First things first

Before I get into the achievements I feel I should preface that I fully believe this game should be experienced blind at first. Secondly I will try and include as little spoilers as possible in this guide :)

Work safety

Should come at the beginning of the game

Achieved after sending your first squirrel picture

Found in chapter 5 just find the fallen logs and around the area will be a steel cabin. Inside there will be a broken guitar. Pick it up and the achievement will pop

Missed opportunity

keep faxing things that aren't squirrel shots until they say this
images must be taken with the SLR not the handheld camera (thanks to TheCell.ch λ³)
It also doesnt count if you fax pictures of the squirrels path (credit to Hanni for this info)
Also, you need to do it on chapter 2 after you open the SLR box (thanks to Nanofu and Matt Fraggle for working this out)

this achievement can now be earned :) you need to use the fax machine ten times in one session (thanks to Hanni for this)

In order to get Work Safety in the first chapter, after you fax Nina the picture of the caravan the cutscene shouldn't be skipped.

I got "Missed Opportunity" during day 3 after faxing in a picture of the sky twice (same picture both times).

I can't seem to get "Work Safety" one at all. I replayed the game from scratch twice, jumped around on the vehicle and sent pics of just it too. It doesn't trigger for me :(

for "Missed opportunity" you can just fax the paper with the text "fax this" it will count as a photo with no squirrel in it. just fax it a few times.

The other suggestions for "Missed opportunity" didn't seem to work for me, but I found a quicker way that doesn't involve answering the phone at all.

In chapter 2, unpack the camera and turn it on, then go inside and switch the printer channel to 2 or 3 (no camera connected), then print that. It will print a blue error image. Keep faxing that til you get the achievement. She won't call at all.

For fax_sound.wav you have to fax 10 times in the same session ( dev post here ).

I've got Missed opportunity now. It is important to use the SLR camera and print photos with the printer next to the TV and not the handheld camera. 2 Photos with the SLR was enough, 10 photos with the Handheld did not do anything. Please note that in the achievement for others :)
