After gaining control of Willie open up his desk to acquire the Cake and Diary.
Talk to Julie and select 'Ask her to leave' then 'Bribe with the cake'.
Inspect Julie's Desk to get the glass.
Walk to Curve Wrecker's Desk and select 'Search' after interacting with it two times to get Jigsaw Piece 2.
Walk to the Filing Cabinet and select 'Check the books' then 'No Longer Human' to get Jigsaw Piece 3.
Walk to the Teachers Desk to get Jigsaw Piece 4.
Walk to the Water Dispenser and select 'Yes'.
Walk to the planter and select 'Water the plant' to get the random code.
Walk to both Curtains at the west wall and close both of them.
Walk to the Notebook and select 'Check' then 'Put them in'.
Enter the random code to get the Drawer Key.
Walk to the center desk in the middle row to get the Jigsaw Piece 1.
Walk to the Filing Cabinet and select 'Check the drawers' to get the Bloody Knife.
Talk to Liza.
When asked 'Should I gather up my courage and confess my love to her?', answer 'Yes' then 'Confess my love'.

In the Steam Library Page, right-click Confess My Love.
Select Properties. Select Local Files.
Select Browse Local Files.
Select System.
Copy System.rvdata.
Rename the copy System.rvdata.bk (sometimes the achievement doesn't unlock so making a backup is important).
Delete System.rvdata2.
Start Confess My Love.
Complete one playthrough.

Acquired by inspecting the Teachers Desk and reading the control scheme.

Acquired by inspecting the desk located at the second row directly below Liza.

Secret Achievement

'Wait, what?', you have to finish the game first and have gotten every ending:
Go to the games install folder normally:
[C:\Program Files (x86)\Steam\SteamApps\common\ConfessMyLove\System]
delete your save-game [System.rvdata2]and start a new game

Note on Mutated Room.

You know you're in the mutated room whenever you begin a playthrough in which Willie comments that his head hurts and the screen flashes red.

the bloody knife causes the harm ending, it doesnt unlock the mutated room. it has to be the normal knife. I believe nightmare causes it to spawn as the bloody knife but im not sure yet

@Makaio It does nothing except unlocks the final achievement. A pain to find unless you read someone else's guide.

What does the secret even do? You delete the thingy and you open the game and finish a playthrough and it gives you the achievement, at least that's what happened to me, is there any special dialogue that I missed or something?

You have to be really quick, just keep trying to get faster if you don't get it again.

"Unlocking the Mutated Room" seems to be in the wrong order. You have checking the notebook before finding the last jigsaw piece.
