Introduction
This is a comprehensive guide to the Binding of Isaac. This guide will not differentiate or specify between the vanilla game and Wrath of the Lamb add-on. This guide will contain information about everything in the game. Ranging from base knowledge to item descriptions.
If you see any kind of mistake, be it information, sentence structure or grammar, please leave a comment below. It'll be fixed as soon as I can!
This guide contains information based heavily on my own experience and knowledge, with some data taken from the Binding of Isaac wiki. See credits for more information on that.

Note
If an image doesn't load, it's Steam's fault for being Steam. I can't really help that, sorry!

Movement
You can use WASD to move in any of the standard four directions.

Environment - Introduction
The first thing in this guide, except for the controls, will be the common environmental "hazards". Items / objects that you will commonly find on each and every run. They're not that hard to understand.
Environment - Rocks
There are three types of rocks
There are normal (brown), tinted (blue) and unbreakable (black) rocks.
Here's a picture showing a tinted rock (left) with three normal ones, and four unbreakable ones on the right.

A rock functions as an obstacle, unless you have specific items, you yourself or your tears cannot go through or over them. They can be blown up with bombs, and tinted rocks will always yield some kind of treasure when doing so.

This particular tinted rock gave us a key and a bomb. Commonly, tinted rocks are known to give spirit hearts but other drops include keys, bombs and chests. Sometimes several of them combined.

It should be noted that rocks, except for unbreakable ones, in the Womb (and Utero) are always coloured red. This means you cannot tell the difference between standard and tinted ones.
Environment - Gaps
Gaps (and Rocks)
Gaps, or holes, are very much like rocks. They're an obstacle that cannot be traversed without special items. You can however, shoot across them.

Luckily, rocks and gaps have a useful synergy.
By placing a bomb correctly, as shown in the images below, you can fill up a gap with a rock.

You can fill up any gap as long as a rock is adjacent to it, by placing a bomb to the opposite side of the gap. (See the images above)
Environment - Poop
Poop, next to rocks, might just be the most common object you'll find in the game.
There are four versions, standard, corned, golden and red poop.

Standard poop
Standard poop is the most common - and by far the ♥♥♥♥♥♥♥♥♥, it's comparable to a rock - You can't walk or shoot through it, but you can destroy it with your projectiles. When doing so, there's a slight chance of a pickup dropping from it.

Corned poop
Corned poop is the second most common and acts the same as its standard counterpart. The only addition is that of an Eternal Fly circling it. This white fly is immune to any attacks but can still damage you. Upon destroying the corned poop, the fly will turn into an Attack Fly and will target you like any other enemy.

Red poop
Very rare, but still more common than golden poop. Red poop is only spawned by one particular boss - the Carrion Queen - and can only be found on one floor in the game, the Chest.
You take one hit of damage upon touching it, and it will regenate itself infinitely until the room is cleared. Red poop does not drop any pickups.

Golden poop
The rarest of 'em all. Golden poop is very durable, and upon destruction will spawn either a trinket (Counterfeit Penny) if it's unlocked, or a bunch of coins.
It should be noted that the game determines the spawning of a golden poop every time you enter a room, and that any corned poop (if left untouched) has a chance to turn into one. This means that, if you'd keep re-entering the same room where a corned poop is present, it might turn into gold after enough tries.

Environment - Fire
Fire is a lot like poop in this game. It's like a mixture of a standard and red poop. It can drop a pickup when extinguished, it damages on contact, and it doesn't allow you to move through it.
There are only two types of fire, standard (yellow) and red fire.
Red fire behaves the same as its yellow counterpart, except it'll often shoot a slow moving projectile at you, dealing one hit of damage.
Environment - Spikes
Technically there are two types of spikes. Or rather, two pairs of two spikes.
The first pair is the stationary spike:
Pictured left is the common spike, and pictured in the middle is the crooked spike. If you walk over them, you take 1 heart of damage. Non-flying enemies take massive damage from spikes.

The second pair is the moving spike.
Blue ones will quickly slide towards you if you're in their line of sight. Gray ones (pictured above, right) will continuously slide around the room until all other enemies in it, are defeated. Then it'll disappear. If you get hit, you take 1 hit of damage

Seems legit.

It should be noted that moving spikes are considered enemies.
Environment - Creep
There are 5 different colours of creep. They're all produced by (certain) enemies.
Every colour of creep flickers (see .gif)

Damaging creep
The most important ones are the ones that directly harm you, for one hit of damage. These are the red, yellow and green creep. Red creep is more dangerous than green, because it blends in with blood. Apart from that, all three function the same.

Flickering red, yellow and green creep.

Slowing creep
The other two versions of creep are black and white, and both slow anything on it - Much like cobwebs - down.
White creep is more common, being produced by a few certain enemies while black creep is only produced by one boss.

Environment - Leftovers
Here will be the leftover environmental objects that are too small to warrant their own chapter

Cobwebs
Cobwebs are fairly simply. They slow down anything above them, be it you, enemies or projectiles. They can be blown up with a bomb, but 100% of the time, this'll be a waste of either a bomb or your time.

TNT
TNT (or trinitrotoluene, if you want) barrels may randomly appear during your game. After taking enough damage, they'll explode, dealing the same amount of damage as a standard bomb would, but with a slightly bigger radius.

Troll Bombs
There are two versions of troll bombs, standard ones and super troll bombs.

Standard troll bombs cannot be pushed around with your tears (like normal bombs can), but you can knock them away by walking into them if you're fast enough. They'll explode a few seconds after being dropped.

Super troll bombs are larger and actively follow you around until they explode.
They both do a full heart of damage.

Grimace
A Grimace looks like a circular rock or stone with an expresionless face carved in.
There are two versions, gray ones that shoot normal, red tears and green ones that shoot Ipecac shots (Which are green, arcing bombs). The green bombs can be used effectively to kill enemies in the room, but do a full heart of damage against you. The Grimaces look identical except for the colour.

Locks
A stone with a padlock in it. By paying a key you can get rid of it, allowing you to move onwards. This will always be optional, you will never need to get rid of one to progress.

From left to right, in the top row we can see cobwebs, TNT and a Troll bomb.
In the bottom row there's a Super Troll bomb, a Grimace (gray) and a lock.
Pickups - Introduction / Summary
Pickups - or consumables - are commonly found items that you'll need on each floor.
There's heart, bomb, key, pills, cards and trinkets.
After clearing a room, there's a reasonable chance of a pickup - or a chest - to drop in the middle.
Chests can also be found normally in rooms, and they'll often contain a few pickups each time.
Pickups - Hearts
Heart Containers

The Life Bar. Three heart containers are present, of which two are filled. Spirit hearts would show up behind the last heart container and Eternal hearts between them.

There was intended to be room for only 20 hearts in total, this is not the case however, as you can continue to collect more. They'll simply be "off the screen", so you won't see them.
There's currently a maximum amount of 12 heart containers. After that, you can only increase your health by picking up Spirit hearts.

Types of hearts
There are three types of hearts in the game, as pictured.

From left to right, a normal red heart, a Spirit heart and an Eternal heart.

Red hearts can only be picked up to refill your empty heart containers. They can appear as either a full heart, or a half-heart. Spirit hearts only exist as full hearts, and act as armour. If you take damage, that doesn't actively prioritize red hearts, blue ones are the first to go. They cannot be refilled and don't count as heart containers.

Eternal hearts are the rarest ones. They exist only as half-hearts, and will function as a red heart in priority, but a blue heart as usefulness when picked up. If you happen to find a second half, they'll combine and form one new heart container.
If you take one half down with you to the next floor, it will also change into a heart container.
Pickups - Coins
Very common and used for a lot, there are 3 different version of coins.
Pennies, nickels and dimes. They're worth 1, 5 and 10 pennies, respectively.
Pennies are very common, nickels are rare and dimes even rarer

Coins can be either used in shops to purchase items or pickups, or to be gambled.

From left to right: A penny, a nickel and a dime.
Pickups - Keys
Probably the most important pickup besides hearts, keys are used to open locked Shops, Treasure Rooms or Libraries, or golden chests.

There are three versions of keys, one of which is not a pickup but rather an item that grants you 99 keys in total. That's the silver Skeleton Key.

The two other keys are the common, silver key. And the pretty rare golden (skeleton) key. The latter will grant you infinite keys for the duration of the floor.

A normal key and a golden (skeleton) key. A silver skeleton key is identical, except for the colour.
Pickups - Bombs
Another common item you'll find while you play, bombs can be used to deal significant damage to enemies and bosses alike. They can also be used to destroy environmental hazards such as rocks, cobwebs or fires. Or to blow up things such as slot machines and fortune tellers.

Commonly a bomb drops as a single bomb, but a "1+1 free" double-bomb drop is not rare.

A normal, single bomb pickup in the Womb, surrounded by some rocks.

Troll bombs and super troll bombs are explained above, as I consider them to be hazards.
Pickups - Cards
There are two types of cards, playing cards and tarot cards. Both can have either one of 5, or one of 21 different effect which is revealed on pick up.

Playing cards
Also known as Suit cards. There are 5 versions.

Pickups - Pills
There are 17 pill effects, 16 of which are in the game currently.
At the start of every run, only 6 effects are chosen to appear at all during the game.
These 6 will not change unless you pick up either the Luck Foot ot PHD, both of which will turn any negative pill into a positive. (Luck Down -> Luck Up)

Pill appearance
The colour and angle of a pill are completely random.
Upon picking up a pill, without owning the PHD, it will display "???". After using it, you'll learn its effect, which will then be displayed instead of the question marks, would you find it again.

This pill might be speed down now, and health up in the next run.

Pickups - Trinkets
Trinkets are holdable items that have slight effects, but can change your play drastically. Most of them give you a chance to get something, such as the Bloody, Burnt or Counterfeit Penny, which give you a 50% to receive either a heart, bomb or coin pickup whenever you collect a coin,

Pickups - Chests
Just like actual pickups, chest have a chance of spawning after clearing a room. They can, of course, be found randomly in any room as well.

There are three types of chests. Regular, Golden and Red chests.

Regular Chest
A regular chest can contain one to three of the following pickups: - hearts, bombs, keys and coins. It can also contain either a single card, trinket, pill or another type of chest completely.

Golden Chest
These chests require a key to be opened. They have the chance to spawn up to 5 pickups, excluding coins. (There can be more coins than 5, or more coins + up to 5 other pickups).
There's also a chance it can contain a specific item, like the ones found in Treasure Rooms.

Red Chests
A less common chest, this one is primarily found in Curse Rooms.
A red chest can either pay out with something objectively great - any of the Guppy Items (including Dead Cat) or an instant teleport to the Devil or Angel Room.
Furthermore, it can contain up to two spirit hearts, three (friendly) attack flies or two pills as good pay-outs.
Bad pay-outs include two enemy spiders or two (or one super) troll bombs.

Curses
Chance of a Curse
There are a few curses possible in the game. You will be notified of them as you start the floor.
The chance of a curse happening is 1,25%, normally.
This chance is replaced depending on a few things.
  • If you are on the Womb / Utero, the chance becomes 3,33%.
  • If you have the Everything is Terrible! Achievement, the chance becomes 10%.
  • If you have defeaten Isaac at least once, the chance becomes 20%.

Here's a list of possible curses:

Curse of Darkness
Will disable your map completely. Items such as the Treasure Map or Compass have no effect here.

Curse of the Labyrinth
This means you're on an XL Floor.
An XL floor is part I and II of that floor, combined. That means it'll also have two Boss Rooms and two Treasure Rooms, but also twice the amount of normal rooms.
An XL floor will continue to have only one Shop and Library / Curse Room / Self-Sacrfice Room / Challenge Room / Boss Challenge Room / Arcade, if appliable at all. (See "Rooms")

Curse of the Lost
The current floor becomes one "size" bigger.
Example: The Caves would become just as big as the Depths.

The Map

  • The room you are currently in, is white,
  • Without special items, you can only see rooms adjacent to rooms you've been in, on the map,
  • These, unexplored and uncleared rooms are dark grey,
  • The rooms you've cleared are light grey,
  • The Treasure Map item shows all the rooms - all the squares.
  • Because they'll be unexplored, the Treasure Map will show them as dark grey.
  • The Compass item will show all the Room Symbols on the map, but not the squares.
  • The Secret Room - which is also shown by the Treasure Map - will stay as a black square with a white question mark, regardless of having visited or even cleared it.
  • The Super Secret Room - which is not shown by the Treasure Map - will also stay as a black square with a white question mark.
Rooms - Arcade Room - Gambling!
In this part of the guide, the rooms will be explained. We'll start of with the Arcade Room as it allows me to nicely explain the gambling aspect, too.

Gambling, what for?
If you ever stumble upon a Slot Machine, Fortune Teller or a Beggar, you can invest your money to gain pickups - Bombs, keys, cards, trinkets or more money!
Stocked up on everything? Worry not. There's always one grand pay-out available at these facilities.

A little bit about the Arcade Room
An Arcade Room can only spawn if you start a floor with 5 pennies or more. Note that this doesn't mean it will spawn, only that there's a chance. On a map, an Arcade Room is shown with a die.

It costs one penny to enter an Arcade.

The following can be found in Arcade Rooms:
Slot Machines, Fortune Tellers, Shell Games and Blood Donation Machines.

Slot Machines and Fortune Tellers

Left, a Slot Machine and right, a Fortune Teller.

Only one of these is present in any given Arcade. If you walk into one of them, you'll lose one coin and play the machine. In case of the Slot Machine, there's a 55% to win nothing. The Fortune Teller has a 60% chance to give nothing, in the form of a fortune.

The Slot Machine can pay-out with either one, or multiple combined pickups; hearts, bombs, keys, coins and pills. The two grand pay-outs include a Pretty Fly - which is an orbital - and the Dollar (which is basically 99 coins). Upon receiving the Dollar, the machine will self-destruct.
The two "bad" pay-outs, aside from nothing, include a harmless, black fly and selfdestruction.

The Fortune Teller, our Slot Machine's more sought-after brother, can give you trinkets, cards, and blue hearts. But only one of each, each time.
The grand pay-out includes the Fortune Teller selfdestructing, and leaving a Crystal Ball in its wake. The bad pay-out is the aforementioned selfdestruction, without a Crystal Ball.

The Blood Donation Machine

A working one and a broken one. The broken one paid out with an IV-bag.

The Blood Donation Machine will always pay out - You touch it and it'll take half a heart away in exchange for one to three coins. It'll always prioritize your red hearts and will only start taking your blue ones if you only have one half of a red heart left.
The grand pay-out of a Blood Bank (Which is what it's also known as), is the Blood Bag. Which is a speed and health upgrade. The "bad" pay-out is an IV-Bag.
The IV-Bag functions like a portable Blood Bank, allowing you to trade in excess hearts for money, anywhere.

It should be noted that, if any Blood Bank gave you an IV-Bag, any other Donation Machine will give you a Blood Bag if it pays out.

The Shell Game

Winning the Shell Game.

Shell Games are a lot like Slot Machines. They can pay out with either multiple hearts, bombs, keys or coins. Upon giving the little feller a coin, he'll promptly shove any of the aforementioned items under a skull and switch them around. Don't be alarmed - It's impossible to follow it with your eyes. Instead, there's always a 33% chance to win.
The grand pay-out of the Shell Game is the Skatole item. Better known as Fly Love or the Poop Helmet. It will turn any flies, save for the ones that explode, friendly: They won't damage you anymore.

The following is not found in Arcade Rooms

Beggars and Devil Beggars

Beggars look like the shopkeeper, holding a sign with the symbol of a coin on it. By giving them coins, there's a small chance of them dropping any pickup. Giving them enough money will result in them paying out. Usually, this is a health-upgrade or an item commonly found in Shops or Libraries.

Devil Beggars look like little demons, holding a sign with the symbol of half a red heart. It'd be clear you're paying them with your life. Less prone to paying out, these little dudes can give you trinkets. Their main pay-out, though, include the chance of receiving a Devil or Angel item.

Other methods of gambling include the Self-Sacrifice Room and the Curse Room. Which will be explained in their own section.
Rooms - Self-Sacrifice & Curse Rooms - Gambling.. With your life!
The next two ways to gamble, are two complete rooms on their own.

The Self-Sacrifice Room
This room has a standard chance of spawning. In the middle, there's a damaging spike - even if you have the ability to fly. By moving over the spike, you will take one heart of damage, and there'll be a slight chance of a chest - be it normal, golden or red - spawning.
A chest will always spawn if your health is reduced to half a heart via sacrificial means here.

The pay-out isn't really worth it, in my opinion.

The Curse Room
It's not really gambling, but it's a gamble to enter.
The Curse Room inflicts one hit of damage upon entry (and exit) and contains either one or two red chests. It can also contain a free Devil or Angel Room item, although this is very, very rare.

Inside of a Curse Room. That chest contained two spiders. Not a good gamble.
Rooms - Treasure Room, Library & Shop
These three rooms, the Treasure Room, Library and Shop, will be your most sought-after places. They're the ones that reliably - with Shop as exception - give you an item.

Treasure Room
The Treasure room, except on either Basement I or Cellar I, is always locked and requires one key to open. Inside, you will find an item from the Treasure Pool (See: Item Pools). Any item you have found in a previous playthrough has a smaller chance of appearing.

The door to a Treasure Room. On the Basement / Cellar I, there's no keyhole.

Shop
In the Shop you may find a number of random pickups to buy. These can include any pickup. There will also, always, be either a passive or active item for sale.
The prices are as follows:
  • 3 pennies for a regular, red heart. 2 pennies if it's discounted.
  • 5 pennies for any other pickup, 3 if it's discounted.
  • 15 pennies for an active or passive item. 7 if it's discounted.

The first three floors in the game, (Basement I + II, Caves I, Cellar I + II and Catacombs I) will always have a proper shop. Any shop after those floors has a high chance to spawn a miniboss - Greed or Super Greed - instead.

Inside a shop. A red price means it's a discounted item.

Library
A library has a small chance of appearing, only 5%. 25% upon holding any of the active Book items. It'll always contains two (usually different) books, from the Library Pool.

The Shop and the Library both have the same door, it costs a key to enter.
Rooms - The Secret & Super Secret Room
These rooms are always present on any given map, you only need to find them!
In order to find them, you must first understand the map.

Secret Room
This room is located 'between' rooms, and always adjacent to three or two.. It is possible that one, or several of the bordering rooms adjacent to a Secret Room is a special one - like a Shop or Treasure Room.

Do you see how the Secret Room, the ?, is adjacent to three rooms?

Inside, you will usually either find 3, 6 or 9 coins, a slot machine, an item from the Secret Pool or Greed - a miniboss.

In order to enter a Secret, or Super Secret, Room, you must place a bomb near the wall and blow it open.

Super Secret Room
Also known as the Top Secret Room, this room is adjacent to only one, regular, room.
It can contain a multitude of things, each with their own visual style.
  • Cathedral-styled: The room will contain one half of an Eternal heart.
  • Cellar-styled: The room will contain one random trinket.
  • Library-styled: The room will contain one random Tarot card
  • Necropolis-styled: The room will contain a Fortune Teller.
  • Sheol-styled: The room will contain one or two red chests.
  • Womb-styled: The room will contain 7 to 8 red heart pickups.
Rooms - Challenge & Boss Challenge Rooms
Challenge Rooms and Boss Challenge Rooms are very similar. Both have a specific requirement to be fulfilled in order to enter. A Challenge Room will only open if you have full health, whereas a Boss Challenge Room will only allow entry if you're down to one, single heart.

Challenge Room
Inside a Challenge Room there'll commonly be either a regular or golden chest. Upon opening these, the door will lock and you will have to fight three waves of enemies.
The lower down you are in the game, the harder these waves are.

It's possible for a Challenge Room to contain an item, too. Although this is rare.

Inside a Challenge Room we find an item! If I pick it up, the fight begins.

Boss Challenge Room
The three major differences between these two rooms, aside from the door's appearance, are:
  • Boss Challenge Rooms will always contain an item.
  • Boss Challenge Rooms will have two (and sometimes three) waves of easier bosses (such as Larry Jr.)
  • Boss Challenge Rooms are a bit rarer than standard ones.

Left is an unlocked door to a Challenge Room, and right the locked one to a Boss version.
Rooms - The (I Am) Error Room
The I am Error Room is the rarest room in the game. The only way to get there is to teleport using telepills. No other means of teleportation can get you in, and the only way out is to teleport, too.

In the I am Error Room you can find one of the following:
  • A random, free, item and 8 random pills.
  • A random, free, item and 4 unlocked golden chests.
  • A random, free, item, 13 coins and 3 priced things. These things may be items or pickups.
  • Two random, free items.
  • Two priced items and 5 coins.

The room also contains a trapdoor to the next level. Considering you will usually be unable to leave this room without going down it, it's wise to always save telepills until you've done what you want on the current floor. That usually includes collecting the Boss Room / Treasure Room item and other room's loot, if appliable.

The I am Error Room.

Note: If encountered on the Womb II or Utero II, there will still only be a trapdoor to Sheol. This means you cannot access the Cathedral and by extension, the Chest.

Also, this room cannot be encountered in the Cathedral or Sheol, but can be found in the Chest. If you find this room in the Chest and you have no means to teleport out, you will be stuck.

Rooms - The Devil & Angel Room
The Devil Room, also referred to as the Deal with the Devil Room or one of the two Deity Rooms (the other one obviously being the Angel Room) is a very important and helpful room.

A Devil Room has a chance of spawning upon defeating the boss, without taking any red heart damage on the current floor. You don't need to clear the entire floor, just not take any damage to your red hearts in the Boss Room and any room before getting there.
Self-inflicted pain, such as Blood Donation Machines, does not count.

Inside the Devil Room you will find either a Deal with the Devil opportunity or a boss - Krampus.

Deals with the Devil are paid with either one or two red heart containers or three Spirit hearts. This means you lose them completely if you take the deal.

The Monster Manual for three Spirit hearts or the Razor Blade for one container? I'll pass.

If you happen to find Krampus in the Devil Room, the door will automatically lock and the fight starts. If you beat him, you will always gain the same reward. A passive item called Coal.

The Angel Room, also referred to as the God Room, Deal with the Angel Room and the other one of the two Deity Rooms, is a bit less common.

It has the exact same requirements - To not take any damage to your red hearts - plus you need to have turned down a Deal with the Devil opportunity before, in the same run.
Angel Rooms will always give you one, free item.

Inside the Angel Room we find the Dead Dove, a very good item.
Item Pools - Understanding
An Item Pool decides which item is to be found, where.
For instance, you can only find Treasure Room Pool items in Treasure Rooms.

Here's an image containing all the different item pools. Source[lparchive.org]. Click to expand.

Items will only show up once per run, with the exception of a select few, if and only if you've picked them up. This is why you should always pick up spacebar items, regardless of if you want them or not. This will prevent them from reappearing later.

What happens if an Item Pool is empty?
The best way to explain this is the Library. There are only a few books in there. If you happen to have picked them all up before, the Library will have no items in its pool. When that happens, it will draw items from the Treasure Room.

If every single item in the game is depleted (Which shouldn't happen without cheating/hacking or breaking the game) and the game needs to spawn one, it will crash.
Items - Passive & Active
The two biggest differences in items are active and passive items.

Active Items
These are items which you hold in the spacebar slot. You have to actively use them in order for them to do anything.

Every active item, also known as spacebar item, has its own recharge. This is shown with little squares next to the item. These squares are known as ticks.

Using the images above as example. The left one shows our spacebar item, the Dice, is fully charged and ready to be used. The middle image shows it just after usage, the ticks have been depleted and they need to be filled up before we can use the item again.
The third image shows that, after clearing one room, two ticks have recharged. So we'll need to finish three rooms to use the Dice once.

Items may recharge either one, two, three or six ticks per room. One exception is the Candle, which recharges based on time, and not the amount of rooms cleared.

Passive Items
These are fairly simple. These are items you pick up, and they have an automatic effect. Any item that increases any of your statistics, or modifies how or what you shoot is considered a passive item. Or in short, any item you don't use via spacebar, which is not a pickup, is a passive item.
Items - Introduction
Here's how I will explain certain items.

There'll be an image of the item taken from the Item Pool image earlier in this guide. This is what the item looks like, in game, too. Its name will be shown next to it.
Then the following information will be displayed:.
Line: The sole purpose of this line is to keep every box equal in size. You can disregard it completely.
Type: Either Passive or Active.
Recharge: The amount of rooms that need to be cleared to fully recharge this item again.
Pools: All of the pools this item can be found in.
Effect: What the item does upon activation (if active) or pickup (if passive).
Explanation: An explanation of its effect, in more technical terms. Numbers won't tell you things directly, but are there for comparison's sake. If one item increases damage with 1, and another with 2, you'll see which one is better.
Requirements: Some items are locked, here it'll tell you how to unlock these items.
Category: Categories the item belongs to. Here's a list of categories and what they mean.
  • Collectible - An item that gives you pickups.
  • Defense - An item that helps you defend yourself before or after being hit.
  • Familiar - A little character or object that follows you around. Characters will shoot with you, while others will simply follow you around with other effects.
  • Orbital - An object or entity that orbits the player and blocks incoming shots.
  • Weapon - An active item that is primarily used as a weapon. Usually related to bombs.
  • Modifiers - An item that changes how you or your shots behave. The following modifiers are possible:
    • Flight - Grants you the ability to fly. You can fly over creep, spikes, rocks, gaps. You cannot fly over enemies.
    • Freezing - Freezing shots are shots that have a small chance to freeze an enemy in place, for a limited amount of time.
    • Homing - Homing shots curve towards / home in on enemies.
    • Piercing - Piercing shots are shots that can pass through multiple enemies.
    • Slowing - Slowing shots are shots that have a small chance to slow down enemies for a limited amount of time.
    • Spectral - Spectral shots are shots that can pass through solid objects.
  • Upgrade - An item that directly upgrades any of your statistics. The following upgrades are possible:
    • Health up / down - Increases or decreases your heart containers, usually by one. If playing as ???, will increase your Spirit hearts by one.
    • Range up / down - Increases or decreases the distance your shots will travel before landing.
    • Speed up / down - Increases or decreases the speed of which you move.
    • Tears up / down - Increases or decreases the fire rate of your projectiles

Every pool's list will be in alphabetical order. If you want to find a specific type of item, you can use the control+F functions to search for it. (For instance, "Tears up" or "Damage".) Otherwise, you can search by name or picture. The word "The" will be disregarded. ("The Sad Onion will be "Sad Onion")

If an item is in multiple pools, it will be listed under each.

Enemies - Introduction / What are Champions?
During the course of your play, you'll encounter rooms filled to the brim with enemies.
Every enemy functions differently - although some are quite similar, too.

What is a Champion?
A champion enemy is an enemy with a different colour and size. They function the same as their regular counterpart, with minor differences.
These differences may include:
  • Dropping a (different) pickup (than usual),
  • Faster / slower movement,
  • Shooting different (such as homing) shots,
  • Spawning different enemies than they usually do,
  • Increased or decreased health, and
  • Increased damage.

This applies to normal enemies and bosses.

A normal Spitty and two different champion Spitties.

Important!
A few things to remember
  • Certain enemies fire a V-shot. A V-shot means it's two bullets that don't actually aim for you.

Two Fat Flies firing V-shots. V-shots are meant to hit moving targets.

  • A triple shot, will function like a normal shot - It'll hit you if you stand still. The main difference is, well, there are two other projectiles to keep in mind when strafing away from it.

  • A quadruple shot, which is rare, functions more like a V-shot. It's avoided completely when standing still, but if you need to move due to other obstacles, it will be harder to not get hit than it is for triple shots.

  • A Brimstone-like laser, or, Blood Laser, is a thick red laser that will go over obstacles and pierce enemies. For an example, see Double Vis or Super Gluttony.

  • An Ipecac shot, is an arced bomb.

Here will be a list of enemies. I've grouped them together if they are somehow connected. It'll list where they're commonly found and how they behave.
Bestiary - Common Enemies #1
Flies
Flies are by far the most common enemy in the game.

Attack Fly
These slowly move towards you with the intention to damage you on contact. They're often accompanied by other flies, but have low health and are more of an anoyance than a threat.
They're found in the Basement, Caves, Depths and Womb.

Black Fly
The Attack Fly's harmless brother. They hover around randomly and deal no damage whatsoever.
They're found on the same floors as an Attack Fly, where they can replace the aforementioned randomly.

Eternal Fly
This little, white fly is only seen circling either an enemy or corned poop. If their host is destroyed, they'll turn into a larger version of the Attack Fly, with more health.

Moter
A Moter is two Attack Flies stuck together. They behave the same, and split into two Attack Flies on death. They can be found in the Caves.

From left to right, an Attack Fly, Black Fly and Moter.

Pooter
Pooters do not deal contact damage. Instead, they fire a single shot at you while slowly moving towards you. They're found in the Basement.

Fat Fly
Somewhat similar to Pooters, these fat ones shoot V-shots while moving away from you. They're found in the Catacombs, but may replace a Pooter in the Basement or Cellar.

Left, a Pooter and right, a Fat Fly.

Boom Fly
Boom Flies fly around the room diagonally, bouncing off walls and other Boom Flies. They explode on death. They're found in the Caves, Catacacombs, Depths and Necropolis.

Red Boom Fly
Identical to Boom Flies, the only difference is that these ones explode in a 6 direction shot. They're found on the same floors as their bomblike counterparts.

Sucker
Suckers slowly fly towards you. Upon death, they explode into a +-pattern projectile shot.

Spit
Similar to Suckers, except that they explode into an Ipecac shot upon death. This shot is targeted either towards you, or another Split. Both Suckers and Spits can be found on every floor.

From left to right, a Boom Fly, Red Boom Fly, Sucker and Spit.

Spiders
Spiders all share the common trait that they move fast and will probably catch new players by surprise.

Spider
If we'd compare all spiders to flies, this would be the Attack Fly. It scuttles around the room, towards you. It can cover large distances in a single scuttle and is often accompanied by others. They're much faster than flies. They can be found in the Cellar, Catacombs, Necropolis, Utero and Chest.

B. Spider
Comparable to a Moter, this spider will split into two normal Spiders upon death. They move somewhat slower and cover less distance in their scuttles than normal Spiders. They can be found on the same floors where Spiders can be found - the Cellar, Catacombs, Necropolis and Utero.

Trite
This spider will jump from place to place, able to cover the entire length of a room in one hop.
They're comparable to Flaming Hoppers, or to a lesser extent, their normal counterpart, the Hopper. They can be found on the same floors where Spiders and B. Spiders can be found, except for Utero.

On left is a Spider and a B. Spider, on the right a Trite.

Turrets
There are two types of Turrets, both of which have the same three versions.
Stationary turrets will shrink when damaged. They can only fire when at full size, and, if left alone will slowly regrow to full size. They don't deal contact damage.
Walking turrets don't shrink and have a finite amount of health instead. They fire more frequently and aimlessly walk around the room.

Boil (Stationary)
The most common and, in my own opinion, most dangerous. That's because the shots they fire are random and scattered. They're found in the Caves, Catacombs, Depths and Necropolis.

Gut
Not to be confused with the enemy called Guts, these green versions of the boil shoot Ipecac shots, aimed at you, instead. They're found on the same floors as Boils.

Sack
The white Sack spawns Spiders instead of shooting and can be found in the Cellar, Caves, Catacombs, Depths and Necropolis.

Walking Versions
Walking versions are identical to their stationary counterparts, except for the differences explained before.

From left to right in the top row, Boils, a Gut and a Sack. And in the bottom row their walking counterparts.
Bestiary - Common Enemies #2
Deformed Humanoids
These, except for the Mulliboom, could be seen as the different stages of having an infestation of flies. It's not uncommon for any of these to suicide.

Mulligan
A harmless enemy that tries to flee from you. It can neither attack nor deal contact damage and spawns several Black Flies upon death. Mulligans can be found in the Basement, Cellar, Caves and Catacombs.

Mulligoon
Mulligoons will try to flee from you too, and don't deal contact damage either. They do have an attack though, which is to suicide and explode in a 6-projectile shot while dropping a Troll Bomb. If you kill them before they can kill themself, they'll explode in a +-patterned shot, instead.
They can be found in the Basement, Cellar, Caves and Catacombs.

Mulliboom
This version will run towards you immediatly, exploding on contact. They're considerably faster than their cowardly brothers. Luckily, their explosion only does half a heart of damage. if you kill them yourself, they'll, obviously, explode too. Like Boom Flies. They can be found on the same floors as their counterparts above.

Hive
Much like Mulligans and Mulligoons, these poor enemies will try to run away from you as you slaughter them. They do have an attack though - If you are in their line of sight, they'll cough up flies. (Usually Attack or Black Flies). On death, they spawn up to 5 flies, composed out of Attack / Black Flies, Pooters and Fat Flies, randomly. They can be found in the Caves, Catacombs, Depths and Necropolis.

Swarmer
Slowly hovering around the room, these abominations will randomly spawn regular Attack Flies. Whenever hit, they will spawn flies more frequently, and Pooters or Fat Flies instead of Attack Flies. Upon death, they live either a Boom or Red Boom Fly in their wake.
Swarmers are immune to contact damage, such as from the Virus, but also doesn't deal contact damage itself. They're commonly found in Necropolis, and to a much lesser extent in the Caves, Catacombs and Depths.

Mulligan; Mulligoon; Mulliboom; Hive and Swarmer, in that order.
Bestiary - Common Enemies #3
Humans (Or humanlike)
May include deformations.

Hopper
These enemies will randomly jump around the room. If you get close though, they'll jump towards you. They can be found in the Basement and Caves, and can only damage by contact.

F. Hopper
If a Hopper touches a fire, it will turn into an F(laming) Hopper. These versions are quicker, cover more distance with their jumps and have double their original health. They are never found 'naturally'.

Leaper
The Hopper's more dangerous brother, Leapers will act like Hoppers but occasionally jump up very high (off the screen) and land on your current position, firing shots in a +-pattern as they land. They're found in the Caves and Depths.

Gaper
Gapers will walk towards you in the most direct route available. They can only damage by contact and upon death may leave either a Pacer or Gusher in their wake. They're found in the Basement and Cellar.

Pacer
Pacers aimlessly walk around the room, they're found in the Basement and Cellar.

Gusher
A Gusher is similar to a Pacer, except they'll occasionally fire aimless shots from their neck.
They're found on the same floors.

Gurgle
They behave similar to Gapers, except that they're slower and shoot Ipecac projectiles at you. They're found in the Catacombs and Necropolis.

From left to right, a Hopper (and a green Champion), Flaming Hopper, Gaper, Pacer, Gusher and Gurgle.

Mr. Maw
These enemies will walk towards you and launch their heads at you. The heads will still be connected to their body via a long cord. On death, they may leave either a Maw or Pacer in their wake. They're found in the Depths and Necropolis

Maw
A flying, severed head that will float towards you and shoot projectiles. They're found in the Caves, Catacombs, Depths and Necropolis.

Red Maw
Floats towards you faster than regular Maws and does not shoot, instead, explodes in a +-pattern of projectiles upon death. Like normal Maws, they can be found in the Caves, Catacombs, Depths and Necropolis.

Psychic Maw
A very rare version of the Maw, they behave like regular ones, except that they shoot homing shots instead of regular ones, and have an Eternal Fly circling them.

Horf
Horfs are a lot like Maws, except they don't float towards you. Instead, they stay immobile while firing at you. They're found in the Basement.

From left to right, Mr. Maw, Maw, Red Maw, Psychic Maw, Horf, and a Mr. Maw showing its attack.
Bestiary - Common Enemies #4
Regenerating Creatures (Humanoids)
Creatures, apart from the stationary Turrets, that can regenerate indefinitely.

Globin
This enemy will walk towards you, much like Gapers. Will do a full heart of damage on contact, and, once defeaten will turn into a pile of goo. Unless this pile is destroyed, a weaker version will regenerate from it. They're found in the Caves, Catacombs, Depths, Necropolis, Womb, Utero and Sheol.

Glazin Globin
This enemy behaves like a Globin does. Compared, it has a higher movement speed, health and once turned into goo will slide away from you. They appear naturally in the Depths, but may replace a Globin on any floor.

Left, a Globin and right, a Glazin Globin.

(Semi-)Invincible Enemies
Enemies that are either invincible, or invincible under special occasions.
Does not include the Grimaces, Poky or Slide, which are explained under Environments - Leftovers and Environments - Spikes.

Knight
Knights are invincible up front and can only be damaged from behind. They generally move around the room randomly, but once they notice you - be it in their line of sight or just being close in general - they will move towards you with increased speed. They're found in the Caves, Depths, Womb and Sheol.

Selfless Knight
These enemies behave just like Knights, except they'll move towards you at a higher speed if detected. They're found naturally in Sheol, but may replace a standard Knight on any of their floors.

Left, a Knight and right, a Selfless Knight.

Mask + Heart
While a Heart may appear alone, a Mask is always accompanied by a Heart. A mask functions like a Selfless Knight, except it's completely invulnerable. The only way to kill a Mask is by destroying its coupled Heart. Mask + Heart pairs are found in Utero and Necropolis.

Heart
A heart, wether or not coupled with a Mask, will slowly try to move away from you and, when you get to close, shoot out projectiles in a +-pattern. They can be found in Utero and Necropolis when paired with a Mask, or alone in the Womb and Utero.

Left, a Mask + Heart pair and right, a single Heart.

Host
Hosts are immobile skulls that reveal a flesh body once they rise up to shoot you. They fire triple shots, and only rise up when not under fire. The skulls cannot be damaged. They're found in the Caves, Depths, Womb and Sheol.

Red Host
Not invincible or invulnerable in any way, the Red Host is much like its counterpart. No longer invulnerable with its skull down, it rises up much more frequently to fire shots of 5 tears at you.

From left to right, a standard Host, a standard Host revealing its flesh body, a Red Host and a Red Host revealing its flesh body.
Bestiary - Common Enemies #5
Blobs
Because really, what else could I call them?

Clotty
Clotties slowly crawl (if you can call it that) around in random directions, occasionally shooting projectiles in a +-pattern. They're findable on every floor.

Clot
Clots are a lot like Clotties, except they shoot projectiles in an X-pattern. They're findable on the Basement, Cellar, Caves, Depths, Catacombs, Utero and Cathedral.

I. Blob
A rare enemy, kind of a combination of the Clot and Clotty. It behaves the same except it shoots out a + and an X-pattern at the same time, and more frequently. It can replace either a Clot or Clotty on their floors.

From left to right, a Clotty, Clot and I. Blob.

Maggots
Because even though one is actually called Maggot, that's what they all are.

Maggot
This enemy moves around the room aimlessly, only being able to damage via contact. It's findable in the Caves and Catacombs.

Charger
They behave just like Maggots until you get in their line of sight, in which case they'll charge towards you with increased speed. They're findable in the Caves, Catacombs and Depths.

Spitty
Spitties will, also, aimlessly move around the room. Sometimes they'll randomly shoot a projectile forwards. They're findable on the same floors as a Charger.

From left to right, a Maggot, Charger and a Spitty.

Bodyparts
Because that's what they are.

Brain
Slowly slides around the room, leaving a trail of red creep. This enemy's present in the Depths and Womb.

Guts
Rolls around the room, connected to a rock, spikes or a wall. If the object they're attached to is broken, they'll quickly attach themself to another one and continue rolling. Just like Brains, they're present in the Depths and Womb.

MemBrain
MemBrain slowly moves around the room, occasionally hopping up to fire 8 large projectiles in a circle around itself. Upon death, will leave two Brains in its wake. Findable in the Depths, Womb, Utero and Chest.

Mama Guts
Functions the same as a Membrain, except it leaves two Guts in its place upon death. Finable in Necropolis and Utero.

From left to right in the upper row, a Brain and Guts. And in the bottom row MemBrain and Mama Guts.
Bestiary - Common Enemies #6
B. Lickers
Because I had no idea where else to put them.

The B. Licker will move around the room randomly, aimlessly and quickly. Usually found in lines of 5 or 3 parts. Killing one part of the line will make them all seperate. They may bump into poop or TNT-barrels, destroying them. They're found on the Catacombs, Utero and Necropolis.

Burrowing Enemies
Enemies that are encountered only on the Womb or Utero and use its fleshy floor to burrow under.

Para-Bite
These Diglets will move towards you and often burrow underground to move away from you again. Can only damage on contact.

Fred
These creatures, of whom I can only assume they were named after a friend of the developer or something, can fire an Ipecac-shot at you before jumping up to burrow down, to reappear somewhere else. When burrowing down, they release projectiles in an X-pattern.

Lump
I've always called them Cysts. Anyway, it fires a triple shot at you if they see you. Will run away after shooting them, via burrowing.

From left to right, a Para-Bite, Fred and two Lumps.

Leeches
They don't suck blood, don't worry.

Leech
Behaves like a Charger, except it can fly. Findable in the Depths, Womb and Utero.

Kamikaze Leech
Also known as Bomb Leech, it behaves like a standard Leech except it explodes on death. It's findable in the Depths, Womb, Utero and Sheol.

Holy Leech
Also known as the Holy Bomb Leech, it behaves exactly like a Kamikaze Leech, except it has an Eternal Fly circling it. Findable on the same floors as the Kamikaze version, plus the Cathedral.

From left to right, a standard Leech, Bomb Leech and a Holy Leech.

Vis
Or would the plural be Visses? Visii? Visaea?

Vis
This enemy will charge up, and fire, a Brimstone-like laser once you're in their line of sight. Apart from that, they walk around slowly and randomly in the room. They're findable in the Depths and Womb.

Double Vis
This enemy functions like a normal Vis except it also shoots a laser out the back. They're findable in the Depths, Necropolis and Womb.

Chubber
Chubbers fire off a Little Chubby-like familar, or a Charger that can fly. Apart from that they function the same as a normal Vis and are findable in the Catacombs, Depths, Womb and Necropolis.

From left to right, a Vis, Double Vis and three Chubbers.

Mirror Movers
You'll understand why.

Dople
This enemy mirrors your movement completely. It only fires when you do, and has the same range and fire rate as you do, too. They don't deal damage on contact, though. Dople is findable in the Womb and Utero.

Evil Twin
While it's supposed to be an upgraded version of Dople, it's actually even easier to kill. It mirrors your movements and shoots when you do, but, it will always shoot a triple projectile that moves much slower than yours. Note that it can fly, though. You can find your Evil Twin most commonly in Sheol, although they might appear in the Womb and Utero, replacing a Dople, too.

A Dople on the left and an Evil Twin on the right.
Bestiary - Common Enemies #7
Eyes. Stationary turrets.

Eye
The immobile Eye will function like a stationary turret but is much more dangerous than others I've called Turrets, such as the Boil, Gut or Sack. An eye will track you and fire a Technology-like laser at you in quick succession. They're only findable in the Womb and Utero.

Bloodshot Eye
if you're wondering why it's called Bloodshot, that's because the Brimstone-like laser it fires, as opposed to a Technology-like laser, is also known as Blood laser. Anyway, this version of the Eye tracks somewhat faster and fires faster too. Paired with the thicker laser it fires, it's quite dangerous. It can be found naturally in Sheol but may replace a normal Eye in the Womb or Utero.

I spy with my little eye an Eye on the left, and a Bloodshot Eye on the right.

Baby
You. When you find Lemon Mishap in a Treasure Room again.
Kidding! <3

Baby
These flying enemies fire a single shot at you whilst slowly hovering towards you . Occasionally, they may teleport away from you. They're findable in the Depths, Necropolis, Womb and Utero.

Angel(ic) Baby
Angel Babies are much more agressive than their non-winged brothers, blessed with much more health, they shoot triple shots while hovering in on you. They'll teleport more frequently too, but to get closer to you instead of running away. They're findable on the same floors as the standard Baby, plus the Cathedral.

Baby on the left, Angel Baby on the right.

Thiefs
If they hit you, you lose ánd drop some coins.

Keeper
Also known as Greed Heads, these nasty enemies will hop around the room while firing V-shots at you. They'll usually try to hop into you to deal damage, too. They're seen in the Catacombs and Necropolis.

Hanger
Hangers float around the room, slowly towards you as they fire triple shots. The Hanger is immune to touch-damage and doesn't inflict it itself either. They always have an Eternal Fly circling them, and they're hangin' out in the Catacombs, Necropolis and Cathedral.

A Keeper on the left and on the right, a Hanger.

Disclaimer
You might find certain bosses as normal enemy in the early floors too. These include Larry Jrs, Blastocyst and Fistula.
Bestiary - Minibosses
The Seven Deadly Sins act as minibosses. Their health is tied to the floor you find them on (earlier floors mean less health).
Minibosses can be found on every floor.

Envy
In my opinion, the most annoying of all seven regular minibosses. Envy will split into two segments when destroyed. Then those segments will split into two again, and finally, even those will split into two. The smaller a segment is, the faster it will move around the room, diagonally. They'll bounce off walls and each other, making it hard to keep track off where they'll be if many segments are on the screen at once. Their only way of damaging is via contact.
Commonly, it will drop bombs or the item Shoop Da Whoop!.

The different segments you can find Envy in. Left is his starting form. It will split into two faces (second picture) which will then split into two smaller faces, (third picture). Finally, those faces will split into the smallest ones, visible on the fourth picture.

Gluttony
Gluttony has two attacks, one is firing a Vis-like, Brimstone-like laser if you're infront of him, and the other is an 8-way projectile shot around him.
Commonly, it will drop (red) heart pickups, but it there's a chance of the <3 item dropping.

Gluttony.

Greed
By far the most common and hardest miniboss, Greed will often show up in Shops and Secret Rooms. Greed can fire a difficult-to-dodge triple shot, if he hits you, you'll lose coins similar to a Keeper or Hanger. He can also spawn two Hoppers.
Upon death, he'll often drop a number of coins or the Steam Sale item.

Greed.

Lust
Lust is quite fast, and will relentlessly pursue you in the quest for hugs. If he touches you, you take damage. Lust never slows down in speed.
After defeat, Lust will either drop a pill or the Virus item.

Lust.

Pride
Pride might be considered the easiest of the minibosses, but his ability to spawn 4 Troll Bombs randomly in the room should be watched out for. His only other attack is to shoot medium range, diagonal beams from the center of his body.
Commonly, it will drop (troll) bombs, (tarot) cards or the Anarchist Cookbook item when defeated.

Pride.

Sloth
Sloth can fire out an Ipecac shot at you or spawn a Charger. When defeated, it will drop a tarot card or Bob's Rotten Head.

Sloth is just a recoloured Globin. Lazy, huh?

Wrath
Wrath will try to either damage you by contact, or spawn a normal, active, bomb which he'll kick towards you. Upon death he'll drop bombs or the Mr. Boom item.

Bomberman Wrath.
Bestiary - Super Minibosses
Much rarer than their normal counterpart, but functionally the same. The health that Super miniboses have is tied to their floor, and they can appear on every floor.

Super Envy
Functionally the same as Envy, except it starts off with one extra of the largest segment. It's also slightly bigger and coloured blue. There's a reasonable chance of it dropping the Forever Alone item when defeated, apart from the common Envy drops.

The smallest segment of a Super Envy.

Super Gluttony
Where Gluttony functions as a Vis with its Brimstone-like laser, Super Gluttony functions like a double-vis, firing its laser in both directions at once. Gluttony's 8-way shot is accompanied by a random scattering of shots now. There's a chance to receive the Bucket of Lard after killing him.

Super Gluttony firing its laser like a Double Vis.

Super Greed
Just like Greed, Super Greed can appear in the Shop or Secret Rooms quite often, and is therefor the most common Super miniboss. Instead of a triple shot, Super Greed fires a difficult-to-dodge quadruple shot, which also steal coins on hit. He can also spawn Keepers, as opposed to Hoppers.
On death, there's a reasonably chance he'll drop A Quarter, apart from the common Greed drops.

Prepare your wallet.

Super Lust
Super Lust is even faster than his (her?) regular counterpart. He'll also leave a trail of damaging creep behind him now, making him very dangerous for anyone who scoffed at speed upgrades.

Hugs?

Super Pride
Pride's spawning of a few troll bombs is replaced by spawning 2 Super troll bombs instead. Its diagonal beam is now also accompanied by a random scattering of shots in any direction.
Apart from the common Pride drops, Super Pride can give you the Dead Cat item.

This picture was taken right after he killed me with his super troll bombs, jerk.

Super Sloth
Super Sloth looks to be the least upgraded miniboss. Firing two Ipecac projectiles instead of one, and spawning two spiders instead of one charger now. Instead of a chance of dropping Bob's Rotten Head, he can now drop Bob's Curse.

He doesn't look that different, but if he was looking the other way, you would be able to see he has an eye. Similar to a Glazin Globin.

Super Wrath
Functions the same as normal Wrath, except he'll spawn super troll bombs now. Can drop Mr. Mega.

Hey, who turned out the lights?

Ultra Pride
Styled after the two creators of the Binding of Isaac, Ultra Pride is the hardest miniboss in the game. Hell, I find him to be more annoying and harder than a lot of actual bosses!
Ultra Pride is two entities, the main Sloth-like creature (Edmund McMillen) and the Baby familiar (Florian Himsl).

Edmund can fire off three, big homing projectiles and fire off a line of green creep if you're next to him. Do not stand left or right from him because this attack has quite some range to it and will hit you.
Florian will teleport around the room, much like a Baby or Angelic Baby, and often fire a single homing shot.

Upon death, you can claim Edmund's Left Hand (trinket) for yourself.

Left we see Edmund McMillen, and right we see Florian Himsl.
Bestiary - Bosses #1
Information
At the end of every floor you have to fight one, or two if on an XL floor, boss. Upon defeat, this boss will drop either an item from the Boss Pool or a specialized item. For instance, any of the four Horsemen of the Apocalypse will always drop a Cube of Meat, which not in any item pool.

Here'll be a list of every boss in alphabetical order, except for final bosses, which have their own segment.

Blastocyst
Fightable in: the Womb / Utero
Relatable bosses: Gish, Monstro, Monstro II
  • Blastocyst, in his largest state, hops around, sometimes scattering bullets around the room. It can also perform a large jump, shooting out 8 projectiles around itself when it lands.
Upon taking enough damage, it will split in two, medium versions.
  • These medium states, can shoot projectiles in +-pattern randomly, or in X-pattern after landing from a jump. After taking enough damage, they'll split into the smallest Blastocyst state.
  • In their smallest state, they can only shoot projectiles in a +-pattern. After taking enough damage, the bubble of goo surrounding the Embryo, which is inside, will pop, leaving it defenseless.
  • As Embryo, it can only hop around, hoping to hit you with contact damage.

The different stages a Blastocyst can be found in.

Blighted Ovum
Fightable in: the Cellar / Catacombs
Relatable bosses: Gemini, Steven

  • The larger one of the two siblings will continuously pursuit you at high speed, wanting to deal contact damage.
  • While moving, he'll leave a trail of damaging creep behind him.
  • The smaller sibling might occasionally fire a Brimstone-like laser at you.
  • Upon death, both siblings will explode in a radial burst of bullets.

Left, the larger sibling in the Blighted Ovum pair, and right, the smaller sibling.

Bloat
Fightable in: the Womb / Utero / Necropolis
Relatable bosses: Peep

There is no shame in dying against the Bloat.
  • The Bloat can fire two Brimstone-like lasers either left, right, or downwards. However, he'll do this without any kind of warning or charge up animation, so try not to be in any of those directions from him. Ever.
  • He can also splatter damaging, red creep around him. Watch out, this creep could easily fill 1/4th of the entire room at once.
  • He can fire a radial burst of bullets.
  • He can launch up into the air and drop down somewhere else, usually at where you're located. Once dropped down, or while dropping down, he might release more creep or a radial burst of bullets immediatly.
  • There are two other entities in the room, his detached eyes. They'll block your shots and bounce around the room diagonally. They deal contact damage and cannot be destroyed.

Champion version(s):
  • Green: Launches 3 Ipecac projectiles randomly instead of bullets when landing. Also spawns 2 Chargers when producing creep.

Standard Bloat, firing its Brimstone-like lasers.

C.H.A.D.
Fightable in: the Caves
Relatable bosses: Chub, Carrion Queen
Unlock: Complete the Caves 20 times
Standard item drop: Little C.H.A.D.

C.H.A.D. behaves exactly like Chub does, except he shrinks in size and increases in speed the less health he has.
  • C.H.A.D. will charge towards you if you're in his line of sight.
  • He can spawn two Suckers during the fight.

Just like Chub and the Carrion Queen, placing a bomb in his way as he charges makes him swallow it, dealing significant damage.

Two images of C.H.A.D., notice how he's much smaller in the right image, where is health is nearly depleted?

Carrion Queen
Fightable in: Catacombs / Necropolis
Relatable bosses: C.H.A.D., Chub

The Carrion Queen behaves, for the first half of the fight, exactly like C.H.A.D. or Chub.
Just like the aforementioned, placing a bomb in the way of her charge makes her swallow it, dealing significant damage. Apart from that, she can only be damaged on her last segment. Hit 'r on the butt!
  • During the first phase of the fight, she will charge towards you if you're in her line of sight.
  • She may spawn red poop randomly during the fight.
  • At 50% health, she'll start bouncing around the room diagonally, like a Boom Fly, at high speeds. She'll bounce off walls and red poop. Shooting her tail during this phase makes her change directions immediatly, too.

Champion version(s):
  • Pink: Smaller in size and will not start moving diagonally at 50% health. Spawns tiny versions of the Heart enemy, instead of red poop. Damaging these Hearts will also damage her.

Left, a standard Carrion Queen and right, the pink Champion version.

Chub
Fightable in: the Caves
Relatable bosses: C.H.A.D., Carrion Queen

Chub is the boss of who C.H.A.D. and the Carrion Queen are based off
You can drop a bomb in his mouth as he's charging to deal significant damage.
  • He'll charge towards you if you're in his line of sight.
  • Chub can spawn Chargers during the fight.

Champion version(s):
  • Blue: Larger in size, more resistant to knockback and slower. Spawns Boils instead of Chargers.
  • Orange: Slightly smaller, faster and unpredictable in his movements. Spawns Spitties instead of Chargers.

Standard Chub.

Relatable bosses: Famine, Pestilence, War, Death, Headless Horseman
Unlock: Destroy 30 tinted rocks in total + beat Mom once
Standard item drop: White Pony

Conquest is not one of the Four Horsemen in this game, and as such will not drop a Cube of Meat as result. Out of all the Horseman-characters though, he is the hardest.
  • Conquest may shoot four homing shots during the fight.
  • He may leave the screen to summon beams of light on random spots in the room, which deal damage. They're similar to the ones from Crack the Sky. After doing this, he'll charge back onto the screen from either left or right.
  • He may charge off the screen only to charge from right to left with multiple copies of himself

Conquest.

Relatable bosses: Triachnid

Daddy Long Legs hoves in the middle of the room, attached to the ceiling. He has four different attacks.
  • He may stomp his four, damagable, feet around the room, randomly.
  • He may retract his head into the air and stomp one of his feet down at a time, at your location.
  • He may cough up two Spiders.
  • And he may slam his head down to stomp you, usually he'll do this after stomping all his feet down one at a time. Once his head lands, it'll fire a circle of bullets around him.

Daddy Long Legs is also one of the largest, non final, bosses.

Death
Fightable in: the Womb / Utero
Relatable bosses: Famine, Pestilence, War, Headless Horseman, Conquest
Unlock requirement: Defeat Mom once
Standard item drop: Cube of Meat

One the four Horseman of the Apocalypse. He has two phases.
Phase 1, he will ride on his horse.
  • Death may summon a scythe at the center of each wall in the room (so, four scythes) to travel across the room.
  • He may also summon a pair of scythes to follow you. Scythes damage on contact.
  • Death can use the active item Hourglass to slow you and your tears down.
Upon damaging him for 50% of his health, he'll dismount his horse which will constantly sweep across the screen.
He himself, will occasionally summon pairs of Knights.

Champion version(s):
  • Black: Increased health, will always spawn four scythes instead of two, and spawns Kamikaze Leeches instead of using the Hourglass. Once dismounted, will spawn pairs of Red Maws instead of Knights.

Death.

Duke of Flies
Fightable in: the Basement / Cellar
Relatable bosses: Husk

The Duke of Flies himself will harmlessly and slowly float around the room, coughing up flies that will circle him, for defense. Once he has enough flies for defense, he'll cough up Attack Flies to attack you. Sometimes, when there are too many flies around him, he'll cough to release them all. This means they'll stop circling him and start targeting you. Upon death, it will leave a small selection of flies, accompanied by Pooters, in its wake.

Champion version(s):
  • Orange: Will spawn Suckers instead of Attack Flies.
  • Green: Spawns Moters instead of Attack Flies and fires a circle of shots out around him when he coughs.

Left is a regular Duke of Flies while on the right, the orange Champion version is coughing up some flies.

Relatable bosses: Krampus

The Fallen can be found on any floor randomly, except for the Cathedral, Chest and Sheol.
He has two phases, single and double.
In his first phase, he'll attack either
  • By shooting out a triple and quadruple shot at you in quick succession.
  • By chasing you until enough damage has been taken.
  • Firing out Brimstone-like shots in the four cardinal directions. He may do this randomly, but will always do it after chasing you.
Once his health drops to 50%, he will split in 2. This is his second phase.
  • Either of the two Fallen can perform any of the three attacks randomly.

The Fallen in his first and second phase.

Famine
Fightable in: the Basement / Cellar
Relatable bosses: Pestilence, War, Death, Conquest, Headless Horseman
Unlock: Beat Mom once
Standard item drop: Cube of Meat

Famine has two stages:
Stage 1:
At the start of the fight, Famine will charge eiher to the left or right, going through the wall and appearing on the other side until he's back in his original location.
From there, he may spawn either a Pooter or Fat Fly, up to two total.
If you get in his line of sight, he will use his charge attack again, which can be used to kill his own flies.

Stage 2: At 33% of his health, his body and horse will be destroyed, and he will no longer spawn flies or use his charge attack. Instead, he'll continuously fire triple shots at you.

Champion version(s)
  • Blue: In phase one, he'll often release a burst of shots around him, and he'll charge much longer and more frequently. In his second phase, instead of shooting triple shots, he'll scatter a hail of bullets in your direction.

The different versions, phases and shot technique used by Famine.

Fistula
Fightable in: the Caves
Relatable bosses: Teratoma

Fistula will appear as a single, large piece, floating around the room slowly.
After taking some damage, the large Fistula piece will split into four medium parts, which move faster. Those will eventually all split into two smaller pieces which are even faster, and upon destruction of those, they'll spawn a single Charger.

The four different stages of Fistula, from left to right.

Gemini
Fightable in: the Basement
Relatable bosses: Blighted Ovum, Steven

The larger of the two will pursue you, sometimes speeding up his movements only to eventually catch his breath again, while the smaller one will occasionally shoot a single shot at you.

Killing the larger twin first will enrage the smaller one, which will then stop shooting and pursue you instead, as it flies.
Killing the smaller twin first will not change the larger one's behaviour.

Champion version(s):
  • Blue: Decreased speed, increased health.
  • Green: The two twins are detached, normally they are attached. The rest of the fight is the same.

A blue Champion Gemini on the left, and an enraged smaller twin of a regular Gemini pair on the right.

Watch out, the sliding attack is unpredictable, fast and not particularly choreographed.

Champion version(s)
  • Blue: Her size is decreased a great bit, and an additional Gurdy Jr., rarely even a third, will spawn in the room. They will only use their sliding attack.
  • Red: Increased size and health, does not shoot shots.

  • His scatter shot has been upgraded to a Blood Beam, which he will fire left or right depending on your alignment.
  • His launch has been upgraded, he'll jump twice in quick succession. The first time he lands, he'll release 8 projectiles in a circle around him, and the second time, he'll spawn four Suckers. These effects may alternate in order.

The exploit to defeating Monstro II with great ease is to coax him into using his laser attack. Simply stand to his left, he'll grin and fire a laser. As he does this, move over to his other side as you shoot him. He'll then fire his laser again. You can keep doing this until he's dead.

Red Mom: Will stomp much faster and much more frequently. Her Eye will now shoot a triple shot, and she'll no longer spawn enemies.
Blue Mom: The enemies she may spawn is changed to Trites, Keepers, B. Spiders and B. Lickers.

After defeating Mom once, the game will end. if you beat Mom again, and any time after, a trap door leading to the Womb / Utero will be provided.

The three different versions of Mom (Click to expand).

Notes:
  • Mom can be instantly defeated by using the Bible's active effect in the room.
  • After defeating her for the second time, and any after that, the room will be locked. Unless you can teleport out, no door will be provided to exit. If you do teleport out, however, a door will reappear again.
  • Mom will always drop the Polaroid trinket, if unlocked.
  • Using the II High Priestess tarot card on Mom will not work.
  • You can use the Blue Candle to attack closed doors in the Mom fight, which will damage her.
Bestiary - Final Bosses - Mom's Heart
Mom's Heart
Mom's Heart is the final boss of the Womb / Utero, and will be found on the second floor of those segments until you've defeaten it 9 times.

At the start of the battle, itt will be accompanied by two Eyes.
Mom's Heart follows a very easy pattern. The Heart will hang in the middle of the room, where it's damagable, and spawn two to four different, standard enemies and then retract back to the ceiling, where it's invulnerable.
Once you've disposed of the enemies, it'll come back to spawn more, and so forth.

Once dropped down to 33% of its health, it will no longer retract back to the ceiling and instead continously fire ipecac shots at you while spawning waves of enemies.

Mom's Heart.

Notes
  • Using the Bible's active effect in the room will instantly kill Mom's Heart.
  • You can use the Blue Candle to damage it while it's retracted to the ceiling.
Bestiary - Final Bosses - It Lives
It Lives!
It Lives will replace Mom's Heart after it has been defeated once. It behaves like similar to Mom's Heart, and as such is also accompanies by two Eyes at the start, except it has more health and spawns more difficult enemies.

The enemies it will spawn include:
  • 2 Eyes*,
  • 2 Freds,
  • 4 Knights**,
  • 4 Suckers**, or
  • 4 Leeches**

After it has taken enough damage, it will start to spawn bosses, in order:
  • Monstro,
  • Larry Jr. (both entities),
  • The Duke of Flies, and
  • Chub

*It will only spawn a pair of Eyes if you damage it too slowly.
**Any of these enemies may be repaced by similar counterparts, such as Kamikaze and Holy Leeches instead of regular ones.

At 33% of its health, it will start to fire ipecac bombs that leave damaging creep in their wake.

After defeating It Lives for the first time, access to Sheol and the Cathedral is guaranteed for future runs, although you may already enter Sheol upon entering a Devil Room in the Womb II or Utero II, where a trap door will be located, instead of a Deal with the Devil.

It Lives with both Eyes killed (See: Mom's Heart for Eye placement).
Bestiary - Final Bosses - Satan
Satan is the final boss of Sheol, he has two phases and the fight has three stages.

Stage 1: The Fallen
  • The Fallen, accompanied by two Kamikaze Leeches, will spawn infront of the Devil statue in the room. This part of the fight is 100% similar to any boss fight against the Fallen.
Stage 2: Satan
  • After killing the Fallen, the statue will transform into Satan who will begin to attack you. He has a few attacks:
    • If you're to his left or right, he will release a patterned wave of shots, first 5, then 4, at you.
    • If you're in front of him, he will fire a thick Bloodlaser from either his mouth or his hands.
  • After depleting his health to 0%, the third stage will begin.
Stage 3: Satan's Hoof[/list]
  • Satan will continuously stomp down on your location with both his hooves in a pattern. First, he will stomp once, and shortly thereafter, again.
  • While this is happening, Kamikaze Leeches will spawn around the room.
Once you deplete his health bar to 0% again, you'll have defeaten Satan.

The first images shows Satan firing his Bloodlasers in the second stage of the fight. The other wo images show, from left to right, the Fallen (the first stage) just after spawning, and Satan stomping down a hoof after already having stomped down one (Click to expand).
Bestiary - Final Bosses - Isaac
Isaac
The fight against Isaac, in the Cathedral, has three stages.

Stage 1: Depression
  • Isaac will frequently shoot a radial burst of 12 shots around him, that move at a higher pace than standard shots.
  • He will fire scattershots of tears back depending on your fire rate. If you fire rate is high, he will fire back at you much more than if it was low.
Stage 2: Hope (66% of health)
  • Isaac's 12-way shot is replaced by a 4-way shot that home.
  • He will still retaliate with tears, as he did before.
Stage 3: Acceptance (33%)
  • Isaac will stop retaliating with tears when you fire at him, although his 4-way homing shot has been replaced with a 6-way homing shot.
  • He will sometimes summon beams of light, similar to Crack the Sky's, around the room.
  • He will spawn Angel Babies, up to six at once.

The first two images showcase the first two stages of the fight. The second two images showcase the third stage, one in which he releases his 6-way homing shot, and the other with the beams and an Angel Baby. (Click to expand)
Bestiary - Final Bosses - ???
???
The fight against ???, which takes place in the Chest, is the final boss fight of the game.
Sadly, it's very, very similar to the fight against Isaac.

In fact, it's complete similar except ??? may retaliate with homing shots during his first two stages, and will spawn attack flies in all three.

The three stages of the fight against ??? are similar to Isaac's. Note the differences: ??? will spawn red attack flies during the fight and will shoot out homing shots in his first stage, too.

Cheat Engine
Very useful to make your own challenge runs or to just goof around after completing the game.

You will need
  • Cheat Engine, which you can download here[www.cheatengine.org], and
  • A cheat table for the Binding of Isaac, which you can download here[forum.cheatengine.org]
Note: This cheat table will only work if you do, in fact, have the Wrath of the Lamb expansion

  • Open up your game and start a game as your character of choice (Image 1)
  • Double click the cheat table, after having downloaded/installed Cheat Engine. It should look like Image 2
  • Click on the little computer icon in the top left, and select Isaac.exe in the Process List (Image 3)
  • Keep the current adress/code list
  • Now you can start activating cheats that will immediatly effect your game. It should be pretty straightforward what they do

Cheats
If you want a specific item in-game, select **Add Item When Entering Next Room and find your item in the list below. (Image 4). Upon entering the next room, you will immediatly be given the item as if you had found it (Image 5). You can give yourself multiple copies of the same item.

Make sure to deselect **Add Item When Entering Next Room to avoid receiving the item on every room.

If you want a specific trinket or card, give yourself a Deck of Cards and check any of the **Deck of Card Always Gives XX (NAME), where XX is replaced with a code, and NAME shows the name of the card or trinket.

Straightforward ones include:
  • Infinite HP
  • Infinite Item Power (Your spacebar item will never deplete)
  • Obtained Items Reappear (Items are not removed from their pools)
  • Max Coins / Bombs / Keys: 99 of them
  • Infinite Coins / Bombs / Keys: 99 of them, they won't deplete
  • 2x Movement Speed
  • Max Damage: You can set your damage to either 10, 50, 100 or 250
  • Max Fire Rate
  • Win at Slot Machine / Shell Game / Blood Bank: You will always win at these
  • The Polaroid is not needed to enter the Chest: This cheat may be bugged, making you skip the Cathedral completely, to immediatly go to the Chest

  • The 198 items were taken from the image in the Item Pools section, which came from here[lparchive.org].

  • Some of the information, notably in the Items, Enemies, Bosses and Secret section, came from the Wiki[bindingofisaac.wikia.com], while I often used it to double-check, too.
