I will be spoiling locations and story reveals in this guide. If you want to have a blind playthrough for the story, I recommend coming back to this guide later.

If you don't know the location of an area I mention throughout the guide, reference this map that has a few other useful annotations.

Fully Integrated:
"Welcome to the sun"

Exit the bunker you start the game in. You are not required to complete the tutorial for this achievement.

A Heart of Meat:
"Finish the pilgrimage and help with the missing heart"

In New Delphi, talk to the Historian in his shack, and then talk to the High Priest about the Secret of New Delphi. He will send you on a Pilgrimage to Titan's Head, Titan's Guts, and Titan's Heart. At each of these sites, you must acquire the seal from the guardian at each location.

• Titan's Head Seal: Acquired by answering the guardian's questions as a zealot of the Titans would. Alternatively, you could pickpocket it off of him or kill him and loot the corpse.
  
• Titan's Guts Seal: Acquired by cooking meat in the stove outside his hut.
  
• Titan's Heart Seal: Acquired from the dying man at the end of the blood trail.

After getting all 3 seals, return to the High Priest of New Delphi. He will give you a key to Godhunter's Prison. Go to the prison, and work your way to the bottom floor, and collect the Seedling Of The Titan. Then, return it to the High Priest. He will tell you to return "in a while", so just travel to a few other locations before returning. Upon returning and speaking to the High Priest, the achievement should unlock.

Fortune Hunter:
"Help Custer collect the core"

Speak to Custer by the statue in the center of Tauris. He will tell you to collect a machine core from Bunker 17. It is in the big terminal on the back right corner of the facility. Return to Custer in Tauris and the achievement should unlock.

Lost And Blind:
"Get the eyes for the blind man"

In Mine 19, take the elevator down and talk to the Blind Man on the lowest floor to have him follow you. When he is hit by an enemy, he will begin to wildly fire his shotgun, and likely get himself killed.

With stealth, walk very far ahead of the Blind Man to clear out the drones and turrets before the Blind Man stumbles into their range. Take him to the end of the facility, where a robot in the office will drop his eyes. Give them to the Blind Man and the achievement will unlock. If the Blind Man dies at any point the quest will fail and you will not get the achievement.

Alternatively, with a High Tech skill level of 5, you can activate various terminals to make the robots passive and simply walk through the facility.

Heightened Senses:
"Reach level 5"

Kill enemies, discover locations, and complete quests for XP.

Transhumanism:
"Fill all implant slots"

Purchase implants from the Machine Merchant inside the shack just to the right of the center of Tauris. Some require Occultism or Cybertech skills of 3 to equip. You just need to fill all of the implant slots to get the achievement.

Exploring Nexus:
"Visit 20 or more locations"

There are a total of 22 locations in the game. Just explore all of the darkened areas of the map and you will find them.

Another Reality:
"Enter a Cyberwomb"

A Cyberwomb exists in Puke Bar, in a room on the far left side, but it needs to be powered up. The battery for this machine can be found in the underground area of Old Trees. Return to Puke Bar with it and put it in the generator in a side room adjacent to the Cyberwomb itself.

A number of Cyberwomb modules exist in-game, but the easiest is available inside the Puke Bar. Talk to the man in the center room of Puke Bar to receive the Combat Simulation Module. Go to the Cyberwomb and interact with it in order to enter the simulation. The achievement should appear as soon as it finishes loading. You are not required to stay in the simulation and can just leave immediately.

Flesh And Bits:
"Get blown to pieces"

There are a number of ways to get this achievement. The most important part is that you die instantly. You can do this by finding a landmine in one of the various locations around the world, by getting hit with the rocket launcher, or by attempting to re-enter the settlement inside the bunker.

Puke Extravaganza:
"Have 5 or more characters puke at the same time"

Puking causes other characters to puke. The best way to get 5 people to puke at once is to go to New Delphi, stand near a large group of Pilgrims, and puke. After a second or two, the achievement should unlock.

Tick-Tock
"Place a grenade in someone’s bag"

Grenades can be purchased in Tauris from the Machine Merchant. While in stealth, walk up behind any character, and interact with them while your weapons are holstered. With 0 Pickpocket skill, you have a base chance of 33% to succeed at a pickpocket. With a higher Pickpocket skill, this chance increases. If you save right before you pickpocket, you can keep reloading if you fail.

Once you succeed at a pickpocket, you have a limited time frame to open your inventory and place a grenade in their pocket. Once the grenade is placed and it explodes, the achievement should unlock.

Workshop Master:
"Defeat the Giant Basher"

The Giant Basher (referred to as the "Deconstructor" in game) is located in the Humanoid Workshop. The outside is guarded by 2 robot drones. They can be dispatched most easily with stealth or the Rocket Launcher. Once inside, there is a console to recharge your abilities, and the boss arena proper. The boss cannot be damaged by ranged weapons until their claw arm has been blown off. This is achieved by shooting one of the red exploding barrels while the boss is near it. Once this happens, they can be shot with any weapon to deal damage. Repeat this cycle until the boss has died.

Flesh Crater Survivor:
"Defeat Glutt"

The Glutt is found in the Flesh Crater. Outside his arena is another console for charging abilities. Once inside the arena, there are multiple yellow worms and several "nests", which spawn these worms. Run around the arena killing these worms and nests, as the boss would otherwise use them to heal during the fight. Once these nests are cleared, you can begin damaging the boss with either melee or ranged weapons.

As the boss gets to low health, they will begin to spawn additional enemies such as the Chargers or generic zombies. The fight gets a lot harder if the boss spawns these extra enemies. Use the flamethrower or rocket launcher to deal large amounts of damage and kill the boss before it can summon too many.

Iron Will:
"Defeat a boss on Hard Difficulty during a Permadeath run"

Create a character with the combat difficulty set to "Hard", and "Permadeath" selected from the "Extra Challenges" menu when making a new profile, then defeat a boss.

I recommend fighting the Giant Basher for this achievement, as they are currently the easiest boss. Higher-tier weapons that help during the fight can be acquired from the Machine Merchant in New Delphi. I recommend the Automatic Rifle in particular. Picking up the Pistol from Puke Bar also helps for shooting the barrels while saving on ammo for the rifle. If you wanted, you could also brave Godhunter's Prison in order to get the Rocket Launcher from the Machine Merchant on the bottom floor. The Rocket Launcher can also be acquired from an enemy in Crossroads, but it doesn't come with much ammo.

If you so desire, you can still technically "save" on permadeath by copying your save data into another folder, and copying it back into your game folder if you die.

Just enter the Perished City in the east side of the map next to (Location) The Wall
(seems you cant enter with an old character for now or i didnt figure out how)
or Start a new game and choose The Perished City content (drops when spawning in)

Survive all waves during the virtual combat exercise

try blocking a rocket

go to the Market in Perished City or a other location with traders and purchase Blocking module then head to the crossroads and find the enemy with the rocket launcher
its a bit tricky to get the timing right so do a quicksave before !
equip the block module and try block one of the rockets! Theres your Achievement

Location The Puke Bar
Just survive all the waves of the virtual combat Program at the flesh bar! should be 10 waves!

Tiptoeing around - Achievement
Sneak through the factory undetected

Location The factory (Perished City)
Just dont get detected until you reach the room in the middle at the back of the factory
(works with saves)

Taking One for the Team - Achievement
Let artus make the sacrifice

Location City Metro
at the metro you need to accept quest from ila and search for a special worm in a cave(underground)
after that progress the quest until you can choose between her and the husband pick:
Artus the husband after he dies the Achievement should drop

Electric Trash Digger - Achievement
Defeat the Electric Eye

Location The Dump
Defeat the the Electric Eye in a underground skull cave at the dump
to make this easier just take care of the Turrets in the arena first after hes down 1/3 of his Health Bar!

Sentimental Overload - Achievement
Share at least four memories with the Seneschal

Location at the Villa (perished City)
Talk to the Custodian and share Memories

memory 1 the Flesh Kraken (completed fleshkraken quest)
memory 2 didnt had sex with the queen (queen quest complete)
memory 3 about the almost naked man needs 5 empathy for the skill check
memory 4 seeing two people in ? maybe the Corpse pile at the Heap?

Friend - Achievement
Find a friend for the big guy

Dropped as i loaded in with my old character
i guess its for bringing the Cyborg/Robot head to the Flesh Kraken

Musing around - Achievement
Deciding the muses fate

Location The Market
Talk to the artist,then interact a few times with the living painting until the conclusion option appears
then talk to then artist again the achievement should drop!
May have an alternate solution!

Heptarchs Memorabilia - Achievement
Get an engram of a Heptarch

I just killed the Bouncer ilbert for the Door Key (pickpocket didnt work) ,turns everybody inside hostile! and a Red haired woman with a mask had the Hevla memory item in her inventory, pick it up and the Achievement drops !

Solo Rebel - Achievement
Overcome the Perished City Administration while not siding with a faction

Locations The Villa/The Factory/The Market/Administration
talk to the Seneschal he asks for a favor, accept his task. then head to the factory and make your way to the room in the middle at the back and get the filter.
next get to the market and replace the old filter! near the flesh titan,ask him for a special meat you will recieve Morrowmeat/bring it to the Seneschal/recieve key to back of the villa.then ask if he needs help again and accept his task,go to Residential Area and kill Ilbert The bouncer for his key.
open the door and kill the rest of the people! a woman with red hair and a Mask has the Hevla in her inventory grab it.bring the Hevla to the Seneschal to get access to the last room where the virus is,pick up the virus. Now head to the Administration go inside and go to the backrooms, use the virus on the panel. Enemies will be alerted after the Locked Door opens kill them all and proceed further in the last room will be your target kill him to get the Achievement!

Overcome the spikes in the Temple Dungeon via throwing some meat

locations: The Market/Administration/Residential Area/Temple of the Mother
talk to Protas at the market located in the south and help him,
you have to scout out the administration building.
just go inside and walk to the back and then enter the room in the back scout there
and return to Protas to finish this quest.
he will then direct you to the Residential area,talk to Ilbert The bouncer and he will let you in
then talk to gavas and you get the quest Human resistance!
go to the market and talk to Morrowmeat ask it to help you entering the Temple/
you get Morrowmeat of regret.
then buy some meat at the market or just have some in your inventory!
go to the Temple of the Mother go to the door and try communicate with the flesh,
the door will be cleared and you can enter The Temple of the Mother.
Make your way through to the the next doorway, in this section are the spikes for the Achievement!
