This stated D&D 5e character sheet includes the main page, Details, Spell Sheet and a Companion/Familiar sheet.

Features include an easy click and type interface and convenient reset and lock buttons to completely clear the sheet and prevent accidental destruction of your hard work.

~IMPORTANT NOTES~
Tabletop Simulator's custom UI intercepts all click, drag and scroll functions. Because of this, ensure your camera is not positioned over the character sheet before attempting to interact with the game UI. You will only end up clicking through the game UI onto the sheet.

@captain_uzziah I'm afraid not. That is why I submitted a bug report to the TTS staff. It's not something I can do anything about. The fix will have to come from them.

The loss of data is likely due to Table Top Simulator's v13 update. I will look into it as soon as I can.

The character sheet can no longer be saved and spawned. All data input is lost on spawn, including when combined and state-changed.
