Intro tips and Bastion map
First, complete the story campaign.

Second, complete the Aftershocks DLC (and any other post end game DLC they might make in the future, not counting The Trial because that has no relation to this).

If you're having problems with the Aftershocks mission "Runaround" in Old Town:
From where you spawn, just turn left and look into the ratty old cardboard boxes stacked near the water, loot a key and a picture of a big middle finger, yeah thanks game this puzzle sucked anyway.

Now that survivor spawns won't be interfered with and you have a huge stockpile of crafting materials, we can make all the resources we want and go looking for "survivors in need." These will be lone unarmed humans who are tucked away in small corners of various maps. Their spawns are random, which is why a lot of people find this achievement to be a pain.

Craft two nutria stews, two sterile bandages (no point wasting the good bandages on people), and two herbal medicines, and keep them in your backpack, replacing them as necessary. If you have regular pill bottle medicines and want to use those instead, feel free, but they were way too rare for me to even find enough to take on these runs.

Be prepared to fight enemy humans, so carry your favorite pistol on you, and I'd also recommend either shotgun (double barrel or Nova aka Prova because using it makes you good at video games) on your right shoulder. Bring a blade in case of zombies of course (4th and Pain is a nice one, but the regular clever is good too, because they decapitate which costs less stamina than a brain stab).

Now, go to Bastion and simply follow this path.

The red circle at the top should be your spawn point, meaning the first potential survivor location should be on your right, very close by. Those goofy happy faces represent potential survivor spawns. The one unhappy face doesn't actually mean much, it's still a potential spawn, but it's also almost frequently a mugger.

Now just follow the red path from your initial spawn point at the first skiff to the other skiff.

As far as I know, you always spawn at that north spot, I always did, but if you ever spawn at the other spot, just follow the red lines the other way, from skiff to skiff.

Wait for the survivor to hold their hand out, listen to what they request, and then pull it out of your backpack and put it in their hand. Try not to smack them with anything though, don't be right up inside their nose, and don't pull any weapon, because they'll recoil and pause and you'll have to wait a few seconds for them to reset. Be careful of enemies around. Let the survivor run off after you handed them the item (even if they just run off into a dead end and stand there like a dummy), and then either find an exit or continue the path to see if you can find another survivor, as multiples have a (tiny) chance of spawning in the same run. Survivors will not later spawn in cleared locations in the same visit to a town though, so you can't see a potential survivor spawn point that's empty, walk away, then come back to find that a survivor is there.

You only get progress for this achievement if you go back to your bus camp after helping a survivor.

I've seen some players simply spawn, go past that first truck and turn right at the trailer, and if there's not a survivor there, they'd reload their save. You could do this but I think this made the game crash for me (but my computer is a bit out of date for running VR games so that could just be me). I always wanted to check the other locations just to make sure too.

BE WARNED, the second person on the path has a medium chance of being a mugger, though I've seen them be regular survivors too. A mugger is easy to spot, you'll see them holding a pistol, usually the .30 revolver, and they'll point it at you when you get close and demand an item from you. These do not count towards the achievement, I tested this myself. You can very freely kill these people in any way you want, I would simply pull out my own pistol and shoot them before they could react. Theoretically, any survivor spawn location has a chance of being a mugger. Just keep yourself aware. But also don't be so paranoid that you'll blow a survivor's head off before checking to see if they have a gun.

The third person on this path, the person behind the blue house, is apparently always a mugger. If you see they have a gun, just shoot them. However, it's also important to note that if you go through the map, and this person is the ONLY one you can find, reload your latest save and try it again.

It's important to remember, muggers do not count for this achievement.

If you reload your save multiple times and the only person at all is the mugger behind the blue house every single time, then get to a skiff and exit, go back to your camp and sleep for the night. Try again the next day and see if the spawns change. There seems to be a weird pattern or bug with this game where sometimes on a run, ONLY the mugger will spawn and always in the same place, behind the blue house.

Don't worry about Tower/Reclaimed squads in the map during a run affecting the likelihood of survivor spawns. In fact, I've found the most survivors during death squad warnings in Bastion, oddly enough.

Don't worry about killing a mugger and it making a survivor not spawn. I've had survivors spawn after I killed a mugger, so I guess gunfire doesn't make survivors not spawn.
Via Carolla
IF YOU DON'T THINK BASTION IS WORKING FOR YOU, here's a survivor spawn map of Via Carolla I stole from someone on this page: https://psnprofiles.com/guide/10851-the-walking-dead-saints-sinners-trophy-guide

I didn't try Via Carolla so I can't vouch for this map, but you can consider it if you want.
Conclusion
I once had the second spot in Bastion (next to the truck in the street) have a survivor who just stood there begging me for food and watching as I shotgun blasted four or five hostile Reclaimed soldiers. And after all that he still wanted my nutria stew! I would've blown that survivor's head off if I wasn't going for the achievement, but I guess the meme is true, I got two personalities, the nicest person you'll ever meet and a twisted psychopath, don't mess with this crazy guy haha yano what I'm sayin swrrwrrwr sayin? Well never mind.

Anyway good luck with this one. I had a ton of trouble with this earlier while I was in the middle of the Aftershocks DLC, I couldn't get survivors to spawn for anything and I think DLC missions existing was interfering with survivor spawns. You might have an easier time, and it's always handy to have a map of places to check. There might be more survivor spawn locations in Bastion than this, but this gives a simply reliable path to follow that gets you in and out quick, and I've scoured the map before without finding any other survivors. In fact, I went through the ENTIRE game without ever finding a single survivor, which is why I wish someone else made a guide like this for me before I had to go looking up a ton of message board threads with advice for this achievement.

If anyone thinks this is helpful, maybe I'll do a guide on how I got Axeman's Jazz too. Make sure to thumbs up and give me awards and real life validation.

An important note to help anyone struggling: The beggars seem to only spawn when other non-bell events are on the map. Avoid running base versions of the maps. Every time I ran Bastion or Via Carolla with Deathsquads, Extra Loot, or Supply Caches, there were at least 3 human spawns (muggers and/or beggars). I think the early / late / no bell event by itself still counts as the base version of the map and don't cause beggar spawns.

I'm hoping this game or achievement didn't get broken by patches, but I do remember having to do MANY reruns of the same area over and over and over again just to get a single person to show up. I don't know if there's any extra trick to it, but I'd just say make sure you're bringing resources with you that they might want, just in case that helps them spawn. I do know that this game has some major bugs now, I actually can't play it anymore because sprinting no longer works. It's a shame because I loved this game and would love to replay it but I can't sprint in either this or the sequel, and that's a major aspect of the gameplay that prevents me from playing. Even a fan patch or something would help but I just don't know.

Thank you so much for the guide! Just a heads up to everyone, this achievement took me 3 hours, it's a pain. When taking the Bastion route, for me at least, when you hear gunshots that means it's a guaranteed mugger. Reloading the save also did nothing for me.

I forgot why I drew a sad face there. It's usually a mugger, I think that's why. I had about 20 runs in a row where the only human that would spawn would be the mugger that's right there.

Thanks for the guide. This achievement was driving me absolutely insane, but with the Bastion route I had it done in like half an hour.

Because it's just a sad place. Also I think I drew it that way on accident and I left it like that.
