Achievements here will be listed in general order of complication and difficulty.
Faster and easier ones on top, and difficult, conflicting achievements listed lower.
I made this guide so no one has to open up a dozen tabs and think a dozen things, and go to the wiki page, and the steam discussions to get what they need done.
There will be spoilers obviously.

If you are starting from nothing and want to get everything in one playthrough.
Start by going to the endings section. Your key concerns of being softlocked out of achievements are "A Series of Temporary Measures Strung Together," and "The Path of the Righteous Man" achievement.

And Ctrl+F is your friend.

Remember to SAVE YOUR GAME.
Kill a named character out in the open or waste someone in a compound?
Save your game.
Going into zones of White or Red strain infected?
Save your game.
A day went by?
Save your game.

Remember, if you want all achievements,
Save often and save a lot.
Soft-locking yourself out of achievements and frustrating gameplay will torture you.
Crafting and Construction Achievements
A Present for Vyacheslav
Make a molotov cocktail (requires a rag, cheap vodka and a match).
Materials required for making a Molotov can be found by looting zombie corpses and towns Save your molotovs for the "Some Men Just Want to Watch the World Burn" achievement.

Sterilized
Make a bandage by sterilizing a rag with some cheap vodka.
Usually one of the first achievements everyone makes.

Hindenburg Line
Build a pillbox.
Requires:
4 star builder,
Toolbox,
8 Cement.

Flame-Resistant
Build a concrete watchtower.
Requires:
5 star builder,
Toolbox,
12 Cement.

There's Too Many of Them Out There
Build fences all around your bunker, so that it is completely enclosed.
The fastest way, and easiest to achieve this is to save your game, simply to build a wood fence directly square around your bunker and get the achievement, then reload your game. Otherwise, you're going to be busy building fences and gates around choke points to build your ideal base. Mountains count on enclosing your base.
Destruction and Combat Achievements
Slow Zombies
Cripple 10 infected with legshots. When you are targeting an enemy, move mouse down to select legshots. Firearms skill level 1 required.
Don't forget to stand still. Usually the zombie makes a lunge, or grabs you, you kick it off, and while it's getting back up, you take your time to shoot it.

Shoot Them In The Head
Kill 10 enemies with headshots. When you are targeting an enemy, move mouse up to select headshot. Firearms skill level 3 required.
Your bread and butter once you are able to. Easy Sniper Rifle locations listed on the Map section, and getting easy Firearms skill level listed in Walkthrough under the quest "Learning to shoot"

Some Men Just Want to Watch the World Burn
Burn down 10 buildings with molotov cocktails.
If you've saved your molotovs, Either clear a gang compound by recruiting or murdering them. Then throw molotovs at the shacks. Walls don't count. Doing so is an act of war, keep that in mind.

Splosions
Blow up 10 buildings with a Rocket-Propelled Grenade Launcher.
The three methods of Acquisition:
Looting corpses or recruiting from FEMA
Looting corpses or recruiting from Los Ceros
completing the quest "Selection process" and deciding to help Em Arcine into the inner circle

Tower Fall
Shoot an enemy in the leg, who is in a watchtower, causing them to fall out of the watchtower (Firearms skill level 1 required)
There are many gangs with watch towers. Avoid Marian's Merry Men gang, as that guy has a sniper rifle on his tower. Best done as a surprise attack with multiple characters following you, so you can take your shot without getting stunlocked from being shot. It's an easy one, to save and reload on just for the achievement. The description of Slow zombies tells you how to legshot.

One Shot One Kill
Take down a White Strain Infected with a single shot to the head (requires a Sniper Rifle and Firearms skill level 5)
Firearms skill level 5 will be the chore. The sniper rifle locations, and White strain infected zone are listed in the map section.
What you will most likely need, is gun enthusiast magazines to save you the trouble. Getting easy Firearms skill level listed in Walkthrough under the quest "Learning to shoot"
Simple One-off Quest Achievements
The Forgotten Heist
Somewhere in the area is a getaway vehicle from a long-forgotten heist, containing an assault rifle, a backpack, and a pile of now-worthless cash. Find it.
Directly North East of the bunker, and enclosed off a cliff South West of the Highwaymen gang's intersection. There lies a wrecked car. With useless money and the important assault rifle, and a small backpack. I advise doing this achievement as soon as possible for the gear is useful at the start. Map location in the walkthrough: Maps section

Memories
Find Auntie Abbie's locket and return it to Annie Dillinger.
Annie Dillinger from Fort Kohai provides this quest "Inseparable." Joe Wheeler will not get her quest, so anyone like Alice can speak to her to get the quest. fulfilling her request, loot the zombies for the item and return it to her.

Mind Probe
Find the Brain Scanner.
Related to the quest "Internal Workings of the Mind," given by talking to Isham Seely after you decide what ending you want, which is also after you get enough insulin from talking to Alice. However, you can find the item first, the quest dialogue triggers despite having it in your inventory anyway. This one involves a lot of red strain infected, so bring red strain antigen, and good guns, enough survivors to take on a horde of red strains. Location listed in the map section.

Don't Starve
Steal or destroy all of another community's crops, without ever starting a war with them.
Pairs as an achievement

Grandma's got a Shotgun!
Who is Jim Gallager?
*Or before*
the McCoys v Fat Neils achievements.

After finishing Jim Gallager's 2 quests, recruiting him allows you to loot all of his crops. Burn what you can't take.
If you take the time to plan the recruitment of Connie Reed, the farm is abandoned, and if there are plants left behind, burn or steal them. More details in her stated achievement.

If you're not having any luck with those 2 options, or simply can't find crops to burn, the McCoys, and Fat Neils have crops that are either surrounded by walls, and often unguarded at night, making them easier to steal and burn with no one being any wiser. It will determine the fate of the It's Crispalicious! and You Love It! achievements, so it's best done as one of those "save, achieve, and reload" achievements.

Champion Of The World
Defeat an opponent with skill level 5 at fisticuffs (you'll need to wager 100 gold).
Pairs with freeing character quests,
"Napoleon Complex"
"I Don't Negotiate With Kidnappers"
and the "Maximum Respect" achievements

Fitness Magazines help. Corner the guy into a wall, and time your hits.
It's free money and respect. Challenge everyone you can to fist fights.
Unlike approval, which has you spending money to give gifts, a fight gains you money when you win. If you want to be really scummy with it Savescum to win and not even get hurt.
Gain Medicine skill if you get hurt and self bandage, gain money if they get hurt and you can charge them. If you want more approval than money, have Alice chase after the loser, and heal them for free, because Joe the cheapskate won't.
Collectibles and Insulin Achievements
Crate Expectations
Find and investigate all the crates in the area (there are 47 in total).
Locations listed in the Map Section.
Most crates are guarded by red strain infected, looters, or white strain infected. All of which can be killed to no consequence of other achievements. Bring gear and adjust tactics accordingly. Sniper rifle for looters, and a numerical survivor advantage against fatigue for zombies.
The crate Just south of the FEMA camp is blocked by a wire fence. and cannot be accessed until lategame, when you manage to acquire a Rocket Launcher. See the Splosions achievement for details involving getting one.

A Series of Temporary Measures Strung Together
Find all the insulin in the area and give it to Alice (there are 62 doses in total).
This is going to be a long one. And will be your main obstacle and time limit against the achievement "The Path of the Righteous Man."(TPOTRM Achievement) Keep that in mind. You will need separate saves if you want to complete "TPOTRM" Or speedrun through as much as you can with less of the doses. They need to be in Alice's possession, in her bloodstream or inventory and you should not complete the quest "Missing" as its conclusion will make Alice leave.
There are 62 doses of insulin in the map. That said, never give Alice chocolate.
It's going to be tricky to get do all of this in one playthrough.

You may shakedown or recruit some(Not Amelia Murphy) characters for keys to the pharmacies and hospitals. It does not count against "TPOTRM." However, won't look good to Alice, or the associated faction. Don't forget to trade with these characters so they hand you their stuff, instead of their Gold. Shaking down someone twice in a row usually results in them turning hostile.

40 out of 62 shots of insulin are achievable without voiding "TPOTRM." Make a separate save to get the rest if you want to get this achievement over "TPOTRM."

  • 6 in Sienna.
    Speaking to Isham Seely via Alice to obtain entrance into the Pharmacy.

  • 20 in Fort Kohai
    You can buy them all off Clay Parker, or make him love you so much that he joins you, netting all of them for free.
  • 5 in Red Sands
    Access to the Hospital is provided by Bud Anderson, after completing his quest "Alliance." Accepting his quest will lock you out of TPOTRM.

  • 2 in Santa Maddalena
    Sam Deetz will give them to you for free. The sweetheart.
  • 10 in Pequeno Canyon
    Access to the Pharmacy there is provided by Amelia Murphy After completion of the quest "The Death Penalty" after confronting her about lying, where she offers the key as an added incentive to complete the quest in her favor. Killing Tom 'n Butch will lock you out of TPOTRM.

  • 2 in McCoy's Compound, East of Fort Kohai.
    Johnny Casey will give them to you in the "Eye For An Eye" quest, but will lock you out of TPOTRM. Read: Fat Neils V McCoys Achievements section, on how to avoid this.
  • 5 in Johnny Casey’s house, SouthEast of McCoy Compound.
    After you finish Johnny Casey's quest, "Eye For An Eye." He will provide with you with the key to his house. Recruit or slaughter the squatters of Casey's house, up to you.
  • 2 in Fat Neil's Compound, East of Fort Kohai.
    Josephine Jackson will give them to you in the "Tit for Tat" quest, but will lock you out of TPOTRM. Read: Fat Neils V McCoys Achievements section, on how to avoid this.
  • 5 in Buckeye
    The locked Hospital. After you finish Josephine Jackson's quest "Tit for Tat." She will provide with you with the key to the Hospital.

  • 5 in Monkey Wrench Gulch
    Inside a locked Pharmacy, but Juanita Baca located in the Los Ceros base, will provide access after the quest "Monkey Wrench Gulch." Again, this quest can lock you out of TPOTRM.
    This quest is best done without her present, as she willmurder Alonzo Musgrave, during this quest. Which in turn, requires you to kill herif you want to avoid being locked out of TPOTRM.
Approval and Respect Achievements
Maximum Respect
Awarded if some character's opinion of Joe Wheeler reaches 100% Respect.
Ties with the "Champion Of The World" Achievement. Fighting people in fisticuffs gains you respect. Do that enough times in a bigger faction, and you can max it out easy as respect influences all characters of that faction. Don't lose, or you lose respect. Killing zombies in front of characters has the same effect. So luring a horde towards a friendly faction and killing the zombies in front of members of that faction works out too. The brain scanner is helpful here.

Stamp of Approval
Awarded if some character's opinion of Joe Wheeler reaches 100% Approval.
Gifts are the main way of increasing approval. Ask what's up for them to discuss what they like, or use the brain scanner on them to find out their traits that tell you what gift items they like.

  • Tasteful Erotic Novels(Characters with the sex-addict trait)
  • Sci-fi Novel(Characters with the nerd trait)

  • Box of Chocolates(Characters with the Chocoholic trait)
  • Fine wine(Characters with the alcoholic trait)
  • Pack of Cigarettes(Characters with the Smoker trait) Be careful, causes lung cancer in characters, and will kill the character if they consume enough to fill their "lung cancer bar"

AchievementIf you vote for Bud, which is necessary for the "I'm only interested in the gold!" achievement, he will die after disarming mines in the back passage of the Los Ceros compound. Everyone will blame you for it, decreasing their approval of you, Bud will tell Fort Kohai members to stop talking to you unless you pass each and everyone's approval check.Make sure you've gotten the key to the hollow rock bunker, for the "El Dorado" quest and bring him there to disarm the bomb first. Which will give you the "I'm only interested in the gold!" achievement.

Recruit any member of Santa Maddalena to your community.
7 People are not recruitable. Barring the collapse of the entire compound, but God won't let happen, right?

There are achievements for recruiting members of the Fat Neils V McCoys, "They started it!" and "Of course you realize this means war!" They are in the the Fat Neils V McCoys Achievements section, as there are also achievements for recruiting all of them without any deaths for one side or another

Who is Jim Gallager?
Recruit Jim Gallager to your community.
The easiest one of the named recruitment quests
Speak to Al Martin, residing in Santa Maddalena, who tells you to check up on him, in the quest
"The Rational Man," while doing that quest, help Jim do his own quest of building his town "Gallager's Gulch." After that, give him gifts if you still can't get him to join.

Just a Patsy
Recruit Bat Jacobs to your community.
Requires the "El Dorado" quest, given by Rachael Scull. During their stand off, select "I don't care about her. I'm only interested in the gold as a response. It won't be a big deal to say that to her if she already has some respect and approval for you. To recruit Jacobs, you can shakedown Rachel in front of him, or give him gifts, to increase either his respect or approval. Shaking down Rachel, will result in her giving you no reward for her quest, but not shaking her down will get you 100 Gold. Having him around when you findKit Wakemangives you extra dialogue.

Grandma's got a Shotgun!
Recruit Connie Reed to your community.
This one's going to take awhile, and requires the death of her Husband, Toby Reed at the completion of his "The Photograph" quest. It is required that you use Alice to give Toby reed the Photograph, and apologize, or she will be unrecruitable when he dies, because Wheeler has no people skills.

I recommend that you do not do his quest to begin with. Take your time, and Increase her approval by gifts and trading.
"The Classics" quest by Ed Hutchinson will help with this.
Trading her spare pistols you taken from looters and looters hordes increases approval. Moreover she's one of the few people who buy pistols for the higher price of 5 gold.

She also requires respect, which you cannot gain from fisticuffs, because granny don't bring fists to a shotgun fight. The strategy for getting respect is to leave Wheeler with an assault rifle next to her, and have another character lure chasing zombies up to her, where Wheeler automatically cuts them down in front of her. Luring hordes coming from the East, like cactus springs will do fine as well. Her respect is easily gained with less risk this way. Alternatively, you can do her husband's quest, where she will be alone to venture out on her own after her food runs out, and you'll be playing escort mission until she respects you enough to be recruited.
Gang Quest Achievements
I Guess I Have No Choice
Recruit a looter to your community.
Use gifts for approval, and fisticuffs for respect. All gangs except the Knights of Purity, and the Leader of The Grand Army are recruitable. You can alternatively, just slaughter a gang that tries to rob you to the last man, and force the guy to work for you, recruiting him or her.

I Don't Negotiate With Kidnappers
Free Bridget Cassidy without giving the kidnappers any ransom money or gifts.
The "White Knights" quest, given by Cathay Starr in Santa Maddalena.
The favoured method of peacefully freeing her, would be to challenge them to fisticuffs over and over, until their respect is so high, that upon attempting to purchase their ransom, would cost 1 gold. After that, you can be sure that if you shakedown their leader, they won't turn hostile. Make sure to trade all the gold out of their inventory, so you need not shake them down more than once. Take the key from shaking them down, and free the prisoner without consequence.
Or you could just slaughter all of them free her.
None of the Knights of Purity are Recruitable.

Napoleon Complex
Free Willard Williams without giving the kidnappers any ransom money or gifts
Vincente Alvarez of the Los Ceros gives the "The Grand Army" quest.
The same technique used in the "I Don't Negotiate With Kidnappers" Achievement can be used.
However, unlike the Knights of Purity, all except the the Leader of the Grand Army, D*ck Short, are recruitable. Steam is censoring what is literally his name.

Anyway, to complete the "Grand army" quest without getting blood on your hands, for what could potentially void the "TPOTRM" achievement. lead a Red Strain infected into the lone guy after recruiting everyone else, and shoot him after he turns into a zombie. Accidents happen.

I Am The Law!
Complete all the quests that involve taking down a gang of looters.
If you want to establish a friendly relation or want to recruit all looters.
When approaching a gang compound, Save. When you get close enough, one of them will walk up to you, and attempt to shake you down, tell them to try it. If they back down, that gang is now a non-hostile faction. If they don't, you're in for a fight. Reload a save, and attempt to enter the Compound from a different angle to get challenged by different gang members. It's apparently random, but there are some people with just a higher chance of backing down when trying to rob you.
Recruiting all of them counts as getting taking them down. So all of these quests can be completed peacefully.
Sorted by Location, Quest name, and quest giver.
Fort Kohai:
  • "Duck Hunt" given by Sarah Slaughter
  • "Looters Stole My Locket" given by Zee Franklin
  • "The Road to Santa Maddalena" given by Eliza West
  • "Devil's Gulley" given by Charlie Cashman. He dies at the completion of this quest.

Santa Maddalena:
  • "Big Sister given" by Dorcas Williams
  • "The Lord's Work" given by Maud Gilbert

  • "White Knights?" given by Cathay Starr. Not possible to recruit them, but not necessary to kill them either, related to the "I Don't Negotiate With Kidnappers" achievement. The quest might not even be necessary for this achievement.
  • "The Death Penalty?" given by Amelia Murphy. Not necessary for this achievement.

Fat Neil's Compound, East of Fort Kohai:
  • "The River" given by Jane Leroy.
McCoy's Compound, East of Fort Kohai:
  • "Circle the Wagons" given by Manuel Gregg

Los Ceros:
  • "Dune Bandits" given by Robert Fink
  • "The Gas Station" Mafia given by Ned Jackson
  • "Merry Men" given by Chris Nixon (Alonzo Doyle of Marion's Merry Men has a sniper rifle.)
  • "Kill Cassidy" given by Elfego Arcine. Part of the "selection process" quest.
  • "The Graybill Family" given by Em Arcine. Part of the "selection process" quest.

Vince's Gang and Cassidy's Gang are irrelevant.
Fat Neils V McCoys Achievements
You can keep all Fat Neils, and McCoys Alive, Not trigger a war(TPOTRM), complete both of their Tit for Tat, and Eye for an Eye quests, gaining all 14 shots of Insulin from both factions but however, this method, will bar yourself from "Crispalicious!" and "You Love It!" achievements.
You want to Save/reload for the "Crispalicious!" and "You Love It!" achievements, and follow this path for TPOTRM Achievement.
  • Complete Fat Neils Vs McCoys quest given by Wyatt Ingalls before accepting Tit for Tat or Eye for an Eye, selecting "Can't we all just get along" during the truce negotiations, with an outcome of a truce based on mutual hatred.
  • Now Complete Tit for Tat or Eye for an Eye.
  • War averted, and insulin gained, but you will not be able to recruit every member of either the Fat Neils, or the McCoys.

They started it!
Recruit any member of the McCoys to your community.
Gifts or fisticuffs. Get approval and/or respect, then ask them to join you. Or do the "Crispalicious!" achievement killing everyone, and recruiting the last Surrendering McCoy.

Of course you realize this means war!
Recruit any member of the Fat Neils to your community.
Gifts or fisticuffs. Get approval and/or respect, then ask them to join you. Or do the "You Love It!" achievement killing everyone, and recruiting the last Surrendering Fat Neil.

You Love It!
Recruit all the McCoys to your community (without any of them dying).
Complete "Eye for an Eye," quest, then do the Fat Neils Vs McCoys quest, siding with the Mcoys, by slandering the Fat Neils when asked your opinion. You will be locked out of TPOTRM for starting this war.

Crispalicious!
Recruit all the Fat Neils to your community (without any of them dying).
Complete "Tit for tat," quest, then do the Fat Neils Vs McCoys quest, siding with the Fat Neils by slandering the McCoys when asked your opinion. You will be locked out of TPOTRM for starting this war.
I'm only interested in the gold! Achievement
I'm only interested in the gold! (secret)
Find a way to get the gold from Hollow Rock Farm without setting off the I.E.D.
The completion of this quest will require the death of Chris Kane,
and another male expendable member of your faction.
And Kate Barlow, just because.
  • You first get this quest from Rachael Scull, who has you interrogate Bat Jacobs. Tied to the "Just a Patsy" achievement. read that for more details for this part of the quest.
  • After that, you need to find a guy called Kit Wakeman. Who can be found in Los Ceros Compound. Find him yourself, patroling the southern outer walls, or pay someone in the Los Ceros 50 gold to get a quest marker on his location.
  • He'll join you, double cross you, and try to get you killed with his gang. Bring Bulletproof vests, and heavy firepower. Expect heavy resistance.
  • After slaughtering them all to a man, the last guy will lie to you about where the gold is. Get their approval up to have them tell you the truth. Do whatever you like with the guy after that.
  • The truth is, that Kate Barlow has the key and location, so that's your next stop. You require to have a bunch of chocolates, generally higher approval with Los Ceros, and a male expendable character who will feed Kate Barlow Chocolates and die for her affection. The lowest stats I've seen, belonged to a Frank Howe, who I recruited off the Grand army.
  • Confront her with the fact that she just shot a man she liked, and she'll give you the key.
  • The Bunker with the gold however, is rigged to blow, so you can't access the gold without dying.
  • Complete the "Selection Process," with either Em or Elfego Arcine, to progress the "Missing" quest.
  • The Missing quest has an election of either Bud or Wyatt. Side with Bud, and Chris kane will follow you. Have him go to the bunker before you continue the "missing" quest.
  • Bombs disarmed, Gold's yours. You get the achievement, and will help with the "All You Need Is Gold" Achievement too.
  • When at war with the Los Ceros, find Kate Barlow and shoot her. May have something to do with the TPOTRM achievement, but All Los Ceros are fair game during your war.
Ending Achievements
All You Need Is Gold
Complete the game by amassing a vast amount of gold.
Money. Easiest method is via the "El Dorado" quest. tied to the "I’m only interested in the gold!" Achievement. Don't need to concern yourself with it. Pick love, and the option to do it later will appear. More information in the "Happy Ending" Achievement.

The Great Dictator
Complete the game by becoming the most powerful community leader in the area.
Recruitment. Or reduction. You want over 80% of the population to be in your faction, via either recruitment, or being wiped off the face of the map.
Don't need to concern yourself with it. Pick love, and the option to do it later will appear. More information in the "Happy Ending" Achievement.

Happy Ending
Complete the game by following your heart.
You require a higher Approval from Alice, which means less shaking people down, giving gifts, and getting approval and respect from other around you. Getting her as much insulin as possible, and she will speak to you about what you want to do. Pick love. She will also have a
4 part questline simply called "Alice Ivers" in this order.
  • learning to shoot
  • going for a run
  • building the solar-powered fridge
  • planting the safflower seeds

    Invade FEMA ONLY AFTER these things are completed.
Clara Allison, the Leader of FEMA will either tell Alice something that makes her leave, or drop a notepad upon death that when investigated, will make Alice leave. Talking to Isham Seely after this, will open up a dialogue allowing you to re-choose your ending,Forgetting about Alice,letting you complete Either Money, or Power Objective endings, orgoing after Alice,which is the Love Ending. This ties into the Secret achievement "What The Hell, Right," Just below.

What The Hell, Right? (secret)
This awardment is obtained if Isham joins you on your journey to Vancouver.
High Approval and respect from Isham Seely opens up some dialogue options after initially choosing Love. Just don't be a jerk about Alice, he'll give you the "Internal Workings of the Mind" quest, to be completed with him during your assault on the FEMA base, during the "Missing" quest. If complete, he will join you, when you decide to go after Alice in Vancouver, and thus your achievement.

The Path of the Righteous Man
Complete the game without killing anyone who didn't deserve it, executing those who do, and without allowing Alice to die.
    According to the dev:
  • Not kill Tom and Butch
  • Not accept Bud Anderson's mission to make an Alliance with Santa Maddalena
  • Not trigger a war between Fat Neils and McCoys
  • Either not do Juanita Baca's quest, or kill her when she murders Alonzo
  • Either vote for Bud, so Wyatt doesn't get assassinated, or somehow execute Ida for doing it
  • Kill Felipe Espinosa
  • Kill Mike Doolin
  • Kill Clara Allison
  • Not attack any other communities apart from looters, FEMA and Los Ceros
I have listed achievements that can conflict with getting this achievement, and anything that would be important in regards to this achievement with "TPOTRM" Best use Ctrl+F to look through.
But I'll go through most things of note here.

-On the safe side, selecting "Die Scum" is not a dialogue option unless they are part of Los Ceros, or Fema. Looters are always hostile.

-Any gang that tries to shake you down, can be slaughtered to a man on the basis of self-defense.

-You can kill Cornelius Chacon, as a necessary part of the "Selection Process" quest, along with anyone you are sent to kill in that questline because they all, even the Lone Wacko conveniently have blood on their hands.

-The three people listed, Felipe Espinosa, Mike Doolin, and Clara Allison must die. It doesn't matter how. If you recruit them, you can take away all their guns, and run them into a horde of red strains.

-Kate Barlowas part of the El Dorado questline will kill one of your members. It may or may not be part of the achievement, but she definitelydeserves to die.

-D*ck Short. That's his name. cannot be recruited, but is required to be killed for the "Grand Army" quest. It would be real unfortunate if a red strain infected chasing you, just happened to make their way to his base, found him all by his lonesome, and bit him.

"White Knights" quest is not necessary for "I Am the Law"

I think "What The Hell, Right?" requires Isham Seely to be present when you explore the military convoy. If he is not with you, he ends the remote conversation with something like "I hate you" and that blocks the achievement.

Incredible guide but your crate map is missing one on the raised peninsula right next to / inside peninsula. It's #31 on the other guide you have linked right below your map.
