Preface
The achievements are ordered by increasing rarity (as of the time of writing this guide). I created this guide to provide the first complete in-depth guide for the little playerbase the game has. Additional information from comments is much appreciated as some unlocking conditions remain partially unclear.

Game difficulty: 3/10
Trophy difficulty: 6/10 (if "Cartographer" had any invalidity indicator 2/10)
Glitched trophies: Maybe "Cartographer"(unlocking conditions unclear and no invalidity indicator)
Minimum playthroughs: 1

Short notice:
Unfortunately, Steam decided to randomly delete some of my screenshots, and I was not prepared for this to happen. Sadly, I do not have any backup images saved on my computer, and I have no plans to recreate them (seems like they could disappear again at any time, so why go through this hassle again). Sorry. However, I will do my best to make up for that by being more descriptive in the text.
Hidden Place
I noticed that this achievement was unlocked only a few minutes after it actually happened. With regard to the achievement's image, it probably unlocked when I touched a teleporting tree, similar to the one shown in the picture to the left.

What sets it apart is its different color from the others (not all teleporting trees have a distinct color from its surroundings, probably the color does not matter) and the particles it emits. When I walked through it, the tree teleported me to a secluded place with no other entry or exit than another teleporting tree (maybe only such places count, maybe any teleporting tree will do, who knows).

Invalidity/ Occurency:
There are usually multiple opportunities in a single playthrough to obtain this one, so do not worry if you missed a chance.
Maverick
For this one, the player character has to perform a cannonball jump into deep water. Water counts as deep, if the player starts swimming when entering it. Either locate a nearby hoppable bush or sheep, interact with it to trigger the jump and aim for the deep part of a water body. The sheep option is usually the faster/ more likely option, because even if the sheep is far away from water, you can befriend- and lead it to a water body (just interact with it, it will follow you even across rooms but do not run too far away from it). As soon as the sheep comes into contact with water, its wool will expand, and you can now use it as a platform to jump from.

Invalidity/ Occurency:
There are usually multiple opportunities in a single playthrough to obtain this one, so do not worry if you missed a chance.
Creation of a Forest
When your character dies, a tree will sprout at your death location. I do not know whether the game counts total deaths, deaths in one playthrough, deaths in one gaming session or only within a short period of time. For me it popped up after dying five times in a row. My total death count was probably around 30.

Invalidity/ Occurency:
You can obtain it at any time during a playthrough (as long there is a way to die).
Listen
Throughout the game you will come across several large white animals. Approaching them will trigger a short written monologue. Once an animal has exhausted all of its text bubbles, it will disappear.

A comment mentioned that the achievement popped up after talking to all animals of a single level.

In my case, it popped up after listening to around 20 of these animals. If the comment is true, then I had to talk to these much animals because I was not exhaustive on the first few levels.

Invalidity/ Occurency:
On each level, you have the chance to earn this achievement - meaning, it is not really missable. If it is still missing after one playthrough just start a new one and be thorough on the first level.
Lost in the Woods
For me, it popped up seemingly randomly.
I theorize that the unlocking condition requires you to spend 5-10 consecutive minutes in areas of the world marked with trees on the map. This probably indicates what classifies as forest.

While I did not confirm how important it is, better walk to a point where the player character is completely surrounded by trees (on the map) deep within a forest, like in the picture to the left.

If this does not unlock it, then it is possible that the game measures the overall time you spent in forested parts of the map. In that case, I can not give a reliable estimate of the required duration.

Invalidity/ Occurency:
You can obtain it at any time during a playthrough (as long there is a huge area full of trees).
In Circles
First, find a flower field. Now run a few small circles within the field (with a radius just a few times the size of the player). Doing this not only assists you in walking nice, closed circles (the flowers disappear for a short period of time and leave you a trail), but you can also run indefinitely as long as you remain in contact with a flower while running. Run in circles, until the achievement pops up.

I do not know whether the game counts total circles, circles in one playthrough, circles in one gaming session or only within a short period of time. With enough stamina, it may not even be necessary to run in a flower field, but I did not check whether the flowers are a mandatory part of the unlocking condition.

It popped up for me after running a few continuous circles in a flower field. It neither popped up when I walked nor when I run circles outside a flower field (but maybe my stamina was not sufficient or the circles were not good enough).

Invalidity/ Occurency:
You can obtain it at any time during a playthrough (as long there is a flower field).
Making Friends
For this achievement you have to be befriended with at least one white rabbit, one black rabbit, and one pink rabbit concurrently. White rabbits are very common, black rabbits and pink rabbits are a littler rarer.

Refer to the picture on the left to see what they look like.

Do not bother having rabbits follow you at all times until you found a specimen of each color. There are various actions that cause rabbits to stop following you, such as riding a deer or dying. Sometimes they even get lost randomly. Good luck finding them again, then. Also wolves can kill your rabbits while they follow you.

Do not worry about keeping track of locations of black and white rabbits either, because the mechanics explained in Something Went Terribly Wrong allow you to spawn black and white rabbits infinitely. Just prepare a black and a white rabbit through multiplying and transforming animals that way. Then, all you have to do is find a pink rabbit and lead it to the prepared rabbits.

Note that some rabbits may not initially befriend you, but instead start to run around, dissolving any plant they touch until they return back to their default behavior. If I deduced their behavior correctly, you can befriend them after solving a specific nearby puzzle where rabbits need to dissolve some obstacles (they always spawn with that behavior for a reason).

Invalidity/ Occurency:
There are usually multiple opportunities in a single playthrough to obtain this one, so do not worry if you missed a chance.
Walk, Don't Run
At multiple occassions, you will face wolves in the game. While they chase you, they can even start multiplying themselves as long as you close enough to them to keep the aggro going. The picture to the left shows a sleeping wolf. Every wolf is asleep at first. You can make them sleep again by either quitting and restarting the game or by re-entering the current part of the map. All clones of the original wolf will be merged into a single being in this process (in my experience). Wolves can not follow you to other parts of the map.

When finding a wolf, awaken it by walking near it. It will start hunting you. Once the wolf starts moving, walk, don't run - as the achievement title says. Let it pursue you and try to stay alive until the achievement pops up (around 30 to 60 seconds) but avoid changing the current part of the map until you got the achievement. I tried it multiple times, but it seems to me that the wolf needs to multiply at least once for it to work. A good tactic for this achievement is to hide between trees as the path finding for the wolves ca not really handle too many tightly placed obstacles.

If a wolf comes into contact with a non-wolf creature, both will permanently disappear. However, the player character will always respawn after contact with a wolf. This means you could potentially exterminate all wolves on the map by touching each wolf before it can multply. Beware that wolves can also erase your befriended animals or other useful creatures.

If you can not find wolves or accidentally wiped them out, use the mechanics explained in Something Went Terribly Wrong.

Invalidity/ Occurency:
You can obtain it at any time during a playthrough (as long there is a wolf left).
Something Went Terribly Wrong
At some point you will stumble upon a few pink interactive flowers. Mine looked like the ones in the picture to the left and were surrounded by wolves. Interacting with the flowers transforms all creatures on the screen (possibly even a bit further) to other creatures. Currently, I found the following transformations.
  • A wolf ➔ Rabbit (randomly black or white)
  • Any rabbit ➔ Deer (randomly male or female)
  • Any deer ➔ Wolf (sleeping)

For this achievement, you have to befriend a rabbit. The picture shows a white rabbit. Lead the rabbit to the flowers, then transform it to a wolf and let the wolf kill you. If you do not want to search for a rabbit, just transform any animal near the flowers into a rabbit, befriend with this rabbit, and continue as described.

Invalidity/ Occurency:
There are usually only a few opportunities in a single playthrough to obtain this one, so if you missed a chance, you may have to start another playthrough after finishing you current one.

About infinitely multiplying creatures:
The flowers even allow you to multiply animals as much as you want. For this, provoke some wolves as described in Walk, Don't Run and let them multiply. After they multiplied, interact with the flowers. This process will turn all wolf clones into autonomous creatures thatmerge with the original wolf/ wolves.

Repeat this process to create an theoretically infinite amount of wolves/ deers/ rabbits. This can help you with other achievements like Walk, Don't Run or Making Friends. However, the more animals are on-screen concurrently, the harder it is to manage them and keep them within the range of the flowers' effect. Even if you start with creatures of a single species, you will likely end up with animals of different species, because some will leave and enter the effect radius as they please between transformations.

In my opinion, the best way is to gather a bunch of rabbits near the flowers, because once befriended, the rabbits will stay close to you. This way, you will have more playroom for the inevitable random spread of the animals between transformations and multiplications. It also enables you to start multiplying with creatures of only a single species.

Be aware that having multiple different species in the range of the flowers' (which is likely to happen) can result in wolves killing other creatures, as described in Walk, Don't Run. This is why it is only theoretically possible obtain an infinite amount of creatures.
The Only Way Out is to Finish the Game
Just finish the game by completing all 10 levels.

Invalidity/ Occurency:
It may be possible to soft-lock you inside a run, but you would have to actively provoke it. Even with knowing all mechanics, it would still pose a huge challenge. It will probably never happen by accident.
Practice
After a few seconds of doing absolutely nothing ingame, the player character will sit down to meditate, as shown in the picture to the left.

Leaving the player character in this state for about 60 minutes will unlock the achievement (I did not explicitly measure it and just estimate the time).

Just pass the time with something else than your computer.

Invalidity/ Occurency:
You can obtain it at any time during a playthrough.
Cartographer
This is by far the most cryptic, time-consuming and hardest achievement of the game. If you manage to fulfill its condition, it will only unlock at the very end of a playthrough. To unlock it, you probably have to enter every room on every level within a whole single playthrough.

Since everything in the game is randomly generated, you will not know in advance how many rooms there will be.
Moreover, there is no indication of how many rooms you have already entered (or are unvisited) or whether you already invalidated the achievement for the current playthrough. Make sure to search every nook and cranny during your playthrough, as the generation could place a cave entry, teleporter, or whatever, anywhere on the map, which (potentially) could lead to an unexplored room. Until you are sure to have found everything, do not touch the level exit.

Invalidity/ Occurency:
You probably can invalidate the achievement any time you exit a level for the current playthrough. Only at the end of the playthrough (can take about 10 hours!) you will know for sure. If the achievement did not unlock, you have to try again in a new playthrough.

My personal experience:
I had to replay the entire game just for this achievement.
Very often, I found myself out of bounds. And sometimes, I thought I was out of bounds, but the level generator still placed something in the weirdest most unexpected random location. I do not think that you have to map out every square inch necessarily, because the level bounds are very blurry and unclear (even to the game itself). It may be that it is sufficient to visit some specific key rooms or that some very small rooms need not to be entered. However, I would advise to leave as little as possible to chance by going through each level thoroughly.

And it was on your first run? Or a second playthrough?

I got Listen on the first world. I assume you have to find all animals in a given world. I think there were four or five.

I updated and polished the guide. It should now hold all necessary information

Very often, I got out of bounds. And sometimes I thought I was out of bounds, but the level generator still put something there.
The achievement is nothing but tedious busy work with no indicator on your progress or whether you already invalidated the achievement for the current playthrough.
You have to be borderline thoroughly and even then I'd advise to pray not only to one god but to multiple deities of different beliefs at once to maximize your chances.

It's a little speculative, but I think the unlock condition is to enter every room on every level in one playthrough.
However, since everything is randomly generated, you don't know beforehand how many rooms there will be.
I had to replay the entire game just for that and I searched everything up and down, because the generation could put a cave entry, teleporter, whatever, anywhere on the map which, potentially, could still lead to an unentered room.

How do you exactly get Cartographer?
