[All Acts] Ghost's Encyclopedia of Loot and where to find them

Unmarked Spoilers ahead! Baldur's Gate 3 has a lot of secret or hidden items for you to equip. At this moment, Baldur's Gate 3 does not emulate the dreaded attunement slots that are common with particularly powerful magical items. You only have to deal with juggling conflicting item locations (you can only wear one helmet, one set of armor etc.)

Here's everything of note I've found so far, and where to get it. As some of these rewards are dependent on how you approach certain quests, it may not be 100% accurate. Please note that I have not included items or gear that I have not personally encountered.

Using This Guide
Baldur's Gate 3 is rather open-ended about how you can acquire things. You can handle quests the "proper way" and be rewarded with things at the end of quest lines or you can murder hobo to victory!

Control+F is your friend. Every named magic item listed will include
  • The in-game picture/text box saying what it does
  • Where I found it and how I got it (again not necessarily the only way)
  • The Name of the Object for searching
  • The Quality and Type of the Object for searching

Please be mindful that as Larian updates and improves the game, the UI for items changes and evolves, meaning that the presentation of how the item looks may not be the same as it was whenever I took my picture of it. I will do my best to update things for an effect change, but not if the item card looks different.

One last thing to point out: as you finish quests and the situation of things change through the game, opportunities will change as characters move elsewhere, die, or become hostile. You should expect that there will be missed opportunities if you fail to do something in time, or a particular way. Even as a minor example of this, Vendors will update their inventory. I'm not sure if it's tied to your character level or the passage of time, but I have gone back to Dammon and Arron in Druid Grove multiple times during Act 1 to pawn off garbage I've found just to see that they have new items of a higher quality to sell.
Act 1: (Missable) Nautiloid Ship: Defeat Commander Zhalk
That's right!

If you manage to kill the devil boss at the end of the tutorial, you can loot from him the uncommon martial two-handed melee weapon, Everburn Blade.

There are two strategies to get your hands on this, the simple way (kill him) and the easy way (get lucky).

The Simple Way:
If you want to do this, I suggest freeing 'Us,' the Intellect Devourer and Shadowheart. Kill the three imps at the front of the room with basic attacks and cantrips and then focus fire him down. Use all spell slots you have on damage: the Commander will focus the Mind Flayer until he dies, and Cambions will eventually spawn in meaning you can't stall for time or try a control strategy.

If you can time Shadowheart's 'Guiding Bolt' spell before the Mind Flayer gets to go, you can help ensure it's getting big damage on the Commander to help close out the fight. If you really want this weapon, you're probably going to need to save scum Shadowheart's Guiding Bolts and hope you get some crits in the fight. You should also level up almost immediately from killing him.

If your mind flayer "friend" is still alive by the time you kill the Commander (unlikely on Tactician), he will turn on you.

This is right out of the game a strong weapon to give to Karlach, Lae'zel, a Fighter, Paladin, or Ranger PC. It also just looks really cool in the overworld, and offers some of the best melee damage you can do in Act 1.

The Easy Way:
Before entering the Helm, switch one of Shadowheart's spells to Command and use it to have the Commander DROP the blade. With a 55% chance per casting, your odds on a single run are about 79.25% chance of success. Good luck to anyone doing an Iron Man run!
Wilderness, Forest
Next to the cliff where you recruit Asterion, there is an area that you can jump down to. If you go over to the little alcove and remove the rock, you will find a chest inside with a Ruby worth ~100 gold (reduced from Early Access' 1600).

This can be a good way to get your hands on some early game shop items. The amount of money you get for selling things is based on the Charisma of the character performing the transaction, with a 20 CHA character getting 50% of the listed value of an object from Traders. While it's not possible to do this in Early Access, Wyll - a Charisma based Warlock - is easily recruited within the first major location (Druid Grove) right next to where the earliest Traders are. You can wait to hit Level 4 and hit the 18-19 benchmark if you want, but given the useful items from Ethel (see below) I think it's worth recruiting Wyll and giving him the gem to sell.

Gale's recruitment is now missable. If you fail - or refuse - to pull him from the rock, he will be unavailable throughout Act 1.
Dank Crypt
Going into the Upper Layers of the Temple where you find Shadowheart (you can enter by either tricking the door man, picking the lock, or breaking the tiling with the rock) you will eventually find a library looking room with a statue. If your crew passes a Perception check, you will find a button to unlock the deeper section of the Temple.

One side of the temple has a large sarcophagus with a booby trap, a lance and a key. Traps will shoot exploding fire balls at you from either side if you do not disable them after opening the Sarcophagus. There are there are at least six grease vents, one by the exit, one by the way you entered the room, and two on each side. You can
  • Use the boxes and vases throughout the room to subdue the gas vents
  • Outmaneuver this by separating your cast to one side of the room, and leaving one character to stop the traps
  • Enter Turn-based mode after triggering the traps, and press the button on the nearby column, if your party managed to perceive it.

Inside the Sarcophagus is the Uncommon Versatile melee spear weapon, The Watcher's Guide and a Key.

In regards, to the lance, the True Strike effect activation appears to work even if you make a ranged attack or a spell attack. It has actually been buffed to be able to be wielded Versatile, making it a straight upgrade for Shadowheart's starting Mace.

Use the key on the large doors opposite the Sarcophagus room to find another place to loot. This area is safe to explore until you push a button in the alcove, at the top of the stairs, left of the statue (which also requires a perception check to find). 4-5 Skeletons will wake up (one with about triple health). Defeating them all will let you safely explore the opened room. Inside is a necklace that lets you cast the Level 3 Speak With Dead spell. You can cast it as many times per day, but it only works on any given corpse once.

If you open the sacrophagus in the room you will find the Very Rare Amulet, The Amulet of Lost Voices, and you can make an undead skeleton friend, Withers. Withers does three services for you:
  • Resurrects dead party members for 200 gold.
  • Allows you to respec a character for 100 gold.
  • Lets you recruit a hireling for 100 gold.

Just Outside Druid Grove
After the skirmish outside the Druid Grove, the defeated Goblins can be looted. The goblin Boss, Za'Krug has Uncommon Gloves, Gloves of Power.

Of your party members, Asterion is one of the best users of these: if you give him two Light one-handed weapons, he can make up to Four attacks - as a Thief, using a Potion of Speed - in one turn using both Bonus Actions to make Off-hand Attacks. This means he'll have up to four chances to activate the Bane effect.

The actual effect of receiving the brand has changed a couple of times during the course of Early Access into launch, so I'm a little unsure as of [2023-08-09] what the flavor text implies.

~~~
There is a tall rock plateau that must be traversed to by jumping just outside Druid Grove (X: ~170, Y: ~370). At the top of the area is a wooden battlement over looking a campsite. If you examine the skeleton in the center, you will find an uncommon amulet, Silver Pendant.
Guidance is one of the best spells in the game and is a fantastic option for any character that is not normally concentrating on things: you can use it as a bonus 1d4 for ANY skill check. You can also use it when you need to roll a social skill against Shadowheart where you'd want the 1d4, but she wouldn't offer it to you.
~~~

Farther down the path in the Forest area, outside the Blighted Village, on the left side (not Scratch's side) down the rocks is a hidden treasure chest at X:80 Y:349. It was marked "Hidden Goblin Cache" on my map sometime after interacting with something at the Shattered Sanctum (a letter probably?). Inside were uncommon Gauntlets, Fleetfingers.
(Missable) Druid Grove, Traders: Arron and Dammon
Walking into the Druid Grove, you will find a Halfling Merchant standing in front of a statue.
You can barter with him for healing potions, scrolls, and some magic items.

As a trader, Arron's inventory is semi-random and updates as time goes on. But most first visits he seems to have the following unique items:
  • The rare gloves, Gloves of Missile Snaring
  • The uncommon versatile melee quarterstaff weapon, Rain Dancer.
  • The uncommon ring, Ring of Flinging
  • The Uncommon one-handed melee weapon, Dragon's Grasp.
  • The uncommon longbow weapon Spellthief
  • The uncommon medium armor Hedge Wanderer Armour.

~~~
Dammon the Blacksmith has a variety of +1 weapons and armors, but his two starting exclusives seem to be:
  • The uncommon shield, Safeguard Shield
  • The uncommon two-handed shortbow weapon, Hunting Shortbow
~~~

At some point (after your first visit?), Auntie Ethel will leave her little store front for her house in Sunlit Wetlands.

When you first arrive, she is willing to sell you an uncommon two-handed melee quarterstaff weapon, Corellon's Grace. It's generally preferable over Rain Dancer, but is clearly meant for a Monk character. One offers an almost constantly useful bonus and the other is a niche utility option. Both of them cost money to purchase at this moment in the story.

If you miss it...
If you miss the opportunity to purchase it from her now, you can loot it from her or buy it later depending on how you interact with her at her house (see below).
(Missable) Druid Grove: Quests and NPCs
Next to where Wyll is recruited, the trio of Tieflings are arguing (loudly) about where or not you should stay or go, there is a Strange Ox. If you have the ability to talk to animals he will comment about his plans. It is unclear to me in Act 1 if this ox returns later, but if you kill him he will have an uncommon ring Shapeshifter's Boon Ring.

~~~
Volo should first appear here talking to a bear named Bosk. He is not immediately or obviously a trader, but there is a button with coins on it that should appear in the buttom left for you to trade with him. He will sell you the uncommon ring The Whispering Promise.
~~~

There is a locked chest behind a pillar near Alfira, on the path away from the Beach.

You can unlock it for an uncommon helmet, The Cap of Curing.

~~~
If you go down toward the beach from the Druid's Grove, you'll encounter a sidequest as a boy is attacked by Harpies.
If you open the chest at the bottom of the first rock plateau, you can find an item randomly.
If you follow the rock outcroppings, you will find their nest which hides an Uncommon ring, Ring of Colour Spray.
~~~

Proceeding in the opposite direction of the Harpy Quest is a second smaller shoreline where a bear named Ormn is fishing. If you move the flat rock, you'll find a stealable uncommon amulet, Amulet of Silvanus underneath.
This one eluded me for awhile, so here's a picture of it.

Note that if you simply walk up to the rock and take it, Ormn will attack you.

If you proceed into the water area with a character with a non-negative STR mod, find a wooden chest with a bloodstone worth about 75 gp.

~~~
Between the Druid's Inner Sanctum, the Wilderness, and the platform behind Senna's jail cell is a room.
In Druid Grove, find the Two Tieflings contemplating ending a Goblin prisoner's life. Resolve the situation as you wish (Note that some Paladins can break their Oath by mishandling this situation...)
When the conversation is over, go to the back of the cage to find a platform. Jump to it. If you pass a perception check, the door will be openable.
Inside is a cavern with 4 Goblins harassing a gnome (who is at 1 HP, it is not necessary to save him).
Be careful entering this area: the statue on the right side will shoot fireballs at you when you reach the first mud patch. You can apparently shoot the statue until it breaks with bows but I've found that you can also appear to sneak past the statue. There are also blue glowing plaques near the statues labelled Aspect of the X, which can be interacted with to disable the turrets.
Resolve the goblins' presence. On the right side, near the bend that leads upward is a spot that you can jump from to an island (you will likely take fall damage: casting Feather Fall or using a Thief to reduce the damage can help if your health is low). The island has a corpse with a key for the chest at the top of the bend. Jump back across and climb the bend for your prize. The uncommon versatile melee quarterstaff weapon, Nature's Snare.
It is possible you might find your way to this passageway if you seek Nettie for help, but I haven't tested it.
If the Gnome, Findal dies, you can loot an uncommon helmet, Key of the Ancients off of him that gives +1 to Nature. If he survives, he will be an ally present in the Emerald Grove if you confront Kagha. (He basically just heals himself but his presence will draw some fire off of you.)
The circlet is missable if you save Findal's life. The +1 bonus to Nature checks isn't particularly worth pursuing in general. If he dies, take it for Gale, but I find him more useful as a meat shield for confronting the Shadow Druids.
~~~

As part of the interaction with some of your favorite neighborhood wannabe Thieves' Guild, you can get the 'Magic' Ring. This is most easily done with Astarion or a Rogue. It is in fact, not magic.

~~~
If you manage to persuade Kagha to not kill Arabella, her mother will give you the uncommon amulet, Komira's Locket.
If Arabella dies, the mother will not give you anything.
~~~

If you're open and honest with Nettie and your condition, it is possible for her to give you a vial of Wyvern Poison as a solution to the "becoming a Mind Flayer" problem you have. It's a nice one time use item this early, but its non-unique.

~~~
If Arabella dies, her mother will take revenge on Kagha by poisoning her.
If you go into the chest somewhat hidden off from the room Nettie isn't in (be mindful about stealth), you'll find a note telling you that Kagha is supposed to be meet someone by the Twisted Tree in the Bog (see below). This will require you enter the Sunlit Wetlands.
There is a fight with some Mephits and Moads where you can then pick up an uncommon shield, Wood Woad Shield.
You'll find a secret panel in the Twisted Tree revealing that Kagha is planning a coup with the Shadow Druids. The three rats will reveal themselves to be Shadow Druids. It is possible to convince her to make her see reason with Nature/Persuasion checks, but Loic and Marcoyl will follow Kagha's decision. If you saved Findal in the secret tunnel where Nature's Wrath was, he'll be an ally in this fight.
Anyway, Kagha drops this uncommon amulet, Broodmother's Revenge on her death.
I do not believe you can get Broodmother's Revenge without Kagha dying (but I'm not 100% certain).
~~~

If you investigate Kagha (see above), you'll discover the Shadow Druid plot. Regardless of whether or not Kagha lives, Zevlor will give you some uncommon gloves, Hellrider's Pride. They can also be purchased earlier from him.

If you do not get the gloves (dealing with Kagha, killing Zevlor, etc.) before the Tieflings evacuate they may be lost.

~~~
You can get access to a Hidden Vault by returning Halsin to the Grove, or by stealing the Wolf Rune from Rath.
After acquiring the Wolf Rune, you can place it back into the statue near where Nettie is. It will activate a secret stair case into a hidden vault. On the table is the rare two-handed martial melee glaive weapon, Sorrow.
In a box on the right, are the uncommon clothing, Robes of Summer.
In one of the three stacked crates on the left side, is a single vial of the rare potion Wyvern Toxin.
Owlbear Cave and Reforging the True Soul Spear
There is a skeleton in the back that can be looted safely after defeating the Mother Owlbear (it is not necessary to kill the cub to explore safely). It is wearing some uncommon Medium Armour, The Oak Father's Embrace.
In the shrine area, there is a magically sealed box. Jump over to the statue's little area. If your characters can spot the prayer sheet with a Perception check, you can have any character (except Shadowheart) open the box by reading the prayer sheet next to the box. Shadowheart will protest but can be convinced using a DC 10 Persuade. Inside are some goodies like the uncommon necklace, Moondrop Pendant.
There are two parts of this weapon that can be found inside the body of a dying dwarf (True Soul M...something)f, and the mother Owlbear. When you have the two pieces, you can reform them into an uncommon versatile melee weapon, Vision of the Absolute.
There aren't many creatures with multiple eyes wandering around freely (one in the Underdark and one if you open the bottle for the Zhentarim escort mission), but the chance to blind is useful.
Sunlit Wetlands / Putrid Bog
On the island where some Mud Mephits and are standing next to a tree across the river, (the one you go to for Kagha's Conspiracy quest), there is a chest facing away from the mainland behind the tree.
It can contain a pair of uncommon gloves, The Sparkle Hands.
~~~

Attacking and defeating Gandrel will reward you with the uncommon weapon Gandrel's Aspiration.

~~~
Something isn't quite right at Auntie Ethel's Teahouse. If you pursue the matter, Auntie Ethel will have to make a showing. If you didn't buy it from her in Druid Grove, she'll drop an uncommon two-handed melee quarterstaff weapon, Corellon's Grace if you did not buy it from her earlier and an uncommon necklace Tarnished Charm.
At the bottom of Ancient Abode, you will find the uncommon amulet, Ever-Seeing Eye on an altar, and the rare two-handed melee quarterstaff weapon Staff of Crones across from a set of potions.
There is also a wand on the altar (Bitter Divorce) that you can pick up for the sidequest to give to the woman, or keep yourself. I didn't keep the wand, but I believe it works as an infinite use spell scroll to summon a zombie body. (Paladins beware, using the wand - even to help Mayrina - can break your Oath!)
Blighted Village, Cellar, and Whispering Depths
A trio of Orcs stand guard inside a house in the Blighted Village. While the other two Orcs can be kited more cleverly from outside the houses' windows with bows and spells, Lurk himself will cast spells like Mirror Image, Firebolt on top of making use of his great strength. His uncommon helmet, Warped Headband of Intellect can be looted from.
If you instead befriend Lump, he will give you his War Horn, which you can use once to summon him into battle with his friends.
~~~

In a locked chest almost right next to the teleportation rune, there is a rare helmet, Haste Helm A+ item for speedy characters like Astarion.

A group of goblins and Wargs are hounding a gnome on a windmill in the back of the town. If you attack and kill Fezzerk, you can acquire an uncommon heavy martial weapon Very Heavy Greataxe.

If you open the hatch into the basement of the mill, you can find a heavy chest that contains the rare boots, The Speedy Lightfeet.

~~~
Looking inside the bottom of the Well can reveal a mini-dungeon (requires some skill checks to access) with Ettins, Shift Spiders and a beefy Spider queen with around ~130 HP. A lab near the dungeon's entrance has uncommon boots Spiderstep Boots which will protect you from being affected by Web and similar effects.
There is a skeleton in the central room right here
where you can pick up the uncommon ring the Spurred Band.
At the end of the dungeon is the Spider Queen who will summon small shift Spiders with 5 HP to swarm you. I recommend using arrows and throwables to kill off the three groups of eggs before engaging with her, then shooting out the webs beneath her to deal ~50 damage per fall. After defeating her, you will find uncommon clothing, Poisoner's Robes, the Dark Amethyst for the Necromancy of Thay, and a Rune to warp outside.
There is also a Dark Amethyst that you can use to open the Necromancy of Thay, if you want (see below).
Connecting the large double doors of the building across from the Blighted Village warp room is a secret workshop in someone's cellar that will also lead into the Whispering Depths. Inside is some Infernal Iron (bring it to Dammon in Druid Grove with Karlach in your party) and a partially made "Masterwork" weapon that sits as such: Uncommon Martial Melee Weapon, Steelforged Sword. If you open the chest on the ground floor, it'll explode! But inside is a journal with specific instructions on using Sussur Bark (an Underdark tree) to finish the weapon.
There are three options: Sussur Greatsword, Sussur Dagger, and Sussur Sickle. All three are formed by taking a common, ordinary variant of weapon and combining it with the forge. The result is a Rare +1 variant. The Sussur weapons have a chance to activate a Silence effect, making the target unable to cast spells and immune to thunder damage. You can use it as a Mage Killer.
~~~

In the cellar of the building of the glyph, there is a mini-dungeon where you can obtain The Necromancy of Thay, a book. The key to the gate can be found on a shelf leading back to the ladder outside. It can be found behind the stone slab protected by gargoyles (either disarm the plate the book is on or escape by turning on turn-based mode). You can destroy it by casting Guiding Bolt on it from Shadowheart, or from a scroll (even without opening the door). You may choose to read it instead for Forbidden Knowledge...

~~~
There is also a chest in the primary laboratory that has the rare gauntlets, Bracers of Defense.
Risen Road and Waukeen's Rest
There is a sidequest that involves examining the stories of an injured Tiefling and a party of four defending a house near by.
Regardless of whether you choose to side with the injured Tiefling or Anders, Sword of Tyr, you can get his uncommon martial two-handed melee greatsword weapon, Sword of Justice as either a reward or loot.
If you travel into the building's basement where you meet Anders, you'll find a secret cache of weapons, armor and the uncommon gloves, Gloves of Heroism.
~~~

There are some gnolls attacking two guys in a cave in the mountains of Risen Road. If you defeat the Gnoll Warlord, she'll drop a rare one-handed melee weapon, Shattered Flail.

On the corpse of a dead caravan agent near the melee with Flind, is the uncommon magic weapon Speedy Reply.

When you save the two guys, you can easily convince / kill them to see inside the chest. Inside is a canteen that's holding a trapped Spectator (Beholder variant 90 HP eyeball monster). The chest isn't worth opening for any immediate upside but whether or not you want to interfere with this Zhentarim scheme may be more obvious when we get to Act 2.

If you keep the bottle and don't open it, OR let just let them keep it.you may be able to convince the people in the Zhentarim lookout to sell things to you in their hideout near the Githyanki patrol. See the Zhenarim Hideout for more.

In a locked chest against the back wall of the cave the two guys were hiding in, you can find the uncommon gauntlets, Reason's Grasp

~~~
On the river level, away from the one-horned Tiefling, is a skeleton hidden in a bush. On it is an uncommon ring, Smuggler's Ring.
~~~

The Flaming Fist are tryng to put out a fire a pit westward of the gnolls. Save the Advisor to the Archduke and she'll give you the choice of three rare weapons:

The Sparky Points, The Joltshooter, and The Spellsparkler.

All three have a similar effect, the weapon type is just different.

~~~
Inside the burning building, on the second floor, *across* from where the Flaming Fist gather are a pair of rooms. The left room has a man trapped beneath some rubble, the right room has a locked chest. Inside that chest is an uncommon weapon, Hamarhraft. If you find it too difficult to pick the lock with the fire going on, you can always just pick up the entire chest and lockpick it open later.
(Missable) Zhentarim Hideout: Trader, Brem
If you saved the two guys in Risen Road from Flind and her Gnolls, and left them with their cargo alive, they will positively report back to their boss Zarys. Talk to her and she will give you permission to access Brem's magic items and the rare weapon, Harold.
You can also get access to Brem by saving them, but successfully persuading/intimidating them to let you have the cargo.
His inventory as a vendor seems to be pseudo-random, some items he always carries, while a couple others are random.
Uncommon Medium Armor, The Jolty Vest
Rare Heavy Crossbow, Giantbreaker
Rare Bow, Titanstring Bow
Uncommon Gloves Gloves of Thievery
Uncommon Weapon Rupturing Blade
~~~
The "office" area in the far back right contains a chest with the very rare Abyss Beckoners.

The left room has a chunk of Infernal Iron for buffing Karlach. Be warned that accessing either of these rooms means sneaking around the Zhentarim or fighting them.
(Missable) Mountain Pass
At the edge of the map leading out of the overworld area are some Githyanki Knights. If you fight them (and it's a rather tough fight, even at Level 5) you can defeat the Githyanki warriors for their weapons.

The Greatsword is a good weapon for Lae'Zel if you somehow missed The Everburn Blade on the Nautiloid, the Longsword and Crossbow don't have any unique properties beyond the +1 bonus.

If you manage to get Voss to drop it (Command: Drop spell, Disarming Attack you can acquire extremely early the
Legendary Weapon Silver Sword of the Astral Plane.

I have not managed this myself but I understand the timing is extremely tricky. If you cannot manage it, you can acquire it later at Act 3.
(Missable) Goblin Camp
Crusher the Goblin has a rare ring, Crusher's Ring. When equipped, it gives the wearer more base movement speed.

This is most useful on characters that like to fight in melee, as they need to be able to move into position in order to attack. A Thief Astarion benefits from this a lot: the extra movement is applied to every Dash action as well, allowing him to zoom around the place.

Wood Elves/Half-Elves are also big fans of this, getting a 13.5 m base speed (150% the normal value)!

Might be overkill on Wood Elf Monks but it's funny.

~~~
The Trader, Grat, seems to sell a couple of basic +1 weapons, but I also found him selling the following:
uncommon Martial Two-Handed Weapon, Doom Hammer.
uncommon boots, Boots of Aid and Comfort
uncommon gloves Gloves of Archery
uncommon weapon Returning Pike.
Boots Swiresy Shoes
If you get caught harming Goblin leaders, he will become hostile along with the rest of the camp.
~~~

Near the Teleportation Circle, "inside" the statue area for Selune is a ladder that leads upwards. On that first level, past the skeleton is an uncommon, Glowing Shield.
(Some Missable) Defiled Temple / Shattered Sanctum
If you fight and defeat Princess Gut, she will have a number of things in her belongings, including these two goodies. The uncommon shield, Absolute's Warboard and the uncommon amulet, Absolute's Talisman.

Priestess Gut is a tough fight on her throne, but you can talk to her privately in her chambers with a single character. Have your intended face for the interaction split off from the rest of your party by dragging them off of the chains in the bottom left corner. Your other characters can enter separately, hide in the corner and waiting for the fight to start. Once it does, taken your hidden characters and open fire. You can also place traps around her while she waits for you to talk to her (explosive barrels) and blow her up.

~~~
Defeating Dror Ragzlin, Warlord of the Absolute, will provide you with the uncommon versatile melee weapon, Faithbreaker. A nice melee upgrade for Shadowheart from her default mace or a generic shortsword.
~~~
If you freed Sazza in Druid Grove, and then talk to Minthara, you can have Minthara spare her to earn an uncommon weapon, Assassin's Touch.

Defeating Minthara, the Drow Cleric of the Absolute, will give you the uncommon one-handed melee weapon, Xyanyde, an uncommon Light Armour Spidersilk Armour, and uncommon boots, Boots of Striding.

There is also a gilded chest to the side of her "desk" that contains the rare boots The Watersparkers.

The boots are a good pick for Shadowheart, Eldritch Knights, Clerics, Rangers and Paladins who have Medium Armor Proficiency and Spellcasting.

~~~
Leaning up against a brazier near Torturer Spike,is an uncommon melee weapon, Jagged Spear.
One last note about the Goblin Leaders: if any of them manage to get a call for help off, the rest of the Shattered Sanctum, and the Goblin forces outside the keep will become hostile to you, seemingly permanently. If you want to keep the grunts none the wiser, you need to eliminate them before that happens, with no witnesses. Priestess Gut is the easiest, as she gladly walks into her chambers to talk to you alone. Making good use of the Stealth / Surprise round ambushes may help with this.
~~~

A Halfing trader inside the Goblin Fortress, Roah Moonglow offers a variety of magic items.
Rare Weapon Bloodied Greataxe

Rare Weapon Monster Slayer Glaive

Rare Weapon Gold Wyrmling Staff

Uncommon Necklace Amulet of Elemental Torment

Uncommon Weapon Bow of Awareness

[uUncommon Weapon[/u] Hunter's Dagger

IMPORTANT TO NOTE: Roah will leave as a merchant if you slay the Goblin Leaders, but she will reappear in Moonrise Towers later.

~~~
Outside the Worg Pens is a Skeleton lying in the remains of an open casket. It has a rare ring, Ring of Poison Resistance. Poison doesn't seem to be a particularly significant threat, but it's a magic ring!
In the pen with the two worgs (not Halsin/The Bear's) there are remains of people. If you examine one of the piles, you will find the uncommon melee weapon, Worgfang. A rather useful off-hand weapon for clearing out Shattered Sanctum.
In a chest near the Suspicious Meat where the two Goblins stand when you come in is a chest containing an amulet. It is considered stealing to take it while the Goblins are alive, and they will warn you not to touch it.
Uncommon Amulet Beastmaster's Chain
Beastmasker Zurk is wearing a set of rare boots, Linebreaker Boots
~~~

There is a small balcony behind the Warlord's throne that is filled with ornate chests. In them is an uncommon amulet, Amulet of Selune's Chosen, rare boots, Springstep Boots, and the uncommon gloves, Gloves of the Growling Underdog. Getting access to this amulet likely requires at least meeting the Warlord (I don't think you can walk up the stairs on either side without triggering his introduction event).

~~~
On the table to the right of Abdirak are an uncommon melee weapon, Ritual Dagger, and an uncommon melee weapon, Ritual Axe. He himself carries the uncommon weapon Loviathar's Scourge.
~~~

In a chest in the Defiled Temple area behind True Soul Gut's chamber, I found a chest holding the uncommon necklace Amulet of Misty Step.

Friendly NPCs Post Shattered Sanctum
For rescuing Volo from the Shattered Sanctum (and possibly allowing him to examine you?) you will receive uncommon light armour, Blazer of Benevolence.

~~~
If you defeat the three Leaders in the Shattered Sanctum, Zevlor will offer a goodie bag that contains the uncommon helmet, Wapira's Crown. If you deny the gold/reward, the item appears to be lost.
Underdark (In General)
A pair of Minotaurs are wandering the Underdark near the Fort, beyond the Rapport Spores. One of them is carrying an uncommon amulet, Amulet of the Unworthy.
~~~

Outside the Myconid Colony is an executed Deep Gnome corpse that holds a sword.

The uncommon martial melee weapon, Shortsword of First Blood.

~~~
Not too far away, there is an encounter with some Duergar. Their Leader, Gekh Coal, the Slave-Hunter can be fought for a two-handed Axe. When he and his minions are defeated, you can talk to Soverign Shaw for access to a Very Rare Cowl.
The uncommon two-handed martial melee weapon, Exterminator's Axe.
~~~
Hidden behind a spiderweb is a little alcove filled with Spider and Drow sorts of items. Inside a locked chest is some Rare Armour, Drow Studded Leather Armour.

~~~
Inside a cave are some Kuo'Toa worshipping BOOOAL. If you kill Pooldripp the Zealous, he will drop a rare melee weapon, Sickle of BOOOAL That being said, the cool part of the sickle is only worth it if you manage to get it without killing the Kuo-Toa.
Hidden behind some jumpable platforms laced with Torchstalk is a locked chest containing the uncommon armour Slippery Chain Shirt.
~~~

Near the Arcane Tower, there is a small alcove that you can jump to where a corpse is just hanging out. If you loot it, you'll find an uncommon helmet, Helmet of Autonomy.

It's kind of in a weird spot, so here's a picture of the map where I found it. If you climb the rockface downwards, you'll descend into The Festering Cove.

~~~
Outside the Selunite Outpost, a little farther down the path there is the rare martial versatile weapon, Phalar Aluve. It's set up visually as a "sword in the stone" situation.
More obviously outside the hidden wall is a locked chest that contains the uncommon helmet, Helmet of Smiting.
~~~

More properly into the Underdark there is an encounter with a Spectator who will unpetrify Drow in the area to throw as minions at you. One of them, Dhourn - Initiate of the Living Library is carrying an uncommon amulet, The Blast Pendant.

Killing the Spectator will drop the Very Rare The Spectator Eyes.

~~~
Defeating the Bullete that is running around will earn you the uncommon clothing, Bloodguzzler's Garb.
~~~

The very rare quarterstaff, Mourning Frost, can be built by collecting its pieces throughout the Underdark.

One of which can be found on Dhourn (the named Drow from the Specator fight), One of which can be found on the Drow corpse inside the Myconid Colony (either by sneaking in or being let in by Shaw), and the last of which can be found on Filro the Forgotten, wandering near some Hook Horrors near the Sussur Tree. The three pieces are all named Icy ___, but appear as Common items.

They can be forged together in your inventory by using the Combine prompt.
Underdark (Arcane Tower)
Outside the Arcane Tower, there is an unlockable chest behind the 1st Arcane Turret. Inside is a
uncommon one-handed melee weapon, Skybreaker.

~~~
There are a set of uncommon boots, Mystra's Grace, hidden inside a "Chest of the Mundane". Don't be fooled, it is not a spoon.
Inside the tower, in a small chest near the bottom across from one of the elevator entrances, is a rare amulet, Uncovered Mysteries.
On the fourth floor (one above the 'main gate') is a chest in front of a bed. Inside is the uncommon ring, Mage's Friend.
Defeating Bernard, Guardian of the Enchanted Tower will earn you the uncommon Ring, Guiding Light...
and the common martial two-handed melee weapon, Light of Creation.
On the highest floor, in the one corner is a "Stool of Hill Giant Strength". If you break it, a character will roll to see if they can tell that one of the stool legs can be used as the uncommon weapon, Club of Hill Giant Strength.
If you walk near the center ring, it'll reveal a secret invisible up button by equipping the Guiding Light. Take the elevator up, and you'll find another layer of the tower where you can get the uncommon two-handed melee weapon, Staff of Arcane Blessing.
There is also an uncommon ring, The Sparkswall in a nearby chest.
Don't forget to pick up a Tongue of Madness and Tinmask spores (both are mushrooms). You'll bring them to the friendly mindflayer in the Myconid Colony.
There also seems to be a hidden hole in the basement wall that can be perceived with a Perception check, but clicking on it doesn't seem to do anything for me. I think you need a Druid in the party.
(Some Missable) Underdark, Ebonlake Grotto
Living in a Myconoid colony is a Hobgoblin Trader named Blurg. He has several items available for purchase.
If you save her husband from the Bibberbang just outside of town, she'll reward you with a set of rare gloves: Gloves of Uninhibited Kushigo.
~~~

Near the other trader's house/tent, is a hidden "building" who entrance is a mess of vines you can part with a Perception check (although entering appears to be breaking and entering). Inside is a uncommon helmet, Shadow of Menzoberranzan.

If you clear out the Duergar at the Decrepit Village for Soverign Shaw, he will unlock the house and let you take the cowl. Inside is also a drow corpse with a copy of "Flumph Mating Rituals", secretly a book on how to prepare Adamantine at the Grymforge.

If you looted the Duergar antidote off of the slavers, you can give it to the Runaway deep gnome slave. She'll explain that she used these boots to escape. You don't actually need to ask her for them or pressure her, just let her explain her story. You can also heal her with the spell Lesser Restoration.

She will give you her rare boots, Boots of Speed.

~~~
After getting Tinmask Spores and Tongue of Madness mushrooms from the Arcane Tower, return them to Omeluum. He will try to draft a potion with them to remove the parasite. It'll fail, but he'll offer to trade you an alternative solution: the rare ring, Ring of Mind Shielding.
You can buy from him:
  • Rare Weapon, Creation's Echo
The two Roads to Moonrise Tower
Around this part of your game, you should be given two options to proceed:
Take the Mountain Path in the overworld, through cursed shadow lands to reach Moonrise Towers, or travel through the Underdark and come up from the Tower's Basement.
These are both parts of Act 1, but be careful about going too far: once you reach Moonrise Towers, you may not be able to return to anything in Act 1.
Boat Ride from Underdark to Gyrmforge
To get to Gyrmforge, ride the boat led behind by the slavers in the beach encounter in the Underdark.
You will be stopped by another boat of Duergar. If you fight them, you can loot them:
Corsair Graymon:
Uncommon Weapon Shining Staver-of-Skulls
Rare Weapon Bow of the Banshee
Rare Weapon Jorgoral's Greatsword
Watertight Chest:
Uncommon weapon Intrasigent Hammer
Interact with your rudder when you're ready to leave.
Gyrmforge (General)
Running up the stairs to right from the skiff, past the Deep Rotes, are some skeletons belonging to Dark Justicars. On one of their corpses you can find uncommon armour, Dark Justiciar Mail.
Up the stairs towards the last skeleton there is a chest hidden behind some barrels. Unlocking it will reveal a uncommon weapon Light Crossbow+1. Upon picking it up, your character will attempt a skill check automatically. Upon success, it's name will update to its true form, Sharran Crossbow. It does not appear to functionally any different.
Moving a bit deeper into the darkness of the forge by climbing and platforming, you can find a trapped, locked heavy chest. Inside is an uncommon shield, The Real Sparky Sparkswall.
~~~

Talk to Sergeant Thrinn in the "main" chamber, and offer her back the Boots of Speed (see Underdark). She will offer you a choice of one from two items:
Uncommon Armour Armour of Uninhibited Kushigo

You can kill her to reclaim the Boots.

~~~
On the other side of the docks, you can loop around and find two duergar throwing Deep gnome corpses into the water. Ask them if they're checking for loot and they'll sarcastically answer no. A difficult DC 20 sleight of hand check will let you pocket a rare ring, Fetish of Callarduran Smoothhands.
~~~

And lastly in this area, there's a pit area you can jump down into with three mimics and a 'Toy Chest'. One of the Mimics will drop the rare gloves, Wondrous Gloves. The Toy chest can be interacted with to spawn a secret booby trapped evidence that has a scroll with details between the Emerald Enclave and the Harpers.

Nere: Rare Boots, Disintegrating Night Walkers

Uncommon Weapon, Sword of Screams

Uncommon Weapon, Dagger+1

Sergaent Thrinn: Uncommon Ring, Ring of Absolute Force

Thudd: Rare Helmet, Cap of Wrath

~~~
On the other side, near where the Shar skeletons were, there's another cave-in you can open up with Bombs that leads to the upper rafters. One path on the right has a long classic looking video game fire death trap.
Reach the end for a Gilded Chest containing uncommon clothes The Protecty Sparkswall.
~~~

Alternatively, about halfway through an iron ladder drops down to a small platform with levers. Ignore it, and jump across to the platform in the center for a Longsword Mould

Follow the platform to a quick switch puzzle (shoot the right lever with a bow to raise the platform up to you, jump on, shoot the left lever to move across, and shoot the right lever again to arrive at the same level as your destination). When you get across you should find a Mace Mould and a Waypoint!

Go a little farther for an encounter with some Animated Armour to find the Scale Mail Mould.

I lost track of where I found it on this path, but there's an uncommon helmet, Dark Justicar Helm on a corpse somewhere between the bombed wall and the forge.

Just past the Scale Mail Mould is an uncommon Helmet, Dark Justicar Mask

On the other side of the big lava pit, if you go left of the forge, you can find the Splint Mould.

~~~
Alternatively, way back at the first jump you made and found the longsword mould, you can use the nearby lever system to ferry across to the Dormitory level.
you can follow this level back towards the Nere tunnel collapse area to find a skeleton corpse next to the lever system with another Dark Justicar Mail and the Scimitar Mould.
(There may be some other things on the Dormitory level that require passive checks to find, sch as a weapon rack near the Shield Mould, and a crime scene on the highest level, using the iron ladder to the left of the dorms. Alas, I failed those checks.)
The locked room has a fight with some hellboars and a Merregon. He'll drop a Merregon Halberd, and the chest has magic uncommon hand crossbow, Firestoker.
~~~

Going into the Fire Pit, you'll find a Fire Elemental wandering around, agitating the lava. It wanders around somewhat slowly and seems to be passive? i recommend having Shadowheart prepare Create/Destroy Water here in case somewhere you need to jump to catchs fire.

On one of the islands there is an Adamantine chest that contains rare amulet, Sentient Amulet.

On one of the lower platforms, there is an uncommon weapon, Merregon Halberd.

~~~
Deliver Nere's Head back to Soverign Spaw in the Mycoinid Colony to earn the rare amulet, Envoy's Amulet.
If you installed Glut as Sovereign of the Myconid Colony, you will get a different item: Champion's Chain.
Gymforge (Ancient Forge)
Using the forge requires one of the six moulds lying around the area, and a piece of Mithral Ore, of which there are two. Both obtainable Mithral Ore chunks are relaively close to the forge, with one being on the level near the Teleportation Circle and the other being on the side path at the same level of the forge, behind an encounter with Magma Mephits.
There are six things you can make, all are rare. Keep in mind you can only make two (2), and you can make two of the same thing:
Very Rare Armour Adamantine Scale Mail
Very Rare Armour Adamantine Splint Armour
Rare Shield Adamantine Shield
Rare Weapon Adamantine Longsword
Rare Weapon Adamantine Mace
Rare Weapon Adamantine Scimitar
When you're ready to make one, place your mould into the machine, your mithral into the crucible, drop the hammer to descend, split your party, positioning one at each circle and SAVE YOUR GAME.
You'll need to bring lava in to heat the metal but doing so will alert the guardian of the forge, Grym, a 300 HP boss who is naturally immune to all damage types. Grym will target whoever hit him last, so use your party members around the forge to control his movement, keeping one character at the lava valve at one at the lever to drop the hammer. Keep him "Superheated" with the lava and get him to stand underneath the center to deal big damage.
Alternatively, if you defeat him without the Forge's big hammer, you can earn an achievement.
When the fight is over, he'll drop the very rare helmet, Grymskull Helm.
~~~

At the Rosemayn Monastery, you will be locked out from the main door, but there are some broken windows on the left you can enter. Inside are a bunch of very drunk kobolds, one of which is holding a Cerermonial Mace. You can use it as a weapon, but it's really a puzzle piece for a much grander prize.

On the top of the roof are a pair of Giant Eagles guarding a Ceremonial Warhammer.

Back down a level is an altar room filled with four paintings of various "Dawnmasters". Ignore it for now. One of the broken windows leads to a room that is protected by a Guardian of Faith.

Defeat it by using acid, spells like Moonbeam, arrows and the like to pepper it from outside its circle until it dies. You will be rewarded with a Ceremonial Greataxe.

Head back into the room with the four paintings.

Head downstairs and right in front of the original locked entrance is a big statue of Lathander. Read the plaque if you haven't to get the "Find the Blood of Lathander" quest if you haven't already.
~~~
Squirreled off in the corner of the roof, is a Painted Chest that contains the Uncommon Helmet, Holy Lance Helm.
~~~

Proceeding onward will have a set of big crystal energy sources block your path. Destroy them with weapon attacks and disarm the DC 14 trap machines. When you reach the altar, place the Dawnmaker Crest into the Crest slot. FAILING TO DO SO WILL DESTROY THE ENTIRE MONASTERY

Important clarification about letting the vampire spawn live: if you have Oath of the Ancients, you will fall. If you have Oath of Devotion, killing them makes you fall. Not sure about Oath of Vengeance.

Minthara post release is a Paladin. The cache with Fleetfingers is discovered from a map that sits on the desk in front of Minthara in the Shattered Sanctum.

In the Cursed Battleground / Shadowlands area, I found Roland and the Gloomstrand Shield at -56, -116. The coordinates you have are still within the Last Light Inn area, but I ran into it closer to the most South West tip of the right half of the map, *just* South East of the Tollhouse.

"If you complete the conversation/quest about obtaining a Githyanki egg, you will no longer be able to trade with her (there is no way to press the Trade button, and she will not offer the option)."

I completed the conversation by giving her an Owlbear Egg and getting the cash reward. I was then able to talk to her again and buy any of the mentioned items.

For Gloves of Power you note uncertainty over what the flavour text indicates.
"Absolute's Bane" is conditional.
The relevant piece of code is (Act 1 spoiler):
IF(Tagged('BRANDED',context.Source)):ApplyStatus(BANE,100,2,,,,not SavingThrow(Ability.Charisma,11))"
This shows the special ability "Absolute's Bane" only applies is the character has been branded as a follower of the Absolute.

The Sleight of Hand boost applies regardless.

On the screenshot of Gandrel's Aspiration it notes 2 acid damage along with the other benefits. This is not part of the item, it's just that the person in the screenshot was wearing the ring that gives your weapons +2 acid on hit.
