I point to these two endings in particular because they both showed signs that something about them is amiss, as if they're not quite what they seem. The bad ending clearly displays what happens to all three of them, as well as the Anna ending. No pieces of dialogue or scene transitions seem to imply anything further - from what I could notice - unlike the other two endings. I'd love to know what anyone else has to say on the matter, though. How you feel about my interpretations and what your own ones are. I look forward to reading any.
Now, with the happy ending, Jack implies he brings the knife to his neck and is about to end himself. Although the dialogue in the scene that follows it suggests they moved past everything and are simply together on some sort of vacation afterwards, it could also be inferred that what we're seeing here is nothing more than a heaven created by the deceased Jack; his soul wishing for the best reality he couldn't successfully create in life. It would make sense based on how the scene transitioned from his attempt at suicide to all three of them suddenly being healed and accepting of one another. Yes, it's implied that time has progressed and that they're trying to move on, but again, it doesn't have to mean this.
So, in Grace's ending, there's a point right at the end where she suddenly disappears as Jack is talking to her. I think this is telling us that she died in the hospital and what we're seeing of her was Jack pretending she was there in order to cope. The text briefly before and after her sudden disappearance do seem to imply something, and I personally believe this is what it is.
