The lion image isn't featured or credited in the book but is instead on the inside of the jacket
The Jacket's design is credited to Mark Pollard, credited for the book's design and compilation
Adrian Carmack & Kevin Cloud are credited for the graphics & artwork in the game

@spineapple tea I just thought it was interesting to see that there was a skull image in the book. I figured it could have been used somewhere. It does look a lot like the menu icon. Thanks for pointing that out!

i might just be blind but i cant see the skull in the switch in this picture. it kinda looks like the skull in the menu though, the one that indicates what you're selecting.
