This guide is everything you need to get the 100%, it includes all of the achievements, and the collectibles.
Be sure to start the game on the hardest difficulty if you intend to do it in a single playthrough.
The game also has level select so in-case you miss anything or you're short on something you can select any episode at the end of your playthrough.

Mnesist Memory 1/6:
Soon after you learn to climb, you'll drop down past a fence near a wrecked blue car. The first Mnesist Memory is behind it.

Mnesist Memory 2/6:
As soon as you enter a train go to the right and it'll be hidden away in the corner.

SAT Patch 1/5:
After being introduced to Errorist Memories (the white-framed digital monitors) cross the bridge and the SAT patch will be on your right.

Mnesist Memory 3/6:
After defeating the Prowler Leapers with the Regen Pressens ability, exit the area by climbing and jumping over the ledges. Once you are able to walk again (When you are on a platform with a small house) the Mnesist Memory is hidden away in the right corner.

SAT Patch 2/5:
After the next fight, jump over the gap and the SAT patch is on the left side. (Before going up the red pipe)

You missed me
Dodge a Skinner's charge five times in a row.
You'll be introduced to Skinner type enemies around half way through the episode. Simply wait for him to charge and dodge it 5 times.

Mnesist Memory 4/6:
Once you are in a marketplace area, cross the bridge and you should see a man leaning on the railing, and next to him a red boat, the Mnesist memory is behind the boat.

SAT Patch 3/5:
Right after collecting the previous collectible, cross the bridge and take a left towards the 'all you can eat' vendor, the SAT patch is hidden in the corner.

Mnesist Memory 5/6:
After getting the 3rd SAT Patch, run around the corner past the ladder until you reach a dead end. The Mnesist Memory is lying in the corner on the left.

SAT Patch 4/5:
After collecting the previous Mnesist Memory, climb the ladder and hug the right wall until you find the SAT Patch near a robot on the right of the catwalk.

Mnesist Memory 6/6:
After the platforms are falling down and you make a jump to a ladder for safety, slide down the ladder and shimmy across the ledges to the left, when you go past the big red sign and you can go either up or down, go down and shimmy a bit more to the left to find the Mnesist Memory

SAT Patch 5/5:
Once you reach the Leaking Brain before going towards the bar, go straight to the right and the SAT patch is on the table.

In the Olga memory remix: have David awaken and talk to Olga
When you are in the memory sequence and you're introduced on how to change memories find the glitch on the mask and untie it, then watch how the events unfold and the achievement should unlock.

Unbelievable truth
Reach all memory bugs in all memory remixes
I put this achievement in a 'locked' state to show that you're only getting progress for it.
To unlock the achievement you have to get all "Memory Bugs" where you kill the person that you're influencing.
Find the 'Memory Flux' (the Pipe mechanism) and invert it so the memories go the other way and you will kill Olga.

Once you enter the town square you will see a small cut-scene. When you can move again go to the right of the statue infront of you and descend down the stairs, turn around to the right of the steps to find the Mnesist Memory in the corner near some graffiti.

SAT Patch 1/4:
After defeating the Enforcers, climb up the ledges on the right then drop down the wall to find the SAT Patch lying on the ground. (Before going up the red pipe)

Mnesist Memory 2/6:
Eventually you will see cables that are electrified, shimmy to the right while avoiding the electricity and you will see a balcony on the far right with the Mnesist Memory.

SAT Patch 2/4:
Once Edge contacts you after a short cut-scene you'll want to turn around and the SAT patch is hidden near the gate.

Open Mind
Perform your first memory steal on Bad Request

Droning by numbers
Get hit 5 times by a drone during a playthrough, on any given difficulty
Shortly after meeting Bad Request you will see your first security drone. Simply walk into his line of sight 5 times for the achievement.

SAT Patch 3/4:
Right after you go past the security drone you climb up on a platform, be sure to drop down to the left instead of following Bad Request onto the glass.

Focus Boost 1/2:
After getting the 3rd SAT Patch, jump to the slanted glass and you will crash into the apartment below. Exit the room you’re in and head up the stairs directly ahead of you to find the 1st Focus Boost.

Mnesist Memory 3/6:
After collecting the first Focus Boost, descend back down the stairs and enter the kitchen to find the Mnesist Memory on the table opposite the counter.

SAT Patch 4/4
After you unlock a door by synchronizing with a remembrane go through it, then ignore the next remembrane you see. Head down the stairs to the right instead to find a SAT Patch near a gate.

Mnesist Memory 4/6:
After collecting the 4th SAT Patch, run back upstairs and go around the bar and into a bedroom to find the Mnesist Memory on a table.

Mnesist Memory 5/5:
After defeating the two Enforcers, climb up the wall, head up the stairs and then turn right to grab the Mnesist Memory from the bench.

Mnesist Memory 6/6:
After going past the rotating billboard and climbing the pipe all the way to the top. Once at the top proceed to the right to find the Mnesist Memory near a statue.

Focus Boost 2/2:
As soon as you come to a yard take a right before entering the arena. The Focus boost will be sitting on a crate.

Float like a butterfly
Beat Kid X-Mas in under 5 minutes without dying
This might be tough to do on Memory Hunter mode, you will have to evade his charges and then hit him with Power Pressens and you can also use Sensen Fury to damage him. If you do die or you take too long, don't worry you can use level select to choose the episode again and rush it on Easy, but be sure to do it after you beat the main game.

SAT Patch 1/5:
At the beginning of the episode you will see a locked door that you can shoot open. Instead of opening the door, go to the right and the SAT Patch will be in the corner.

Scaramech 1/10:
After collecting the SAT Patch, shoot the door open with the Spammer and proceed forward. You’ll see a white shiny object on the wall straight ahead of you; this is a Scaramech, Shoot it with the Spammer to destroy it.

Mnesist Memory 1/6:
After destroying the first Scaramech, go down the staircase and as soon as you do turn right and the Mnesist Memory is in the half-closed shop.

SAT Patch 2/5:
After you gain access to the 6-Hit combo, defeat the two Prowler Leapers and then turn around to find the SAT Patch behind the stairs to the left.

Scaramech 2/10:
Continue with the main path until you get cornered by an electric wire blocking your path. Shoot the door open with the spammer then go around the voltage current, but don’t descend down the stairs in the next area. Instead, go left to of the stairs and shoot the window with the Spammer to reveal a Scaramech.

Scaramech 3/10:
After learning to shoot Leapers off the walls with the Spammer and defeating the group of Leapers, proceed on by shooting the panels in order to climb upwards. Continue climbing and following the ledges, instead of dropping down to a balcony, follow the path to the left side and then jump backwards and move to the right. Drop down to that platform and instead of going straight towards the SAT Patch, turn to the left and you will see some laundry and a panel you can shoot open. Inside is the Scaramech.

SAT Patch 3/5:
After destroying the 3rd Scaramech, turn around and simply walk forward to pick up the SAT Patch In the corner by some crates

Mnesist Memory 2/6:
After Edge contacts you, you will jump onto a "Jax" sign, proceed after the main path but before going up the ladder, the Mnesist Memory is on the left.

Mix'em all
Land 10 Combos that combine the four different Pressen types
Once you learn the Chain Pressen, you can put each type of pressens into a single combo and use it 10 times.

Focus Boost 1/2:
After defeating the Leapers with your new Chain Pressens ability, shoot the ladder and climb upwards to the top. Once you jump to a platform with a White digital monitor, drop down to the right next to the ladder and you’ll find the Focus Boost.

Scaramech 4/10:
Continue with the main path and leap across the pole and open the door. After opening the door you should see the Scaramech in front of you.

Mnesist Memory 3/6:
After getting a message from Tommy, go past the burning rubble, descend the stairs and turn left to find the Mnesist Memory on a bed in the corner near the stairs.

Scaramech 5/10:
Once you see a panel you can activate by shooting, shoot it, and wait a second before continuing, look up and you should see a Scaramech hiding on the grate that lifted up.

Mnesist Memory 4/6:
After Nilin hurts herself by falling, shoot the door open with the Spammer grab the Mnesist Memory directly ahead of you.

Fiat lux!
Beat 15 Stranglers without using the Sensen DOS
I am putting this here so you can work towards this achievement, whenever you fight against these 'strangler' enemies, try to defeat them using environmental lights, instead of using the Sensen DOS.

Scaramech 6/10:
Shortly after you solve the Spammer Puzzle, you will go up a ladder towards a big fan. Before disabling and jumping through the fan, turn around shoot the Scaramech on the wall.

SAT Patch 4/5:
After jumping through the fan vent continue to the next area and instead of activating the Delivery Drone, go to the left corner and climb up to the platform that contains the SAT Patch.

Queen of the hill
Survive the entire Leaking Brain assault (episode 3) without using Power or Regen Pressens
Once you return to the Leaking Brain bar you will have to survive a short onslaught of leapers, to be safe I'd recommend removing all Power and Regen pressens from your combos so you don't accidentally use them. This is also a good opportunity to work on the Lord of the ring achievement for throwing 15 enemies off the platform.

Lord of the ring
Beat 15 enemies by making them fall off a platform
You'll probably not get 15 in this fight alone, but you should get a good progress for it. Then try to focus on that whenever you see an opportunity to do so.

Scaramech 7/10:
When you’re in the sewers of the tunnels run towards the 3rd grate you see and shoot the movement controls with the Spammer. Now turn around and follow the right wall to another grate, and you should see a Scaramech on the other side. (If you see a White-Digital monitor, then you went too far).

Focus Boost 2/2:
After shooting the 7th Scaramech in the tunnels, go past the White Digital Monitor, and then take a right towards some mannequins, the Focus Boost will be in the corner there.

Scaramech 8/10:
After the train crashes, run towards the right and you’ll spot the Scaramech on the wall, just be sure to shoot it before opening the locked door.

Mnesist Memory 5/6:
Right after destroying the 8th Scaramech, shoot the door open and continue, as soon as you're contacted by Edge go straight towards the grate before going down the stairs.

Scaramech 9/10:
Climb the ladder near the fire and jump onto the upper platforms. Make your way through the train car as soon as you get through it, look inside the train right infront of you to spot the Scaramech.

Mnesist Memory 6/6:
After activating the 2nd Remembrane you will see Johnny guide you through the minefield, pay attention to the right side and grab the Mnesist Memory.

SAT Patch 5/5:
Now this SAT Patch is tricky since it’s very well hidden. Right after the first minefield, climb the ladder in front of you jump towards the first window, but instead of jumping over the gap to the right, jump backwards from the window.

Scaramech 10/10:
Now jump back to the window, and climb to the top, as soon as you're able to stand again you should see the last Scaramech of the episode infront of you.

Out of the friend Zorn
Beat a Zorn in under 120 seconds
You will have to shoot his right hand first, once it is on low health he'll try to hit you and his hand will be stuck in the ground, at that point shoot his hand again with a full charge of the Spammer and he'll tear his hand from his shoulder. Repeat the process for his other hand except this time it won't be torn off but instead he will show you his core that is located in his chest, shoot it with a full charge again and then be prepared for some quick time events. If you feel like you took too long you can always load the last checkpoint.

Jail break-in

Focus Boost 1/4:
As soon as you encounter the Security Drones, hide behind the passage on the right and wait for the drone to go past you. Once the drone goes past, quickly run to the next passage on the right to find the Focus Patch straight ahead of you on the ground.

Mnesist Memory 1/6:
After defeating the Strangler Leapers, climb the wall where it says 08 on the wall. Once you climbed and shimmied your way to the top, you’ll find the Mnesist Memory in the back of a corner at the end of the hallway.

SAT Patch 1/1:
After getting the Drone Detection Module, you will spot a white digital monitor. Follow the drone to the right but at the first corner where the drone goes to the left, go to the right and you'll see the SAT Patch on the ground.

Scaramech 1/10:
Right after the cut-scene where you exit a prison chamber, turn around and go towards the Security Drone that isn't moving, go close to it and shoot the Scaramech that is hiding on the wall in the distance.

Scaramech 2/10:
In the same area, continue towards the right until a prisoner cries out from her cell. Right before you reach the locked door, look over the railing to the left to spot a Scaramech below.

Mnesist Memory 2/6:
Before going through the locked door, move past it towards the end of the hallway, in the last open prison chamber lies the next Mnesist Memory.

Scaramech 3/10:
Now go through the locked door, and straight ahead of you on the ceiling is a Scaramech.

Scaramech 4/10:
Right after destroying the above Scaramech, continue down the stairs and follow the first Security Drone but stop and look up towards the upper left of the wall to spot and quickly shoot the Scaramech before the drone returns.

Focus Boost 2/4:
After destroying the 14th Scaramech, follow the same drone and then turn left to spot another drone in the area. You’ll need to sneak past it to find the Focus Boost on the ground.

I do like spam!
Finish 15 Enforcers off with the Spammer
This is a good chapter to focus on this achievement, whenever you are in a fight try landing the finishing blow with the Spammer.

The shield is down!
Destroy 20 Heavy Enforcer shields
Now you're introduced to Heavy Enforcers shoot a fully charged shot and it will break their shield. Try doing this everytime you encounter a shielded enemy.

Mnesist Memory 3/6:
After defeating the Heavy Enforcer, open the door on the right and inside the room is the next Mnesist Memory.

Fuzzy logic
Hit 5 enemies at once with a Logic Bomb while they are stunned by the Sensen DOS
When you are introduced to the Logic Bomb power there will be 6 Enforcers in the room with you. Make sure you have two Focus Boost bars ready, and then use the Sensen Dos and Logic Bomb right after. You can retry checkpoint until you get it.

Scaramech 5/10:
After you unlock the Logic Bombs ability, Go through the door, but turn around and you should see a Scaramech hiding in the corner on the right.

Scaramech 6/10 & Scaramech 7/10:
After the cutscene where you see Madame activating the Brain Drainer device. Hide behind the pillar and then look for a Scaramech on the device. There are two Scaramachs on the Braine Drainer, one will be below one of the lasers and another will be right above a laser.

Focus Boost 3/4:
After escaping the Brain Drainer device, exit the cell you came out of and go to the right. Sneak past the Security drone and the Focus Boost will be in the corner.

Scaramech 8/10:
After defeating the Enforcers and the Seraphim Robots, continue by going up the first staircase then go to the left corner to spot the Scaramech on the wall.

Focus Boost 4/4:
After destroying the Scaramech, go through the door and immediately turn left to see the Focus Boost in the corner.

Mnesist Memory 4/6:
After stealing Madame's memory, open the first door, then open the second door that is right infront of you, grab the Mnesist Memory off the table on your right.


Machines do it better
Use the Sensen RIP to make robots knock 20 human enemies out
Once you learn the Rust in Pieces ability, you can start progressing towards this achievement, try using it on every robot you encounter.

Mnesist Memory 5/6:
After learning the Rust in Pieces ability, go through the right door and follow the path to the right, and then grab the Mnesist Memory off the table.

Scaramech 9/10 & Scaramech 10/10:
Continue until you activate a Remembrane that shows 2 identical Madame's at the same time, they both go to different rooms where a Scaramech is hiding on the ceiling, be sure to get both of the Scaramechs before going through the door.

8-hit wonder
Perform 40 8-hit Combos
You'll now unlock the 8-Hit combo, whenever you have a chance try working towards this achievement.

The wheat and the chaff
Beat Madame without hitting any of her clones
After you deplete half of Madame's health, she will start cloning herself. You are not allowed to hit a clone, so you will have to look at all the Madame's and spot the one that is moving a little bit, the real one also doesn't have her right hand on her hip but just loosely hanging.

Mnesist Memory 6/6:
After defeating Madame, ignore her and turn around and run towards the left corner to find the final Mnesist Memory of this episode.

In the Forlan memory remix: have Forlan get the gun, argue with Alexia but not kill her

Focus Boost 1/3:
After defeating the Leapers, climb the scaffolding (not inside the red room) to grab the Focus Boost on the ground.

Scaramech 3/10:
After collecting the Focus Boost continue with the main path until Edge contacts Nilin. Then make a left turn into a narrow corridor, you'll see a small fan where you’ll be able to spot the Scaramech on the wall behind it.

Mnesist 1/6:
After you lift an elevator and use it to climb up to a platform, jump over to the other side, and then immediately go to the right to find the next Mnesist Memory.

Focus Boost 2/3:
Avoid the Security Drones detection cones and once you make it to the third drone at the end, you’ll need to shoot the gate open with the Spammer to get past the drone, but instead of continuing wait for the drone to pass, and then enter the gate you just opened and the Focus Boost will be on the ground.

SAT Patch 1/3:
After a short cut-scene showing the flooded area, drop down and follow the red pipe to a balcony.

Scaramech 4/10:
Once you see a sign that says "Jax" climb to the other side of the sign and then shoot the Scaramech that is hiding behind it.

Focus Boost 3/3:
Right after you go past a white digital monitor, you will see a graffiti of a guy holding a baton, climb up the ledge and now you can go either to the left or right, follow the right path to find a balcony that contains the Focus Boost.

SAT Patch 2/3:
Before opening the orange door, turn around and climb up the red pipe, follow the path towards the left to find the SAT Patch on a table.

Mnesist 2/6:
After defeating the first Elite Enforcer (The one with electrified armor) immediately open the glass door in the same hallway, before going through the orange looking door.

Scaramech 5/10:
Once you're at the top of the crane, activate it and you will see a big billboard, and next to it a Scaramech.

Mnesist 3/6:
When you are being chased by Captain Trace, you'll fall through the scaffolding, as soon as you can move again, turn around and grab the Mnesist Memory.

Scaramech 6/10:
Right after you grab the Mnesist Memory, jump over to the small bridge then turn around and shoot the Scaramech that is hiding below the platform that you were on.

Scaramech 7/10:
After defeating the 3 Enforcers, go through the door and the first door on your right will be half open, and the Scaramech is hiding within.

Scaramech 8/10:
Once you reach the rooftop and you are hiding behind some bricks, the Scaramech is hiding behind the 3rd cover you can use.

SAT Patch 3/3:
After stealing Captain Trace’s memory, go slightly to the right and you should spot some white pipes that you can climb, once you climb up you'll see red pipes you can use to go higher, follow them to the rooftop to the left, jump across the gap and you'll see the SAT Patch.

Scaramech 9/10:
Once you enter an apartment through a window, the Scaramech will be hiding near the plants on the left.

Mnesist 4/6:
After you collect the Scaramech, go through the locked door, then take the first left passage in the hallway, right as you go through the blue doors, the Mnesist Memory is in the corner.

Mnesist 5/6:
Once you enter another apartment building you will see a locked door infront of you, instead of opening it, go to the left and take the Mnesist Memory in the dressing room.

Scaramech 10/10:
After solving the door puzzle (The code is: 2058 ) you’ll get a contact message from Edge. While moving forward, take a look at the 3rd light pole on your left side, a Scaramech is hiding behind it.

Mnesist 6/6:
After collecting the Scaramech, continue with the main path and avoid detection from the Security Drones, you will see two open doors on the opposite sides of the hallway, enter the door with the hologram in the middle and behind it is the Mnesist Memory.

When you are moving the belongings to the left, don't search Bad Request's locker, but keep moving the lockers to the left until you find Nilin's locker.

Scaramech 2/12:
After getting the Focus Boost, open the door with the Pick-Socket ability and enter the office on your right. The Scaramech will be on the wall near in corner of the door.

Mnesist 1/5:
After you unlock the next room by activating the panel on the other side of the window, enter the room and hug the right wall to find the Mnesist Memory hidden away.

SAT Patch 1/3:
Once you go through another door that you had to open by shooting through the window, take the power from the newly opened door, and then go down the staircase on your left. Open the maintenance door to find the SAT Patch on the table.

Can't touch this
Defeat 5 Elite Enforcers without using Combos or Spammer attacks
You'll now learn the Sensen Camo ability. It makes you able to kill an Elite Enforcer (Their armors are electrified) in 1 hit. You have to do that 5 times (without having damaged him beforehand).

Mnesist 2/5:
After learning the Sensen Camo ability, take the power from the ladder and open the door to the left of the ladder. Follow the path and you'll see the Mnesist Memory on the table.

Scaramech 3/12:
Now you will have to power up an elevator, exit the elevator to the right side and take the power from it, now go down the hallway until you see a locked cell that you can open with your power. Enter the cell and look at the ceiling above the door to see a Scaramech.

Scaramech 4/12:
Right after collecting the above Scaramech, exit the cell and go to the right towards the end of the hallway, look over to the other side to see a Scaramech hiding behind a pillar.

SAT Patch 2/3:
Once you destroy the Cell Dispatch System, turn around and follow the path to a small hidden corner where a SAT Patch is hiding.

Scaramech 5/12:
Shortly after the SAT Patch, you will move a cell down, and jump ontop of it and it will fall slightly due to Nilin's weight. Now turn around and jump over the gap to find a Scaramech hiding in the corner.

Scaramech 6/12:
After getting the above Scaramech, jump back over the gap, and then to the other side. you'll see a small floating drone, stand next to it and look to the left to spot another Scaramech.

Scaramech 7/12:
After activating a Remembrane you will see a doctor escort Bad Request, synchronize with the doctor to extract power from the machine, don't use it on the Medical Chair, instead go back where you came from and open the cell at the end of the hallway. Inside is a Scaramech hiding on the ceiling.

Focus Boost 2/2:
After defeating the Enforcer and Leapers, activate the Remembrane and take the energy once again. Now use that energy to open the door at the end of the hallway on your right. You’ll find the Focus Boost on a box inside.

Scaramech 8/12:
After collecting the Focus Boost, take the energy and place it into the medical chair and follow the chair down the passage. Once it passes through the security door look above the doorframe to find the Scaramech.

Scaramech 9/12:
After defeating the Heavy Elite Enforcer and his two Leapers, look for a cleaning robot on the same level. This cleaning robot has the Scaramech stuck behind it.

Mnesist 3/5:
After a brief cut-scene you will see a table infront of you with the Mnesist Memory.

Mnesist 4/5:
In the hallway where you are getting flashbacks, walk to the end of the hallway to see Nilin's cell. Enter it and take the Mnesist Memory.

SAT Patch 3/3:
After you extract the power from Nilin's Cell and use it to open the next door, as soon as you enter the door enter the room on the right side, the SAT Patch will be near the corpse.

I know kung fu
Equip 17 Pressens at least once in your Combos
If you've been following the guide, you should be able to unlock this achievement now.

Scaramech 10/12:
After defeating all the enemies in this area, Nilin will contact Edge, walk towards the big machine with the chair in the center and look on the left side of the chair to spot a Scaramech.

Scaramech 11/12:
Raise the next two metal shutters and then look towards your left. You’ll see the Zorn that chased you in the very beginning of the Prologue episode. Destroy the Zorn with a shot from the Junk Bolt, then shoot the Scaramech that was hiding behind it.

The Zorn is located near the end of the episode, it's in the area where you have to raise two metal shutters and it is required to be destroyed to get a Scaramech that is hiding behind it.

Scaramech 12/12:
Now raise one more metal shutter, go all the way towards the end of the hallway but look towards the right to spot a small ventilation shaft, inside is the last Scaramech of the episode.

Mnesist 5/5:
After reaching the bottom of the opening that the Doctor pushes you down, you will see Bad Request lying on the ground, but ignore him and turn around to see a weak structure you can destroy, follow the path towards a fan to find the Mnesist Memory.

Leader of the pack
Beat a Mourner without beating its supporting sidekicks
Now this will be a little tough if you're on the hardest difficulty, put as many power pressens into your 8-hit combo as you can, and make your 3-hit combo into only regen presens. Now hit the leapers with the 3 hit combo until you have a focus bar ready. Use the Sensen DOS to reveal the Mourner and then shoot him with a fully charged spammer shot, and then hit him with your 8-hit combo. If you don't deal enough damage you can try to damage a few more leapers with a cooldown combo to get the Sensen DOS again.

Dropping L-Bombs
Trigger two Logic Bombs in less than 15 seconds
This can be done at the very start of the episode. First edit your YYYXXXXY combo so it has as many cooldowns and chain pressens as possible. You can also make your standard XXX combo into only regen pressens so you can spam that to build up your focus. Once you have two focus bars ready, Drop a logic bomb and then use your cooldown combo as quick as you can and as soon as the second logic bomb is ready, drop it as well.

Serve the servant

Around this point you should unlock this one, don't worry if it hasn't there will be plenty more robots to come.

Scaramech 1/11:
After defeating the Zorn, shoot the ladder and then climb it, once you're up look over to the left to spot a Scaramech.

SAT Patch 1/4:
After destroying the above Scaramech, go through the doors and down the stairs, then turn left to find the SAT Path underneath the stairs.

Scaramech 2/11:
After making it across to the other side of the platform with the use of a cart, descend down the pipe, then jump the gap. Now head all the way down and turn right to find the Scaramech behind some crates.

SAT Patch 2/4:
Now, this SAT Patch can be a little tricky. When you see the white digital monitor, use the Spammer to shoot the panel to call the cart. Ride the cart until you're close to the caution sign on the wall, stop the cart then jump and grab the ledge on the wall. Follow the path until you find a SAT Patch behind some crates.

Focus Boost 1/2:
After exiting the warehouse, Edge will contact Nilin. Make a turn to your immediate right then make a left in this hallway corridor to find the Focus Boost on a table.

Scaramech 3/11:
After defeating the two Heavy Elite Enforcers, run all the way down the hallway to your right and look at the left corner near the couch to find the Scaramech.

Mnesist 1/5:
After collecting the Scaramech from the corner, run a little towards the left of the hallway until you can shoot a door open on your right. Enter this door and go into the next room to find the Mnesist Memory on a table near the windows.

Scaramech 4/11:
After seeing a brief cut-scene of Trace running away (White enforcer). Run towards the area where the escalators are but do not go down, look at the nearby corner, next to the broken store window for the next Scaramech.

The fallen
Make 50 Leapers fall off walls
If you haven't unlocked this one, this is a good place to finish it. There will be a bunch of leapers that you can shoot down until you get the achievement.

Scaramech 5/11:
After defeating the Leaper enemies, exit through the locked door and go down the first set of stairs, then look towards the right and you’ll find the Scaramech in the corner of the wall near the ceiling.

SAT Patch 3/4:
After the scene where Captain Trace runs away from Nilin, avoid the Security Drones detection cones and go towards the left side of the hallway and enter the office you see. Inside the office is the SAT Patch lying on a chair.

Mnesist 2/5:
You will see Trace next to a bar and he'll run away again, don't follow him but instead go to the left. Wait for the Security Drones to pass by then run up the left platform to grab the Mnesist Memory off the table.

Scaramech 6/11:
After collecting the Mnesist Memory in the disco bar, turn around and face the piano in the center. You should be able to spot the Scaramech on the piano.

Scaramech 7/11:
After defeating the waves of Enforcers and robots, open the door with the Spammer and look to your immediate right underneath the glass fire door is the Scaramech.

Scaramech 8/11:
After collecting the Scaramech from fire door, open the door to your left and avoid the Security Drone’s detection cone as you make your way to the left corner. In the left corner of this hallway you’ll be able to spot the Scaramech on some debris.

Scaramech 9/11:
Once you go through another door, turn around and shoot the picture frame next to the door to reveal another Scaramech.

Mnesist 3/5:
After a short cut-scene make your way down to where Trace is, but ignore him and climb the two crates that you see to on the left. These crates lead to a balcony that contains the Mnesist Memory lying on a table.

Scaramech 10/11:
After you see the Mourner Leapers run through a door, follow them. The Scaramech will be right infront of you on some debris, hard to miss.

Once you are back where Trace is. Before activating the ladder, open the fire door in the same area, and follow the hallway to find the last Focus Boost.

Mnesist 4/5:
Now go up the ladder and through the door infront of you, the Mnesist memory is on the table ahead.

SAT Patch 4/4:
After collecting the Mnesist Memory, open the door to your right, then follow the Security Drone until you see another door on the right. Enter it and hug the right wall to find yet another door that leads to a bathroom. Inside is your last SAT Patch.

A brief cut-scene will happen, before activating the Remembrane, go below the staircase on your left to find your last Mnenist Memory.

Scaramech 11/11:
After collecting the final Mnesist Memory, go up the stairs on your right, then look up at the pillar to spot the Scaramech near the top of the pillar.

This one is a little tricky. Once you are in the server-maze, follow the path until you hear Nilin’s Remembrane say “That’s enough! Now give me your hand.” You’ll find a new path opened immediately behind you and the Scaramech can be spotted on the wall high up on the severs. (If you see the exit of the maze then you are going too far.)

Fury lover
Reach x14 with the Sensen Fury
While you have plenty of opportunities to do this one, I put it here because it will most likely come naturally when you deal damage to the boss since it auto activates the Sensen Fury.
