i didnt make this, shlup did (and he just took it from the HL2 arcade files), but he doesnt know how to upload things to the steam workshop correctly, so. here we are
(steam doesnt let you upload workshop items for games you dont own and he's game sharing gmod).

SHLUP SEZ:
the description should be like "this map was ported over from Half-Life 2: Survivor witch was a Japanese exclusive arcade port of Half-Life 2. What your suppose to do in this game mode is to only use your gravity gun and a shotgun to blow up the zombie houses around the map. But you dont have to, its gmod."
