WARNING
Be advised that this guide might contain spoilers for the game, I'd recommend to read only if you've already beaten the game at least once, I'll try to keep spoiler talk to a minimum. However stuff like party members, locations and bosses WILL be mentioned. I also recommend to tackle this achievement in a New Game Plus run anyways, however it's completely possible to do on a first run, it's just more of a hassle.
ALSO I'M WARNING YOU I TEND TO GO INTO A LOT OF DETAIL WHENEVER I GET INSPIRED. As such, this guide will likely be VERY long. But I'm trying to give as much information as possible, as I haven't found many resources online that give a good dive into HRF.
The current version of the guide is 1.1. Last update made on August 7th, 2020.

Table of Contents:
  • Introduction
  • Quick FAQ: Some general questions regarding the achievement
  • The Setup: Tips to take into account before tackling HRF
  • Line Categories: Split into 5 sections due to Steam limitations, I give great detail onto the known lines that can be obtained, with a total table in the end
  • My Experience: My tale on exactly what I did to get the achievement, I've left some very detailed tips on it so you can replicate them if you wish
  • Possible Improvements: What went wrong on my approach and how you can make it better on your run
  • General tips: Some spare tips for specific groups of lines, you might wanna check this one for info on easy spawns for Ailments and some other lines with specific situations.
  • Useful resources: Some external links of resources you might find useful.
Introduction

I've said this to several people and commented in various places too, so I'd like to start with it.
When talking about HRF, there's two types of people:
1) Players that got it randomly, probably unaware of what it was.
2) The rest of us mortals that had to actively grind for it.

So, what exactly makes this such a headache for a lot of people? As of the writing of this guide, only 1.5% of players have acquired this achievement. It's the 4th "rarest" achievement the playerbase has acquired, which is alright considering the other 2 that aren't the 100% completion one are secret achievements, nevertheless it shows that it's not your everyday thing and people that got it either got lucky, or did the grind like I did.

The description in question is a bit ambiguous, it reads "Hear 250 of Rise's navigation lines", so, what exactly is a Rise navigation line?

In a nutshell, they're the text pop ups that are accompanied by Rise's character icon that usually appears in the top left side of your screen while in battle. Notice the usually as there's a few exceptions to this rule, but overall, you're looking for Rise's icon narrating the text on the bubble. These lines are triggered by a bunch of different factors, mainly starting a battle or Rise commenting on something currently happening, such as a party member suffering from a condition, an action made by them against the enemies, or your party inflicting stuff on enemies.

Your objective is to come across 250 different lines, upon hearing the 250th, the achievement should unlock. Pictured bellow it’s the line that got it for me. There's a funny story behind it which I'll cover later on :^)

I'll be providing an explanation on the types of lines there are to encounter, my personal journey to tackle and acquire the achievement, which you may follow, and some other resources to help you overall. I'll include my list of lines in the end as well.
Quick FAQ
  • Q: Do the lines I have seen carry over in NG+?
A: I believe they do not. Most of the achievements in this game must be completed in a single run, unless whatever the achievement is related to is a carry over to NG+, such as money, the compendium, etc. I can say that when I got the required amount that I had been tracking down, I got the achievement, it was on my 3rd run on the game using the same base file, (2nd start of NG+) so it’s unlikely they carry over. Despite this, some people have claimed they do. But to be safe, assume you’ll start at 0 lines from a fresh game.

  • Q: Is it possible to get this on a first run?
A: Yes, it is! It's very likely someone that battles in a lot of different scenarios can acquire it, the problem is if you're nearing the end of the game and you don't have it yet, it's kind of hard to know what lines should you try to acquire, you'll likely have no idea how many lines you've seen.
This is why I would mainly recommend to do this on a NG+ scenario where you can fully put effort into tracking, personally I think your first playthrough should be something where you take your time, explore and do what you see fit. If you're going for this achievement it can be a bit of a chore since you'll be hunting down specific scenarios, and if you do that it'll likely hinder the way you play. Of course, it's your choice in the end.

  • Q: Does a battle need to be completed for acquired lines to be saved?
A: No, you can safely run away if you're no longer interested in fighting a specific encounter, in a way, running from battle is also a normal end, it’s the same when the enemies escape.

  • Q: Do the lines Rise says while exploring the dungeons outside of battle, such as "*character* has low hp" and such count?
A: No, they don't, they must be inside a battle, even though sometimes you'll hear her voice on these prompts and they're similar to the others, they don't count.

  • Q: Do the quotes she says while in the analyze enemy screen count?
A: You don't see text for these, so they do not.

The Setup
Now then, remember I'm relating my personal experience, something I honestly don't see much elsewhere, people mainly post on which are the lines to hunt down or the scenarios to obtain them, I took notes and documented my process to hopefully make your experience easier if you chose to follow my approach, you can deviate form it of course.

First up, the requirements to hunt for this would be as follows:
  • NG+ recommended, so you can just focus on this without worrying on how the story will unfold. It's possible to do on a first run but I'd honestly recommend you just enjoy the game on your own if you're playing for the first time, if you're lucky, you might get it on your first run.
  • To make everything easier, set the money gain to great. You may also play around with the experience, it's better to go on the lows, just try to never be really overleveled initially. I personally had normal exp gain but avoided most fights. Since it was NG+ I had a pretty beefy Persona to breeze through bosses.
This is also why I'd say do this in NG+, while you choose how to play in the end, I think a better experience would be to beat the game for the first time without messing with the settings. I personally did a first clear on normal. Of course, it’s your choice.
  • Carry over money from a previous playthrough can help too, we'll mainly be using it to purchase some items or pay the fox to recover SP, when needed.
  • Elemental damage items are going to be your best friend, these will allow any character to pretty much cause knocking on most enemies. Before you go in a line hunting session, I’d suggest you go in with 20-30 of each of the items (deal 50 damage to 1 and all enemies, for all 4 element types). These are sold on Shiroku.
  • Now, a VERY important technique you'll want to use is called analyze shuffle, which basically consists into going in the analyze menu for an enemy, then backing out, do this enough times and about every other 3 shuffles, Rise will make a comment on something that's currently going on. When you don't have many lines, you can use this to get several at once, depending on what’s happening. You'll definitely need it to get lines sooner, rather than waiting actual turns for her to say stuff.
  • Consider packing up several healing items, basic medicine works, you'll likely be using a lot of throwaway items on some character turns if you're waiting for a specific condition to be applied to them. Another alternative is to just use weak HP/SP recovery items, such as Peach Seeds or Soul Drops, these however you need to scavenge from chests.
  • Guarding will prevent ailments from being applied to your party, so it's not recommended to do initially.
  • Of course, you might want to equip your party with buff/debuff spells, to just use up turns as well.
  • DO NOT level up your party member's social links, ESPECIALLY RISE, at least until a certain point. Having your party members rank up will make it more annoying to get some status related lines as you go. Yosuke, Chie and Teddie are forced by story progression to be able to pick up fallen allies from the get go, which will make it getting lines for dizzy a bit harder, however there are others you can focus so not much of a problem if you do miss those lines.
  • I would also prioritize doing Teddie's lines ASAP too, as he will always rank up through story, if you take long enough, he'll be able to cure ailments and even endure KO hits, which for the line hunting isn't helpful.
  • A personal tip that you might choose to follow or not is to keep Yosuke relatively weak, have him not surpass level 30 when you get Rise. On a specific enemy spawn you can use him to set up some lines related to defeating several enemies at once, just keep Garu on him and don't make him very strong. This is simply a personal recommendation that I'll explain later.
  • Sometimes several actions will take in rapid succession, or even, at about the same time, Rise will likely not have enough time to comment on all situations. An example of this: In a dungeon an enemy used Mazionga, Yukiko dodged it and Yosuke got hit, getting knocked by the weakness. Rise only commented on Yukiko's dodge at the time.
  • Since some of these situations are random you might want to consider waiting for the line you missed to be acquired again by staying in the same battle or encountering the same group of enemies.
  • Finally, have a method of tracking your progress, I personally wrote down every line I came across myself on a text file, however I found this spreadsheet[docs.google.com] that can make the process a bit easier for you, however not all the actual lines are listed. It's also not made by me so there is a chance there are more lines for a specific situation than what is listed here. Either way, just make sure that you're able to tell if a line you see on screen has come up before or not. Believe me, it's not very easy to track all just by memory
Line Categories I
Now, I'd like to detail the different "categories" for navigation lines you can find. I've grouped into big categories and broken them downfrom there, they are as follows:

Strong enemies
There's a couple of lines you can get when facing a stronger enemy, in other words, the really big shadows you might find in dungeons. There aren't that many but if you can get them to boost the number, go ahead. I'm not 100% sure but from my experience there's one for getting ambushed by a strong enemy, and 2 for normal/advantage encounters. 3 lines.

Battle Start lines
These are lines that Rise will say when a battle starts, and they will not repeat until a different battle with similar conditions is started. There are also 2 subtypes of these lines.

First are the lines that detail the number of enemies that are in the battle, which for this game is any number from 1 to 5 enemies. These are the lines you wanna hold off the Rise Social Link progress on to acquire, as they'll be almost impossible to get later on, besides some very specific encounters.

There's 3 types of encounters, which are Advantage (you hit the enemy from behind and get an extra turn), Ambush (the enemy hits you from behind and they get the first move) or normal (you both clash pretty much in front of each other or from the side). There's 2 lines per type, per enemy number. For instance, if there's a battle with 2 enemies, and you started a normal encounter, Rise will have 2 lines, which are: "Two enemies on the feld! Crush 'em!" and "There are two enemies! Careful now!". This is just an example.

2 Lines per specific encounter X 5 possible enemy numbers to encounter X 3 types of encounters = 30 lines you can nag from this category.
I suggest you go for all of these, as I said, you can pretty much never get them again if you progress Rise's S. Link to 2, so they should be the lines you seek out first.

For the second group of Battle Start Lines, you'll need to progress her Social Link Rank 2 to, I suggest you leave it in 2 only for the remainder of your run, progressing past 6 makes her give you the enemy weaknesses if your current active character has an attack of that type, I don't recall if this will make the following lines not play, but I'd advise you to just not do it until you're done.
I personally kept her at 2 for the rest of my quest, If you raise her more she can start to provide some aids in battle that you ideally don't want as they'll interfere with some lines if triggered right away.

When she's at Rank 2 she learns weakness scan, this means that whenever a battle starts, she'll state an affinity from an enemy in the field. An affinity is either a weakness or immunity an enemy has for a specific attack type. Resistance, Block, Repel and Drain all count as an immunity. Weakness is... well a weakness. There are 7 types of attacks: Fire, Electricity, Ice, Wind, Light, Dark and Physical. Almighty also exists but that one doesn't fit into this.

For each type there's 2 lines per affinity, for instance, when encountering an enemy weak to fire, she will say "That enemy's weak to fire! Keep it up!", or "Hey, the enemy's weak to fire! That's hot!". Anyway, 7 types X 2 lines per type X 2 scenarios (weak or immune) = 28 lines.

The reason the first group of lines become rare if you level Rise is because... there's very few enemies that have no affinities at all, not even bosses which are normally given block for Light and Dark to prevent 1 hit KO's. In my own experience, the first enemy I found with no affinities after getting Rank 2 for Rise was a Hell Knight on the Secret Lab, though I didn't revisit earlier areas, but still, most enemies in the early areas have at least one weakness.

Another important tip, some enemies will have both weaknesses and immunities, or even several of each, though mainly from Secret Lab and onwards, on these cases, Rise seems to always focus on stating the immunity. For instance, Wicked Turrets in Secret Lab 5-8 are immune to dark but weak to light, I never managed to get her to state the weakness to light on them. In fact, doing research for this, I saw that they're actually weak to electricity and immune to wind as well. On my attempts I'm pretty sure I always got the "immune to darkness" lines only.

Battle menu Options
I titled this section like this because they're lines you get from different options of your battle menu.

First up is lines for escaping.First is select “Escape” and if it’s not an instant escape, Rise will start looking for an exit, that’s one.
Second is selecting escape again after doing the previous step, the button will be grayed out, symbolizing it won't really do anything. If you select it, she'll say she's looking around for the escape route.
Third, when she tells you that you can now escape, and you can accept or decline.
Finally, there is another when you attempt to escape from a boss fight, she'll say she can't find anything. You can do this earliest in Void Quest 7 (mini boss) or if you go do any of the optional bosses from the previous dungeons.
It's worth mentioning that in case you never Escape any battles, when Rise reaches level 70 she will learn a skill that will make every Escape attempt succesful, so you would be missing 2 of these. Of course it's unlikely this will happen...

Then there are 3 lines you get when you activate Rush mode, I was confused about this one because nowhere online where I read, they specify about these lines, they only mention "3 lines on rush mode". For this you'll need to basically activate Rush and have it running for a while, she'll eventually start saying the lines. Maybe equip weak weapons so the characters don’t instantly kill everything, it depends on the dungeon you do it of course. So, from this category we have 7.
Line Categories II
Enemy Interactions
There are a BUNCH of lines here. I can't really give the exact number as how many exist, some may be triggered in different ways, but these are the ones you're gonna use to really increase that line count. We have several groups:
  • Party member knocks down (hits weakness) any number of 1-4 enemies in one turn and party member defeats any number of 1-4 enemies in one turn. Try to never knock 5 enemies at onces while line hunting, as this will lead directly into an all out attack prompt, and Rise might not say anything. Of course 5 meaning the max amount of enemies a battle can have, change the number according to the enemy encounter, If all get knocked, an al out attack prompt will appear.
I believe every character has 2 lines for knock/defeat of 1 enemy, for 2-4 they have just one line. Do the math and that’s 35 lines for all 7 party members for knocking, other 35 for defeating. a whopping 70 lines available. To be fair getting all is hard, mainly on Naoto, but I think you can aim for about 50-56 if we rule out the 3 and 4 enemy lines, however there's cases to acquire some of these more easily. There's also 1 line when an enemy that was knocked gets back due to his turn returning. There might be a couple more but I'm confident at least 71 exist for sure.
  • At times if an all-out attack is set to happen, or if for whatever reason an ally offers to use your extra turn, Rise can state an alternate, neutral version of some lines, for example instead of "Three enemies down! You did it, Senpai!", assuming there's more than 3, and an ally wants to user your extra turn, she'll just say "Three enemies down!". Same if there's just 3 and it'll lead into an AoA. These DO count as another line, but don't rely on these. Also, there's a chance no lines from Rise will play at all. I don't exactly know how many they are of these but if I had to assume I'd say 5.
If you don't level any other party member's Social Link Rank, only Teddie can make these lines trigger for non AoA scenarios, after July 26. This is both a blessing and a curse, it will allow you to get these neutral lines, but overall I found it annoying if I was going for a specific "character knocks X enemies" line and it would be blocked out by Teddie pitching in. I personally did Teddie's lines until October so it did screw me over sometimes.
  • There are also some quotes that will play when you "go on defeating sprees" as in you get extra turns and keep defeating more enemies, but I think some of these overlap with the "knocks 1 enemy" lines. It's worth to do initially though, mainly on large groups of similar enemies that you can defeat one by one.
  • The next group is a relatively easy one, Rise will comment how many enemies are left on the field, you can use analyze shuffle to get these out of the way ASAP.
I THINK there are 2 lines for every remaining number of enemies, 1-5. Some of the sources I've seen online say there's only 1 line excepting for the 1 enemy left scenario (which has 2), but I can say I found 2 for all the other cases. So with that said, add 10 lines for this category.
  • There are some cases where enemies will run away from battle, this isn't very common, only a handful of enemies do this, she will sometimes have lines regarding an enemy running away and how many are now left. I don't know exactly how many of these exist, I only came around one of them by pure luck, with the "Rainy" enemies that rarely only spawn on rainy days, the line was: "The enemy ran away, there's two left!" sometimes enemies run in a row and Rise only has the chance to comment on one of them. I can't give you an exact number on these but honestly I wouldn't go my way to actively get them, only if you're lucky you might come across them.
  • Next up Gold Hands, there are 2 lines regarding them. A first when you enter a battle with them, where she mentions they're rare enemies, there's a second one that plays when they escape, it'll however only play if there are 2 or more enemies on the field. If it's just a single hand it will escape and the battle will end before Rise gets a chance to say anything. This quote is indeed exclusive to gold hands and unrelated to the previous escape quotes.
  • There's one more group of lines regarding enemies, which is whenever more enemies join the battle. The first way this can happen is when an enemy summons another, a couple of enemies can do this, the most common are Gold Hands. There's apparently 3 lines for this interaction but from what I saw, you'll need to hunt down different enemies that can summon to get different lines, it seems if an enemy type A uses summon, Rise will only state one of the three lines, I confirmed this on different encounters. You'd need to find an enemy type B that summons and she might use a different line. 3 lines here.
  • The other method more enemies will join is with a reinforcement spawn, this is mainly triggered whenever 1 or more shadows have noticed you and you start an encounter with a different shadow. When you defeat the first group the next will jump in. Same as before, enemy groups can be from 1 to 5, and each have 2 lines for whenever that number is. Meaning 10 lines. I'll admit getting ALL of these is a bit grindy, specially the 4 and 5, but I’ll detail my methods on how I got them later on.
Line Categories III
Character related lines
Here's another beefy group. These lines relate to status or conditions that can be inflicted on your party members, or that they can perform.

PLEASE REMEMBER if a character uses Guard, they will not receive most of these, so consider just using throwaway items like a heal or low SP recover just to eat up their turn. Also you can buff/debuff but try to avoid using the hit/evasion effects, as most of the times you want the enemy to hit you, so it's pointless to increase your own/decrease the enemies hit rate.

First up its ailments: There's 8 of them: Fear, Silence, Enervation, Rage, Dizzy, Exhaustion, Poison and Panic. Dizzy is the hardest one to get the lines for, because most of the times, the only way a character will become dizzy is if they're hit twice with their weakness, before they get their turn, as they'll come up again from simply being knocked down. I believe some specific attacks or scenarios might leave a character dizzy, but it's random.
Also if the members have enough S. Link Rank, they'll pick up other teammates on their own, at times right after they fall, making that possible line you were about to get fade away.
Dizzy has one line when the character gets the status, then another one when their turn arrives and they're still under the effect (they will lose the turn).
Fear also has 6 extra lines that you can acquire. When an enemy is under fear, there's a chance the ailment will make them run away from battle. Your main character won't run away, only your teammates.

I have another weird thing here though, it seems most of the times you will need to reach the MC's turn for Rise to state his ailment lines, it doesn't happen always but I’m fairly certain that for some ailments, a line would not play for me until I reached MC's turn again, no matter how much I shuffled.
Anyway, 8 ailments X 7 characters + 7 still dizzy + 6 escape due to fear makes 69 lines.

Next up is actions the party can perform, such as evading as every character, falling down due to missing an attack (happens more under enervation) or getting a critical hit against an enemy (don't quote me on this as they might group into the knocking lines mentioned before). There's also a line exclusive to MC for getting up by himself after he's been knocked down. 7 evade lines + 7 miss and fall + 1 MC gets back up by himself = 15 lines.

There's lines when characters get hit by their weaknesses and get knocked down. On my experience I didn't get all of these lines, as you kind of need to set up with enemies that have attacks your party is weak to. I will confirm this and edit if needed. For now, let's assume there's one per character, so 7 lines. However i can say I got 2 for Yukiko so after I have all the info i will update.

Getting into low hp has several lines, first there's ones that are played only when an enemy hits you and you go from a "decent number" to about less than 25% of your health. A different line for when the character is still low on HP, different from the "just got hit and entered critical hp". If you use a physical skill that will take you from a healthy to a critical state, the first line will not play. There's another line for when 2 or more party members are low, and she'll just say the party has low hp. Finally another line for every character getting their health points to 0, except MC, if he dies its game over, there MIGHT be a line if you have a way to revive but just don't count on it.
7 enter low hp lines + 7 stay in low hp + 1 several members are low + 6 character is defeated = 21

THERE IS A SPECIAL SET OF LINES REGARDING YOUR MC GETTING INCAPACITATED.
It took some testing but i was able to see there are 3 lines that play when MC goes down. HOWEVER, these lines will only ever play if Rise is at Social Link Rank 10, as she will gain a once per battle revive for you. I tried using the skills Endure/Enduring Soul, however this will just heal you upon getting to 0 HP, they effect is instant and no animation or line plays, you only get a text on the screen telling you the attack was endured.
When you got the revive from the Social Link skill, the line swill play out but likely be interrupted early by Rise's revive animation. I would not advice going for these 3 as it requires Rise's Max Social Rank, but for consistancy I'm including them here.

Then there's debuffs, your characters can get their offense, defense, and agility lowered, each with 1 line. 3 conditions X 7 characters, another 21 lines.

Finally, there's lines that play when you get a victory in battle. These will only play when
a) There is no Shuffle Time at the end, and
b) MC is the one who delivers the final blow to the enemy(s), as any other character will do an animation and they will talk instead.
4 lines appear here. however, there's a chance it's only 3. You see, there's one line that says "Great job", and another one that says "Great work, everyone!" HOWEVER. when the latter appears, the actual voiceline you hear is "Great job", while the text says "Great work, everyone".
I found this really odd, in fact, I got the other 3 lines to play relatively common alternating between each other, but it was only until I was hunting my final lines when I saw the 4th, and to go with it, the voice clip that plays didn't match the actual text on screen. I think I only payed attention to seeing it once or twice, but I’m certain that it didn't play the same text on screen. Odd indeed. For now I'll keep counting it, so we got 4.
Line Categories IV
Boss battles
Some bosses will give unique lines from Rise, some you'll likely never miss, others are very missable if you don't look out for them. A couple of these lines break the established requirements for what navigation lines are, as some, but not all, will appear in a "normal text box", like the one you see in all other moments in the game. However I'm sure they actually count towards the achievement.
I still need to confirm some of these for myself, but they're are as follows:
  • Marukyu Striptease - Shadow Teddie: One line when he uses Ultra Charge. If you do enough damage to him while charging, he'll drop the attack and get knocked. Don't do this, let him charge for one turn and then she'll have another line. After she says it you can try to attack him now so he drops the attack, or just use guard on all your party, if you do you'll block all the damage. 2 lines total.
  • Void Quest - Shadow Mitsuo: I don't really think any lines here are missable, unless you do extreme amounts of damage. There is one at the very beginning, and another when he uses the Whisper attack to start rebuilding his armor. 2 lines.
  • Heaven - Kunino-sagiri: This is a tricky one, when he's at about 75% hp he will start using Quad Coverage, which changes the field so that 3/4 elemental types get weakened, making the 4th more powerful. When he does this you can shuffle to get a line for each element. In fact it's ideal to shuffle to even learn what element is active, if not you're gonna have to guess it. You're gonna have to wait for him to change into all 4 elements to get all 4 lines.
After he receives more damage, he will start using Control, which will make all your active allies to change sides momentarily and attack you. In theory there are lines for every teammate getting controlled one by one, which requires the other 2 teammates to be incapacitated, as well as a line for all 3 allies getting controlled at once, which is the one almost everyone will likely hear. Since you can only do this fight once, only 3 of your teammates can potentially get these lines. This would mean 8 lines. I'm gonna confirm this myself and will update the guide with my findings.
  • Magatsu Inaba - The Culprit: There's 2 lines at the beginning, funny enough unlike the Teddie or Mitsuo ones, these actually play like most other lines, and not on the default text box. The 3rd line is when he uses Heat Riser, which should always be his first attack. 3 lines that aren't missable, unless you somehow KO him before he gets to act.
  • Hollow Forest (Requires maxing Aeon arcana before December ends) - Marie: There's 3 non missable lines right at the beginning. Once again on the default text boxes.
  • Hollow Forest - Kusumi-no-Okami: Once again 3 non missable lines at the beginning. There's two more for when she uses Draining Fog, she must use the attack 2 times. Here's another odd one, each of these final 2 lines have 2 text prompts each, I'm unsure if they count as 1 or 2, since they use the default text box and not the battle one. For consistency just assume they count as 1 each. 5 lines total.
  • Heaven - Margaret (only on a NG+ file): If you met the requirements to fight Margaret, she has 4 intro lines, then a 5th happens when she starts using Personas. The first 4 are also on the default text box and on most resources, I've searched, they claim each text segment counts as 1. It does suck to not have a concise answer for this… 5 lines in total.
  • Yomotsu Hirasaka - Izanami: On her second phase, there is a line at the beginning. Then there is a line for each party member when she uses Thousand Curses. Once again since you only fight this once that’s only 5 lines. Of course, this is the very end of the game so it's not advisable to rely on these lines to complete the achievement.
Confirmed line totals
Here's a concise list of all of what's explained above.
I'm counting only the ones that I'm 100% certain that exist.

My Experience - The Initial Grind
The way I approached the achievement was as follows. I'll write as if it was a walkthrough because reading it back as a journal feels odd, just know that I'll list some corrections/changes to my approach bellow.

I progressed normally until Marukyu Dungeon, I tried to stay at around the same level or a lower of the enemies, I believe I was just level 27 before fighting the dungeon boss. I had a strong enough Persona for it.

On the Shadow Teddie fight, it's the first time you'll have Rise supporting you. The first 2 lines you can pick up are here. I think you cannot get more lines from this fight, as Rise doesn't really know the party members yet so she can't comment on their actions.
After the time of deadline for that dungeon arrives, Rise and Teddie will join the party.

REMEMBER DON'T LEVEL UP RISE'S SOCAL LINK, keep her at minimum for a while. You can go into the TV before you even get her Rank 1, and she will state lines, Rank 1 only has her provide help on all out attacks, which doesn't break anything. However, Rank 2 will make you likely miss about 30 lines, so DON'T.

The game keeps going until Void Quest is unlocked. Here I first went into its first floor to get the "strong enemy" lines, which are like 3.

My party of choice was MC, Yosuke, Chie and Yukiko, basically the first full party. At this point I decreased EXP gain to the minimum. At times I’d adjust my damage as well to do less.

To start tracking lines I started going up Yukiko's Castle again from the first floor, you'll get a bunch as you go, I prioritized getting the battle start lines (advantage, ambush and normal battle starts, for different numbers of enemies), some lines for every character defeating different numbers of enemies, knocking them, some ailment quotes for the characters, low hp, victory lines.

As weird as it sounds, 1 enemy encounters aren't very common, however you can get a single Laughing Table on floors 4-6, I used this spawn for those lines of 1 enemy encounter, as I went up the most common encounters were of 2 or 3.

One aliment type you can get here is silence, by Phantom Mages on floors 5 or 6, I got these here.

After I saw most of the encounters on the castle, I went to the Steamy Bathhouse, I also started climbing from the bottom but I ended up lingering on floors 3 and 4 mostly.
These floors have very helpful spawns, they are not exclusively just on these floors, but I found them very commonly like this:

Floor 3 has a spawn of 4 Monopolizing Cupid, these enemies only have 100 hp and are weak to Ice, by using single hit ice items on them, you can farm the lines for every character defeating 1, 2 or 3 enemies at once, just set up the amount you need, then use the item that hits all, it can be of any element too since the already weakened enemies will just be defeated and the remaining ones will have 50 hp left. Also you can use this for the battle start lines for 4 enemies. A similar spawn can happen of 3 of these Cupids and a single Pursuing Pesce, this last enemy repels ice so you can get the knocks 3 enemies lines here by using the hit all enemies item, before this point I don't believe there's an easy spawn to do it. Of course keep in mind 50 damage will be reflected back to you.

The other helpful spawn can appear in floor 3 too but I found it more common in floor 4, it’s a group of 5 Autonomic Basalt, which are weak to wind. You can get the lines for 5 enemies on battle start here. Now the reason I mentioned the possibility of keeping Yosuke relatively weak it’s because this spawn can help you get the defeating 1, 2, 3 and 4 enemies easily.
When I got here with my about level 27-28 Yosuke, his Garu did just enough damage to not fully wipe the enemy HP, so that almost any other character could just use any AoE attack, preferably the items, and KO them for the line. These enemies have 161 hp so it takes a couple of turns to set them up with items, and since they are Basalt enemies, they can just use Last Resort and screw everything up. I guess you could try to get a relatively weak persona for MC, teach it Garu and hope he deals anything between 111-160 to them, with it you can do this setup as well.
Anyway this is just the way I did it but it happens early and it’s not hard to come by that spawn so it's worth a shot.

Also, Bathhouse 3 has always the same floor layout, and near the stairs heading to Floor 4, there are 2 shadows close to each other, I farmed the reinforcement lines here, I believe the floor has diverse enough spawns to get lines for all groups of enemies. So just approach one of the shadows, get them to notice you then get them close to the other and start the battle, defeat them and then start getting the reinforcement lines. Pictured bellow is the place i mean. As a side note, to unlock this door you first need to have the Bathhouse key and open it from the hallway part, I beleive it's locked if you try to open it through the room with the stairs.

Red shadows on floor 3 can spawn a single Nizam Beast so if you see them amongst these 2 shadows, get them to notice you and go for the other shadow so you get the reinforcement 1 enemy lines.

I also farmed some ailment quotes here, mainly panic and fear. Panic can be inflicted by the Cupids mentioned before. Fear can be inflicted by Pursuing Pesce, I found it more easily on a spawn in floor 4 that can have multiple of them, I believe it can appear on floor 3 too. This enemy can also use summon, I believe they will do so when a party member is under fear. The line I got from this plus some golden hand summon gave me 2 of the 3 summon lines that exist.

At this point I got all battle start lines, all reinforcement lines, and some diverse lines for knocking/defeating different numbers of enemies with these 4 characters. In total I had 139 lines so far.

I then proceeded to go into Marukyu Striptease, but I only wandered around the first 4 floors, mainly to get enervation lines, as the enemy Creation Relic can inflict it. I got these lines plus some of the lines for missing an attack and falling down, since the odds for this increase under enervation.

Finally I proceeded into Void Quest, I got a couple of more lines as I cleared the dungeon. On the boss, Shadow Mitsuo can inflict exhaustion with the bombs he throws in his Hero form, you can nag some lines here. He can also inflict fear but try to cure it ASAP because he can and will use the move that insta KO's all scared enemies. If your MC gets scared please heal him, if you wait for his turn to get his line you might die before you get the chance, guess it depends in your party agility. The line can be acquired elsewhere. Remember that this boss also has 2 unique lines, you probably will never miss them, maybe the 2nd one, just don’t do too much damage and let him cast Whisper.

At this point I had 158 lines. Having claimed all 30 for the battle start with X number of enemies, it was safe to proceed with Rise's Social Link to 2, so I did when I got the chance.
My Experience - Roadway to Victory
I returned to Void Quest after the story section for it ends, now with the ability to obtain affinity lines, I kept the same party as before. I started from the bottom and by the time I got to the 5th floor, I pretty much had all affinity lines besides weak to physical, weak to light, and immune to fire and electricity, I just battled every shadow i found and the lines piled up easily. At this point is where immunities start becoming more prevalent so it’s okay to miss a couple, there aren't enough enemies here to cover all cases, the other lines can be acquired easily on the next dungeon.

Game kept progressing, I arrived at Secret Lab, same party, I got the remaining immunity lines on like the first 3 floors. On floor 6 you can find Immoral Snakes which will give you poison, another ailment.

On floors 5-6 you can find Mind Dice, which will cast Debilitate on your party, lowering all 3 of your stats at once, I grabbed 12 lines here, 3 for each party member. The boss here doesn't have any lines. At this point I had 205 lines.

All I did now was wait for Naoto to join the party, head into Secret Lab again, this time with Kanji, Teddie and Naoto, none of them have any lines yet so it should be easy, there's a lot of spawns that allow you to get 1, 2, 3 enemies knocked, defeated too but since they're stronger it might take more effort. Get some of their ailments, HP lines, in the end, the Mind Dice was the final frontier, Teddie getting low agility was the legendary line that granted me the achievement and ended the ordeal.

Though... looking at my list, the 251st line granted me the achievement, which leaves me to think one of the other 250 i collected doesn't actually count, I'm unsure which one it is though. Honestly my bet is in the victory line for "Great work, everyone" as Rise doesn't actually say that, instead she says "Great job" despite the text being different. Well who knows.
Possible Improvements
Alright, now that you heard my tale, here's some amendments I would make, if I were to do it again, to make everything much simpler.

First up, focus Teddie first, since you can use him from the same time you get Rise, and just like Yosuke and Chie, he can pick up enemies from the very beginning, and in fact he too can choose to take up your turn after you exploit a weakness, though this won’t happen if you go into the TV in between getting him and the Mitsuo dungeon activating, you could choose to enter the TV on a random day to prevent this, as him taking up turns might actually ruin a line you were seeking, in the end it's better to do him first.
To be honest its less work to have just 2 parties (eg Yosuke Yukiko Teddie and Kanji Nato Chie) and get the lines for each group, rather than equipping a single teammate to get their lines... heck, odds are in group 1 you'll get MC's lines so you can just guard with him on the other group. Just probably try to NOT group Yosuke, Chie and Teddie, as it's the one combination that has EVERYONE be able to pick teammates up.

Also, Teddie will learn to cure ailments on the school trip (September 8th), I'll admit he probably only screwed me over by healing an ailment like 2 times when I had him in the party, but to avoid the hassle you can just go into the TV before this and get those lines when Teddie's on the active team.

Also as you saw I always went to the stronger dungeons for new lines, since I detailed some easy ways on early dungeons for some line hunting you can just go there too, it'll probably be quicker in the end.
General tips for some lines
Please forgive if this section is a bit lackluster, I wasn't ready to fully edit the guide but I've been keeping it untoched for some weeks and I got some more findings I had not been able to put out there. I'll improve it over time, I just want more people to possibly use this information.

"Hard" Affinity lines
As i said on my path above, I got most of the affinity lines just by doing the encounters on Void Quest/ Secret Lab, there are however a couple of tricky lines, though the aren't THAT hard to get.

First up: Weak to light enemies, the problem with these lines is that the enemies that have them in earlier stages of the game normally are grouped with other enemies, or have immunities which will forfeit the lines from playing. The first time I got one of those lines was with a group Royal Dancers in Secret Lab 3, this was kind of a rare spawn because that enemy normally spawns with other types, I got very lucky finding an encounter of only them.
I saw it was easier to backtrack to Yukiko's Catle, there are 3 enemies that are weak to light with no immunities, they are:
Phantom Mage (F 5-6)
Burning Beetle (F 5-7)
Positive King (F 5-7)
Try to look for encounters of these enemies, they're not uncommon, and getting these lines is easy.

Second and last its weak to Physical, now THIS is a very special case. The problem is there are very few enemies weak to physical in the game, and ONLY TWO OF THEM don't have any immunities.
The only efficient spawn for these lines are the Killing Hand (they're not golden) on Marukyu Striptease 5-10. They sometimes spawn with Knight enemies that thankfully don't have affinities.
The only other enemy that can potentially get you these lines are the Harmony Giants in Magatsu and Hollow Forest but... come on don't go all the way to the end of the game for these.
The Hands spawn relatively common.
Every other affinity line is easy and you'll likely find them normally just playing. As I said doing Void Quest (1-5) and then Secret Lab (1-2) should get you the rest.

Useful spawns for status ailments
Here's some spawns for getting the different status ailment lines, I've included some of the earlier moments where you can hunt them, but I've also uncluded a list i was provided which I beleive has an enemy casting Stagnant Air, which will make it more likely your party gets the effects. Normally, the single hit moves to inflict ailments have a 30% success rate, and the party wide ones have a 40% rate. Remember DO NOT GUARD to get them-

Ailment
Early Spawn
Alternative
Enervation
Creation Relic (Marukyu 1-4)
Perpetual Sand (Heaven 1-4)
Exhaustion
Constancy Relic (Secret Lab 1-2)
-
Rage
Liberating Idol (Marukyu 5-10)
Carnal Snake (Heaven 2-4)
Fear
Laughing Table(Yukiko's Castle 4-6) or
Persuing Pesce (Bathhouse 1-5)
Dark Eagle (Heaven 2-4)
Silence
Phantom Mage(Yukiko's Castle 5-6)
Lustful Snake (Void Quest 1-4)
Poison
Immoral Snake (Secret Lab 6)
-
Panic
Monopolizing Cupid (Bathhouse 3-4)
Rainbow Twins (Secret Lab 3-4, 7-8, ONLY ON RAINY DAYS)

I'm not sure there's an earlier spawn for Exhaustion, excluding the void quest boss.
I know there was an enemy in the first 2 dungeons that casted Poison but I rarely got them to inflict it, I'll look back and include it later.

Dizzy shenanigans
Dizzy is harder, I need to compile a list of easy spawns of enemies that use each type to farm each character, please check back soon.

For now, I can give you some potential tips on how to get these if you wanna try.
First up is low level Personas with each of the weaknesses, you can equip them alongside the party members also weak to each element, for better chances of enemies getting yet another turn if they knock another party member, they are:
Wind - Izanagi
Fire - Slime
Ice - Ukobach
Electricity - Sandman

Keep in mind that both Teddie and Yosuke are weak to Electricity, but both also can pick up teammates by default, so if you wanna try to get these lines, go with each teammate alone. Naoto does not have a weakness on her Persona so you'd need to rely on enemy crits on your party for that, not worth it.
Anyway, I have been trying to get an efficient spawn for each element but its not that easy, normally the enemies that have elemental spells will normally use something else if they get a free turn. For now I can say that the most succesful way to get some of these lines are for characters weak to electricity.
Laughing Tables on Yukiko's Castle 4-6, sometimes they spawn in groups of 2, and they like to cast Zio and then they might follow up with a physical attack, which will dizzy your party member if they are down. However they might hit a different character, I at least found it easy to get the dizzy lines for MC here. Didn't have much luck with Yosuke as they kept casting Fear on him and he'd run away.

I'm still trying to find better spawns for this, as well as for other types (tested for fire and ice on Yukiko's Castle but none of the enemies like to follow up for a dizzy)
Funny Story
Alright now here's something that happened to me when I was close to finishing. When I went in to get the final group of lines with the 3 party members that had been ignored ever since, I did pretty much the same I mentioned above, fast forward a bit, I was at 246 lines according to my list. I only needed to hunt down the Mind Dice to get the debuffs on Kanji, Teddie and Naoto. I got into a worthless encounter while on the 6th floor, escaped from it, aaaand..... perpetual black screen, my game ended up crashing, that lost me like an hour and a half of progress.

At least I knew the cut point on where I had last saved, so the next day I went and got the 45 lines I needed once again, until I got the achievement. I ended up having 16 spare lines I encountered the first time around but not the second, since I hadn't saved those never got added to my total.

Final words and thanks
Well, I believe that is all for my guide. If you have any information that's not listed here or you think I might be wrong in some aspects, please feel free to comment it bellow, or you can add me and send it over in a chat (If i see you have P4G on your recent games I'll probably accept you lol).
I can adjust the guide If needed.
I plan to confirm the stuff im not 100% sure on, like the Ameno-sagiri lines and stuff like that, I'll revise the guide to complement it. I'll also see if some parts could use some screenshots to show what I'm saying. I can also add some places to get the aliments i didn't mention here. Though if you follow something similar to my path you'll likely get the achievement without the need to step into Heaven.

Some stuff will be hard since I have the achievement already I can't "force situations" to try and confirm the actual number of lines some parts have, mainly the ones I said that appear on the normal text prompt and not the battle one, which appear on the bosses.

If you wish to see the lines I got on my attempt, I've put them on a pastebin linked in the previous section. There are some comments at the top and bottom of the file. I also included some extra lines that i got initially before my game crashed, and I did not get back on my re-doing of the final line hunting session. Also the lines aren't written exactly like they are ingame, i kept most of them about the same but i might have messed up here and then, also ingame the use a hyphen when writting stuff like "Yosuke-senpai", I just used a blank space instead.

I'm also trying to track all the lines in the game on a spreadsheet also linked above, it's still a work in progress, however I've already made some findings while collecting lines and updated the guide with them.

I hope you enjoyed this guide! Since I put so much effort into getting this achievement I thought it made sense to share my findings and process to hopefully help others with this achievement. I did something similar for a stupidly dumb achievement in Danganronpa 2, lol.
In total i THINK i took like around 15-20 hours since the line hunting began, i know its a big interval but i DID lose progress on the crash. Also writting this guide took me several afternoons as well haha.

Many thanks to a lot of Steam users from the discussion forum that have contributed to some of my research, you've made some of my efforts easier! (i swear i'll name them here soon)

I'm planning on making a video version and upload it to YouTube, there is seriously nothing regarding HRF on there, I checked, but I wanna have more info before that, it's easy to edit the guide but not a video. Also I an hopefully simply stuff more by showing stuff on a video, this ended up being way longer than what I initially thought.
Please leave your feedback bellow if this was helpful and I wish you luck on your HRF quest!

By the way in the FAQ I can confirm that using NG+ resets the number of rissette lines. I'm using a save file tool to look at the values for lines that are counted, I loaded in a NG+ file that's at 55 hours with no progress and it read in 0 lines for the achievement. Idk if you're still updating this but regardless super great guide thanks for the help!

Thank you so much. It took me like 15 hours . My 250th line was "Three enemies defeated! You did it Naoto-kun!"

Same, got this achievement accidentally. Didn't realize it was that big of a deal until I saw that it had such a low percentage. However this is a fantastic guide. Incredibly in depth, hope yall searching for this one find it.

I feel so bad that I got this achievement by accident and i see all these people struggling out there.

Yooo, just got this, thanks a lot for the guide!

Because of sheer laziness, I did a variation of your guide without writting anything down, it worked pretty well! I was gonna put everything in this comment but it was too much text, so I wrote a new guide . I mention yours a lot since it was my main source of info.

Congrats again on the awesome guide and thanks for the help!

I'm playing the game for the very first time and beat Ameno-Sagiri last night, but apparently I also stumbled into this achievement at some point yesterday?? Like, I did zero intentional groundwork, I didn't even know this was a thing. I guess I get to brag now!
