Important: achievements may not work with the patch installed, and save files are not shared between the patched and unpatched versions. I suggest playing through the patched game and then going back and skipping through the text in the unpatched game to unlock any achievements you've missed. (You can change the skip option in the settings to skip unread text and press Control to skip or N to fast skip, or you can just hold down Enter.) If you're not sure how to unpatch the game/what files to remove, to be safe uninstall it and delete all the files, then reinstall the Steam version. Also note that the final achievement is buggy.

Endings
See Katya's walkthrough for a guide and suggested route order. After following it, you should have all of the ending achievements:


Zenya End
Reach Zenya's ending.


Makoto End
Reach Makoto's ending.


Tetsuo End 3
Reach Tetsuo's third ending. [Diving Deep]


Tetsuo End 1
Reach Tetsuo's first ending. [Red Road]


Tetsuo End 2
Reach Tetsuo's second ending. [Miracles May (violin version)]


Miracles may Route

Signifying that a new choice for the final ending has been unlocked. You should be able to load the last quicksave from Tetsuo's second end to get the final ending achievement.

CONGRATULATIONS!

Once you've gotten all the endings, click Extras for a bonus CG.


CG Completion
Unlock all CGs.

If you played the patched version, you might have gotten this already. If not, open CG Library in Extras after completing all endings.


Song Completion
Unlock all music.

If you played the patched version, you might have gotten this already. If not, open Media Library in Extras after completing all endings.


I'm in blue
Unlock the full version of the opening song.

After you've unlocked all the music, in the Media Library, click Full Ver. next to Movie 001 and watch the video, or if you've enabled movie skip in the Config menu, you can click to skip it.
Final achievement

Complete all achievements.

I got all the other achievements and tried a bunch of things (playing through the game again, opening the galleries, checking the liner notes, quitting the game and restarting, etc.) but couldn't get this to pop. According to this thread, you can get the achievement if you re-install the patch after completing all the other achievements, then open the Recollection Library in Extras, which isn't available in the censored version.

I can't imagine this was intentional, so hopefully it will get fixed so it's unlockable without the patch. If it is (or if you find another workaround), please leave a comment so I can update the guide!

Hm, try going into scene replay in the uncensored version and opening the congrats CG there?

any idea how to get the CONGRATS achievement? I completed the whole game originally with the uncensored version, then beat it entirely again with the censored version, but I got the congrats CG only with the uncensored version (though in both versions it shows me with the congrats picture), any idea how to fix that? I thought me going to the picture in the CG section would make it work but it still isn't even though it is right in front of my face :/ i am just trying to get all the achievements for the game, so this last one is irritating me
