Why the game crashing ?
There are tons of reasons that cause instability or crash, but the main one is the refresh rate for sure! 25 hz and 30 hz for consoles and average computers from 2011, now we are in average to 50/60hz what cause a conflict.

the solution is simple: vsync
block the limit at 30hz unless your graphics card does not allow it, my gtx 1050 cannot do so below 40hz, in this case choose the lowest refresh rate will fix this problem.

My game won't let me change my hz at all, its stuck at 60

59hz fixed the crashing for me

Windows recovery mode is the real answer

I am playing it on a 120hz TV with 115fps NVidia cap and Vsync on in Nvidia panel and it's stable for me.

I play it at 144hz and it crash every time the chapter ends, i´ll try that

this didn't work for me. In game I changed my settings to 30 hz and it is still crashing.

Thanks for the help, I was having a hard time figuring out why my game was crashing so consistently, but this seemed to work.
