Guide
Step 1: Create a new character on Easy difficulty.
++++++++++++++++++
Step 2: Make an extra save of this character in the Elf Song Tavern (you now have two saves of this character).
++++++++++++++++++
Step 3: Activate the cheat ''Level Warp & Invincibility'' (''At any time during gameplay, hold L1 + R2 + Left (on D pad) + Triangle and press START. After entering the code correctly, a special menu will appear.''). Regardless of the controller - the buttons match their supposed place. For example, on an Xbox controller Triangle would be the Y button. (No, I don't know the combination for keyboard players).
++++++++++++++++++
Step 4: Turn on Invincibility and select level warp. There will be a list of levels in which you can vertically scroll through via UP and DOWN on the D pad. While scrolling, you will hear the menu scrolling sound per every different level you scroll to. However, the level screen will be locked in place. As such, you must scroll all the way to the bottom (where you won't be able to see the name of the level due to the locked programming of the level warp cheat menu) and you can do this by holding down the DOWN for a few moments. The level you would be highlighting is Eldriths Boss Fight. Go there and kill Eldrith. Note, if you don't wish to fight Eldrith with a wet noodle you can re-input the level warp. Once more hold the DOWN until you feel you've hit the end of the list, then press UP twice. Pressing it once gets you to the Pre Boss room where that ghost guy is. The room above that is where the big mini boss robot thing is. Kill that and it drops the Onyx Sword which is the bane of Eldrith.
++++++++++++++++++
Step 5: After you have finished the game, complete the ''Gauntlet'' by killing all of the enemies, within 15 minutes of time (just has to be under 15, and then leave through the exit door). Note that unless you turned off your invincibility, it will still be on. Also, note that any loot you grab during the gauntlet is worthless. Only magic potions were of value to me because the Icy Orb spell grants massive use when you're being swarmed.
++++++++++++++++++
Step 6: Once the Gauntlet has been completed you will have unlocked Extreme mode. Launch Extreme mode with the save file you used to beat Eldrith. Then in the Elf Song Tavern, Import the fresh level 1 backup save you made at the beginning of this guide. This brings the added difficulty because you've got a level 1 character in extreme mode.
++++++++++++++++++
Step 7: Bask in the delight of having a truly extreme Baldur's Gate experience my friend. Note, completing extreme mode grants you the character Drizzt Do'urden (yes you can level warp and wack Eldrith just like you did before for quick access to Drizzt).
++
Step 8 (Optional): There are some natural game handicaps you're probably using and had no idea of. While Waukeen may praise your ability to seize opportunity, know that Illmatter cries and flagellates himself every night because of it.

Things to stop doing are:

Jumping everywhere: For 1.5 speed, (seriously how did you normalize listening to that sound). Now you know why Kobolds run from you when they see splashing water bounding around a sewer corner). Simply, only jump when its roleplay appropriate and know that enemies can catch you while running.

The 360 advantage: You're bullying a bugbear by running behind them so they can't swing at you. Run to the sides of them if you want, but stop circling around them mid-fight.

Backpedaling: You run in and run out, sure enemies chase you but not as far as you can backpedal. To make this more interesting, keep backpedaling but give yourself an imaginary space that you limit yourself by. In environments where the map is split into many rooms or in areas where different scenery such as islands or campsites are present; allow yourself to only fight in those areas as if an invisible wall was there.

Multiplayer Backpedling: So your co-op player died. Stop running back to the save station, instead revive them when you reach the next save station. Make an exception of this only for boss fights and if you believe the next save station is far then use a recall potion for roleplay value. Now recall potions are potential life savers for the team instead of just a means to offload more cumbersome loot. (This challenge is probably most extreme at the beginning of Act 2 in the sunset mountains when you've got nowhere to recall to). Also, if your co-op friend complains about xp level differences...then tell them to get good or start using you as a human shield to tank the enemies.

Stop or limit your use of shields: Shields allow you to indefinitely block (only in the direction you are facing). In a multiplayer game it can be as easy as block in a corner while your buddy shoots the dumb enemies swiping at the shield. No weapon has block durability. Yes, some enemies can circumvent any block. However, it's not challenging enough because of the rarity. Therefore, make up an imaginary shield durability value and drop the shield once you think it would have broken. Or, block with it then release the block to simulate yourself losing stamina, or just forgo it altogether.

Randomize the Store: When at the store and wanting to buy something, count how many items there are available to buy in your store section (weapons, armor, potions, ect). Make sure you have enough gold to purchase the most expensive thing. Then on a randomizer roll, select the item in accordance with your roll number ( https://www.random.org/ ).

Climbing to safety every single chance you can get: (I'm talking to you soyboy Bartley fanboys who skipped past Alyth's melons so you could go smash up a cellar and grab gold...only to find R.O.U.S or otherwise known as rats! ( https://youtu.be/OXQwx1EolD8 )

Put a life limit or time limit on the duration of each level.

Level up one different ability at a time with each new level (instead of leveling one ability to max strength).

Play all three characters at a time, swapping them out after each level.

Play Hard mode and Extreme mode at the same time: Play the level on hard mode, then load in your save on Extreme mode at the beginning of the level, and import the character you just beat the level with on hard mode.

Don't enable alcohol addiction: Don't pick up the side quest bottle for that stinking drunk Nebbish.

Don't be a Simp: Don't pick up the spice for that she-lackey of a guard.

Do enable Ipswitch to continue his nicotine addiction by not completing his side q.

Note: If two players are loading into the game you might get an infinite black screen. Have both players press any button and the game will load. It's a strange bug that makes you think the game is loading, when in actuality the game is waiting for both players to initiate loading into the game via pressing any button (I press A personally).

Remember to give this guide a thumbs up, favorite it, and comment what you think is or was the most challenging Baldur's Gate experience that you had on the journey! Don't forget to mute the volume when shopping at Ole Bart's, and seriously, be a good Harper and the Zhentarim won't swipe your precious oceanic urn tonight!

Lastly, and this is a real dozy; but for those serious adventurers out there who think Extreme mode was EZ...here is your final challenge. May you never hope to pass this burden on to your next of kin...should you live long after embracing the most difficult quest of them all..................... Wake up Lortimer

When we played this on PS2 with a friend, we genuinely thought that Lortimer would be the mastermind behind the whole conspiracy.

I believe attacks are missed because of armor rating. Such as, your weapon has a damage amount. However, the enemy can reduce that final amount per its armor rating value. Therefore, you technically arnt missing attacks. Rather, the random chance roll that your weapon makes when you get a successful hit isint able to overcome the enemies armor rating; hence you score a 0. Even if you have the damage number visibility turned on, I have never personally seen a 0. That means it just appears as though you missed in which you really just scored a 0. I have to test to see if keen weapons and improved critical-strike-ability character scores help your chance to hit. Thanks for posting this topic to the thread.

Is this difficulty suppose to have you miss attacks? Never played extreme back in the day

Good idea to start the game at NG+ as a "fresh" level one character! ;D
I have played this game a lot in my childhood but this little twist forces you to actually play the game as intended.
You can't just rush through it spamming the attack-button.
No, you actually have to time your blocks correctly, dodge their attacks and use your environment to hide. You also can't be to wasteful with your ressources like healing and arrows.

Would recommend!
