For people here looking for the new upgrade:

Fast reload - in the cabin with log entry 15
Grenade Pouch - at the same campsite as log entry 3
Frag Grenade - in the railcar with log entry 4
Armor plating - top of tower in the south of the map (climb onto the roof)
Ammo bags - On the roof of the abandoned house, east end edge of the trail north of the rail car

my code was 4936. its probably completely random everytime so theres no use keeping note unless you want to type out every possible combo of four digits.

In mine the code was 4278 or so it looked like. It was actually 4218. The one looked like a seven. Hope this helps

so can you not have every weapon in inventory? pistol is 1 and 4 but shotty ar and sniper can only go 2 and 3....

Nevermind, found a review complaining about crouch jumping and he explained it… INCREDIBLY obtuse. I never would have tried hitting the crouch button in mid air to jump higher.
