Brief
The longest you can normally hold a note in the game is 4.5 seconds. When you run out of breath, the game denies any input (other than mouse movement) and you're left flushed for 3 seconds. As a result, the game's standard song list never has a note that exceeds this limitation, however, when you download Custom Songs, those limits are non-existent. The Charters (the ones who make the song's chart map) have free reign to make any note last longer than those 4.5 seconds.

This is the issue that many inexperienced players end up stumbling on, unable to surpass the 4.5 second note barrier.
No Modding Route
This section is for those who do not wish to use mods to surpass the 4.5 second note limit, and will mainly utilize the gameplay mechanics the game has offered you.

The Indicator
The game doesn't tell you, (I think) but there is an indicator for how long you've held a note and how much time you have left before you blush.

It can be seen in the left side of the display, here.

Every note you play, this wavy line as shown within the span of 4.5 seconds will move towards the left edge of the screen.

Once this wavy line is off-screen, your character will run out of breath, unable to play for 3 seconds.

We want to prevent the wavy line from reaching the left of the screen, and there's an easy method for doing so.

Shorting the Toot
🢃 Take a look here 🢃

The wavy line fell back, and we got a Perfect score.

You can probably can tell through the compression, this was done by simply releasing the input for a brief moment.

The game is lenient enough to let you toot before the note reaches your cursor, the same will apply when it comes to playing that very long note, however, if you continuously shorten your toots, regardless of the length, the game will begin to penalize you with a lower score.

That's basically it, Shorting the Toot is the solution if you don't want to use any mods.
Modding Route
This section is for those who mainly do not want to deal with the specifics of the game's mechanics and want to bypass the restrictions that are placed on the player.

Follow the guide below until you finish the Run the Game and Quit section. If you've already done so, you can skip right over it.

Download the DLL file here: https://cdn.discordapp.com/attachments/1022946803550793781/1023810893844324383/CircularBreathing.dll

What you are downloading is the Circular Breathing Mod. As the image above states, and if you're well-versed in your wind instruments, it's a breathing technique used by players to produce a continuous tone without interruption.

Now, I can understand how extremely sus this link is, but this is the solution if you really don't want to deal with the game's mechanics. Plus, this is literally the only source for this mod, no GitHub or Drive Link. Once a proper page for this mod is in set, the guide will be updated accordingly.

Once you have the DLL, place it in your plugins folder.

You're basically done, run the game and you can toot as long as your heart desires. You'll never have to worry about running out of breath.
FAQ
This section will answer questions regarding the mod or techniques in this guide.

Why is the game lenient about timing and holding notes?
I don't have a good answer for that, though, in my personal opinion, this was likely an effort to future proof for custom songs. Obviously the devs didn't think the game would receive widespread attention, this is just a silly trombone game.

Why is the Circular Breathing Mod a thing?
A lot of Charters tend to use another mod, called Autotoot, to benchmark their charts, mainly to save the effort of playing the chart themselves. The chart could be too difficult for them, maybe they played enough of the chart that they're sick of it, or they need to work out the timing of the notes as DAWs have a tendency to be inaccurate.

Autotoot doesn't have Circular Breathing by default, and that poses a few issues when playtesting charts. It could be said Autotoot's limitations are what Charters should follow, either the playability or the accuracy of the chart, though that's a discussion for another day.

To quickly short the toot while using the keyboard, continue to hold the first key you used, quickly tap any other keyboard key.
