1. Download D3Dx by Chiri:
D3Dx[helixmod.blogspot.com]

2. Copy d3d9.dll and d3dx.ini to the game folder
**Copy d3d11 if you are running the game in DirectX 11**

3. Open d3dx.ini

4. Change the resolution and refresh rate to your desired values

Example: 1080p/144Hz

width=1920
;height=1080
refresh_rate=144
full_screen=1
;force_stereo=1

5. Save the .ini file with your edits

This works as long you enable Vsync and DX9 mode, DX11 does not work.
Going from 60Hz now to 240Hz (with framerate going above 400FPS) looks so smooth, it's night and day.

This Guide didn't work on my Nvidia.
To get above 60fps (on DX11) all I had to do was:

- Disable G-Sync (set "Fixed refresh rate" on Nvidia control panel ( NCP),
- Set Vsync to Fast, on NCP.
- Set Vsync to ON, in your game settings.

Set your graphics options so that fps will always be above the refresh rate of your screen.
>> game fps will be > 60 but your screen refresh will still be locked to 60Hz by the game<<

- Fast Vsync >> works,
-"disabled" or "use 3D aplication settings" >> Fps above 60 + tearing.
-"Enabled" or " Adaptative" >> locked to 60fps.

Don't change anything on your screen settings, only on NCP..

i've added the following lines to my default.cfg:
