You can play Story Mode on any difficulty, it doesn't matter.
You can use "A" and "D" to move left and right, even though the game does not tell you that.
Challenges and Arcade will start unlocking as you progress through the Story Mode.
Story Mode
Bamboo Ranger
Kill Stomper in Story Mode


Stomper is the boss at the end of Chapter 1. Cannot be missed.

High Noon
Kill Gunslinger in Story Mode


Gunslinger is the boss at the end of Chapter 2. Cannot be missed.

Monkey Spider
Kill Octo in Story Mode


Octo is the boss at the end of Chapter 3. Cannot be missed.

Minefield
Kill Hangman in Story Mode


Hangman is the boss in the middle of Chapter 4. Cannot be missed.

Spooky Keeper
Kill Phantom in Story Mode


Phantom is the boss at the end of Chapter 5. Cannot be missed.

Way of the Ninja
Kill Demon Samurai in Story Mode


Demon Samurai is the final boss in Chapter 7. Can be missed if you decide to spare him. Replay chapter to get the other achievement.

Way of the Samurai
Do not kill Demon Samurai in Story Mode


Same as previews achievement. Can be missed if you decide to kill him. Replay chapter to get the other achievement.

Shadow Warrior
Complete Story Mode


Unlocks after you have completed all 7 Story Chapters, on any difficulty. Cannot be missed.
Challenges
1st Chamber
Obtain at least one flame in all basic Challenges


This refers to the first 19 challenges. Get at least 1 of 3 flames on each challenge to unlock this achievement.

36th Chamber
Master all Challenges


This is the hardest, most time consuming and beyond frustrating achievement of them all. To complete this one, you have to get all 3 flames on all challenges. There are total of 24 challenges. By the end of Story Mode you should have unlocked 19 out of 24. The rest are unlocked by maxing out your Ninjutsu skills (all 5 of them). You do that by leveling up. Note that challenges won't give you any exp so you will have to do it through Story and Arcade. Good luck, you will need it.
Arcade
Grilled Bear
Play all Arcade Modes


After completing Chapter 4 in Story Mode, the 2 Arcade Mode games, Gatekeeper and Survival, will be unlocked. Simply complete one round in each mode to get the achievement.
Others
The Cleaner
Kill 5000 Pirate Monkey Zombies™


All kills in the game count towards this number. That means Story Mode, Challenges and Arcade. At best you will get about 40% after completing Story Mode. You can farm the rest of the kills either by playing New Game +, Challenges or/and Arcade modes. The fastest way is to play Arcade mode Gatekeeper.

No Remorse
Play as Samurai


After completing Story Mode, Demon Samurai will be unlocked. You can use him in any of Challenge or/and Arcade modes. Play either to get the achievement

For me the Linux native version didn't show the cutscenes. Therefore it was impossible to "Way of the Samurai" unless I enabled Proton emulation.

One tip in general is to use your phone as a input by streaming the game from the PC using Steam Link or similar. Specifically the Samurai boss was easy if you farm some levels to increase the health and/or spam multiple finger swipes to hit all shadows.

A nice guide, but the challenges are pretty annoying with a mouse. Unfortunately no PC with touchscreen around to finish them in classic manner.
