You have to try to have this happen and there is less than a 0.000...01% (about thirteen 0s) chance of this occuring. To see this occur, you need to unlock a hidden character, use a specific skill in combat (probably several times), use a specific item, and get some of the most debilitating effects over 12 times within 3 turns. Then you need to either allow the attacks to go through or have enough bad luck to get this as an effect from the result of that item.
