This guide will include clues for puzzles along with the answers to the puzzles. Some of the puzzles have alternate ways for you to find the answer and I hope to cover that all in this guide.

Introduction
Thanks for taking the time to check out this guide. It will cover all of the major puzzles in the game along with the clues you can find for the puzzles if you are interested in finding more information.

The Digital Clock
The first puzzle you will be tasked with is finding the time for this digital clock. The note on the desk will tell you the room to go to that has the answer.


Inside this room you will find a clock that is stuck at seemingly 11:55 but a clue on the wall adjacent to this clock gives the hint of it being relative to a 24hr clock.

With this info, if you return to the digital clock and enter the 24hr time which would be 23:55 a secret wall/door will open to the next area right behind the digital clock.



Time to feed the dog.
The next puzzle has you feeding the dog. To start you need to be in the cafeteria and reading the note next to the microwave gives tell you the location of the Meatchop that you'll be feeding to the dog.


After grabbing the Meatchop from the freezer in the same room throw it into the microwave to warm the meat up for the dog.


To get to the dog go outside of the cooking room and the door leading outside is to the right.
Drop the warm meat in the dogs food bowl and grab the key that the dog is guarding. This key should open a door in the main lobby on the 2nd floor.


Solution for a "solution"
The next puzzle will involve you going to the chemistry lab on the 3rd floor though you can find a billboard on the 2nd floor with the 2nd floor map along with the assistants first note that tells you about the concoction you will be making on the 3rd floor.

In the chemistry lab you'll learn you need three different ingredients to make a concoction to get rid of creatures blocking a door on the first floor. In the chemistry lab you can find the assistant's second note which tells you the location of all three ingredients starting with the first the detergent which is up the stairs in a cabinet.

The next item, the drain cleaner is found in the girls bathroom on the second floor in the last stall. This is also where you might expect to find the red liquid in a tub but unfortunately it has drained into the first floor girls bathroom. The second you grab the drain cleaner a nurse will spawn and the door will lock for a few seconds and while in the first floor girls room when you grab the red liquid a creature will spawn the moment you pick up the liquid.


After gathering all three liquids you may want to save on the first floor to the door immediately left
before the stairwell. Heading back to the chemistry lab on the 3rd floor you can pour all three ingredients into their test tubes to get ready to make a chemical solution. Before you can make the mixture you have to light the fire under the cauldron and after that you just interact with cauldron to input the formula. In the chemistry lab you can also find clues on the needed combination to make the chemical solution As well, in front of the test tubes is the formula for finding the right combination. Which is Detergent(blue) - Drain Cleaner(green) = Strange Liquid(red)


For every wrong guess inside of the cauldron you will take damage, so if you don't want to take damage the answer is provided in the photo below. This will make you the chemical solution and will spawn on the small pedestal to the right of the cauldron. The potion can be used on the door on the first floor that is covered in the creature to open the pathway to the park.
It's the statue puzzle
Some people myself included may have gotten caught here. At first it seems as though only three of the four statues are movable, but it turns out all four are movable. the tomb stone in the middle of the statues tells you which statues you need to move but before we do that we should unlock the fourth statue. To get to the dial for the last statue walk across the wooden board above the gate with a red light once you get to the other-side take a left turn and go upstairs.


After going up stairs there will be two more wooden boards that you can cross though one is covered by normal wooden planks that fall from underneath you. On this one you will want to jump down the hole and turn around to see the button for the dial.


The puzzle is now just reading the tombstone and all the statues descriptions. The child looks to the concerned mother statue while the concerned mother looks to the business man statue who looks to the older psychiatrist statue who looks at the future the child


After putting this together the red light on the gate turns green and the new pathway is opened up. In this area you will meet Ophelia's friend Samantha who has a green key that you can pick up after talking to her. it will be on the wooden barrier in front of her so make sure you pick it up.

The numbered photo puzzle
This puzzle involves a library with a spirit that can damage you so it might feel stressful if you aren't prepared. The good thing to know is that if you haven't read the clue in picture #2 below the spirit wont spawn in so get a good look at the room before reading that clue! In the adjacent room you will be able to find a layout for the puzzle along with the third strange scribbles hint for the puzzle. The photo's are marked with roman numerals on the puzzle as for the proper order for the numbers and for reference the sun symbol on the bottom of the picture is the candle in the library so that helps give you the orientation of the room in this puzzle. you will need to write down or remember this code for the keypad to open the door to the next area.


If you are finding getting the code is hard because of the spirit beating you up, there is a candle in the library that you can light as long as you have matches that will ward off the spirit when it touches and subsequently puts out the candle. It wont last forever so you will have to be quick after the candle goes out in finding the right photos but for those that just want the answer for this puzzle see the following 4 photos.


You will then enter the numbers from those photos 4927 in the keypadded door to get through to the next area.
Ophelia the Pianist
One of my favorite puzzles in the game. Once you find the piano you will also see a photo of piano keys that are labeled on the couch along a note from the music teacher on top of the piano which has foreshadowing for the end of the puzzle.

Leaving this room and going to the PA room down the hall on the left you will find four speakers that you can activate from a button panel.

The first and main way of doing the puzzle includes turning the speakers on and interacting manually with each to see what note each speaker plays while also paying attention to the symbols on each speaker then looking closely at the sheet music for the Tune of Memories as each of the notes has a symbol.


For those that are having troubles with this puzzle I wrote the answer on the next photo just press the numbered keys in order from 1-4. When you are at the piano and you enter the correct the combination a cut-scene will play where the dog will open the next pathway which you can get to from the backyard through the cafeteria.

Just what the doctor ordered
close to some of the last mandatory puzzles and with this one you will have to search a emergency ward for patients who have very particular wounds. The hint that you are shown is as if you are facing the people with these injuries so instead of a left arm wound and a right leg wound you want to find the patient with a right arm wound and a left leg wound along with the chest wound.


In the Emergency ward you will need to read the patients diagnoses on their medical papers near their stations then push buttons to activate their light which is shown in above the door you are trying to open in the previous area. You will know if you have gotten the right combination when all 3 lights are green, but if any lights show up as red then you have turned the light on for the wrong patient so you have to go back and turn it off and find the right patient.

The vined door with the eye scanner
This puzzle has a few steps starting off by reading the Maintenance Note clue attached on the eye scanner to the left of the door you will want to do a 180 and walk down the stairs and take the first right.


Once you enter this room you'll see a cart on your right, next to the cart is the doorway you will want to enter, through here on the back wall is a hole covered by wooden planks on the wall that you can open up by hitting it with your melee weapon.


Once you open this area up you will have to crawl through until you find the hive that is causing the vines. As long as you have a match with you you can burn this and the vines on the door will disappear. From here you will want to exit the crawl space and walk straight from this door way to the one adjacent from this room


From this room you can go up the stairs on the right and break some wooden boards to get into the room that powers up the eye scanner.

The only thing left to do for this puzzle is getting the eye!
Head back downstairs one last time and aim slightly to the left and run into the doorway in the back of the room on the left. You will see a hole through this doorway in front of you slightly to your right. Inside this room you can find the eyeball on the desk. You can now use the eye on the retina scanner.


Through the door you will be able to push a blue button in the middle of the room with the guard. before it shows you the door it opened in a previous room.
OPTIONAL: Saving the boy
If you feel inclined to be a good person unlike me then this will help you understand how to save the boy locked in Cell 104. In the command room with the 4 arrows is a Cell Block Report that tells you to ask Grandmother Delaney for the combination. To find the location for the next hint, turn around and go out and down the stairs and immediately take a left to open a door and continue forward to the next door and this is where you can find a note on a wheelchair that subtly tells you the correct combination for the puzzle.


But if you prefer to see the less subtle answer, shoot the giant mouth creature in this room and it will gladly point you in the right direction with the answer to the puzzle.
When you add that information to the dials you can then go and save the boy from cell 104 in the back left corner of the cell block. You need to open his door and talk to him for him to follow you and you will be able to take the boy to his father who will be in the next area right before the elevator. You can also talk to the father at this point if you feel inclined to.
Restoring our childhood
You are getting close to the end of the game but before that we have to rebuild Ophelia's doll that can be found in her room. We have the technology to rebuild it! After leaving Ophelia's room open the door immediately to your left. Run straight through the opening on the left side and go through the curtains to the kitchen. You can find someone had been cooking up some of Ophelias doll here!

The next piece you can find by going out of the kitchen and turning left and going through the double door with the red light above it. The door in this room is locked so instead of going through the locked room back up to the door you entered from go to the left in the darkness and behind the book shelf you will find a hole covered with some wood planks that you will break with your melee weapon. The room you want to enter from here is the double door with the red lights.

Be careful to sneak through this area unless you like trouble. Take a right going through the walls with blinds and go through the double door at the end of the hallway on the right. When you get into this area find the girls bathroom it should be on your right a little bit down the hall. Inside you can get the dolls legs in a locker behind a little creature that will shoot fire balls at you.

When you walk out of the bathroom there will be a doorway you can enter to the left of the writing on the wall. From here go up the stairs on the left side in the room to find a chest that holds the dolls other arm.

The last piece you will need now is the dolls head which can be found in the room with the giant demon we wanted to hide from earlier. Go down the stairs and go out the double to the right of the room with dolls arm you just grabbed, run down the hallway and take a right and run down this hallway to see a red light, take two left turns and you will be facing the dolls head.

To check if you have all the doll pieces open your inventory with the i key and look in the bottom right corner and if you see all four piece of the doll then you're ready to head back to Ophelia's room to put the doll back together! After attaching all pieces you will get the red master key

OPTIONAL: Saving an old friend
We're at the final stretch so we might as well help our friend who helped us earlier right? To help Samantha we will have to talk to her inside of the nursery. If we talk to her from the side she isn't on we can help her so if you're in this room turn around and go towards the room with the elevator and take a left to find the locked door to the nursery. After talking to Samantha you'll have to sacrifice a syringe as it's just what she needs to feel better. If you haven't found any of the free syringes you can buy one from the vending machine on the third floor.

The final puzzle
For the final puzzle before the boss battle you will need to solve the code on the keypad. The Assumption of Power clue to the right of the keypad will a vague description of what keys you will need to push. Around this room are several insignia's with only four being needed for the puzzle.



Starting from the first hint in the note "It all began when you were born" the hint here is referring to your Creation.

The second hint being "A house, marked by a shadow". This hint is more direct as there is only one icon that has the house shape and it is "Family".

The Third hint "The future faces the past" is similar to the last clue where it is very direct with it's answer being the icon titled "Time".

The final clue is "To fulfill your purpose, you must become out Queen" to which the answer in this room is the icon titled "Sovereignty".

For those that just want the answer see the bottom photo and press the buttons in order from 1-4

After entering the combination the door will unlock letting you battle the boss!
Hard Mode Puzzles
While a good portion of the game stays the same with the puzzles on "Hard" difficulty, there are a handful of changes. Below will have all the Hard mode puzzle adjustments.


Clock Puzzle

This puzzle is similar to how it occurs in the normal difficulty, the only change is the time.
So just check the time on the clock and then convert it to 24 hours. In this case, 7:45 becomes 19:45


Chemistry room puzzle

The locations for all the concoction components stay the same on this puzzle (refer to 'Solution for a "solution" ' portion of the guide for their locations) though the solution (pun unintended) is moved around inside the chem room, one hint being downstairs to the back left corner of the room in the storage closet/dresser and the other being upstairs on a desk near the back left of the room. The solution is presented in a math equation.
To keep things simple the values needed are Green 2, Blue 5, Red 4

Statue Puzzle

This one is fairly easy of a concept as you just need to match up the posture of the etchings to those of the people.

Using the Tombstone as reference for direction (looking at the tombstone is 'up' and facing away from it would be 'down') going from left to right the businessman faces to the right. the concerned mother statue faces to the left. The child faces up and the older psychiatrist statue faces right.

Numbered Photos Puzzle

I actually find this one to be pretty easy compared the normal difficulty version.
The code for this just requires you to put the characters listed I-IV in order then the numbers on their photos in the next room is the doors code. The sequence becomes 5693

Ophelia the Pianist Puzzle

For musicians this one is a no brainer, but for anyone not so fluent in reading sheet music, below will be a easy to read chart of the C scale where you can try to read the notes as they appear in game. The keys you need to play in order are D, E, A, F (in order as labeled, 1, 2, 3, 4.

The ER Room Puzzle
Instead of the body with body parts circled we are given a seemingly cryptic puzzle for finding the patients lights we need to turn on. Going in order from top to bottom see the photos below for the patient lights that need to be activated.

The final puzzle
So using 5(Balance) as a reference, going right from there is clockwise and going left is counter clockwise.
That being said the one immediately to the right would be 6 which is on the other wall. so 6 would be Time continuing this trend, below you can see the 4 numbers and their corresponding icon for the keypad.

I believe that is everything for the the hard puzzles, though if you think I missed something please leave a comment and I will update asap!

@purple61830101: To save your game you need to find a tape recorder, and have at least one empty tape in your inventory. "Combining" tape and recorder initiates the saving process.

Thanks for the guide, I would never have solved the portrait puzzle because I thought the hint "The portraits show the faces of the people still bound here" meant that the faces still had to be recognizable and not be scratched up or damaged. Came here when the ghost got me the moment I walked through the door trying to figure out the puzzle.

If memory serves me right, it should be between the "Time to feed the dog" and "Solution for a "solution" " chapters of the guide.
