WARNING
This guide is not spoiler friendly. Below is a complete and detailed guide for everything you will need to 100% the game. I still advise you to explore and enjoy the game at your own pace. If you want a more spoiler-free version, simply skip to the achievements specific section below the guide. Note, however, that some will still spoil things simply due to having to explain the process.

Info
Due to this game having a number of unique mechanics and caveats, I've decided to include this section so you have a better idea of what you're going into.

Endings
There are 4 endings in the game, however you can get 2 in one playthrough when going for Dweller in the Dark. First, you must NOT restore all of the Eos vestiges. I recommend not restoring any to be safe. You will be able to get the ending (Good), then continue from the final boss after doing so. Once you restart, restore all of the Eos vestiges and defeat the final boss again for the second version of the ending (True). If you do the True version first, you will need to do 4 playthroughs instead of 3. I go over this more thoroughly in my walkthrough + the achievement sections.

Gifts and XP
Gifts are essentially your skills in the game. You unlock them with Haze (currency) and can unlock them to use on any class by maxing out their XP bar. This is called Inheriting. However, the game punishes you for levelling up too much and will give you ZERO Gift XP if you're too a high a level for the area you're in. To mitigate this, the community created the 7 levels per boss rule. After killing the first boss you can level up to 8. After the 2nd boss you can level to 15. So on and so forth. At least until you unlock the achievement for inheriting 50 Gifts.

Use your Gifts!
Gifts such as attack power up, elemental effects, and defense are incredibly useful. Don't be afraid to spend your Ichor as you'll be regenerating it constantly.

This is not a Souls game
Use the AI partner. This game was not balanced for solo play and you will be making your playthroughs infinitely harder by not utilizing them. It is possible to solo the game, but it's not recommended.
Walkthrough I
Ruined City Underground
Mistle (1/51) - Underground Area
After completing the intro sequence and being thrown into the Undeground, you can find this Mistle right in fron of you.

Mistle (2/51) - Natural Cavern
After the cutscene where Oliver gets attacked, progress through the cave system ahead. At the end near a fork will be the Mistle.

Mistle (3/51) - Natural Cavern Depths
Continue through the linear caverns until you restore the Bloodspring. The Mistle is just ahead.

Berserker Blood Code (4/32)
After exiting outside you will meet back with Oliver who's become Lost. His fight is rather simple, as his choice of weapon is a large heavy hammer. Dodging his attacks and getting a few pokes in shouldn't prove difficult. About half way through he'll transform into a large Lost, but the strategy remains the same.

Oliver Collins

Berserker Blood Code
After Oliver's fight pick his core off the floor in front of you.

Prometheus Blood Code (5/32)
Storyline. Talk to Louis to acquire this.

Mistle (4-6/51) - Home Base, Training Area, Outer Crossroads.
After arriving at Home Base, you can find this Mistle near Davis.

Depths Map (1/12) - Den of Darkness
After warping back tot eh Outer Crossroads, a new NPC will be near by. Speak with Shang to receive the map.

Depths Map (2/12) - Den of the Dead
Head straight past Shang into a long tunnel and follow it through until you come to the first fork. The map is on the left next to a corpse.

Ruined City Entrance
Mistle (7/51) - Ruined City Entrance
Progress through the tunnel to the far end to automatically find this one.

Prometheus Vestige A
After ascending the stairs, hug the right wall to find this under a destroyed bus.

Hunter Vestige C
As soon as you reach the parking garage, break the boxes to the left to find this behind them.

Mistle (8/51) - Parking Garage
After killing the large Lost, descend a level in the garage to trigger a cutscene. Afterwards, turn around to find the Mistle.

Hunter Blood Code (6/32)
Drop down the ladder in front of you and progress forward until you spot a patrol of enemies. Hook a right and follow the path to your first Lost Invasion. Clear out the enemies, and then the Blood Code can be found directly across from the chest.

Hunter Vestige B
Cut directly across the cross section to the far left side this time. If you head left, it leads to a giant hold. We're going to ignore this for now. instead, head right and you will see 2 red tanks on your left. Break them to find the vestige. You'll know you're in the right are if you see a large hammer Lost and a bunch of dogs.

Prometheus Vestige C
After killing the large Lost, continue straight towards a cliff face where you'll be ambushed by hanging monkey enemies. From here, go straight to where a bunch of Lost are digging. The vestige is next tot he 2 on the right.

Hunter Vestige A
head down the right path this time to find yourself under the parking garage. To the left is a staircase with a bunch of boxes in front of it. Break the boxes to find the vestige. The stairs also lead to a shortcut.

Hunter vestige D
To the right of the stairs across a giant chasm is a large Lost and a few snipers. Take them out and head across to find this on the right most overhang.

Mistle (9/51)
Hook around the left side and follow the path to a large staircase. At the top you'll find a small destroyed park. The Mistle is to the top left of the park.

Prometheus Vestige D
From the park hug the cliff face on the right to find a ladder you can descend. The vestige is just behind the angel statue ahead.

Prometheus Vestige B
Head back up the ladder and follow the path until you reach a fork in the road. To the right is a large Lost. The vestige is behind her.

Down the left path is the Butterfly of Delirium boss. She can actually be quite tough and is a roadblock for many players. A lot of her attacks have very little tell or wind up, often causing you to get hit before you think you should. She can spew poisonous balls of varying style, summon a poison tornado, and leave poison trails. However, there is a pretty easy way to deal with her if you're struggling. If you keep your distance, she has a tail charge attack that you can dodge to the right, then dodge towards her to get in 2-3 free attacks. This attack is extremely easy to spot and dodge.

I used the Fighter code and Yakumo as my partner for his high defense, but Louis is also a viable choice as he can shoot her with fire, dealing massive damage to her.

Butterfly of Delirium

Mistle (10/51) - Poisonous Butterfly Nest
Directly in front of you after killing the boss.

Mistle (11/51) - Dried Up Trenches Entrance
Directly in front of you after the cutscene.

Howling Pit
Mistle (12/51) - Howling Pit Entrance
Warp back to the Parking Garage Mistle. Remember that giant hole in the ground at the crossroads I told you to ignore? We're going to head there now. There's a ladder near it that's pretty much worthless to us at the moment, so we're going to have to Dark Souls 2 it and plummet into the giant hole ledge to ledge. At least at the bottom is an elevator back up, huh? Follow the path through to a cutscene and the Mistle.

Mercury Blood Code (7/32)
It's time to take a quick trip back to Home Base. Talk to Coco and she will give you her Code.

Hermes Blood Code (8/32)
After talking to Coco, talk to Davis to get his code.

Atlas Blood Code (9/32)
Finally, talk to Yakumo to receive his code.

Atlas Vestige A
Warp back to the Howling Pit and drop off the right-hand side near the lever. Head straight across to find a rocky ramp leading up to 2 winged Lost huddled in the left corner. The vestige is between them.

To make exploring a little easier, head down the opposite ramp, and hug the wall to the right all the way around to a corrupted Mistle.

Atlas Vestige C
From here, head right and you'll see a small enclosed area with only 1 way in. It's kind of a circular section of the map that looks like a C. The vestige is on that center bit behind a large Lost.

Assassin Vestige D
From here, head back to the ramp we just came from. Coming off of this ramp, head as straight as you can and you should see another ramp ahead. At the top is another large Lost with the vestige next to it.
Walkthrough II
Assassin Blood Code (10/32)
Head down the opposite ramp of this island and hook right to find another ramp very close by. At the top is a chest. We're going to use this as a marker. Head down the opposite ramp, and follow the right wall all the way through and you'll eventually reach a section with a large Lost and 3 rolling Lost ahead. Ignore the large Lost for now and continue following the right wall to find another Corrupted Mistle. Now head back to the large Lost and kill him to find the Code.

Assassin Vestige B
Head back to the chest and down the first ramp. Follow the left wall all the way around. You will pass a taxi directly across from a lamp. Keep following the left wall around and you'll pass another lamp followed by a small metal door. The vestige is right behind.

Mistle (13/51) - Hills of Deception
180 and head up the left ramp to trigger a cutscene. After it ends the Mistle is in front of you.

Assassin Vestige C
To the left of the Mistle.

Atlas Vestige B
To the right of the Mistle near the lever.

Assassin vestige A
After acquiring the Eagle Key and opening the metal door, you should see this straight ahead of you. Follow the path around to the right to reach it.

Atlas vestige D
After using the Moss Covered Key to drain the sluice, drop off the immediate right side and continue to follow the path right. You'll spot this at the very end.

Mistle (14/51) - Bottomless Shore
Progress through this area until you reach a lever. The path here will split. Take the right side to find this on top of the raised platform with the flaming barrel.

Mistle (15/51) - Derelict Oil Rig
See below.

Head back to where the path split and take the left path to our next boss, Invading Executioner. This one is much easier than the Butterfly and shouldn't take too many attempts to kill. She herself is fairly nimble, but her attacks are deliberate, taking some time to actually come out. The 2 attacks to really be careful of are her pole spin move and when she slides across the floor. I've been one shot by both of these. Once you kill her and finish the memory, you will receive the vestige and the Mistle.

Invading Executioner

Dried-Up Trenches
Darkseeker Vestige B
Warp back to the Dried-Up Trenches and proceed forward a short distance. You'll see a pile of boxes on your left, and a heap of scrap metal to your right. Drop off the right side on to the root-like platform below you and follow it to a ladder. past the 3 enemies up top is the vestige.

Mistle (16/51) - Decayed Ship Interior
Progress through the level however you see fit until you reach a decrepit shack with a bunch of enemies around it. Head around the left side of the shack and drop down to the root like platform. Once it splits, take the right path tp the Mistle.

Depths Map (3/12) - Town of Sacrifice
Climb up the ladder to find this on the corpse to your right.

Darkseeker Vestige A
Drop off the roof and continue down the path ahead. You'll turn left into a section where 2 large Lost climb up from over the edge. The vestige is behind the boxes to the left.

Mistle (17/51) - Frozen Seabed
Push ahead of this area to find a ladder down to the next Mistle.

Darkseeker Blood Code (11/32)
Continue forward to our next boss, Insatiable Despot. This boss is incredibly easy as his moveset is pretty limited. You want to make sure you're constantly pressuring him during his first phase, or he will summon adds to help him out and they can deal quite a bit of damage. During his second phase, I found sticking to his ass pretty much shut him down completely.

Insatiable Despot

Cathedral of Sacred Blood
Depths Map (4/12) - Flood of Impurity
After the long cutscene at Home Base, warp back to the Park Ruins Mistle, and interact with the statue we found a vestige near. Follow the linear path until you climb a long ladder. Once at the top, look in the cranny behind you to find the map.

Mistle (18/51) - Cathedral of Sacred Blood Entrance
After grabbing the map, continue on the path to find the first Mistle of the area.

Isis Vestige B
After the Mistle, head down and kill the lone enemy in the circular area. Head up the center path and take a right, followed by a left, and another right. You will be invaded here but you can see the vestige in this room.

Mistle (19/51) - Artifice Tower
Exit out of the door we came in and head straight. Go up the far staircase, then follow the path through to the tower with the halberd Lost. Exit and hook and immediate right. Going straight will lead to a shortcut and an enemy you can kill to reveal the map. Now head back and take the other path down a ladder. Continue on to find another corrupted Mistle and a tower you can climb for another shortcut. Follow the path forward to the Mistle.

Isis Vestige C
From here, follow the path forward through a series of towers until you drop down to a lower ledge. If you push ahead you'll find a door you can open for a shortcut. At the ledge we just dropped to, look down again, for another ledge. Inside is a large Lost and the vestige.

Mistle (20/51) - Outlook Tower
Continue down the only path forward to find this Mistle at the top of a circular staircase.

Mistle (21/51) - Gated Room
The next section is kind of labyrinthian, so I won't go into too much detail here. Thankfully every path eventually meets up at the same place, so feel free to explore and get lost as you like. Once you reach a large circular room with a long ladder in the center, climb the ladder, then drop down the following ladder to the Mistle.

Memories of Player
Mistle (22/51) - Research Facility Area
Once again, the next are is pretty windy, but thankfully it all converges into one spot. Make your way through at your own pace until you arrive at the Argent Wolf Berserker fight. Much like the last boss, he has a very basic and slow moveset that you can easily dodge and poke between attacks. Once you've killed him, interact with the vestige ahead and you'll receive a choice. Choose yes. Follow the linear section through to the Mistle.

Mistle (23/51) - Military Storehouse
After progressing through this area, you'll come to a section with a couple of trucks lined up on the left. The Mistle will be in the right building up ahead, just before the giant spike protruding from the ground.

Mistle (24/51) - Main Battlefield
Head back to where the trucks were lined up and make the first right you can to find a ladder. Hook right into the warehouse, take the ladder up, then cross over into the next one. Once outside, loop around the entire building to find an enemy we can kill for the Old Shutter Key. Head back tot he Mistle we just activated and open the shutter next to it. Follow the path from here to find the Mistle next to a small metal bridge.

Queenslayer Blood Code (12/32)
Drop down the ladder for the only optional boss of the game; The Queens Knight. This dude's pretty generic as far as fights go. He has a few slash attacks, and a large thrust that can deal decent damage. During his second phase he'll gain the ability to warp around for aerial slams that he can do once, or 3 times. I found baiting his 2 hit slash and rolling behind him was best for both of his phases.

Queen's Knight
Walkthrough III
Cathedral of Sacred Blood Continued
Mistle (25/51) - Cathedral Perimeter
Continue forward into the tower ahead of you. The Mistle is to the right.

Isis Vestige E
Okay, this one's really far away in this mess of a level, so bare with me here. Head up the ladder to the 2nd floor. Don't take the next ladder, continue across into another tower. Continue up the following staircase and take the ladder to your left to the outside. Climb up the 2 ladders to grab the Shoddy Key at the top (for a later vestige), then head all the way down all 4 ladders to the very bottom. Cross the bridge and open the large door. We can't open the next one, so go down the stairs in the center of this tower, instead. Head up the next 2 sets of stairs and cross into yet another tower. Don't take the path left. Instead, take the stairs in the center of this tower up and cross into the room with the large Lost. Look down the center of this room to see the vestige.

Mistle (26/51) - Inner Tower
Head back to the same room where you dropped down for the previous vestige, and this time follow the path to the next tower. Take the stairs in the center, and then hook a right to drop out of the window. Head up the staircase to find the Mistle.

Isis Vestige A
Head into the next area and climb the ladder to find the door the Shoddy Key opens. Drop down the hole in the center, and then drop out of any window. Don't drop to the lower section! From here, look for the path with the corrupted enemy in it. Kill her, then break the crates to find the vestige.

Mistle (27/51)
Head back and take the opposite path this time, again not dropping down to the lower level. Ahead you'll spot a corrupted Mistle. Take the ladder to its left and open the chest ahead for the Ornate Key. At this point you can feel free to explore any paths you might not have. Once you're ready, head back to the locked door just past the Shoddy Key door and open it. Follow the path straight to the Mistle.

Isis Vestige D
Progress forward to trigger a cutscene. You will receive this at the end.

IMPORTANT
Since this is our "perfect" playthrough, you MUST talk to Leda and restore all 3 of the Isis vestiges BEFORE opening the large orange door. After defeating the boss, you will enter a final memory sequence. Make sure you restore their memories at the end!

Isis Blood Code (13/32)
Enter the orange door to meet Successor of the Ribcage. This boss is purely a ranged enemy so you can safely position yourself behind or to the side of her and go to town. Just be sure to avoid any AoE attacks she places near you. Once you've killed her, you will have to do so again, and this time she has some new moves + attacks much faster. The same strat applies. Stick behind her and you'll have little issue. Once you kill her, choose to restore her memories for the Blood Code.

Successor of the Ribcage

Mistle (28/51) - Crypt of the Ribcage
Enter the large door to trigger a cutscene. Once done, return to where Aurora is for the Mistle.

Artemis Blood Code (14/32)
Now's a good time to stop by the Home Base and talk to Mia for her Blood Code.

Ridge of Frozen Souls
Mistle (29/51) - Ridge of Frozen Souls Entrance
Once you're ready, warp back to the Inner Tower in the Cathedral and open the large door nearby. Warp to the next area to find the Mistle immediately ahead.

Fionn Vestige A
Very shortly after, you'll pass a large Lost, followed by 2 dogs. Just ahead of the dogs are a bunch of Lost around a few cars. Look to the left for a ledge you can roll off to. The vestige is just below.

Fionn Vestige B
Head back to where the cars were, and just ahead you'll see a chest. be careful of the patch of ground that breaks away! Continue up the hill past the chest and the path will split left. Don't go left yet, go straight to find the vestige.

Mistle (30/51) - Iceless Falls
head back and take the left path this time. The Mistle is at the end near the waterfall.

Fionn Vestige C
Take the downward path to the right of the Mistle to enter a cave. Eventually it will split left and right, with the right having purple bioluminescent crystals. We can avoid that for now as it leads to a door we can't open. instead, head left to find the vestige.

Mistle (31/51) - Snowfield Summit
Head back to the Iceless Falls and this time take path left of the Mistle into another cave. Ahead will be a very powerful Lost followed shortly by a ladder. Follow the path up to trigger a cutscen. Afterwards, turn around to find the Mistle.

Fionn Vestige D
From the Mistle head straight down to the destroyed building nearby. The vestige is right next to the powerful ice Lost.

Mistle (32/51) - Observatory Site
Head back and loop around the opposite side of this building. You'll come to a smaller destroyed building, and just around the side of it a split path that either leads to a cave system, or to more destroyed ruins. Take the upper path to the ruins. The Mistle is at the end near a ladder.

IMPORTANT
Coming up is another Successor. Since this is our "perfect" playthrough, you MUST talk to Cyllene and restore both (2) of the Fionn vestiges BEFORE entering the boss fight. After defeating the boss, you will enter a final memory sequence. Make sure you restore their memories at the end!

Fionn Blood Code (15/32)
From the Observatory Site Mistle, head down the ladder and follow the cave out to a path leading to the Successor of the Breath. This boss bolsters an incredibly high defense. I strongly recommend using Flame Weapon and Flame Spike to counter that a bit. Most of his phase 1 attacks are pretty simple to dodge, however there is one that stands out. He will glow red and spawn a swarm of icicles. I found dodging this attack to be pretty hard, but found sprinting to work every time. When he hits phase 2 he will gain rocket boosters and all of his attacks will be amplified in either size or quantity. After defeating him, restore his memories for the Blood Code.

Successor of the Breath

Mistle (33/51) - Crypt of the Breath
After all of the cutscenes with Mia and Nicola you can find the Mistle in front of you.

Eos Vestige A
Before leaving the area, head to where Cyllene was to find this vestige.

Eos Blood Code (16/32)
Return to Home Base and speak with Io for a cutscene. Afterwards, talk to her again for her Blood Code.

Ashen Cavern
Mistle (34/51) - Ashen Cavern Entrance
Warp back to the Gated Room in the Cathedral and open the door to your right. Take the elevator down to find the Mistle not too far ahead.

Artemis Vestige C
Directly ahead is 2 paths, one that continues straight and one that goes right. Take the straight path to find the vestige.

Artemis Vestige A
Head to the right path this time for a cutscene. Afterwards, take the right path, followed by an immediate left to a low overhang with 3 poisonous blobs. The vestige is around the corner to the left.
Walkthrough IV
Artemis Vestige D
Head back and up the rocky slope to where the sniper Lost are. Continue up the second slope and drop off the left side at the end to find the vestige.

Survivor Vestige B
Loop around the right corner to find this next to a corrupted Mistle.

Mistle (35/51) - Towering Crags
Drop down and head up the first slope again, this time taking the cavern path to the left. Follow the only path forward until you reach a chest. In the next room, take the first left you can up a slope to find the Mistle tucked away.

Survivor Vestige A
From the Mistle, drop down and take the slope up until the path splits. The right leads to a Regen increase, and the left continues forward to the vestige. Once you emerge in the large room covered in thorns, look straight and slightly to your right to spot the vestige.

Artemis vestige B
Head back to where we entered this area and take the left path this time. You should immediately see this vestige. Continue forward until you can make a left near 2 dragon head Lost. Kill the knight and grab the vestige. Keep the metal door here in mind.

Mistle (36/51) - Accumulator Ruins
From the vestige you just picked up, head to where the dragon head Lost were and hook an immediate left to find this Mistle tucked away in a corner.

Survivor Blood Code (17/32)
From the Mistle you should spot a vestige straight ahead. Don't head to that just yet, instead go right to where the large red Lost is sitting near the ledge. Look to the side to see another dragon head Lost with the Blood Code behind it.

Survivor Vestige C
From here you should see a cavern ahead filled with blue thorns. Don't go into that path just yet, instead go past it to the left to find the vestige we saw earlier.

Dark Knight Blood Code (18/32)
Head down the blue path this time to find the Gilded Hunter boss fight. I HIGHLY recommend bringing any ice Gifts you can, such as Frost Weapon and Ice Barrage, as well as Blood Guard, as most of his attacks deal blood damage. His phase 1 is fairly simple. Use Frost Weapon to deal extra damage to him and poke him between attacks. His second phase, however, he is unrelenting and will give you almost no room to breathe. Before he initiates this phase, reapply Frost Weapon and throw on Blood Guard. Just focus on dodging and avoiding his attacks until his focus shifts from trying to relentlessly murder you, then go in for a few strikes.

Gilded Hunter

Mistle (37/51) - Putrid Surge Tank
Directly in front of you after the fight.

Mistle (38/51) - City of Falling Flame Entrance
Head back to the metal door we couldn't open earlier and pass through it. Follow the linear path through to the Mitle.

Scathach Vestige A
From the Mistle head left past the fire geysers to reach a ladder. Follow the path around to another ladder down. Continue forward until you reach a large stone staircase. The vestige is to the right before heading up.

Scathach Vestige C
Continue through the level a bit until the path splits left or right. Go right to find a Regen increase and a chest shortly after. From the chest, drop down to where the blue Lost is. Loop around to the left to climb a ladder. At the top, hook another left and drop down the first chance you get to find this next to the red and white barriers.

Scathach Vestige D
From here, press forward and take the first left you can. Follow the path through to find a corrupted Mistle. Once you turn around, you should spot this to your left near a destroyed taxi.

Mistle (39/51) - Razed Cityscape
Proceed through the level until you come across a cutscene. Afterwards, head forward past the giant blue Lost to find a ladder. At the top, head to the far right end of the platform, then look right to see the Mistle below you.

Scathach Vestige E
From the Mistle, continue forward until you reach a large greatsword wielding Lost. Past him the path splits in 2. You want to go left, past a bunch of urchin-like enemies, to a 3-way crossroads. The right path leads to the vestige.

Scathach Vestige B
Head back to the crossroads and take the path straight ahead this time to find this vestige.

Mistle (40/51) - Back Alley
Return to the crossroads one last time and take the left path this time. Head past the large blue Lost to find a staircase. Hook a left at the top to find the Mistle below you.

IMPORTANT
Coming up is another Successor. Since this is our "perfect" playthrough, you MUST talk to Thebe and restore all 3 of the Scathach vestiges (II, II, and IV) BEFORE entering the boss fight. After defeating the boss, you will enter a final memory sequence. Make sure you restore their memories at the end!

Scatach Blood Code (19/32)
After talking with Thebe, enter the Successor of the Claws arena. Just like Gilded Hunter, I recommend using Frost Weapon to deal extra damage. She's a pretty easy boss as her moveset is fairly limited and easy to dodge. If she ever uses a volley of fire, I suggest dodging into them as they're homing and will explode behind you. During phase 2 she has a super charged dash that does a lot of damage. Be sure to keep an eye out for that too.

Successor of the Claw

Eos Vestige B
After the fight, return to where Thebe was to find this vestige.

Crown of Sand
Mistle (41/51) - Crown of Sand Entrance
It's time to warp back to the Iceless Falls in Ridge of Frozen Souls. Head right from the Mistle to the downward slope and back to that purple cave we ignored earlier. Follow the series of ladders down into the cave system to eventually find this Mistle.

Harmonia Vestige B
Immediately after the Mistle is a staircase that leads to a split path. Ignore the right side and continue straight until you reach another intersection. Take the left path to find this behind a large insect-like Lost.

Harmonia vestige A
Head back to where the path split at the stairs and take the right path this time. Head all the way to the end of the street and head right to find the vestige.

Harmonia Vestige C
Take the left path now, and you'll see a pile of boxes to the left where the Lost wake up from. Take the ladder up to find the vestige at the end.

Mistle (42/51) - Sunken Ruins
Take the other path to a large sand-filled plaza. In the center is a Regen increase. From here take the right path between the buildings, then look to your right after dropping down.
Walkthrough V
Harmonia Vestige D
Continue forward to a section where you have to bob and weave through allies and buildings to avoid giant boulders. Once you're past him you will come to a 3 way intersection. The left path loops around, and the right path leads to a chest. Instead, continue straight to where the dragon head enemy is. To your right is a large bug and straight ahead is another dragon head. The vestige is straight ahead behind the dragon head enemies.

Harmonia Vestige E
Head back to where the large insect was and continue past it to the large sandy area. Pass through and take take the left path to another large insect enemy. All the way at the end is the vestige, next to a 3rd insect Lost.

IMPORTANT
Coming up is our last Successor. Since this is our "perfect" playthrough, you MUST talk to Themisto and restore all 3 of the Harmonia vestiges BEFORE entering the boss fight. After defeating the boss, you will enter a final memory sequence. Make sure you restore their memories at the end!

Harmonia Blood Code (20/32)
From the vestige, follow the only available path. Straight ahead leads to a ladder and elevator shortcut. The right path leads to Themisto and the Successor of the Throat. This boss doesn't pose much of a threat as most of its attacks are large AoEs that can be easily spotted and dodged. Sticking close to her worked well for me as she would use either a large close range swipe or her multi-sand fist attack which left her vulnerable. I also used Frost Weapon for the entirety of the fight.

Successor of the Throat

Mistle (43/51) - Crypt of the Throat
After the fight, this will be right in front of you.

Eos Vestige C
Return to where Themisto stood to find this vestige.

Heimdall Blood Code (21/32)
Return to Home Base and speak with Jack to receive his Blood Code.

Demeter Blood Code (22/32)
Return to the Crypt of the Ribcage and go down the stairs to Karen for the Blood Code.

Crypt Spire
Mistle (44/51) - Crypt Spire Entrance
Head back to the Crypt of the Breath and take the purple Mistle to the next area. Once there, proceed up the first set of stairs to the Mistle.

Warrior Blood Code (23/32)
Progress through the area until you reach a lever that lowers a ladder. Climb up and follow the path until it splits. The left side leads to a Regen increase, and the right leads to a lever. Head to the lever but don't pull it yet. First, head out the right side to find a ladder up to the Blood Code.

Depths Map (5/12) - Misty Ruins
Use the lever we just passed to open the door. At the split ahead, take the right path to find the map.

Warrior Vestige A
This one's quite a ways away, so I'll explain the whole path to be safe. Head down the other path now and open the following 2 doors via their levers. At the next split, take the right staircase up and head into the center of the tower to find a lever and a chest. Open the door and kill the large knight in the following tower for the Bloodstained Key. Open the door and you'll shortly be greeted by a ladder and lever. The ladder leads to an infinite use home warp item that I recommend grabbing. Head back and pull the lever this time. Run down the following hall and loop around to the right side. Break the boxes to find the vestige.

Hephaestus Vestige B
Pull the lever in the center of the same room and continue forward to 3 knights. Kill them to acquire the Warped Key. Use the next lever, and enter the center of this tower to find an elevator. once at the bottom, exit out of the door and hook a right to find this inside one of the large red tanks.

Hephaestus Vestige C
Take the elevator back up and continue down the path. At the split, take the left path into the next tower. Exit out to the left to find a ladder up to the Argent Wolf Key. Head back down and continue onward to another lever. The vestige is to the right of the next room.

Hephaestus Vestige A
Use the lever to drop the ladder ahead and climb up. Follow the path through to another elevator, but don't take it down just yet. Instead, head behind it to find the vestige under a bunch of wooden boxes.

Mistle (45/51) - Upper Level
Take the elevator up to the next floor. Once at the top, check behind the elevator for the Mistle.

From here, follow the path forward to what many believe to be the hardest boss in the game; Blade Bearer and Cannoneer. This is basically the Ornstein and Smough of this game, if you're familiar with Dark Souls. I highly recommend bringing Flame Weapon + Ice Armor for Blade Bearer, as well as Frost Weapon + Flame Protection for Cannoneer.

The Blade Bearer is incredibly fasr, dodges around constantly, and can cover large distances pretty quick. Meanwhile, the Cannoneer is relatively slow, but he makes up for it by constantly shooting high damage AoE attacks from the sidelines. I opted to focus the Blade Bearer first as I didn't want to deal with her leapfrogging around the whole time. Try your best to separate the two, but ALWAYS keep them both in your vision. You can use the towers around the area to create space or heal up if you need to. Once you do engage the Cannoneer, be careful as he also has some strong close quarter moves with quick wind up times.

Blade Bearer and Cannoneer

After killing the duo, continue ahead for the Jizo Mido boss fight. I went in with Flame Weapon and Blood Guard, as a lot of his attacks do blood damage. His fight is pretty basic; bait out his swings or various blood orbs and take pot shots between. His phase 2 is the same as the first phase, only he get's a little more aggressive and will now put a rotating wheel of explosive blood orbs around you occasionally.

Juzo Mido

Provisional Government Outskirts
Mistle (46/51) - Provisional Government Outskirts Entrance
After killing Mido, open up the purple door to enter the next area. The stairs going down leads to a bit of DLC content if you own it. Continue straight across the large bridge and all the way up to the next zone of the game, where you can find this Mistle.

Hephaestus Blood Code (24/32)
Now's a good time to return to Home Base and talk to Murasame for her Blood Code.

Eos Vestige D
Return to the Mistle and progress through the area until you come to your first split in the road. Take the left path and drop down behind the large Lost. After crossing the bridge, take the left path across another bridge and you'll drop into a yard filled with shipping containers. Head to the top right section and you'll spot the vestige on a ledge. Hook around the right side to find a ladder to the ledge.

Mistle (47/51) - Research Facility Ruins
From the ladder, head forward to spot a large Lost. Behind her is a chest, corrupted Mistle, and ladder shortcut. Head back to where the large Lost was and locate the path forward behind a destroyed truck. Head through until you're given the choice to follow a ramp down, or continue along the ledge. Both of these paths lead to the the Mistle, so feel free to explore both.

Ishtar Blood Code (25/32)
Follow the path forward to the Queen's Knight Reborn boss fight. This guy is almost identical to his fight in Memory of Player, so I won't go into much detail here about him. After killing him you'll be given a memory which grants the Code.

Queen's Knight Reborn
Walkthrough VI
Mistle (48/51) - Government Square
Directly in front of you after the fight.

Depths Map (6/12) - Zero District
From the boss arena, continue forward until you go down a ladder. The map is directly to the left as you get off.

Provisional Government Center
Mistle (49/51) - Provisional Government Center Entrance
Continue forward though the tunnel to reach the Mistle.

Depths Map (7/12) - Void District
From the Mistle, take the right path and follow it through until you reach a long corridor that eventually branches off to the left. If you continue straight you will find Dominic, an NPC who gives you a sidequest. You can use him as a landmark. Head down the left branch instead, and head all the way to the end where the boxes and crates are. break them to reveal a ladder. The right side staircase leads to a corrupted Mistle and a shortcut back to the Entrance Mistle. Take the left path and destroy the boxes at the far end to find another ladder with the map at the top.

Mistle (50/51) - Connecting Bridge
Return to the top of the ladder that lead to this section, and hook an immediate left down a staircase. Follow the path into a large room with knights and take the staircase ahead to the right. Pass through the long corridor filled with wall spike traps until you descend a staircase. The left side has a ladder hidden behind the destructible objects that leads to a shortcut. The rest of this section has nothing important, but you can find a Queen Tungsten in one of the nooks. Once you're set, take the right path instead, and continue forward until you reach a large Lost where the path splits. The left path leads to a dead end room. Instead, take the right-side stairs up. You can hook a left to drop off on top of a Queen Tungsten, but the right side is the way we need to go. Pass over the rafters and down the stairs to find the path splits. The left side leads to the boss, so ignore that for now. Take the path straight and eventually it will split yet again. The right path leads to the Mistle.

Eos Vestige E
Head back to the "boss" now. Killing her is as easy as spamming light attack for 10 seconds. Once she's dead you will receive the vestige.

Eos Vestige F
Exit out of the far end of the arena and progress until you reach an elevator. Don't use it yet, You can break the boxes to the left for a Regen increase, and break the boxes to the right to reveal a path to a ladder. You can lower it, but don't use it. Instead, look left for the vestige.

IMPORTANT
At no point during this playthrough should you restore the Eos vestige. We are able to get 2 endings during this playthrough by killing the final boss under certain circumstances, but if you restore the vestige you will lock yourself out of one of them. This will increase your total playthroughs from 3 to 4.

Mistle (51/51) - Gaol of the Stagnant Blood
Return to the elevator and take the long ride down to trigger a cutscene. Open the large door and progress forward to the Mistle.

Miasma Manager

Progress forward to the final boss of the game, ensuring you never repaired the Eos vestiges. The first part of this fight is rather simple. I only used an attack up Gift, but Blood Guard could come in handy too. His moveset consists mainly of large heavy swipes that can easily be dodged.

Skull King

Hades Blood Code (26/32)
The 2nd phase is a bit different and can prove to be a bit of a challenge. His standard attacks are long swipes, a leap attack, and a pounce. If you attack under him too much he will cause a large explosive AoE, and later in the fight he will summon a hailstorm of crystals. Attacking the Virgin Born from the side or behind does decreased damage, so you're almost forced to face him head on unless you want the fight to really drag out. I found the best thing was to bait out the swipe attacks and attack his front arms for the most part. If I found a safe time to attack his face, I would. His deadliest move is the hailstorm. If he does it, hiding under him seemed to work well for me. The fight might take a few tries to get the hang of, but once you kill him you'll be greeted with a great cutscene. Afterwards, you'll be back at Home base with all your friends. Interact with the front door to trigger the ending scene, the credits, and receive the Blood Code.

Dweller in the Dark

NG Clean Up
After returning to the title screen, reload your save to find yourself back before the final boss. We have a bit of clean up to do before we officially end our NG run. Warp back to Home Base and speak with Io. Restore any and all vestiges you'd like to, especially those of your friends for the achievements (Prometheus, Atlas, Artemis, Hephaestus, Eos). For Coco you'll need to watch either the Hunter or Survivor memories that specify she's in it. Davis we won't be able to do until a little later. The most important one is Eos. We need to restore all 7 for our next ending.

After restoring all the memories, it's time to collect the last few Depths maps and vestiges.

Depths Map (8/12) - Cliffs of Rust
  • Warp to the Parking Garage in Ruined City Center to find the Self-Important Revenant from early into the game. Talk with him and he'll tell you of a powerful Lost at the park.
  • Warp to the Park Ruins and kill the large Lost, then head back and talk to him again.
  • Finally, warp to the Decayed Ship Interior in Dried-Up Trenches to find him once more. Speak with him for the map.

Depths Map (9/12) - Rocky Ruins + Hermes Vestige D
  • For this map we'll need to continue the Self-Important Revenant's quest, as well as the quest of another NPC. Start by going to Home Base and talking to Davis, then travel to the Cliffs of Rust.
  • Once there, head down the center path and kill the monster marked by the blue cross.
  • After killing the marked enemy, head up the staircase directly to your right. At the end of this path is the vestige.
  • Warp back to the Decayed Ship Interior and report that the monster is dead. Finally, warp to the Poisonous Butterfly Nest and speak with him again. Travel to the Research Facility Area in Memory of Player. Head back to the camp where the blue cross is on the map and grab the Mistle Agent. Take this back to the Revenant to finish his quest.
  • Now it's time to travel to the Hills of Deception in the Howling Pit to meet Richard. After talking to him, follow the blue cross on your map to find his sleeping bag and deliver it to him.
  • Warp to the Ridge of Frozen Souls Entrance and head up the hill to where the 2 dogs are. Roll off the ledge where we grabbed a vestige our first time here to find Richard at the bottom. Talk to him, kill the marked Lost, then loop around to talk to him again and progress his quest.
  • Travel to the Natural Cavern Depths in Ruined City Underground to find him once more. Talk to him, kill the marked enemy around the corner, then talk to him again for the map.
  • Finally, warp to Home Base then back again to speak with him for the map. If he isn't there, talk to Self-Important Revenant again, then warp back and he should appear.
Walkthrough VII
Depths Map (10/12) - Arachnid Grotto
  • Warp to the Dried-Up Trenches Entrance and speak with Gustav to start his quest.
  • The first item we need is at the Parking Garage in the Ruined City. Head up the ramp to where the blue icon on your minimap is to find the report.
  • Warp to the Outlook Tower in the Cathedral of Blood. Head straight down and to the left for the report.
  • Warp to the Observatory Site in the Ridge of Frozen Souls. Head straight up a few feet to find the report on your right.
  • Finally, head back to Gustav and turn in all 3 items. Afterwards, warp to the Accumulator Ruins in the Ashen Cavern to speak with him one last time for the map.

Depths Map (11/12) - Swirling Flood
  • For this map we need to complete the first part of Shang's quest. You should have spoken with him earlier, but you can double check that you've talked with him enough by visiting him at the Outer Crossroads in the Ruined City Underground.
  • Head to Home Base and speak with Davis to travel to the Den of Darkness. Once there, locate the item denoted by the blue cross and deliver it to Shang.
  • Warp to Home Base, then back to Shang and talk with him to trigger an invasion. After clearing it, speak with him for a reward, then warp to Home Base and back again for the map.

Depths Map (12/12) - Silent White + Hermes Vestige C
  • After speaking with Shang for the last map, head to Home Base and speak with Davis to travel to Swirling Flood. After arriving, follow the bottom path to the item for Shang.
  • Head back to the Mistle and take the top path this time. In the next room, head straight to the top where the urchin-like enemies are to find the vestige on your right.
  • Once you've grabbed that, head back to Shang and talk with him. Warp to Home Base then back again and he'll give you the final map. You still need to visit every Depth at least once for the achievement, but we have a few vestiges to grab in them first.

Hermes Vestige A
Talk to Davis and travel to the Den of the Dead. Once there, take the bottom path and you'll arrive at a fork in the road. Go right to come up on a red shipping container. The vestige is around the right corner where all the poisonous slime are.

Hermes Vestige B
Talk to Davis and head to the Town of Sacrifice. Once there, take the path to the right of the metal door (when facing it) and you will arrive at a 3-way split. The vestige is down the center path. You can now use the 4 of these to get your achievement for Davis' memories.

Scout Vestige A
Talk to Davis and head to Silent White. Take the path to the right of the metal door (if facing it) to find the vestige directly before the hall to the mini boss.

Scout Vestige B
Talk to Davis and head to the Misty Ruins. Facing away from the metal door, take the top left path and then head to the top left corner of this area for the vestige.

Scout Vestige C
Talk to Davis and travel to the Arachnid Grotto. Facing away from the metal door, take the path forward to the next room. Continue forward past the staircase to find the vestige behind a large purple Lost.

Scout Vestige D
Talk to Davis and head to the Zero District. Facing away from the metal door, take the path straight. Continue forward until you reach the large white Lost. The vestige is to the left, just past the truck.

Return to Io and rebuild every single memory to unlock the achievement for all vestiges.

Mender of Minds

Scout Blood Code (27/32)
Speak to Davis and travel to the Rocky Ruins. Facing the metal door, take the path to your right and follow it all the way through up the staircase at the far end. The ode is around the boulder to your left, by the large bug Lost.

After that, return to Davis and travel to any Depths map that you haven't yet. You don't need to clear them unless you want to.

Deep Trailblazer

From here you should have the following:
All vestiges.
All Mistles.
All Depths maps.
27/32 Blood Codes.
1/4 endings.

Queen Blood Code (28/32)
Return to the Gaol of the Stagnant Blood and kill the final boss again for your second ending and the Blood Code.

NG+ Bad Playthrough
Once you've gotten both Good endings, load up your save and travel to Home Base. Interact with the front door and start NG+. You can increase the difficulty if you're looking for a challenge, but if you're just looking to get the achievements as quick and efficiently as possible, stay on the same difficulty. During this playthrough we're not going to collect or repair any vestiges, and we're not going to give the memories back to any of the Successors.

Blood Codes 29-32
You will get each of these by killing the Successors and not restoring their memories.

Weaver of Wills

Once you kill the Skull King you will earn the Heirs ending.

Heirs

NG++
And now our final playthrough and ending. This time, we need to restore the memories of 1 Successor. I recommend doing Nicola's memories as his is the shortest level and easiest to collect. As a quick reminder:
  • Part A can be found near the start of the area. Head up the hill to where the dogs are, then roll off the left cliff a little further up.
  • Part B is found shortly after, at a fork in the road. Take the right path to a cliff.
  • Part C is found after the Iceless Falls Mistle. Take the right path down into the cave, and take the left path at the fork.
  • Part D is in front of the first ruined building at the snowfield.

To Eternity

Determiner of Fate
Revenant Preeminent

Unlock every other achievement. Congrats!
Gift Gatherer

Gifts are essentially your skills in this game. Every Blood Code you unlock has a certain amount that you can buy for varying prices. During my first playthrough, I pumped almost all of my Haze into unlocking gifts. 150 is quite a lot, and you'll more than likely unlock it during your 2nd or 3rd playthrough, so long as you pay attention to unlocking them. Note that you need to BUY 150. The pre-unlocked Gifts for Codes do not count toward the total.
Determiner of Fate

There are 4 endings in the game that require you to kill the final boss under certain conditions.

Dweller in the Dark (Good)
  • You cannot restore the Eos vestiges for this ending. You can collect the parts, just not restore them.
  • You must collect all the vestige parts for Isis, Fionn, Scathach, and Harmonia.
  • Once you've collected all the parts, you must restore all the vestiges at either Io, or the Successors attendant who stands just outside their arena before defeating the boss.
  • Assuming you've restored the vestiges, you must then defeat the boss and restore their memories when prompted to.

Dweller in the Dark (True)
  • Restore every Eos vestige at Io.

Heirs
  • Don't restore the vestiges or memories of any Successor.

To Eternity
  • You must restore the vestiges and memories of at least 1, but not all Successors.
Mender of Minds

You must collect and restore every vestige in the game. Please refer to my walkthrough for more info. I recommend going for this on your True ending playthrough as you'll have to collect a decent amount of vestiges for it either way.

  • Assassin A-D
  • Atlas A-D
  • Hunter A-D
  • Fionn A-D
  • Isis A-E
  • Artemis A-D

Together Until Oblivion

This will likely be one of the first achievements you get so long as you don't disable partner voicelines. The AI in this game LOVE to comment on every little thing that happens. I got this early into Ruined City Center.
Proven Devotion

To receive a present from a friend, you first have to talk to them at Home Base and exchange items you find on your journeys with them. Every character likes certain items, so it's best to keep track of who to give what to. When you give items to a friend, you'll gain trade points with them. Think of this as a friendship score. Do not spend these points, or that score will go down. The higher your score (Max 75) the faster you'll receive presents. At 75, you have a 45% chance to receive a present every in-game hour, after 600 meters travelled. I suggest maxing as many people as you can to 75 as this achievement will take quite a while. Don't worry though, the count carries over into NG+ and beyond. You can also buy and trade presents from Shang.

Here are a few max point presents per character:

Io
Curious Novelty, Local Pennant, Tomato Oden Sandwich

Louis
Faded Comics, Tomato Oden Sandwich, Yellowed Book

Yakumo
Aged Brandy, Boutique Sake, Bugarally Doll

Coco
Aromatic Herbs, Chocolate Garlic Flakes, Powerful Spices, Sushi Tacos, Tomato Oden Sandwich

Murasame
Board Game, Bugarally Doll, Retro Game

Davis
Antique LP Record, Bugarally Doll, Chocolate Garlic Plakes, Classic Camera

Mia
Aromatic Herbs, Fragrant Tea

Eva
Flower Seeds, Tomato Oden Sandwich

Jack
Aged Brandy, Antique Coin, Bugarally Doll, Sushi Tacos
Exalted Reputation

This is the achievement that will stop many people from 100%. To reach rank 30 you need 400 Marks of Honor. How do you get them? There are 3 you can find during a normal playthrough, you get 1 per co-op boss kill, certain enemies drop them at an abysmal rate, 100% drop rate from the final bosses in Tower of Trials I through III, and potentially rewarded for clearing the Tower of Trial floors.

No matter how you approach this, this achievement will take a very long time. The fastest method is farming high boss count Depths with a partner, such as Misty Ruins. However, I wasn't able to find anyone to either help or be helped. The second fastest is farming the Tower of Trials, as the chance of these dropping from regular enemies is so disgustingly low.

If you'd like to use the comments section to set up a boosting session please do so.
Burning Spirit

To inherit a Gift, you must first equip the appropriate Blood Code, then equip the Gift and earn XP to max out it's bar. You can also use the Awake items you find throughout the game to automatically max out a Gift. There's a catch, however. If you over-level your character for a specific area you will receive no Gift XP. The community made guideline is 7 levels per boss. That means:

Pre-Oliver: level 1.
Killed 1st boss: level up to 8.
Killed 2nd boss: level up to 15.

So on and so forth. I did this and constantly rotated my Codes and buffs on my first playthrough until I unlocked this somewhere around the Ridge of Frozen Souls. If you already over-levelled, don't worry! You can either join a low level player as you will be scaled to their level, or continue progress in NG+. I do recommend getting this out of the way early, though.

You must first receive the Artemis Blood Code, then restore any Artemis vestige.
Drink Deep

This is likely the first achievement you will get. Either parry, backstab, or launch an enemy. You can do this as early as the tutorial.
Heirs

Heirs is the Bad ending of the game. To receive it you cannot restore the memories of any of the 4 Successors.
To Eternity

To Eternity is the Neutral ending of the game. To receive it you must restore the memories of at least 1, but not all Successors.
Dweller in the Dark

Dweller in the Dark is the Good ending of the game, and there are 2 versions. 1 for restoring all the Eos vestiges, and one for not. To receive this ending you must collect all the vestige parts for Isis, Fionn, Scathach, and Harmonia. Then, restore the vestiges at the bosses respective attendant. Finally, defeat the boss and restore the memories of all 4 Successors.
Resonant Power

To perform a Communal Gift, you must first equip one appropriate for your AI partner in the Gift section. Once you have enough Ichor, use the Gift and the achievement will unlock.

nice guide, would be great to have a "missables section", although the info section technically has it

Nvm I figured out. The game only checks whether the player has all the depths maps when the player decides to teleport to 'Depths :Void District'.

I have all the depths maps and cleared 100%, but the I didn't get the achievement. Do you know what could be causing this?

Scarlet Nexus is on my list of games I'd like to 100%, but I have my hands a bit full with other games and IRL stuff. I can't give a guarantee that I'd get around to it any time soon.

One of the best guides out there by far, super useful and easy to follow. Thank you for such an amazing guide ! Any chance you could do scarlet nexus next? I plan to 100% that game next :)

So, it's only 3 runs and no need to restore the Eos vestiges + 4 successors for the true ending 'See all endings'. I think the guides are outdated and the devs fixed this so that the achievement would unlock by having the other endings.

I just got "Determiner of Fate" AND "To the eternity" at same time WTF? I only restored the memory of one of the successors for the neutral run and nothing else, however Determinant of Fate was also unlocked at the end...
