Introduction
This is a complete guide for obtaining every achievement currently available in World of Horror, with some tips on how to get each one.

01/30/2025 It only took five years, but this guide that was once complete is now complete again. It's done! At least until Panstasz adds more achievements again!

This guide is packed full of spoilers, so reader beware! Use the Table of Contents to jump to a single achievement if you don't want to spoil anything for yourself.

Big addition: You can now reroll mysteries while in-game! Look at your bulletin board and at the top left of the screen, enjoy your new button. Please keep that in mind - during this guide I will say many times to restart your run if you don't have <x> mystery. No longer a requirement!
Chain Smoker
Have 30 Cigarettes in your inventory at once.

This one is pretty easy. Start an Easy difficulty game as Haru (the Yakuza guy), go into the change clothes room and click the little toy thing in the top left to get 1 free FND, and go to the shop. Simply keep rerolling stock/going into and out of the shop until you can buy two more units of Cigarettes, then "use" all three in your inventory (since you start with one).

Unlocks Foreign Cigarettes.
Junior Priest
Purify 2 locations in one playthrough.

With a Holy Candle in your inventory, certain random events can call for using it to purify a location. You only need one Holy Candle; it is not used up when purifying a location. The places I found to purify were in the Mansion during the VICIOUS VERSES OF A VIOLENT VIGIL case, and one random location while investigating the School. There are other events/places that you can purify, those are just the two I recall.

This unlocks the Monument location, where you can trade STAMINA to lower DOOM. Thanks to General Charm for the tip!
Paranoia Agent
Defeat an enemy with the Baseball Bat.

This achievement was bugged in the launch version, but it's now working properly. Simply get the Baseball Bat weapon from the Apartment Stalker (you will almost undoubtedly get this event during the ALARMING ACCOUNT OF ABNORMAL ARMS case) then bludgeon someone to death with it.

Unlocks the Flare Gun.
Peeping Tom
Find the hole in the painting.

During the FAR-OUT FABLE OF A FEAR FESTIVAL case, you can inspect the painting in your room - you must click the actual painting to unlock the option to check it out. You will find a hole and unlock this achievement, and can also move a step further later to find the Karukosa Mask needed to get the Perfect Disguise achievement.

This unlocks the Glass Eye item, which when used gives +1 PER and a random effect.
Perfect Disguise
Enter the Overgrown Factory while wearing a Ritual Robe and Karukosa Mask.

This is easiest to get after you've unlocked the Occult Shop. Buy a Ritual Robe, then do the FAR-OUT FABLE OF A FEAR FESTIVAL case. When in your room at the Inn, you can inspect the painting - you must click the painting in your room to unlock the option to check it out - to find a peephole (also unlocking the Peeping Tom achievement). After this, from the overworld you can choose to explore the Staff-Only area of the Inn, which starts a battle with the perverted innkeeper. Once you defeat him he will drop the Karukosa Mask, which, when worn with the Ritual Robe, will grant you the achievement once this case is over. You can also rarely get the Karukosa mask from the weird statue event while investigating the School.

Unlocks the MIDASU TOUCH spell, which costs 2 STAMINA and gives +1 FND.
Frequent Buyer
Shop for items ten times in one run.

Another easy one. Simply enter, leave and re-enter the shop ten times in one playthrough.

Unlocks the Dog Treats item, which when held lowers all shop prices by 1 FND.
Junior Scout
Investigate 20 locations with a Compass equipped.

Another fairly easy one. Buy a Compass as early in the playthrough as possible and just have it equipped while you play your game, and you'll unlock this.

Unlocks the Map item, which has a chance to lower DOOM while exploring, and is also usable in certain events.
House of Paper
Take a hostage as Miku while wearing the Tokyo outfit.

Playing as Miku, put on the Tokyo outfit and go raise your DOOM rating to 90% or so. Miku's Mayhem perk means that her stats are higher based on the current DOOM level. Simply intimidate a follower in the Schoolyard into following you to unlock the achievement.

This achievement now works, and unlocks the Extra Ammo item, which grants 3 AMMO when used and costs 4 FND.
This Is For My Friends
Defeat the Mad Janitor as Aiko.

Start a game as Aiko, and if the MACABRE MEMOIR OF MORBID MERMAIDS case isn't in your list of cases, restart until it is. Simply play the case normally, and when you defeat him the achievement is yours.

Unlocks Ikumi, a follower.
Wait For It
Prepare an attack during combat, but do not attack in that round.

Seems a little buggy, but I ended up getting it after attempting it several times. Use the Prepare Attack action and nothing else, not even another Prepare Attack. Make sure the turn is 1 Prepare Attack and nothing else.

Unlocks the Pocket Knife, which allows you to escape from combat once with no DOOM penalty.
Team Kill
Backstab a follower.

Very easy achievement to get if you've already unlocked the Witch's Tree location. Start a game as Mizuki Hamasaki (since she starts with a follower) and go use the Witch's Tree; if you don't end up getting the Paranoia curse from the Witch then restart until you do. With this done, select your follower in the menu (make sure the Kill option, which is not clickable, is lit up): click Let Go and you'll kill your follower and get the achievement.

This unlocks the ASHEN CONTRACT spell, which costs 7 DOOM but instantly kills any non-eldritch enemy(!!!!) and can only be cast once. Special thanks to TheOneWaaagh for figuring out how to get this one.
Say Hello
Meet your _other_ neighbor.

I actually just unlocked this while typing this very guide. Simply leave the WOH window unattended for some time while you're in your own apartment, then click it back into focus. On my Win7 machine, fullscreen crashes big-time when alt-tabbing, so it's best to try this in windowed mode.

Unlocks the Bloody Sweater outfit for Kouji.
True Idol
Have 4 followers at once.

Very easy achievement to get. Go to the Schoolyard and simply ask or threaten people to follow you.

Unlocks the SEAL OF SAVVESH spell, which costs 4 REASON and gives +1 STRENGTH permanently.
Classy Ritual
Sacrifice a follower with the Ritual Dagger equipped.

This is easiest done in the HORRIBLE HISTORY OF HOUSEHOLD HELL case. Make sure you have at least one follower, a Flashlight and the Ritual Dagger before starting the case, and play it normally. At the very end, make sure to choose the EXPLORE FURTHER option, set the radio to the frequency noted during the investigation (make sure to pay attention!), then during the battle choose to sacrifice your follower to unlock this achievement.

Unlocks the Tome of Rot, an Item which, when used, deals 8 damage to HUMAN enemies but gives you a new status/injury.

Peeking Tom
Defeat a school wraith haunting the corridor.

During the SPINE-CHILLING STORY OF THE SCHOOL SCISSORS case, in one of the screens you can choose to "Peek Outside" and slide a door open - and behind it is a fun, friendly fellow. Open the door fully to earn this achievement. Thanks to Faceless for the tip.

Unlocks the SEAL OF BRAM'EL, a spell that costs 4 REASON and grants +1 KNOWLEDGE.
Awoo
Call Junko.

Before choosing a new case, go to the Watch TV room and dial 56709 (a lyric from a Junko Ohashi song "Telephone") to unlock this achievement.

This unlocks the spell THREAD OF FATE, which costs 4 REASON but instantly kills a human enemy. Thanks to General Charm for the info on this one!
Paid in Blood
Have a follower die in combat.

When you have a follower, there is an option in Support Actions to have your follower attempt to distract the enemy. This action has a chance to kill the follower, so simply do that until your follower dies.

Unlocks Ryuichi, a follower.
Exorcism
Exorcise a ghost.

Find a Ghost enemy, and use the bow-clap prayer thing to exorcise them. This one is random and while it's not hard, it's kind of a pain to do. I found a Ghost enemy while investigating the School; there is an event where you enter a bathroom and can choose to leave, or close a stall door. Close the door and you'll be attacked by the Toilet Ghost. Thanks again to General Charm for getting me on the right track.

This unlocks Pancakes, a usable item that seems to only be available from the souvenir shop during the FAR-OUT FABLE OF FEAR FESTIVAL case. It costs only 1 FND, and gives +2 STAMINA and -2% DOOM!
Scholar
Research five spells in one playthrough.

Easy achievement. Go to the School Library and simply research five spells.

Unlocks the Army Knife, a PER weapon - which is now equippable!

Transfer Girl
Beat the game as the first character, Kirie Minami.

Unlocks the Eldritch Amulet, an equippable item.
Fit Girl
Beat the game as the second character, Aiko Takahashi.

Unlocks the Cursed Cartridge item. Worn in Slot C, it makes thrown items deal extra damage.
Thug Survivor
Beat the game as the third character, Haru.

Unlocks the Hirie outfit for Kirie Minami.
Idol Survivor
Beat the game as the fourth character, Mizuki Hamasaki.

Unlocks the Private School outfit.
Reporter Survivor
Beat the game as the fifth character, Kouji Tagawa.

Unlocks the Samurai outfit for Haru.
Student Survivor
Beat the game as the sixth character, Mimi.

This unlocks yet another Samurai costume, but again I'm not sure who it's for.
Chaos Survivor
Beat the game as the seventh character, Miku.

As of 0.9.84, this now unlocks Fuel.
Bad Luck
Roll a 1 during an event stat check.

Trust me - this is going to happen eventually.

Unlocks the Hermit, a follower.
From The Brink
Defeat an Old God with over 90% DOOM.

Unlocks the Bone Saw, a weapon.
Need No Education
Burn down the school.

During the battle at the end of the MACABRE MEMOIR OF MORBID MERMAIDS case, in Defensive Actions there is an option to topple a lamp. This will net you this achievement.

Unlocks the Leather outfit for Haru.
Boop
Surprise the dog.

When you're shopping, click that dog's nose and he'll bark at you, unlocking the achievement.

Unlocks the Innocent Student outfit for Mizuki.
World Saved I
Beat the game once.

Unlocks Endless Mode.
World Saved II
Beat the game twice.

Unlocks the playable character Mimi, AKA Best Girl.
World Saved III
Beat the game three times.

Unlocks the WORLD OF HORROR EXTENDED A card pack, which adds 5 more events to the game.
World Saved IV
Beat the game four times.

Unlocks the WORLD OF HORROR EXTENDED B card pack, adding a further 10 new events to the game.
Painted Hospital
Win your fifth playthrough.

Unlocks the WORLD OF HORROR EXTENDED D card pack.

Note that this is a new achievement and it's sorta retroactive - if you've beaten the game more than four times, the next time you successfully finish a playthrough you'll unlock this.
Flooded Nails
Win your sixth playthrough.

Unlocks the FULL card pack.

Win your seventh playthrough.

Unlocks ZHECTAST, an old god.
World Doomed I
Die due to REASON.

Trust me, this is going to happen to you sooner or later.

Unlocks the Pain Medication item.
World Doomed II
Die due to STAMINA loss.

This, too, is going to happen sooner or later.

Unlocks the Experimental Drug item.
World Doomed III
Die due to 100% DOOM.

This, too, is inevitable.

Unlocks the Gruesome Totem item, which lowers DOOM by 10% but take 3 off your max REASON.
Eldritch Backstory I
Beat a game while using the WORLD OF HORROR backstory.

The first time you beat the game, you're also going to get this achievement.

Unlocks the MEDICAL HISTORY backstory.
Eldritch Backstory II
Beat the game while using the MEDICAL HISTORY backstory.

This unlocks the HUNTED BY THE CULT backstory.
Eldritch Backstory III
Beat the game while using the HUNTED BY THE CULT backstory.

This unlocks the SEVENTH CURSE backstory.
Eldritch Backstory IV
Beat the game while using the SEVENTH CURSE backstory.

This unlocks the ILL-FATED backstory.
Eldritch Backstory V
Beat the game while using the ILL-FATED backstory.

Unlocks the WORLD OF HORROR EXTENDED C card pack, which adds 10 more events.
Eldritch Backstory VI
Win a playthrough using the KNIGHT ERRANT backstory.

Unlocks the SCARS backstory.
Eldritch Backstory VII
Win a playthrough using the SCARS backstory.

Unlocks the CURIOUS BIRTHMARK backstory.
Eldritch Backstory VIII
Win a playthrough using the CURIOUS BIRTHMARK backstory.

Unlocks the ELDRITCH PARASITE backstory.
Eldritch Backstory IX
Win a playthrough using the ELDRITCH PARASITE backstory.

Unlocks the EXQUISITE TASTE backstory.
Eldritch Backstory X
Win a playthrough using the EXQUISITE TASTE backstory.

Unlocks the FATALIST backstory.
Eldritch Backstory XXX
Win a playthrough using the FATALIST backstory.

Unlocks UCHIDA, a new ally.
Soft Side
Buy, then return an expensive drug.

When investigating Downtown, you will occasionally be attacked by a mostly peaceful masked female rioter (the chances of this are much higher if you've triggered the RIOTS Old God effect, but it's not required). When defeated, she drops a Prescription item, which can be used at the Pharmacy along with 4 FND to get a drug. Take this drug to the Illegal Den to unlock this achievement.

Unlocks Miku, the Rioter as a playable character. Thanks again to General Charm!
Blood Deal
Feed an Outer Entity.

Once you've unlocked the Monument location, you can trade STAMINA to lower DOOM, and the first time you do so you will unlock this achievement.

Unlocks the Camp Dog, a follower.
Shiokawa History I
Unlock 15 achievements.

Unlocks the City History option, which will let you rearrange (to a point) how the town is laid out.
Shiokawa History II
Unlock 20 achievements.

Unlocks the Shop (Occult) location, whose stock is mainly items like Holy Candles, Ritual Daggers/Masks/Robes, etc.
Shiokawa History III
Unlock 25 achievements.

Unlocks the Illegal Den, a shop whose stock is usually liquor, cigarettes or weaponry with the [ILLICIT] tag.
Shiokawa History IV
Unlock 30 achievements.

Unlocks the Witch's Tree location, where you can take on a curse effect in trade for STAMINA, a SPELL or FND.
Shiokawa History V
Unlock 40 achievements.

Unlocks SCHOOL NURSE, a new "location" you can add to the SCHOOL and who will heal your minor injuries for free. Note that this does not mean she'll refill Stamina or Reason, but will remove Minor Injury cards.
Shiokawa History VI
Unlock 50 achievements.

Unlocks HISTORY CLUB, another location you can add to the SCHOOL. Here you can trade DOOM for new perks - but don't get greedy...
True Despair
Beat the game on Hard difficulty.

Note that the difficulty level to unlock this achievement is actually called True Believer.

Unlocks SMALL CANDLE.
Final Stand I
Reach turn 30 in Endless mode.

Not much to say about methods to get this one - play a character you're good at (Mimi, with a Ritual Dagger, stacking KNW with every level up is pretty broken), hope for good RNG and remember to mitigate that DOOM meter as much as possible.

Unlocks the SHADOW SHROUD spell, which costs 3 REASON and for the duration of the current mystery ignore a location's THREAT level.
Final Stand II
Reach turn 40 in Endless mode.

Keep fighting - if you managed to get to Turn 30 in decent shape, you can make it to turn 40!

Unlocks the ANCESTRAL STRENGTH spell, a ritual that costs 4 REASON and gives +1 damage while active.
Final Stand III
Reach turn 50 in Endless mode.

Pretty hard to get while keeping DOOM from ending your run, but with the right spells and items you can get there.

Unlocks the Hunting Rifle (PER), the most devastating weapon in the game.

DAMAGE: 5
SPEED: 120
CHANCE TO HIT: 80
(damage when shot: 16)
Mira
Solve the Blue Gem riddle.

This one is actually really neat, and the payoff is super worth the pain in the butt it is to get, so maybe consider not spoiling it for yourself...

Go to the dog's store and choose to buy a random discount item, and sooner or later you'll end up with the Blue Gem. With this item in your inventory, explore the Seaside until you've see an event called LIGHTHOUSE RUMOR. Pay very close attention to the words "Blue Gem" in your inventory; they should be shaking very, very slightly. After this, explore the School and pay very close attention (if you're like me and have seen every event, you may be clicking through very quickly): you'll find an event called EMPTY CORRIDOR (OR IS IT?), and the words "Blue Gem" in your inventory will begin to vibrate again. Once this event is over, explore the Forest, noticing that the words "Blue Gem" are vibrating with every event you see, and eventually you'll get your secret ending.

Unlocks the Attack Dog follower. Thanks to everyone for collaborating on this one, but extra special thanks to nothingxs (whose info I got from Petshame's guide) - that's what got me on the right track.
Good Old Times
Equip Carpenter Hammer with both EXPEL EVIL and REGENERATION learned.

This one is easy and is entirely dependent on RNG. Start a game on Easy/Medium as Kirie, and if you're lucky you already know one (or even both!) of the spells you need. Go to the Hardware Store, buy and equip a Carpenter Hammer, then go to the Library and simply trade DOOM to learn spells.

Unlocks SWOLLEN JOINT, a new injury you can randomly receive from events. Enjoy!
Primum Non Nocere
Defeat Dream Devourer without killing a single patient.

This is an easy achievement to get. Start a new game, restart if you don't have CURIOUS CASE OF A CONTAGIOUS COMA in your case list, and do whatever pre-case setup your normally do for your chosen character, whether that's buying new weapons, learning new spells, whatever. When doing the case, you must do the side-quest to investigate the Village twice, as you will receive an item critical to the ending. Play through the rest of the case, and at the end, you are given the option to use the Dust of Seeing to spot the Dream Devourer, which you must then defeat in combat. When that's done, the achievement is yours.

Unlocks Maria, a new ally.
Something Evil
Meet and defeat SOMETHING EVIL.

While investigating certain locations, you have a chance to trigger an event that mentions SOMETHING EVIL, and a small counter will appear at the top of your screen that shows a spooky face and a counter which shows how many SOMETHING EVIL encounters you've seen. There are several, but the following, all located in the Apartments, are what triggered this encounter for me:

MAIL DAY (choose to read your mail)
UNFINISHED PAINTING (I ran into this twice)
STRUGGLING ARTIST (choose to look at the photo).

You can also increase your SOMETHING EVIL meter by 1 every time you use the REST function in your house, which appears to be the easiest way to raise your meter. Note that you must have at least 1 on the SOMETHING EVIL counter for this method to count.

After getting my SOMETHING EVIL counter to 4, I continued exploring the Apartments (note that your character's cute little chibi figure now has a scary face) and eventually ran into SOMETHING EVIL itself. This is a combat where you have only two combat options: sacrifice STAMINA or sacrifice REASON. SOMETHING EVIL will also attack. Once you've sacrificed roughly 10 resources (of any mix; I sacrificed 8 STAMINA and 2 REASON) a new option will appear in Combat Actions, which will end the combat and grant you your achievement. Thanks to Nintendocat for the hints!


A final note on this: you can increase your SOMETHING EVIL counter above 4, presumably to a theoretically unlimited number. I tested this twice and ran into the combat encounter at 4 both times, but some Discord users report that they didn't get it until 5 or even more. I suspect that the counter is actually a stat similar to "magic find" in Diablo-type games, where the higher the number, the more likely you'll encounter the combat, until it is virtually guaranteed. Food for thought, anyway.

Unlocks WITNESS CURSE, a new ritual spell.
Sweater Girl
Find the missing girl.

Very easy achievement to get. Note that you must have completed World Saved IV (complete the game four times) and unlocked Extended Card Set B before you can claim this achievement. Start a game on Easy with the new Ill-Fated backstory, which grants you an immediate level-up, and make sure that you have WORLD OF HORROR EXTENDED CARD SET B enabled - the required event will not show up unless you do. Level up and reroll perks until you can choose the Outdoorsman perk. Then go into your TV room and enter, leave and re-enter until the TV is showing a news story about a girl who went missing around the SHIOKAWA area.

From there, simply investigate the Forest until you find a bloody sweater and your new achievement. Unlocks Moriko, the new character.

Otherworldly Thirst
Drink the blood of a forgotten beast and survive.

This one is pretty much all RNG, hoping that you get the right events. I was lucky and got it on my first try.

Once you've started, make sure you've got the ALARMING ACCOUNT OF ABNORMAL ARMS case, and complete it while making sure to do the side objective - you must have a Tiny Key. After you've got it, investigate the School until you get the Trophy Case event, and use your key to open it and claim a goblet. Make sure that this is the only container in your inventory! Continue investigating the School until you get the HALF OPEN event, which leads you to one of the three Otherworld locations. If you're lucky and got the UPPER KINGDOM one, there is a new option to fill the Goblet, and once that's done just chug it down for your achievement.

Unlocks Yashiro, the new (and very fun) Faith-based character.

We Don't Need Eyes
Destroy your eye and open the third one.

Another RNG achievement, and another one I got very lucky on. Simply go to the Occult Shop and buy a Glass Eye (note that you must have done the Peeping Tom achievement to unlock the Glass Eye for purchase), then use it to pop out your own eye and replace it with the glass one. Then go to the School Library and research spells until you get Third Eye. Casting it while you've got the Glass Eye in your head will grant you the achievement.

Unlocks YGOTHAEG, an old god.

Never-Ending Summer
Change the date and stop the curse.

When in any run, in any game mode, simply click the calendar on the top-left of the screen to change it from May to June 1984.

Unlocks MONKEY WRENCH, a one-handed STR weapon.
DAMAGE: 5
SPEED: 80
CHANCE TO HIT: 60
(HEAVY TOOL): -1 STR
Lightburn
Equip two sources of light at the same time.

Another RNG achievement, but very easy in Timeline B. Start a new game, and make sure to complete the side objective for the SORROWFUL SAGA OF THE MOONLIGHT SAILORS case, which is to discard (not sell) two items. When you complete the objective you will immediately receive a Torch, which can then be equipped along with a Flashlight that you can buy from the dog or the Hardware Store to complete this achievement.

Unlocks HEAVY-DUTY FLASHLIGHT, a 1-handed KNW weapon.
DAMAGE: 4
SPEED: 85
CHANCE TO HIT: 75
(counts as a light source)

Junior Librarian
Discard three books in one playthrough.

Yet another RNG one, and hoo boy this one sucks. I got it by playing as Best Girl Mimi and investigating the Mansion over... and over... and over... and over. Remember that when you discard a book, you must not sell it, but hit the Discard button.

Unlocks the Knight-Errant backstory.
Restless
Complete a playthrough without resting.

Easy peasy. Just don't use the REST command during a playthrough - baths don't count against this, so soak away.

Unlocks Challenge mode.
Onyx Medallion
Gather five curses.

An RNG one, you can hedge your bets to get 5 curses (at the same time) by using the Witch's Tree the maximum of 3 times. Then, explore the Forest until you've got all 5 required for the achievement.

Unlocks Ayaka, a new KNW-based character.
Club Member
Join the club.

Make sure you have the History Club selected (under "School") when you're setting up your game, then simply buy four items from them in the same playthrough. You'll get a neat ending (and be aware, your game will end there) and will unlock Toshiaki, a PER-based character.

Hail to the King
As Moriko, defeat an enemy with a working Chainsaw.

Start a new game as Moriko (the hunter's daughter you unlocked by doing the SWEATER GIRL achievement) and if you don't have the PERILOUS PARABLE OF THE PECULIAR PAINTING case in your list, restart until you do. Play as normal, but do another case before the PERILOUS PARABLE one, making sure that you pick up a Flashlight and have at least 3 FND available. Once you've started the PERILOUS PARABLE you will eventually get to a point where you can explore the WORKSHOP location. Having the Flashlight will enable you to choose to USE YOUR OWN LIGHT SOURCE, resulting in a found Chainsaw (sans fuel). After this case is over, go to the Hardware Store and reroll stock until you can buy FUEL for 3 FND. You can alternatively just buy Fuel before the case, of course. When you have both the fuel-less Chainsaw and the Fuel in your inventory, use the Fuel. Now you can finally live out your Doom fantasies and chainsaw an enemy to death, earning this new achievement.

Unlocks THE HERALD, a new Old God.

Note that the PERILOUS PARABLE OF THE PECULIAR PAINTING case is extremely buggy at the moment, at least for me. I tested this about five times, and each time I chose not to stay with the character you meet during this case my game hard-crashed to the desktop, but if I chose to stay with that character the case went fine. Something to keep in mind until a bugfix.
Real Nightmare
Defeat 3 Dream enemies in one run.

Very easy to get; I landed it on my first try. Choose whichever character you're best with and get your gear (I chose Mimi and got myself a Ritual Dagger, Funny Mask and Compass) then just investigate the Forest. You don't need to encounter all 3 unique Dream enemies in one run, just 3 encounters with any of the Dream enemies: the Dream Eater and the Dream Salesman, who will randomly be encountered in the Forest, or the Dream Devourer (can only be encountered in the CURIOUS CASE OF A CONTAGIOUS COMA mystery).

Unlocks KTU-RUFU, an Old God.
Kick-Box Cutter
Deal 10 damage with a single Kick.

Play as Toshiaki, and make sure you've got the History Club selected for the School location while you're setting up your run. Go into it over and over until you can get the RAGE OF RAGH-ZHULL spell for your Necronomicon. Use it a few times (I used it until my DOOM was at about 70%) then go find someone to dropkick directly into the Shadow Realm.

Unlocks the BOX CUTTER weapon.
Steel Box
Break the blade.

This can be completed easily along with the KICK-BOX CUTTER achievement if your DOOM isn't too high. Regardless of how you choose to go about it, investigate the School until you encounter the Young Witch enemy. When defeated, she drops the BOX CUTTER weapon. Equip it, find some more combat to get into, and make sure you're using the BOX CUTTER to attack. It will eventually break and you'll unlock this achievement.

Adds the FLASHBACKS status.
Welcome to the Family
Let the real Juri in.

Incredibly easy achievement. Play as Juri, and when your DOOM reaches 50% you will be caught by The Horror, which prevents you from gaining EXP for the rest of the run and grants this achievement.

Unlocks the HALLUCINATIONS status.
Can You Trust Them?
Hire two allies in one playthrough and win.

Set up your average run, and make sure you've got the Illegal Den location for the Seaside. Go there, hire two allies, and finish the run as normal.

Unlocks the TAINTED VIOLIN item.
Catch Me If You Can
Handcuff the ultimate opportunist.

Easy to get. Make sure the Police Station is selected for Downtown when setting up your run, then enter it at any point before the Lighthouse and click one of the desk's drawers to get a free set of HANDCUFFS. In the Lighthouse, when fighting Takashi, get his health below 12 (but don't defeat him) then use the HANDCUFFS to defeat him instantly and claim this achievement.

Unlocks the GHOSTLY SPEAR spell.
Insatiable Lust
Defeat a HUMAN enemy in one round.

There's a variety of ways to do this, but the easiest would either be the THREAD OF FATE spell, or setting up for it the same way you did for the Kick-Box Cutter achievement.

Unlocks the BLOOD FRENZY status.
Unfortunate Accident
Own both baseball bats.

Make sure you've got SPINE-CHILLING STORY OF SCHOOL SCISSORS in your mysteries, and start with it. You can immediately get the WOODEN BAT from the middle marked locker in the locker room (remember that the left one has a nasty surprise). Complete the mystery as normal, then simply explore the Apartments until you encounter the APARTMENT STALKER. Defeat him to claim the BASEBALL BAT and your new achievement.

Unlocks Yumiko, a new playable character.
Intrusive Thoughts
Forget five spells in one playthrough.

Very easy. Start as Kirie, who already has 1 spell, and forget it right away. Then go to the Library at the School and learn 4 more, then forget them.

Unlocks the PROTECTIVE CARVINGS spell.

RITUAL
-1 damage during combat encounters
costs 4 STA
Spore Infection
Defeat KINOKO GATHERER without any weapon.

I got this on my first playthrough while hunting for this achievement, but it took ages. Just make sure you don't have a weapon equipped then search the Forest location, where this enemy will randomly appear. When he pops up, just defeat him with plain old kicks or spells. Don't be afraid to just run from every other encounter but this one!

Unlocks the FUNGAL GROWTH status.
Homerun
Complete a playthrough with Yumiko.

Unlocks the old god EH-ZHAL.
Rain of Spiders
As Yashiro, complete a True Believer playthrough with CTHAC-ATORASU as the OLD GOD.

This one isn't too bad if you're experienced and comfortable on True Believer difficulty. Yashiro's gimmicks depend largely on him not taking damage in battle so as to increase/maintain his FTH, so the more hits you can dodge or avoid taking the better you'll be in the long run.

Alternatively, you can use Irrelevant's excellent Power of Friendship guide.

Unlocks the COMMONER backstory.
(H)Experimental Therapy
Complete the Mimi's Little Project challenge.

This is a tough challenge, since at the beginning of every new mystery you've got to essentially cripple yourself in a new and exciting way. I would strongly recommend you absolutely do not do the Vestibular System experiment, because failing every check is basically a death sentence. I chose the following experiments in the following order on my successful run:

1: SELL YOUR KIDNEY
A girl's gotta have some walking around money, right? Negligible STA penalty, and a cash injection at the beginning of the game can really make the difference.

2: IMPLANT A SECOND HUMAN HEART
First of all, it's just pretty heavy metal, and second of all the penalties aren't that bad, especially early on.

3: STAB AN ICEPICK INTO YOUR F'ING BRAIN WHAT ARE YOU DOING
By this point I had plenty of KNW, and a damage reduction was really nice to have moving forward.

4: BREAK YOUR ARM TO FORM A NEW JOINT
STR was never Mimi's build anyhow, and at this point on the run combat speed was fast enough due to a TAINTED VIOLIN that a hit to that was survivable.

5: GRAFT BEAR CLAWS TO YOUR TOES
The final mystery meant that this experiment resulted in one minor injury, and so was basically a non-factor.

Aside from that, the best advice I can give is to try and roll some very gentle mysteries that don't force you to fight a strong boss, because by the end of things poor Mimi can't handle much punishment at all. And take notes during your run, because missing some of the questions in the Lighthouse can end your run right at the finish line.

True Anarchy
Complete the Parole Violation challenge.

I didn't have any trouble with this one at all. The gimmick is that Miku's MAYHEM perk is disabled, but as a trade-off you start with -10% DOOM. Just play this as a normal playthrough with a character who sucks (Miku's starter stats are bad) and you'll get through eventually. As with most challenges, I'd recommend rolling for mysteries that don't have a boss fight.

Unlocks the ARM SLING item.
Lungful of Air
Complete the Cold Turkey challenge.

You play as Haru, but you're not allowed to smoke, which means you will forever have the NICOTINE WITHDRAWAL effect. As above, this isn't too bad - just treat it as a playthrough with a slightly-less-crappy-than-Miku character.

Unlocks the BRICK weapon.
Grounded Forever
Complete the But Dad!! challenge.

Play as Moriko, but you're not allowed to have any guns and will discard them immediately. Another normal playthrough, just build her for a different weapon.

Unlocks the BRAIN WORMS spell.

My Dear Ghost
Complete the Ghastly Presence challenge.

Play as Kirie, but you're not allowed to physically attack - and neither are allies, meaning this is a magic-only run. I struggled a lot with this challenge, but my winning strategy involved acquiring the INVISIBILITY spell, running like a wuss from every battle I could (make sure the Old God you get is not CTHAC-ATORASU) and building a spellbook that would let me deal a ton of quick damage to the enemies I couldn't avoid. Roll mysteries without bosses and hope you get real lucky with your encounters.

Unlocks the INSOMNIA status.
Most Popular Idol
Complete the Ghost Town challenge.

I actually had a really easy time of this one. You play as Mizuki, but the Seventh Curse backstory this challenge forces you into won't let you recruit any allies (except dogs or Kana). You also can't do any actions on any location aside from Resting, which means you can't shop for items, learn spells etc. Seems bad, right? Not as bad as it sounds. I built my character purely into STR and rerolled mysteries until I got the VICIOUS VERSES OF A VIOLENT VIGIL one. Start with that because you can get a free MEAT CLEAVER from the storage room, and once you have an actual weapon this challenge becomes a lot easier. There are, of course, other free weapons available to you in various other mysteries, but VIGIL has very little combat before you can arm yourself and it's guaranteed to be there. Once you have a decent weapon (and the stats to wield it well) this just becomes another sort-of-gimped normal playthrough.

Unlocks the DISLOCATED SHOULDER injury.

For "Junior Librarian" its very easy to just get the History Club and then go in and out until you get 3 "Occult Diary's" discarding them still works

@NightSparrow Mostly spam the forest until you encounter both, but Dream Devourer is only from the boss fight in the Contagious Coma case, which involves completing the sidequest first.

to get the achievement Unfortunate Accident, you need to have 2 different bat weapons at once.
1 is located in a school locker during the Spine Chilling story of School Scissors investigation. (the Wooden bat) The other is a (random? guaranteed?) drop from the Apartment Stalker enemy (the Baseball bat, just investigate apartments over and over until they pop up)

completing unlocks the new character, Yumiko

also yes night sparrow, theres only 3. You fight one as an ending in the coma story (have to do the bonus side card, keep item given to access fight at end), and the other 2 are random encounters found in the forest.
