Achievement Breakdown
Sanitarium has 20 achievements:
  • 16 for story progression (i.e. unmissable)
  • 1 for talking to everyone in the game
  • 3 for simple one-time miscellaneous tasks

This guide's main focus is the "You are not alone" achievement for talking to everyone, but I'll cover the other 3 missables as well for completion's sake.

You are not alone
As mentioned above, this achievement is for talking to every NPC in the game.

Below is a list of all the characters you can talk to, sorted by chapter. It may include some that don't count, but there's no harm talking to them either. Characters are listed by where they start, some move location as you proceed through the chapter.

Chapters not included don't have anyone to talk to.

The achievement unlocks immediately upon initiating conversation with the final person.

Limbo - Find the children in the wood
Chapter 2: The Innocent Abandoned
Head south from where you start to the lemonade stand. Directly opposite the lemonade stand is a hidden path among the trees. Walk down this path to find the children and unlock this achievement.



Dance, dance crazy - Make dancing three friends
Chapter 3: The Courtyard and Chapel
In Doctor Morgan's office is a record player and three records scattered around it on the table. They're called:
  • Belladonna in A flat
  • Rockabilly Swing Thing
  • The Ballad of Captain ♥♥♥♥
Use them on the record player and short scenes will play where a different person dances each time. The achievement will unlock after using the third.


Remember you - Play the good melody for the first time
Chapter 6: The Hive
Save your game after heading up to the second floor and finish talking to Grundle. Talk to the children grubs in the cages and they'll make a melody. Inspect the panel (with a red light) next to the green door and input the melody the children were making.
You must do this successfully on the first attempt for the achievement. If you make a mistake, reload the earlier save.

Buttons are arranged as so:
1 5 2 4 3
The solution is: 5 2 2 4 1 3
or Left, Right, Right, Bottom Left, Top, Bottom Right

Great Guide! Much Appreciated! Especially the Musical puzzle in chapter 6! I could remember the tune in my head, after all these years since i had last played it, but not the console layout itself.

I think you've missed an NPC to talk to: Chik-Nik, one of the bugs operating the furnace in Chapter 6.

I actually prefer this missable achievements only format. In other guides this information is sprinkled with spoilers and much harder to follow. Thanks for taking the time, great guide!
