Introduction
Without any doubt, this is the most complicated achievement in the game. It is far less straightforward than the others and requires scoring in a semi-hidden indicator. Please understand that the guide might be inaccurate. I will gladly take into account your comments for improving or correcting the guide.
How to
There is some agreement on the following requirements
  • Reach Elder Status
  • Bad to Mid Relations Humans (maybe?)*
  • Bad Relations Monsters
  • Low Respect
  • Neurosurgeon Points. There are some missions that yield, along with the common bonuses to primary characteristics, respect and relations, so-called Neurosurgeon Points. They are normally generated by actions that imply taking care of someone's health (not only physical but also emotional). Examples follow below.
NB: There is the impression that "bad" and "low" mean "below 50 in the relevant characteristic" but some people reported otherwise. Please report your stats in the comments so that this guide can be improved.
*For a time, it was believed that Human regard of monsters should be low, but some people are reporting getting the event with 50-odd points. The highest reported yet is 62.


You will get the trigger for this achievement just before the Human-Monster clash. Tell the monsters to fight Humans (telling them to flee, even if it fails, will not trigger the Undercover Neurosurgeon event) but fail due to respect. The trigger for the Undercover Neurosurgeon should come next.

Some missions that yield Neurosurgeon Points (some missions, including at least one that is part of the September 2, 2014 patch, are not listed here):
  • As a Monsterling, helping the wounded Elder (rather than seeking help) that arrives to the cave ends with you removing a knife from its back. It yields a few Neurosurgeon Points and points to some primary statistics. This mission does not yield enough points for getting the trigger; see below for effective ones.
  • As an Elder, journeying into Carmen, helping the woman that you are not very sure whether she is doing funny things or is genuinely sick. Making the medicine yourself (Kindness required; rather than searching human help) not only boosts Human Relations, but also gives some amount of Neurosurgeon Points.
  • As an Elder, journeying into Carmen, saving a baby that has been trapped in a house aflame. Saving it by covering him with your monster muckus (Kindness required; rather than taking the child out of the house by yourself). You get both a boost to Human Relations and some Neurosurgeon Points.

Additional notes
  • It seems that any sole Elder mission that yields neurosurgeon points is enough, regardless of how many points it yields. The Monsterling mission alone will NOT be enough for getting the trigger.
  • Remember you need high Respect and/or good primary statistics to ascend. As such, you might have to actively seek missions that let your Respect to be reduced during the Elder status. Since diminishing Respect as Elder might prove difficult, focusing on maximising Primary statistics before Ascending might carve an easier path towards Undercover Neurosurgeon.

Got Neurosurgeon points from both the house on fire event + helping the human girl event as Elder. Didn't do any events with NS points during other life stages.

After reading this is took me two tries to get the achievement.

Rerolled (exit to menu then return using esc key) to get all the monsterling neurosurgeon events (sword, injured arm, skull) and was lucky in elder stage to get both neurosurgeon events without rerolling (injured arm, surprised face[?]). When the Spine Doctor said let's go, yes I go to fix da hoomans :D

I was able to do all the neurosugreon events listed. The monsterling one with the knife, the old woman one as an elder, and the house on fire with the baby as an elder. I have noticed that, although it's not always possible to guess what event is triggered by what button, certain events will always have a picture on the button.

If you meet as an Elder with the kid with the lollipop, if you just bite into it, and not eat it whole, you get -10% for how humans thinks about monsters, not just -5%. Also, there is a man who is yelling angrily towards you, when come closer - you need bravery to stand before it, then attack him for gain another -10%. These two option is good for countering the +18% what you gain from helping either the women or the child (especially if you do both!).

I think it's good to gain around 10-15% Respect for this achievment, as - if you have enough stat points, and a bit of luck - you will get just enough boost for your test to pass it, but it keep low enough. to easily fail at the end. (If i had at least 20% Respect, then i literally never failed to convince the monsters to go against the humans, even when we lost the war...)

I was only able to pass the Elder quest to save the baby because I failed the other one and it triggered. I also managed to narrowly pass the Elder trial of Ferocity (which mine was much lower) with the 50% attack chance of fighting one of the monsters.

This worked perfectly for me, thank you so much!! I did the two Elder quests, had maxed out kindness and bravery, and 29% respect :)
