unlocks the Martyr character.





All I See Is Red



Hint: Did I do this...? Maybe what they say about me is true.
Effect: I will be better, just give me another chance. I may be a monster but I truly feel a new surge of goodness within me.
Requirement: kill one of the five main gods as the Monster character while inflicted with the Rage status effect.
Benefit: the Monster character starts with one Surge instead of none.
Notes: the Rage status effect can be granted by the Berserk spell (which the Monster starts with), a Rage potion, the Cracked Mirror memento, or Thousandfold's intervention.





Kingslayer



Hint: He may be imprisoned, but that is no justice for his crimes. My army will show him true justice.
Effect: The soldiers drove their broadswords through the husk of their former king.
Requirement: kill Brearg using the Soldier character.
Benefit: the Soldier character starts with a Broadsword in addition to their Spear.





Defensive



Hint: I will live a hundred stolen lives while you break yourself upon my shield.
Effect: Memory of a Soldier.
Requirement: block a number of attacks in a short period of time.
Benefit: unlocks the Soldier character.
Notes: requires a shield. The exact number of attacks is currently unknown but the easiest way to unlock this is against an arrow turret in a dungeon (the ones which fire continously).





Simple Weapon Proficiency



Hint: Who needs all those fancy weapons? I have my trusty blade right here. I call her Godkiller. Never even been sharpened.
Effect: And look at all the materials I saved by not wasting them on her.
Requirement: kill one of the five main gods without upgrading a weapon.
Benefit: you start with +10 wood, metal, stone, and crystal.
Achievement Board - Row 3



The Cursed One



Hint: What are the odds of finding him here? At least a hundred to one.
Effect: Memory of the Rock.
Requirement: kill the Rock.
Benefit: unlocks the Rock character.
Notes: the Rock is an extremely rare encounter who you will encounter somewhere on the map. For reference, I've only encountered him three times in 117 hours and only in overworld areas (never in a dungeon so far).





To the Death



Hint: I am not done yet... And you will be coming with me!
Effect: I cannot bleed out now... There is still so much to do... I will slow the bleeding.
Requirement: kill a boss while bleeding out.
Benefit: slows the rate at which you bleed out.
Notes: the exact rate at which it slows the bleeding process is currently unknown.





Vengeance for the Meek



Hint: I may be a simple farmer, but the warmonger has gone too far. He has to die.
Effect: The peasants revolted, weilding (sic) sickles as weapons.
Requirement: kill Shield using the Peasant character.
Benefit: the Peasant character starts with a Sickle instead of their Wooden Sword.





Death by Flaming Arrow



Hint: I have found a great way to scare off bears and other large beasts. If I'm lucky I may end up cooking dinner too.
Effect: Memory of a Hermit.
Requirement: kill an enemy with a flaming arrow.
Benefit: unlocks the Hermit character.
Notes: one of the easier ways to do this is to shoot bandits through their campfires (or braziers in dungeons). Alternatively, start a forest fire when it isn't raining or snowing and starting firing away. Use bigger bows which have a higher cleave modifier.





Perfect Shot



Hint: Think I can get them both if I aim just right...
Effect: My bow is holding me back. I will need to forgo this hermit's refuge and procure a better one.
Requirement: kill two non-critter enemies with one arrow.
Benefit: the Hermit character starts with a Longbow instead of a Bow.





Back to the Grindstone



Hint: It started out as just a simple weapon. Now I am sure it could slay a god.
Effect: I will only buy mastercrafted weapons from now on.
Requirement: upgrade a weapon to +10.
Benefit: all purchased weapons start with a +1 upgrade.
Achievement Board - Row 4



Elementalist



Hint: The only way to truly appreciate magic is to master its every form.
Effect: And now I can better protect myself against all types magic.
Requirement: use each element of magic.
Benefit: you start with +10 Magic, Fire, Ice, Lightning, and Poison resistance.
Notes: for reference, the elements of magic are Magic (blue), Fire (orange), Ice (light blue), Lightning (yellow), Nature (green), Life (red), Death (black), and Null (colorless).





Pious



Hint: My god loves me. I have dedicated my life to him and he'll surely not abandon me now.
Effect: Memory of a Priest.
Requirement: pray at a shrine until you reach maximum favor with that god (i.e. all candles are lit and burning white).
Benefit: unlocks the Priest character.





Surpassing the Master



Hint: Just how much of a mage could a chunk of obsidian really be anyway? The time for praying is over. Now is the time for some new blood.
Effect: Magic bows to me now. No more neophytic projectiles, I will cast a concentrated beam of pure magic to destroy my enemies.
Requirement: kill Obelisk using the Priest character.
Benefit: the Priest character starts with the Magic Beam spell.





Masterful Lock Picking



Hint: Fine! I don't need a key anyway! I will just smash it open!
Effect: Next time I will remember to bring an extra key.
Requirement: destroy five locked chests.
Benefit: you start with +1 key.
Notes: locked chests are the silver ones.





Experimental Liquids



Hint: Ten vials. All different colors. Let us see what happens.
Effect: Memory of a Chemist.
Requirement: discern what all 10 potions are.
Benefit: unlocks the Chemist character.





Weapon Mastery



Hint: I have a dozen different weapons here. Any of them will suit my needs.
Effect: A true veteran of war can utilize sword or lance.
Requirement: equip 12 different weapons.
Benefit: the Veteran character starts with a Lance in addition to their Falchion.
Achievement Board - Row 5



Storm Brewin'



Hint: I barely had time to react. There was a flash and a deafening noise. A moment later the three bandits laid smoldering on the ground.
Effect: I will never again underestimate the dangers of lightning. Next time it will be five bandits.
Requirement: kill three non-critter enemies with one lightning strike.
Benefit: lightning strikes now jump up to five enemies.





Bloodied Bandit



Hint: I've a sizable bag of coin now, and all it took was five murders.
Effect: Memory of a Criminal.
Requirement: kill five civilians.
Benefit: unlocks the Criminal character.
Notes: unless you get lucky with random civilian spawns, you will need to go to Town, Village, or Castle to get this achievement.





Backstabber



Hint: These soldiers going on about honor on the battlefield. I'll stick to the shadows, thank you. Honor means little when you're dead.
Effect: Memory of a Misanthrope.
Requirement: backstab an enemy while undetected.
Benefit: unlocks the Misanthrope character.
Notes: this requires you to kill the enemy with the attack. You know when an enemy has detected you by a question mark appearing above their head.





Ghost



Hint: I made it through... Nobody even knows I was here.
Effect: Those of cynical nature should hide themselves in an ordinary cloak.
Requirement: complete an area or dungeon without being detected.
Benefit: the Misanthrope character starts with the Ordinary Cloak memento.
Notes: if you have any followers, have them halt where you spawn and then sneak across an entire area or dungeon. The Ordinary Cloak memento and Stealth spell help tremendously with this achievement, as well as the Leather Belt memento as a last resort. You might also get lucky and find a small dungeon with no enemies in it.





Toro



Hint: The boar is beginning its charge. Now is the time to strike!
Effect: I will be quicker in the future.
Requirement: kill a charging boar with a melee weapon.
Benefit: you start with +15% Speed.
Notes: I believe I got this achievement by killing a charging hawk during the Daughter of Sow boss fight (where there are no boars) but that was one of my first runs and can't conclusively confirm it since I don't remember.





Battle Ready



Hint: Never enter a battle without your armor. This suit has protected me for fifty years.
Effect: Memory of a Veteran.
Requirement: have a large amount of armor at once.
Benefit: unlocks the Veteran character.
Notes: the easiest way to get this achievement is to save up metal and pray to Shield, preferably with a forge and a steady supply of stone nearby. Having a higher Knowledge stat reduces the cost when praying to altars. You might also get lucky and find a room in a dungeon with five or more altars which can include duplicates.
Achievement Board - Row 6



Experienced Trader



Hint: I've enough goods to make a hefty profit.
Effect: I would consider myself an experienced merchant. What should I purchase with my new earnings?
Requirement: sell 100 resources of any kind to the market.
Benefit: purchased items are cheaper.
Notes: the exact amount of the discount is currently unknown. Crystal will be the hardest resource to collect and you may have to destroy some mementos to get enough.





Hoarder



Hint: I filled my bags of every good I could find. I will eventually either sell it all or make myself some nice weapons.
Effect: Memory of a Merchant.
Requirement: have 99 of each resource (wood, metal, stone, and crystal).
Benefit: unlocks the Merchant character.





Arms Dealer



Hint: I have recently purchased five marvelous weapons.
Effect: I think I will keep the crossbow. It will complement my short sword and help me protect my wares from roving brigands.
Requirement: buy five weapons using the Merchant character.
Benefit: the Merchant character starts with a Crossbow in addition to their Short Sword.





Weaponsmith



Hint: It is going to take at least a hundred ingots to make every weapon I want.
Effect: Finally done! Careful, it is still a little hot but I can think of no better weapon for this simple blacksmith.
Requirement: spend 100 metal to buy weapons.
Benefit: the Blacksmith character starts with an Embered Cutlass.





Smithy



Hint: Why go to the trouble of purchasing and transporting steel ingots when I can just break down the five or so weapons I keep at home?
Effect: Memory of a Blacksmith.
Requirement: break five weapons.
Benefit: unlocks the Blacksmith character.





Forgemaster



Hint: Day in, run the forge. Day out, run the forge. I have only fifty ingots and already me arms are near to falling off.
Effect: This should provide a score of metal ingots for me to use as I please.
Requirement: smelt 50 stone into 50 metal using the Blacksmith.
Benefit: you start with +20 metal.
Achievement Board - Row 7



My Hands Are Clean



Hint: I need not soil my gloves with trivialities like god-slaying. Others will do it for me.
Effect: Memory of a Tyrant.
Requirement: kill one of the five main gods without using weapons.
Benefit: unlocks the Tyrant character.
Notes: followers, the Sawblade memento, and spells are fine to use.





Cold Snap



Hint: I knew he could see me with those frozen eyes. One quick thrust and his blood welled up through a hole in the ice. He never had a chance.
Effect: The cold is a weapon. I will use it to kill my frostbitten enemies even faster.
Requirement: kill a frozen enemy.
Benefit: you are now guaranteed a critical hit on frozen enemies.
Notes: the easiest ways to freeze an enemy are hitting them three times with a weapon that has the Frozen weapon enchantment (level 2) or standing near an enemy with the Faded Mittens memento for long enough (preferably undetected).





Healthy



Hint: I need to stay as healthy as possible.
Effect: The benefits of healthy living will help me in the coming days. I feel I can live even longer now.
Requirement: get 200 current and maximum Health.
Benefit: you start with +20 Health.
Notes: collect Human Heart mementos or hope you find a Vampire Skull memento early enough.





The Bloody Sacrifice



Hint: I have performed this ritual three times today already.
Effect: Memory of a Cultist.
Requirement: activate three blood rituals.
Benefit: unlocks the Cultist character.
Notes: having the Leech memento helps a lot but isn't necessary.





Treasure Hunter



Hint: I have four treasure maps here. Mayhap I can decipher them all!
Effect: Memory of an Explorer.
Requirement: uncover four buried treasures.
Benefit: unlocks the Explorer character.





No Time to Lose



Hint: I reached my journey's end in only fifteen minutes.
Effect: I have no patience to waste on long treks, these teleport glyphs should help.
Requirement: kill one of the five main gods under 15 minutes.
Benefit: the sanctuary now has teleport glyphs to key areas around it.
Notes:
- Since the final area is typically a larger maze-like area (except for the Grotto), you'll want to try to spend less time on the first two areas.
- Avoid going to dungeons and especially avoid taking any detours (i.e. Forest to Village to Cemetery or Town to Sewer to Marsh).
- You always start at the southern cardinal point in the Town and Hedge Maze and their exits are at the northern cardinal point.
- You always start in the eastern cardinal point in the Labyrinth and its exit is at the western cardinal point.
- In the Tomb, Castle, Prison, and Crypt, the path to the boss will never have locked doors (presumably as a failsafe in case you don't have, or can't get, any keys) so you can use that to your advantage and ignore all locked doors unless you have time to spare and need to power up a little.
- I recommend going with weapons that you're familiar with and upgrading them in the sanctuary prior to starting. You should also use the altars around the area (if they're still around), particularly Thousandfold's just for the stats.
Achievement Board - Row 8



No Weapon Required



Hint: A weapon is far too impersonal for this denizen of violence. I only trust my fists.
Effect: And they will serve me well in the days to come.
Requirement: kill a boss with your fists.
Benefit: your fists deal more damage.
Notes: I believe you can damage the boss by other means and only need to land the killing blow with your fists.





I Survived the Plague



Hint: For three score days the disease ate away at my body and my lifespan has certainly been reduced to a fraction of what it once was
Effect: Memory of a Doctor.
Requirement: lose a certain amount of maximum Health to poison.
Benefit: unlocks the Doctor character.
Notes: the exact amount of Health that you need to lose is currently unknown.





A World Dies



Hint: The children of Sow shall stand no longer. Destroy a hundred and half again as many and let us see this sleeping god react.
Effect: As a bonus, we've now more lumber than we will ever need. I kept twenty logs for myself.
Requirement: cut down 150 trees.
Benefit: you start with +20 wood.





The Greatest Memory



Hint: The crystallized experience is scattered everywhere. I've counted at least five hundred so far. Think of all the memories I will have!
Effect: Some memories I will never forget. I will begin my new life with additional experiences to draw from.
Requirement: have 500 experience at once.
Benefit: you start with +40 experience.





Learned



Hint: I poured through the library and found five books that should be of great help to me. Now to deal with my insatiable thirst...
Effect: Memory of a Historian.
Requirement: learn five spells.
Benefit: unlocks the Historian character.





Fractured Mind



Hint: All these different memories... I do not know who I am anymore.
Effect: I suppose I will just let the memories come as they will. At the very least it should be easier to obtain them.
Requirement: collect 36 unique mementos.
Benefit: mementos cost 20% less to purchase.
Discovery



Monster Hunter



Discover 35 enemies.
Notes: discovering an enemy requires you to kill one.





Weapon Enthusiast



Discover 60 weapons.
Notes: discovering a weapon requires you to pick it up.





Necessary Accessories



Discover 15 equipment items.
Notes: discovering an equipment item requires you to pick it up.





Conflicting Memories



Discover 100 mementos.
Notes: discovering a memento requires you to pick it up.





Magician



Discover 35 spells.
Notes: discovering a spell requires you to cast it.





Explorer



Discover 100,000 map tiles.
Collection



Recovering Amnesiac | Wise Beyond Words

Notes: praying at Obelisk's altar will sometimes yield lore pages at the highest level of favor.
Analyze



Zoologist



Completely analyze 10 enemies.
Notes: analyzing an enemy requires you to kill 100 of them.





Weapon Expert



Completely analyze 20 weapons.
Notes: analyzing a weapon requires you to kill 101 enemies with it.





Ready for Anything



Completely analyze 5 equipment items.
Notes: analyzing an equipment item requires you to pick it up 10 times.





Should Probably Make a Guide



Completely analyze 25 mementos.
Notes: analyzing a memento requires you to pick it up 15 times.





Wizard



Completely analyze 15 spells.
Notes: analyzing a spell requires you to cast it 50 times.
Damage and Killing

The Death of Violence



Kill Thousandfold.
Notes: refer to the One Solution achievement for the steps required to unlock Thousandfold. Once you have done so, a new portal called "The True End" will have appeared in the usually-empty hut directly to the north of Song's chapel. When you enter the portal, you will have kill yourself by either running into the spike, bleeding out, or using the Blink spell to teleport out of bounds whereupon you will get the achievement.





The Ultimate Irony



Kill Song.
Notes: in order to unlock Song, you must complete the three blood rituals located in the area around the sanctuary (at least one of which requires you to be on a New Game+ loop). Once completed, a fresh blood trail will be on the ground leading from the starting point to Song's chapel where a new portal called "The Beginning" will have appeared. Enter the portal to face Song. The blood rituals are marked as a, b, and c on the following map:

- b requires you to have at least 150 Dexterity and a weapon with at least 100 Weapon Power in order to destroy a fleshy wall which is blocking access to it. The less Dexterity you have, the more Weapon Power you will need and vice-versa. For instance, if you have 200 Dexterity then you only need 75 Weapon Power.
- c requires you to be on a New Game+ loop since the guillotine deals 300 damage to you. Either build up Health via the Vampire Skull, have the Egg memento, or have the Blink spell.
- Lure any of the enemies around the sanctuary into the blood circles and use them as fuel for the rituals. If you run out of enemies in the sanctuary then you'll have to finish the process on another New Game+ loop.





Hero of the Day | Party of Champions



Kill 5 different gods with one character. | Kill 5 different gods with ten different characters.
Notes: the Monster character does not count towards these achievements.
Miscellaneous



Gravekeeper



Fill the graveyard.
Notes: this requires you to die 24 times. You will see the graveyard around Veil's altar become populated with gravestones which represent your deaths.





I Am...



Unlock 15 characters.
Notes: there are a total of 17 unlockable characters so you can avoid ones which are hard or tedious to unlock such as the Tyrant, Doctor, and Martyr characters.





Self Reflection



Become a Monster.
Notes: this requires you to either activate Thousandfold's intervention or die with the Mirror shield equipped. Note that interacting with the pit after killing one of the five main gods counts as dying (as opposed to interacting with the clock which brings you back to the sanctuary for a New Game+ loop). This lets you complete a playthrough (and possibly unlock other character-related achievements or gain progress towards godslaying achievements) while still unlocking this achievement. If you accidentally unlock the Monster character and don't want to play as them, quitting and re-launching the game will return the normal selection of characters.





Plunderer



Uncover 100 treasures.
Notes: if you are going for all other achievements then you will get this organically. If you want it sooner then play as the Explorer character since they start with a Map Scroll memento.





Surgeon



Remove 16 tumors.
Notes: the 16 tumors are located in the area surrounding the sanctuary and some require you to be on a New Game+ loop. See the following map for their locations along with some notes for a few of the harder-to-reach ones:

- 3 and 4 are guarded by a Ribbed Horror.
- 6 requires you to have at least 150 Dexterity and a weapon with at least 100 Weapon Power in order to destroy a fleshy wall which is blocking access to it. The less Dexterity you have, the more Weapon Power you will need and vice-versa. For instance, if you have 200 Dexterity then you only need 75 Weapon Power.
- 7 requires you to be on a New Game+ loop since the guillotine deals 300 damage to you. Either build up Health via collecting Human Hearts or Vampire Skull, or have the Egg memento.
- 9 has a pressure plate on the west end of the corridor which lowers a stone door on the east end for a total of three seconds. You require at least 180 Speed, be sprinting prior to activating the pressure plate, and dodge when you're close to the end of the corridor (you'll have to dodge while sprinting a few times in order to get used to the distance you cover so you can learn when to time your dodge) in order to make it to the other side before the stone wall closes.
- 11 requires you to be on a New Game+ loop and have the Blink spell.
- 12 requires you to have a weapon with at least 250 Weapon Power in order to destroy obsidian or activate Veil or Obelisk's intervention to destroy all breakable objects in the area of effect.
- 13 requires you to either have at least 200 Speed so you can jump over the pressure plates or be on a New Game+ loop and have the Crystal Shield or the Mirror with a number of Surges.
- 14 requires you to either have at least 200 Speed with a number of Surges to heal when you get hit between dodges or be on a New Game+ loop and have the Stone Shield, Tower Shield, Greatshield, any other shield with a number of Surges, or the Blindfold equipment.
- 6, 7, 9, 12, 13 and 14 can also be done on a New Game+ loop with the Blink spell.
Conclusion
One of the reasons I decided to make this guide is because I found that information for various achievements (as well as just unlocking things in general) was spread out all over the discussion forums and the unofficial wiki mostly has ripped data (some of which is actually incorrect). Does anybody know if Simple Weapon Proficiency requires you to never even pick up a +1 weapon? Is it fine to use an upgraded weapon as long as you don't upgrade it yourself?
