To answer your question, Yes, he senses the pestilence in other survivors, and he comments on it depending on the character and their reputation. Maxwell is special because 049 is always vocal about him being ill.

The way I set up the quotes, he's more on a "truce" with them, since they're a liability toward his survival.
With players that have the Attacker/Murderer tag, however, 049 isn't so friendly.

Also, 049 really, really doesn't like Maxwell.

I wonder if 049 will simply not sense the pestillence/disease in the other survivors.
