This is a guide to help you get all the 20 Achievements in the game.
It contains spoilers of course.
You'll have to do at least 2 playthroughs to get them all.

Potions, their ingredients and where to find them (3 achievements)
I am a Werewolf...
Learn how to use the werewolf form


After you left the Catacombs and entered the Kitchen area you'll get a list of ingredients of Filch:
  • Apple Seeds (you'll get this from Filch after freeing him)
  • Animal Fur (can be found in a barrel in the Dungeons)
  • Gray Mold (can be found on the table in the Kitchen)
  • Celery (can be found on the table in the Kitchen)
Drop these ingredients in the cauldron, fill an empty vial and get your first potion.
Now you're able to control your Werewolf form and step into the moonlight outside the kitchen.

Alchemist
Prepare the “Night Watchman”


List of ingredients:
  • Tree Bark (can be found outside the Kitchen on the trees to your left)
  • Horse Chestnuts (can be found in the Left Wing, downstairs)
  • Thistle Thorns (can be found in the West Wing, after jumping the large gap from the East Wing continue to the West Wing, turn right inside and left)
  • Holly (can be found inside the cabinet in the Kitchen)

Senior Alchemist
Prepare the “Wolfsbane”


List of ingredients:
  • Feathers (can be found in the West Wing, after you have opened the gate by turning the wheels in the correct sequence, you can find them on the ground before the beacon)
  • Wolf Blood (can be found in the secret Alchemy room in the Library, turn left when you enter it, you'll find a bucket in the corner, use a vial to collect the blood)
  • Moon Flower (can be found on the table in the Kitchen)
  • Aconitum (you'll get this from Isabelle)
  • Morning Glory (can be found in the Chapel)
Beacon Locations (1 achievement)
Destroyer
Complete the stone beacons task


Towards the end of the game you are told by Filch to destroy all the stone beacons throughout the castle. (This is also mentioned in the letters, which can be found in the second Guest Room)
There's a total of 20 beacons. 10 inside the castle, 10 outside. They can be destroyed after you have learned to master your Werewolf form, just by attacking them.

Inside the castle:
  • 1: Outside the Kitchen, next to the well.
  • 2: Courtyard, to the left of the frontgate.
  • 3 & 4: Chapel, two beacons. Go upstairs and find the first one, jump the gap and find your second one.
  • 5: East Wing, upstairs.
  • 6: West Wing, on the balcony outside.
  • 7: Outside, after exiting the Brewery, jump the gaps.
  • 8: Dungeons, room to your right.
  • 9 & 10: Catacombs, at the start where you first met the Werewolf, and behind a locked gate.

Outside the castle:
  • 11 & 12: When you cross the bridge, there are two beacons in front of you, right from Laszlo's body.
  • 13: Turn left after crossing the bridge, all the way in the left corner of the walls.
  • 14, 15 & 16: At Laszlo's body go up the hill to the wall, turn right, keep moving straight with the wall to your left.
  • 17, 18 & 19: At the spot where you found the axe, keep moving straight, with the wall to your left.
  • 20: Close to the spot where you started the game.
Card memory game (1 achievement)
Excellent Memory
Solve the card puzzle


After you have entered the Library and picked up the book and the card you can play this puzzle. Interact with the card with the board on the wall (between the first and second bookcase on your left). The puzzle is exactly the same everytime.

Lock-picking Artist
Unlock all chests


There are 8 treasure chests to be found in- and outside the castle:
  • When you start outside there's a little cabin near you. Inside it is your first chest.
  • When you enter the Courtyard, go left and at the end there's your second chest.
  • When you enter the Dungeons and you have your axe, open the door to your right and there's your third chest.
  • When you enter the Catacombs, follow the hallways and find your fourth chest.
  • When you enter the Kitchen, you'll find your fifth chest under a window.
  • When you enter the Library, turn left and find your sixth chest.
  • When you enter the Chapel, use your Werewolf strenth to crash the gate to your right, go up the stairs, jump the gap and wait till you transform to a human again. There's your seventh chest.
  • When you have opened the sixth chest, open the door (enter the East Wing), go downstairs and find your last and eighth chest.

Coin Collector
Collect 230 Gold


This achievement can only be unlocked if you pickpocket every guard and open every treasure chest there is to be found. Don't pay the guard at the frontgate to get outside, cause you'll never get to 230 then, you can of course after you have collected it all.

Explorer
Collect an optional item


At the start of the game you can walk around the thrashed carriage to a bridge. Cross the bridge and go uphill, to your right you'll have some treestumps with an axe. Pick it up.

Realization
Realize your new identity


When you have entered the castle and woke up in a Guest Room and had your conversation with Isabelle, walk to the cabinet with the mirror above it next to your bed. Look at your reflection.

Investigator
Discover the reason for your invitation


After you have left your Guest Room, turn left to the second Guest Room and enter it.
There's a large closet in the corner to your left, open it. Pick up the empy vial and the letters.

Barrier Breaker
Unlock a normal sized wooden barrier without transformation


Only possible if you have collected the axe outside before entering the castle. When you have collected the Rusty Key and entered the Dungeons, go to the room on your right. Open the door with your axe.

Survivor
Survive the werewolf attack


After you have entered the Dungeon, freed Filch, crawled into the cell and entered the Catacombs, the Werewolf will chase you. Run and jump some gaps and close the gate so the Werewolf can't follow you.

Escaper
Run and hide from the werewolf


When you have entered the Brewery go to your left and use the lever to open the gate. Immediately start running, cause the Werewolf appears behind you, and jump the gap and run straight ahead to a hidingspot behind a basket, duck and hide.

Good Reader
Find and read a book about your curse


When you have entered the Library and let Filch in, he asks you to find a book. The book is located to the left, in the back of the Library, on a shelf (with your back to the entrance). Pick up the book (and the card) and read the book. Be sure to read it all, cause the achievement actually takes some time to unlock.

Master Thief
Rob all pockets


There are four guards in the Courtyard. Pickpocket all four of them for some gold and the Rusty Key (CTRL to sneak). There are also three guards upstairs and outside in the East Wing. Before turning into a Werewolf to jump the gap, be sure to pickpocket the first one before the gap.

Wild Beast
Kill more than 10 humans


Kill more than 10 humans is quite easy, as soon as you turn for the first time you can kill the maid and a couple of guards in the Courtyard. You can keep on killing the guards in the Courtyard (they will respawn everytime you re-enter the Courtyard), just keep on killing until the achievement pops up.

Two endings (4 achievements)
Achiever
Complete the game without falling or being killed


Exactly what the description says, do not die PERIOD! If you by any chance turn up dead, quit the game and load your game again.

Human
Complete the game with less than 12 transformations


If you time everything right, destroying the beacons when you are already transformed, this one would be quite easy.
  • 1st: When you have learned to control your Werewolf form, crash the beacon next to the well and destroy the barricade inside the kitchen to the left.
  • 2nd: In the Courtyard to destroy the beacon.
  • 3rd: In the Chapel, to crash the gate and the two beacons on your way to the door to the East Wing.
  • 4th: In the East Wing, take the second moonlight spot, run upstairs, crash the beacon and run back down to the balcony part and jump the gap.
  • 5th: In the West Wing, open the gate so you can go outside, transform, crash the beacon and run to the door on your right, crash it as well and keep following the hallway to the barricade and crash it as well.
  • 6th: After you get the Cog in the Brewery and raised the gate, transform, crash the other gate, run outside, keep following the path, crash the beacon along the way and wait till you can get into the Library.
  • 7th: One time for the outside beacons (if you think you will run out of time, run back to a spot in the moonlight on one of the bridges)
  • 8th: When you enter the Dungeons again, follow the hallway to the moonlight spot, transform and run back to the entrance, to the room on the right, to destroy the beacon.
  • 9th: Enter the Catacombs through the Well outside the Kitchen. Run all the way to the beginning, where you first met the Werewolf. transform, go to the door and turn right to crash the barricade and beacon, run back into the Catacombs (you can wait a while in the moonlight to get a full moon) and follow the hallway till you can crash the gate and the beacon inside.
  • 10th: At the ending

Path of Darkness
Unlock the bad ending


Pretty easy to get, you don't even have to be extremely evil. Killing humans helps, pickpocketing as much as you can helps.

Path of Freedom
Unlock the good ending


Ok, this one took me a while to figure out! This is what I did:
  • Kill NO humans, you can run past guards in your Werewolf form as long as you don't get too close.
  • Pickpocket only the absolute necessary. The guard on the leftside of the frontgate has the Rusty Key, pickpocket two of the other guards for the remaining 15 gold (you should have collected 185 gold from all the treasure chests), cause you need to be able to get outside to destroy the beacons, which will cost you 200 gold.
  • Be sure to read every book and every letter or list. And really READ it, read the book till you hear the chime of unlocking the book-achievement (cause it won't pop-up again if you already got it).
  • I don't know if this is required as well, but I also chose every top response in conversations, cause to me it seemed like the bad ending response was at the bottom and the good ending response was at the top. Could be related, or not at all, but it's what I did.
  • Don't transform more than you absolutely have to (see Human Achievement for details).

yes, yes and yes^^ and i dont kill any human because i got the "Human" Archiv too.^^ i think this game hates me and thats it hihi xD

Oh that one. There's a lever for it somewhere in the catacombs. At least I think you're talking about that one.
