The Location Of All Endings
All endings can be achieved at the very, very end of the game.
Specifically, this room:
The Negative Ending
The easiest to get and one you probably don't even need a guide for.
Hint: Become the new mother
Solution: Do nothing. Let the end scene play out.
The Neutral Ending
The neutral ending requires you to have 1 bullet for your gun.
Hint: Deny becoming the new mother
Solution: Shoot yourself (hold right mouse buttonto aim at yourself, then pull the trigger)
The Positive Ending
This ending requires your bring along 5 bullets for your gun.
Hint: Release the 5 spirits.
Solution: Dolls. Shoot all 5 dolls to release their spirits.

Hey, I honestly don't remember much about the game's achievements other than dying in particular ways in each chapter and the endings.
It looks like maybe this thread has something about the Medic's curse.
For "Maximum Pain", are there a bunch of rats or something that you can shoot? I'm just going by the achievement picture, but don't remember. sorry
