This is a collection of all of the items currently in Risk of Rain (CURRENT WITH: Seekers of the Storm) and what they do, as well as how to get each one (just kidding I don't do that anymore). There are spoilers within, so open at your own risk!

Intended to be used as a reference for when you are in the middle of a game, if you need more information on the exact statistics of an item or are having trouble remembering what an item does before you pick it up.

Introduction
Friendly reminder that I'm not a dev for this game, and it's not really the place to ask about potential bugs. Risk of Rain 2 has its own official discord, which I recommend everyone joins. They have channels you can direct questions to where actual devs and people with game knowledge will assist you.

There will be spoilers in this guide!

If playing multiplayer, I believe you will encounter all items that the host has unlocked, regardless of whether or not you have unlocked them yourself. You must still unlock the items via their requirements to find them in your own singleplayer games, or when you host.

Items are listed in order, left to right, top to bottom, as they appear in the in-game logbook. All items that have unlock requirements have the unlock condition listed beneath them.

White Tier Items

Warped Echo
  • On damage taken 50% of the damage is dealt immediately while the other 50% is delayed for 2 seconds. This effect has a cooldown of 10 seconds. Every additional item increases how many instances of this effect the player has. (+7.5% per stack) and movement speed by 7% (+7% per stack).

Chronic Expansion
  • Every 5 enemies slain grants a damage counter bonus increasing the players damage by 10% (+5% per item stack).

Knockback Fin
  • 7.5% (+7.5% per stack) chance on hit to knock enemies into the air. Height increases per stack.

Bolstering Lantern
  • When below 50% health receive a damage increase of 20% (+20% damage bonus per item stack).

Antler Shield
  • 55% (+5% per stack) chance on being hit to reflect 10% (+10% per stack) damage back to the attacker.


Green Items

Chance Doll
  • 20% (+8% per stack) chance to get higher rarity items from chance shrines on success.

Prayer Beads
  • Grows in power by absorbing experience, storing 20% (+5% per item stack) of player stats per item level up. On removal, gain all stored stats.

Luminous Shot
  • Using the secondary skill charges the next primary attack, dealing +150% damage (+25% per item stack) per charge up to 5 times (+1 per item stack).

Sale Star
  • Gain an extra item on the first chest opened per stage. Each additional Sale Star increases the chance of getting more items by 5%.

Unstable Transmitter
  • When reaching below 25% health the Survivor explodes, dealing 350% (+350% per item stack) damage to nearby enemies and teleporting to a random location. This effect has a cooldown of 2 minutes (-5% per item stack).

Noxious Thorn
  • 25% chance on being hit to inflict nearby enemies with 2 (+1 per stack) stacks of bleed. If enemies are already inflicted with one or more stackable debuffs, add 1 (+1 per stack) to those debuffs instead.


Red Items

Growth Nectar
  • While benefiting from 5+ unique buffs gain 20% (+20% per stack) bonus stats for 5 seconds.

War Bonds
  • Yields 75 gold (+25 gold per item stack) based on current difficulty level and the survivor's level at the start of every stage.

Runic Lens
  • Overspill. 5% chance (+1% per 100% done on initial hit plus 1% per item stack) on hit to summon a meteor on the target, dealing 2000% damage (+50% damage per 100% damage done on initial hit plus 50% damage per item stack).

Sonorous Whispers
  • When a large monster is killed it will always drop an item. All elites have a 15% chance of dropping an item (+5% per item stack).

Electric Boomerang
  • 15% chance on hit to fire an electric boomerang that travels a short distance before returning, dealing 30% (+30% per stack) damage and stunning all enemies hit.


Lunar Items

Longstanding Solitude
  • On level up gain a free unlock for the next purchase (+1 free unlock per item stack) but all gold is converted to experience. Anything that costs gold is increased by 100% (+100% per item stack).


Active Items (Equipment)

Seed of Life
  • Revives the user when killed or revives dead allies on activation. Consumed on use.
Common Items (White)
Soldier's Syringe
  • Increases attack speed by 15% (+15% per stack).

Tougher Times
  • 15% (+15% per stack) chance to block incoming damage. Unaffected by luck.
  • Notes: The stacking for this item appears to be more complex than simply +15% per stack, and I have been given an algorithm for determining the actual effectiveness of the item. It is as follows.
    Tougher Times stacking algorithm : (1 - 1 / (0.15 * [ITEMNUMBER] + 1)) * 100)
  • Notes: Interesting statistics on the stacking algorithm. "As is, you need 16 for 70% reduction, 27 for 80%, 38 for 85%, 60 for 90%, 127 for 95%, over 327 for 98%, over 661 for 99%, and over 6665 sad teddy bears to have a 99.9% chance to block incoming damage."
  • Requirements: Die 5 times.

Monster Tooth
  • Killing an enemy spawns a healing orb that heals for 8 plus an additional 2% (+2% per stack) of max health.

Lens-Maker's Glasses
  • Your attacks have a 10% (+10% per stack) chance to 'Critically Strike', dealing double damage.

Paul's Goat Hoof
  • Increases movement speed by 14% (+14% per stack).
  • Requirements: Fail the Shrine of Chance 3 times in a row.
  • Notes: Shrines of Chance look like THIS[i.imgur.com] and can be found on any map. There is no surefire way to guarantee 3 fails in a row, you simply have to get lucky (or rather, unlucky).

Bustling Fungus
  • After standing still for 2 seconds, create a zone that heals for 4.5% (+2.25% per stack) of your health every second to all allies within 3m (+1.5m per stack).

Crowbar
  • Deal +75% (+75% per stack) damage to enemies above 90% health.
  • Requirements: Discover 10 unique white items.

Tri-Tip Dagger
  • 10% (+10% per stack) chance to bleed an enemy for 240% base damage.

Warbanner
  • On level up or starting the Teleporter event, drop a banner that strengthens all allies within 16m (+8m per stack). Raise attack and movement speed by 30%.

Cautious Slug
  • Increases base health regeneration by +3 hp/s (+3hp/s per stack) while outside of combat.

Personal Shield Generator
  • Gain a shield equal to 8% (+8% per stack) of your maximum health. Recharges outside of danger.

Medkit
  • 2 seconds after getting hurt, heal for 20 plus an additional 5% (+5% per stack) of maximum health.
  • Requirements: Defeat an Elite-type monster.

Gasoline
  • Killing an enemy ignites all enemies within 12m (+4m perstack) for 150% base damage. Additionally, enemies burn for 150% (+75% per stack) base damage.

Stun Grenade
  • 5% (+5% per stack) chance on hit to stun enemies for 2 seconds.

Bundle of Fireworks
  • Activating an interactable launches 8 (+4 per stack) fireworks that deal 300% base damage.
  • Requirements: Duplicate the same item 7 times in a row with a 3D printer.

Energy Drink
  • Sprint speed is improved by 25% (+25% per stack).

Backup Magazine
  • Add +1 (+1 per stack) charge of your Secondary skill.
  • Requirements: Fully charge a teleporter without getting hit.

Sticky Bomb
  • 5% (+5% per stack) chance on hit to attach a bomb to an enemy, detonating for 180% total damage.

Rusted Key
  • A hidden cache containing an item (80% chance for green, 20% chance for equipment) will appear in a random location in each stage. Opening the cache consumes this item.
  • Requirements: Defeat the teleporter boss in under 15 seconds.

Armor-Piercing Rounds
  • Deal an additional 20% damage (+20% per stack) to bosses.
  • Requirements: Complete a teleporter event.

Topaz Brooch
  • Gain a temporary barrier on kill for 15 health (+15 per stack).

Focus Crystal
  • Increase damage to enemies within 13m by 20% (+20% per stack).

Bison Steak
  • Increases maximum health by 25 (+25 per stack).

Repulsion Armor Plate
  • Reduce all incoming damage by 5 (+5 per stack). Damage taken cannot be reduced below 1.

Mocha
  • Increases attack speed by 7.5% (+7.5% per stack) and movement speed by 7% (+7% per stack).

Delicate Watch
  • Increase damage by 20% (+20% per stack). Taking damage below 25% health breaks this item.

Roll of Pennies
  • Gain 3 (+3 per stack) gold on taking damage from an enemy. Scales over time.

Power Elixir
  • Taking damage to below 25% health consumes this item, healing you for 75% of maximum health.

Oddly-Shaped Opal
  • Increase armor by 100 (+100 per stack) while out of danger.

Hopoo Feather
  • Gain +1 (+1 per stack) maximum jump count.

Ukulele
  • 25% chance to fire chain lightning for 80% total damage on up to 3 (+2 per stack) targets within 20m (+2m per stack).

Leeching Seed
  • Dealing damage heals you for 1 (+1 per stack) health.

Predatory Instincts
  • Critical strikes increase attack speed by 12%. Maximum cap of 36% (+24% per stack) attack speed.
  • Requirements: Reach +200% attack speed.

Red Whip
  • Leaving combat boosts your movement speed by 30% (+30% per stack).

Old War Stealthkit
  • Falling below 25% health causes you to gain 40% movement speed and invisibility for 5s. Recharges every 30 seconds (-50% per stack).

Harvester's Scythe
  • Gain 5% critical chance. Critical strikes heal for 8 (+4 per stack) health.
  • Requirements: Complete a Prismatic Trial.

Fuel Cell
  • Hold an additional equipment charge (+1 per stack). Reduce equipment cooldown by 15% (+15% per stack).
  • Requirements: Pick up 5 different types of Equipment (Active Items).

Infusion
  • Killing an enemy increases your health permanently by 1 (+1 per stack), up to a maximum of 100 (+100 per stack) health.
  • Requirements: Defeat 3000 enemies.

Bandolier
  • 18% (+10% per stack) chance on kill to drop an ammo pack that resets all skill cooldowns.

Berzerker's Pauldron
  • Killing 3 enemies within 1 second sends you into a frenzy for 6s (+4s per stack). Increases movement speed by 50% and attack speed by 100% for the duration.
  • Requirements: Charge the teleporter with less than 10% health.

Rose Buckler
  • Increase armor by 30 (+30 per stack) while sprinting.

Runald's Band
  • Hits that deal more than 400% damage also blast enemies with a runic ice blast, slowing them by 80% for 3s (+3s per stack) and dealing 250% (+250% per stack) total damage. Recharges every 10s.
  • Requirements: Discover the hidden chamber in the Abandoned Aqueduct.

Kjaro's Band
  • Hits that deal more than 400% damage also blasts enemies with a runic flame tornado, dealing 300% (+300% per stack) total damage over time. Recharges every 10s.
  • Requirements: Discover the hidden chamber in the Abandoned Aqueduct.

Chronobauble
  • Slow enemies on hit for -60% movement speed for 2s (+2s per stack).

Wax Quail
  • Jumping while sprinting boosts you forward by 10m (+10m per stack).
  • Requirements: Reach +300% movespeed (includes sprinting).

Old Guillotine
  • Instantly kill Elite monsters below 13% health (+13% per stack).

War Horn
  • Activating your Equipment (Active Item) gives you +70% attack speed for 8s (+4s per stack).

Lepton Daisy
  • Release a healing nova during the Teleporter event, healing all nearby allies for 50% of their maximum health. Occurs 1 time (+1 per stack).

Razorwire
  • Getting hit causes you to explode in a burst of razors, dealing 160% damage. Hits up to 5 (+2 per stack) targets in a 25m (+10m per stack) radius.

Ghor's Tome
  • 4% (+4% per stack) chance on kill to receive treasure worth $25. Money amount scales over time.

Squid Polyp
  • Activating an interactable summons a Squid Turret that attacks nearby enemies at 100% (+100% per stack) attack speed. Lasts 30 seconds.

Death Mark
  • Enemies with 4 or more debuffs are marked for death, increasing damage taken by 50% from all sources for 7 seconds (+7s per stack).

Shipping Request Form
  • A delivery containing 2 items will appear in a random location on each stage. (79% chance for a white tier item, 20% chance for green tier item, and 1% chance for red tier item.)
  • Increases rarity chances of items per stack.

Hunter's Harpoon
  • Killing an enemy increases movement speed by 125%, fading over 1 second (+0.5 second per stack).

Shuriken
  • Activating your Primary skill (Left Click by default) also throws a shuriken that deals 400% (+100% per stack) base damage. You can hold up to 3 (+1 per stack) shurikens which all reload over 10 seconds.

Regenerating Scrap
  • Does nothing. Prioritized when used with Uncommon (Green Tier) 3D Printers. At the start of each stage, it regenerates.

Ignition Tank
  • Ignite effects deal +300% (+300% per stack) more damage over time.

Chance Doll
  • 20% (+8% per stack) chance to get higher rarity items from chance shrines on success.

  • All of your attacks explode in a 4m (+1.5m per stack) radius for a bonus 60% total damage to nearby enemies.

Ceremonial Dagger
  • Killing an enemy fires out 3 homing daggers that deal 150% (+150% per stack) base damage.

Frost Relic
  • Killing an enemy surrounds you with an ice storm that deal 600% damage per second and slows enemies by 80% for 1.5s. The storm grows with every kill, increasing its radius by 1m. Stacks up to 6m (+6m per stack).

Happiest Mask
  • Killing enemies has a 7% chance to spawn a ghost of the killed enemy with 1500% damage. Lasts 30s (+30s per stack).

H3AD-5T v2
  • Increase jump height. Creates a 5m-100m radius kinetic explosion on hitting the ground, dealing 1000%-10000% base damage that scales up with speed. Recharges in 10 (-50% per stack) seconds.

N'Kuhana's Opinion
  • Store 100% (+100% per stack) of healing as Soul Energy. After your Soul Energy reaches 10% of your maximum health, fire a skull that deals 250% of your Soul Energy as damage.
  • Requirements: Find the Altar to N'Kuhana. (In the swamp map, below the edge of the cliff - Right below the huge carved piece of stone that hangs out over the edge.)

Unstable Tesla Coil
  • Fire out lightning that hits 3 (+2 per stack) enemies for 200% base damage every 0.5 seconds. The Tesla Coil switches off every 10 seconds.
  • Requirements: Deal 5000 damage in one shot.

57 Leaf Clover
  • All random effects are rolled +1 times (+1 per stack) for a more favorable outcome.
  • Requirements: Complete 20 stages in one run.
  • Due to the counteracting luck-based effects, Purity and 57 Leaf Clover cancel each other out, leading to no increase or decrease in luck.

Sentient Meat Hook
  • 20% (+20% per stack) chance on hit to fire homing hooks at up to 10 (+5 per stack) enemies for 100% total damage.
  • Requirements: Loop back to the first stage.

Alien Head
  • Reduce skill cooldowns by 25% (+25% per stack).
  • Notes: Through anecdotal evidence and a few sources backing me up, I believe this item may cap at 3 stacks (for a total of 75% cooldown reduction).

Soulbound Catalyst
  • Kills reduce equipment cooldown by 4s (+2s per stack).
  • Requirements: Discover and activate 8 unique Newt Altars.

Dio's Best Friend
  • Upon death, this item will be consumed and you will return to life with 3 seconds of invulnerability.
  • Requirements: Stay alive for 30 consecutive minutes.

Hardlight Afterburner
  • Add +2 (+2 per stack) charges of your Utility skill. Reduces Utility skill cooldown by 33%.

Wake of Vultures
  • Gain the power of any killed elite monster for 8s (+5s per stack).

Brainstalks
  • Upon killing an elite monster, enter a frenzy for 4s (+4s per stack) where skills have no cooldowns.
  • Requirements: Defeat an Elite boss on Monsoon difficulty.

Rejuvenation Rack
  • Increase all healing received by 100% (+100% per stack).
  • Requirements: Without healing, reach and complete the 3rd teleporter event.

Aegis
  • Healing past full health grants you a temporary barrier for up to 50% (+50% per stack) of the amount you healed.

Shattering Justice
  • After hitting an enemy 5 times, reduce their armor by 60 for 8 seconds (+8s per stack).

Resonance Disk
  • Killing enemies charges the Resonance Disc. The disc launches itself toward a target for 300% base damage (+300% per stack), piercing all enemies it doesn't kill, and then explodes for 1000% base damage (+1000% per stack). Returns to the user, striking all enemies along the way for 300% base damage (+300% per stack).

Interstellar Desk Plant
  • On kill, plant a healing fruit seed that grows into a plant after 5 seconds.
  • The plant heals for 10% of maximum health every second to all allies within 5m (+5m per stack). Lasts 10 seconds.

Defensive Microbots
  • Shoot down 1 (+1 per stack) projectiles within 20m every 0.5 seconds. Recharge rate scales with attack speed.

Laser Scope
  • Critical strikes deal an additional 100% damage (+100% per stack).

Spare Drone Parts
  • Gain Colonel Droneman, a modified gunner drone.
  • Drones gain +50% (+50% per stack) attack speed and cooldown reduction.
  • Drones gain 10% chance to fire a missile on hit, dealing 300% total damage.
  • Drones gain an automatic chain gun that deals 6x100% damage, bouncing to 2 enemies.

Ben's Raincoat
  • Whenever you would receive a debuff, prevent it. Increases maximum health by 100 (+100 per stack).

Pocket I.C.B.M.
  • All missile items and equipment fire an additional 2 missiles. Increase missile damage by 0% (+50% per stack).

Symbiotic Scorpion
  • 100% chance on hit to reduce armor by 2 (+2 per stack) permanently.

Bottled Chaos
  • When using equipment, trigger a random equipment effect 1 time (+1 time per stack).

Growth Nectar
  • While benefiting from 5+ unique buffs gain 20% (+20% per stack) bonus stats for 5 seconds.

Legendary Items (Red) cntd.

Void Items (Purple)
  • All Void tier items are "corrupted" versions of other items. They are akin to Lunar items, in that most have a positive and negative effect. Sometimes that negative effect is just that it "corrupts" your other items, meaning it replaces all of your existing and future copies of another item.

Safer Spaces
  • Blocks incoming damage once. Recharges after 15 seconds (-10% per stack).
  • Corrupts all Tougher Times.

Needletick
  • 10% (+10% per stack) chance to collapse an enemy for 400% base damage.
  • Corrupts all Tri-Tip Daggers.

Lost Seer's Lenses
  • Your attacks have a 0.5% (+0.5% per stack) chance to instantly kill a non-Boss enemy.
  • Corrupts all Lens-Maker's Glasses.

Weeping Fungus
  • Heals for 2% (+2% per stack) of your health every second while sprinting.
  • Corrupts all Bustling Fungus.

Encrusted Key
  • A hidden cache containing an item (60% chance for a white tier item, 30% chance for green tier item, and 10% chance for red tier item.) will appear in a random location on each stage. Opening the cache consumes this item.
  • Corrupts all Rusted Keys.

Polylute
  • 25% chance to fire lightning for 60% total damage up to 3 (+3 per stack) times.
  • Corrupts all Ukuleles.

Singularity Band
  • Hits that deal more than 400% damage also fire a black hole that draws enemies within 15m into its center. Lasts 5 seconds before collapsing, dealing 100% (+100% per stack) total damage. Recharges every 20 seconds.
  • Corrupts all Runald's Bands and Kjaro's Bands (Not confirmed, just my best guess.)

Lysate Cell
  • Add +1 (+1 per stack) charge of your Special skill.
  • Corrupts all Fuel Cells.

Voidsent Flame
  • Upon hitting an enemy above 90% health, spawn a lava pillar in a 12m (+2.4m per stack) radius for 350% (+280% per stack) base damage.
  • Corrupts all Will-o'-the-wisps.

Plasma Shrimp
  • Gain a shield equal to 10% of your maximum health. While you have a shield, hitting an enemy fires a missile that deals 40% (+50% per stack) total damage.
  • Corrupts all AtG Missile Mk.1.

Tentabauble
  • 5% (+5% per stack) chance on hit to root enemies for 1 second (+1 second per stack).
  • Corrupts all Chronobaubles.

Benthic Bloom
  • Upgrades 3 (+3 per stack) random items to items of the next higher rarity at the start of each stage.
  • Corrupts all 57 Leaf Clovers.

Pluripotent Larva
  • Upon death, this item will be consumed and you will return to life with 3 seconds of invulnerability, and all of your items that can be corrupted will be.
  • Corrupts all Dio's Best Friends.

Newly Hatched Zoea
  • Every 60 seconds (-50% per stack), gain a random Void ally. Can have up to 1 (+1 per stack) allies at a time.
Boss Items (Yellow)
Titanic Knurl
  • Increase maximum health by 40 (+40 per stack) and health regeneration by 1.6 (+1.6 per stack) health per second.
  • Requirements: Kill the Titanic Guardian boss, and get lucky with the drop rate.

Queen's Gland
  • Every 30 seconds, summon a Beetle Guard with bonus 300% damage and 100% health. Can have up to 1 (+1 per stack) Guards at a time.
  • Requirements: Kill the Beetle Queen boss, and get lucky with the drop rate.

Halcyon Seed
  • Summon Aurelionite during the teleporter event. It has 100% (+50% per stack) damage and 100% (+100% per stack) health.
  • Requirements: Kill the Aurelionite boss. One of these will drop for each player in your game, guaranteed upon killing Aurelionite.

Little Discipline
  • Fires a tracking wisp for 300% (+300% per stack) damage. Fires every 1.6 seconds while sprinting. Fire rate increases with movement speed.
  • Note: This is a boss item dropped uniquely from the brand new boss, the Grovetender.

Pearl
  • Increases maximum health by 10% (+10% per stack).

Irradiant Pearl
  • Increases all stats by 10% (+10% per stack).

Genesis Loop
  • Falling below 25% health causes you to explode, dealing 6000% base damage. Recharges every 30 seconds (-50% per stack).

Artifact Key
  • Apparently used to damage the new boss inside of the new secret world. Testing required, but does not appear to have uses outside of that specific boss fight.

Molten Perforator
  • 10% chance on hit to call forth 3 magma balls from an enemy, dealing 300% (+300% per stack) damage each.

Shatterspleen
  • Critical Strikes bleed enemies for 240% base damage. Bleeding enemies explode on death for 400% (+400% per stack) damage, plus an additional 15% (+15% per stack) of their maximum health.

Mired Urn
  • While in combat, the nearest 1 (+1 per stack) characters to you within 13m will be tethered to you, dealing 100% damage per second, applying tar (assumed slowing affect), and healing you for 100% of the damage dealt.

Charged Perforator
  • 10% chance on hit to call down a lightning strike, dealing 500% (+500% per stack) damage.

Planula
  • Heal from incoming damage for 15 (+15 per stack) health.

Empathy Cores
  • Every 30 seconds, summon two Solus Probes (floating ball enemies) that gain +100% (+100% per stack) damage per ally on your team.

Defense Nucleus
  • Killing elite monsters spawns an Alpha Construct. Limited to 4 (+4 per stack).
Lunar Items (Blue)
Shaped Glass
  • Increase base damage by 100% (+100% per stack). Reduce maximum health by 50% (50% per stack).

Brittle Crown
  • 30% chance on hit to gain 2 (+2 per stack) gold. Scales over time.
  • Lose gold on taking damage equal to 100% (+100% per stack) of the maximum health percentage you lost.

Transcendence
  • Convert all but 1 health into regenerating shields. Gain 50% (+25% per stack) maximum health.

Corpsebloom
  • Heal +100% (+100% per stack) more. All healing is applied over time. Can heal for a maximum of 10% (reduced by 50% per stack) of your health per second.

Gesture of the Drowned
  • Reduce Equipment cooldown by 50% (+15% per stack). Forces your Equipment to activate whenever it is off cooldown.
  • Requirements: Kill 20 hermit crabs by chasing them off the edge of the map.
  • Notes: For those having trouble finding a map with hermit crabs to chase off of an edge, you must loop once and get back to the Titanic Plains level, the hermit crabs will be spawning there.

Strides of Heresy
  • Replace your Utility Skill (Slot 3) with Shadowfade.
  • Fade away, becoming intangible and gaining +30% movement speed. Heal for 25% (+25% per stack) of your maximum health. Lasts 3 seconds (+3s per stack).
  • Hold all 4 Heresy items to transform your character into the Heretic.

Visions of Heresy
  • Replace your Primary Skill (Slot 1) with Hungering Gaze.
  • Fire a flurry of tracking shards that detonate after a delay, dealing 120% base damage. Hold up to 12 charges (+12 per stack) that reload after 2 seconds (+2 per stack).
  • Note: This item functions similarly to the Artificer's Primary Skill, with the way it fires and recharges, but reloads the entire "magazine" at once as opposed to the Artificer's one by one reload.
  • Hold all 4 Heresy items to transform your character into the Heretic.

Hooks of Heresy
  • Replace your Secondary Skill (Slot 2) with Slicing Maelstrom.
  • Charge up a projectile that deals 175% damage per second to nearby enemies, exploding after 3 seconds to deal 700% damage and root enemies for 3 (+3 per stack) seconds. Recharges after 5 (+5 per stack) seconds.
  • Hold all 4 Heresy items to transform your character into the Heretic.

Essence of Heresy
  • Replaces your Special Skill (Slot 4) with Ruin.
  • Dealing damage adds stacks of Ruin for 10 (+10 per stack) seconds. Activating the skill detonates all Ruin stacks at unlimited range, dealing 300% damage plus 120% damage per stack of Ruin. Recharges after 8 (+8 per stack) seconds.
  • Hold all 4 Heresy items to transform your character into the Heretic.

Beads of Fealty
  • When you obliterate yourself at the Obelisk while holding one of these items, you will be teleported to a new "final boss" level. This level holds a Scavenger boss with several upgrades over the normal one, and when you defeat this enemy it will drop 10 lunar coins. Afterwards, the game will fade to black and "end".
  • Stacking this item does nothing. Only one person in a multiplayer game must have this item for it to work, but I personally have not tested whether or not the one person holding the item has to be the one to activate the Obelisk or not.

Focused Convergence
  • Teleporters charge 30% (+30% per stack) faster, but the size of the Teleporter zone is 50% (-50% per stack) smaller.

Defiant Gouge
  • Using a shrine summons enemies nearby. Scales over time.

Mercurial Rachis
  • Creates a Ward of Power in a random location nearby that buffs both enemies and allies within 16m (+50% per stack), causing them to deal +50% damage.
  • Note: This is also an Active item.

Purity
  • All skill cooldowns are reduced by 2s (+1s per stack). All random effects are rolled +1 (+1 per stack) times for an unfavorable outcome.
  • Due to the counteracting luck-based effects, Purity and 57 Leaf Clover cancel each other out, leading to no increase or decrease in luck.

Glowing Meteorite
  • Rain meteors from the sky, damaging ALL characters for 600% damage per blast. Lasts 20 seconds.
  • Requirements: Carry 5 Lunar items in one run.
  • Note: This is also an Active item.

Helfire Tincture
  • Ignite ALL characters within 8m. Deal 5% of your maximum health per second as burning damage to yourself. The burn is 0.5x stronger on allies, and 24x stronger on enemies.
  • Note: This is also an Active item.

Effigy of Grief
  • ALL characters within are slowed by 50% and have their armor reduced by 20.
  • Note: This is also an Active item.

Spinel Tonic
  • When activated, you "drink" the tonic, gaining a boost for 20 seconds. This boost increases damage by +100%, attack speed by +70%, armor by +20, maximum health by +50%, passive health regen by +300%, and movespeed by +30%.
  • When the Tonic wears off, you have a 20% chance to gain a Tonic Affliction, reducing all of your stats by -5%.
  • Note: This is also an Active item.

Light Flux Pauldron
  • Decrease skill cooldowns by 50% (+50% per stack). Decrease attack speed by 50% (+50% per stack).

Stone Flux Pauldron
  • Increase max health by 100% (+100% per stack). Reduce movement speed by 50% (+50% per stack).

Egocentrism
  • Every 3 seconds (-50% per stack), gain an orbiting bomb that detonates on impact for 360% damage, up to a maximum of 3 (+1 per stack) bombs.
  • Every 60 seconds, a random item is converted into this item.

Eulogy Zero
  • Items have a 5% (+5% per stack) chance to become a Lunar item instead.
Lunar Items (Blue) cntd.
Longstanding Solitude
  • On level up gain a free unlock for the next purchase (+1 free unlock per item stack) but all gold is converted to experience. Anything that costs gold is increased by 100% (+100% per item stack).
Active Items (Orange)
Disposable Missile Launcher
  • Fire a swarm of 12 missiles that each deals 300% damage.
  • Cooldown: 45 seconds

Foreign Fruit
  • Instantly heal for 50% of your maximum health.
  • Cooldown: 45 seconds

Primordial Cube
  • Fire a black hole that draws enemies within 30m into its center. Lasts 10 seconds.
  • Cooldown: 60 seconds

Ocular HUD
  • Gain +100% Critical Strike Chance for 8 seconds.
  • Cooldown: 60 seconds

The Back-Up
  • Call 4 Strike Drones to fight for you. Lasts 25 seconds.
  • Cooldown: 100 seconds
  • Requirements: Repair 30 drones or turrets.

Preon Accumulator
  • Fires preon tendrils, zapping enemies within 35m for up to 600% damage per second. On contact, detonate in an enormous 20m explosion for 4000% damage.
  • Cooldown: 140 seconds
  • Requirements: Open the Timed Security Chest on Rallypoint Delta.
  • Notes: The Timed Security Chest locks after 10 minutes. Gotta go fast!

Milky Chrysalis
  • Sprout wings and fly for 15 seconds. Gain +20% movement speed for the duration.
  • Cooldown: 60 seconds

Royal Capacitor
  • Call down a lightning strike on a targeted monster, dealing 3000% damage and stunning nearby monsters.
  • Cooldown: 20 seconds
  • Requirements: Defeat the teleporter bosses after activating 2 Shrines of the Mountain.
  • Note: Shrines of the Mountain can be found on any map (excluding hidden levels) and look like THIS[i.imgur.com].

The Crowdfunder
  • Fires a continuous barrage that deals 100% damage per bullet. Costs $1 per bullet. Cost increases over time.
  • Cooldown: 5 seconds

Gnarled Woodsprite
  • Gain a Woodsprite follower that heals for 1.5% of your maximum health per second. Can be sent to an ally to heal them for 10% of their maximum health.
  • Cooldown: 15 seconds
  • Requirements: Fully upgrade a Shrine of the Woods.
  • Notes: Shrines of the Woods appear in the Distant Roost map. They look like THIS[i.imgur.com].

Radar Scanner
  • Reveal all interactables within 500m for 10 seconds.
  • Cooldown: 45 seconds
  • Requirements: Collect 10 monster or environment logs.

Eccentric Vase
  • Create what is functionally a 2-way zipline of up to 1000m in length. Lasts 30 seconds, and is accessible from either end.
  • Cooldown: 45 seconds
  • Requirements: Defeat the guardian of Gilded Coast without any beacons deactivating. AKA, kill Aurelionite without losing any totems. You can enter the Gilded Coast by activating what will look like a golden version of a chance shrine. This is usually very expensive to activate, and it will generate a golden portal for you to enter once the teleporter event has been completed.

Blast Shower
  • Cleanse all negative effects. Includes debuffs, damage over time, and nearby projectiles.
  • Cooldown: 20 seconds
  • Requirements: Die to fire damage 3 times. (Easily done against Elder Lemurians.)

Volcanic Egg
  • Turn into a draconic fireball for 5 seconds. Deal 500% damage on impact. Detonates at the end for 800% damage.
  • Cooldown: 30 seconds

Jade Elephant
  • Gain 500 armor for 5 seconds.
  • Cooldown: 45 seconds

Sawmerang
  • Throw three large saw blades that slice through enemies for 3x400% damage. Also deals an additional 3x100% damage per second while bleeding enemies. Can strike enemies again on the way back to you.
  • Cooldown: 45s

Recycler
  • Transform an Item or Equipment into a different one. Item stays within the same tier (Common, Uncommon, etc). Can only recycle once.

Super Massive Leech
  • Heal for 20% of the damage you deal. Lasts 8 seconds.
  • Cooldown: 60 seconds

Gorag's Opus
  • All allies enter a frenzy for 7 seconds. Increases movement speed by 50% and attack speed by 100%.
  • Cooldown: 45 seconds

Forgive Me Please
  • Throw a cursed doll out that triggers any On-Kill effects you have every second for 8 seconds.
  • Cooldown: 45 seconds

Trophy Hunter's Tricorn
  • Execute any enemy capable of spawning a unique reward, and it will drop that item. Equipment is consumed on use.
  • Cooldown; 0 seconds

Goobo Jr.
  • Spawn a gummy clone that has 700% damage and 700% health. Expires in 30 seconds.
  • Shaped like a friend.
  • Cooldown: 100 seconds

Molotov (6-Pack)
  • Throw 6 molotov cocktails that ignites enemies for 500% base damage. Each molotov leaves a burning area for 200% damage per second.
  • Cooldown: 45 seconds

Executive Card
  • Whenever you make a gold purchase, get 10% of the spent gold back. If the purchase is a multishop terminal, the other terminals will remain open.
  • Cooldown: 0.1 seconds

Remote Caffeinator
  • Request an Eclipse Zero Vending Machine from the UES Safe Travels. Delivery Guaranteed in 5 seconds, dealing 2000% damage. Heal up to 3 targets for 25% of their maximum health.
  • Cooldown: 60 seconds

Seed of Life
  • Revives the user when killed or revives dead allies on activation. Consumed on use.

Aspect Items
This is a section created specially for any items that have been encountered but have no logbook entries. There will likely be missing data in several parts on these items, as they do not appear in the logbook. Each of these items drop from killing elites of the same type.

The common theory is that these "Aspect" items are a more permanent version of what happens when you kill certain elites (lightning, fire, ice) while using the Wake of Vultures legendary (red) tier item.

UPDATE: Aspect items are a pain to collect data on, and they don't show up in the logbook like the rest of the items. I'm going to go ahead and just call this section defunct for now until I can find a convenient way to collect information on them, but the gist of these is that they're items that I'm pretty sure drop from elites, and mimic the different effects those elites have attached to them.

Aurelionite's Blessing
  • Turns you gold!
  • ???


Her Biting Embrace
  • Applies slowing effect on hit to damaged enemies.
  • This is dropped from ice elites/bosses.




Silence Between Two Strikes
  • It HALVES your health, and turns the other half into quick-recharge shields. For every attack it creates a dot that explodes after 1 or 2 seconds.
  • This is dropped from electric (enemies that also have the exploding dot effect) elites/bosses.




Ifrit's Distinction
  • It turns your character red/on fire, gives them horns like the "Blazing" elites, and greatly increases attack speed + move speed. Your character also set enemies on fire with attacks, and leaves a trail of fire behind them that burns enemies.
  • This is dropped from fire elites/bosses.




N'Kuhana's Retort
  • You have the same effects on enemies as Malachite enemies do on you. You disable enemy healing, and you occasionally shoot out spiky Malachite blasts in a handful of different directions that deal damage when stepped on by enemies.
  • This is dropped from Malachite elites/bosses.




Spectral Circlet
  • You gain a hefty slow on hit effect (stronger than that given by Her Biting Embrace) and make nearby friendlies invisible. Not sure what this actually does for those invisible friendlies, did some testing and enemies definitely still target them.
  • Note: Yes the image above is blank, that is how it appears in game in your equipment slot.

the aspects are a huge pain to actually collect data on, section is essentially defunct for now unless i can figure out a convenient way to collect data / pics on them... i'll slap a little update in there that acknowledges this and the one that you found!

The dlc comes with new aspects for the new elite types. I picked up Aurelianite's blessing during a run and it turned me gold. No idea what is actually does as it bricked my run soon after, causing me to be unable to pickup items until I crashed

The Beads of Fealty come into effect regardless whether or not the player holding them activated the Obelisk. As I found when trying to help my friend unlock the Mercenary, Only I held the beads, and when they activated the Obelisk we were both transported.
