This is the Avohkii, the mask of light, from Bionicle.
This mask is bonemergable and goes to the Team Fortress 2 Soldier.
Models included: 1
Collision model: Yes
Phong shading: Yes
Self-illumination: No
Multiple skins: No
Jigglebones: No

Model and texture: Populus89
Hexing and packaging: Adam We

Since this has had a suprising effect, I thought I should put this link here:

Its a Skyrim mod that makes a tribe of Toa, extensive models with color variations and options, masks and weapons.
Sadly I know nothing of hexxing, so if anyone can find someone to hex that, it would be a greatly popular to all Bionicle fans.
