I think the blessings hurt whip is the most likely explanation, but there's also a bug (feature?) where if you get to the shrine room without the Devouring Rage spawning, then he will spawn after you enter the shrine room. Maybe this person went AFK thinking they were safe and the DR gave them a hug.
