Endless Nameless
When your bullets leave the screen from one side, they will wrap around to the other side of the screen.
Police Badge
Monsters have a chance to drop money upon being killed.
Snuff Film
Every few seconds, all monsters on screen will take damage. This will be indicated by your screen flashing red.
Isaac's Head
Spawns a friendly turret that follows behind you and shoots bullets that will hurt monsters.
Firwork
Turns your bullets into Explosive Missles, which deals damage to a large area.
Safety Pin
Makes your bullets pierce through monsters, allowing you to hit multiple targets with one bullet.
Wrench
When used, changes all uncollected Items on screen to another Item.
The Anarchist Cookbook
When used, grants a temporary Damage upgrade for the Board.
Cigarettes
When used, grants you a temporary Movement Speed upgrade and makes you invulnerable for a few seconds.
Lottery Ticket
When playing the Slot Machine with this item, the machine will pay out with prizes more often. Upgrade Rooms also have a chance to have more or less Items.
Anxiolytic
Acts as a Damage upgrade, so long as you can keep a Combo Timer from hitting 0. This timer gets reset at the start of a new Board and when killing an enemy.
Kerosene
Turns your bullets into a short-ranged spread of flames.

Blocks most enemy projectiles, can still be destroyed with certain attacks.

Every handful of monsters you kill you gain +1 Heart.

Increases your Fire Rate.
House Key
When used, grants you acess to the Secret Room at the end of a Dimension if you lost it by taking damage.

Scry Bullets
Makes your bullets phase through walls.
Rubber Bullets
Makes your bullets bounce off enemies and walls.
Heroin
Allows you to manipulate your stats in any way you want at the cost of 1 Heart. Press 'CTRL' to change what stat to manipulate.
Severed Finger
Shoots a short-ranged, weak bullet in the opposite direction you are facing.
Lube
When used, temporarily increases your Fire Rate for the Board.
Molotov Cocktail
When used spawns a projectile that explodes upon impact with a wall or monster.

Counterfeit Money
Doubles your current money amount.
Orbitoclast
When used, deals 1 Heart of damage to you but insta-kills all monsters on screen and halves the health of all bosses.
Coinfriend
When used, grants you +1 Coin.
IV Bag
Upon taking damage, you gain money. It also fills up a tank for free in Blood Donation Machines.
Undefined
Corrupts your bullets.
Dead Butterfly
Gives your bullets wings and makes them move in a up-and-down type pattern as they travel.
Stolen Panties
When used, temporarily increases your Bullet Speed for the Board.
Rat
Gives you a pet rat. :)
Spoiled Milk
Grants +3 Hearts.
Envelope
Grants you a random Stat Upgrade for every 25 Coins you gain and don't spend.

Reveals the truth.

Increases your Damage.
Meat Scythe
Changes your bullet into a scythe that you throw, and can only be thrown again when it returns to you again.
Paddle
Increases your Fire Rate, along with temporarily increasing your Fire-Rate for every enemy you kill on a Board.
Estrogen
Increase your Bullet Speed, all Healing Items grant +1 extra Heart.
Apple
Upon use, depending on how rotten it is, will either grant Hearts or take them away to gain a Damage increase.
