I think the issue people may have been running into is if the game RNGed from 1 day of tired to sick the next day which limits the total amount of days to 3 which can of course cause issues with maintaining the schedule. Make sure you pay attention to changes in status between days. It should go "you feel tired/still tired/sick/still sick" from day to day. If it skips a tired say and escalates to sick, reload the save and try again.
In Emperor rank 6, picking "I'll help out, too!" will prompt Hidetoshi to confess in rank 9, and for him to have extra dialogue in March
On the other hand, in Temperance rank 1 if you pick "Aishitemasu." ("I love you") as Bebe's farewell line, he will admit to have always known what it meant, and that he meant it every time he said it in rank 10
There's not much I can do to help with carrying over your sickness as it's pretty much luck-based. The only thing I can suggest is to go play the crane game once, save, and then continue with your daily activity 10x. If none of them carried over your sickness, play another crane game, then save and repeat the process again.
