The Basics
This guide is about automating the M1 hold. This can be useful for various reasons, e.g. less strain on your hands, freeing up a hand so you can do other things on the side like taking a drink, etc.

The automated M1 hold can be used on the following actions:
  • generator repairs
  • healing / mending / recovering when downed
  • totem cleansing
  • opening chests
  • unhooking / uncaging
  • cleansing from Plague's sickness
  • snapping out of Doctor's madness
  • searching Pig's Jigsaw boxes
  • waking up from Nightmare's dream state
  • closing Demogorgon's portals
  • disabling / freeing from Trapper's traps

Depending on the action the automation may be more or less useful. But for the main Survivor objectives (generators, healing, totems) it certainly is useful for me.

You will still need to manually hit any skillchecks that may pop up.
In-game Solution
  1. start an M1 action
  2. keep M1 pressed
  3. press ESC to enter the menu
  4. let go of M1
  5. press ESC again to exit the menu
  6. the action will now progress automatically

As of now (14.01.2021) this still works, but the current PTB has removed this "feature".
Windows
Windows has a mouse option called "ClickLock". Once activated the mouse will go into a hold option if you press M1 for a certain amount of time. To activate this option and adjust the time needed to start the hold go to your mouse settings.

For modern Windows systems do the following:
  1. press Windows key
  2. type "mouse"
  3. open the mouse settings
  4. on the right hand side open "additional mouse options"
  5. go to the tab "Buttons"
  6. make sure "Turn on ClickLock" is ticked
  7. to adjust the hold timer click "settings" to the right of the checkbox

Please be aware that this means this option is constantly on, unless you disable it again. Consequently it may have an effect on your non-DBD related operations.
macOS
Not quite the same as Window's ClickLock, but macOS has an option called "Drag Lock" using the trackpad. For more information and further alternatives check here[apple.stackexchange.com].

Linux
