Intro and Building 1
The Crooked Man: “David appears.” Impossible to miss, it pops up when David from the Crooked Man shows up.

I’ve Seen You Before: “Speak to the counter worker at Mary’s kitchen.” (Missable) When you enter Mary’s Kitchen for some breakfast, go up to the blonde girl at the counter and try to speak to her before you sit down.

The Sand Man: “Sophie appears.” Impossible to miss, it pops up when Sophie from the Sand Man shows up.

The Boogie Man: “Keith appears.” Impossible to miss, it pops up when Keith from the Boogie Man shows up.

Loopy Stairs: “Solve the ‘Loop stairs’ puzzle.” After the scene with Keith, leave the Dining Room and go up the stairs to the left. Keep going up stairs, and Will will eventually realize he's going in circles. Check the black smear next to the stairs, then use the Spray Can to mark the third floor and end the loop.

That’s What the Knife Is For: “Solve the ‘Patient’s Knife” puzzle.” After you’ve solved the “Loop Stairs” puzzle, go downstairs to the second floor (one floor below the spraypaint-marked third floor). Head right, and blood will appear from one of the doors. Go inside and look under the bed to find a letter to Alec. Backtrack a little to the Visting Room hallway. Enter the left door (now ajar). Use the Key (Visiting Room 3) from the staff’s lounge on the door to the opposite side of the Visiting Room. Get the Knife under the table by the leftmost chair. (You must have read the letter.)

Ghost in The Bell: “Solve the ‘Patient who doesn’t want to leave’ puzzle.” Go upstairs to the fifth floor. You'll hear sobbing. Enter Room 1508 (8th room from the left), in which it stops. Read the writing on the desk. Go up to the sixth and get Area Map 2 from the bulletin board. Try to leave, and the lower-right door will open. Enter the open door and proceed to the back Room. Try to open the door to the other examination room, then walk back.
Building 2
And Then There Were None: (Missable) “Look at the picture book once again after opening the doll’s bellies.” After finding and cutting open all of the dolls for the “Boy Who Cried Wolf” puzzle, go back to the pediatrics room and look at the child’s drawings again.

The Hanged Man: (Missable, but it takes more effort to miss it than to get it) “Listen to the explanation of the ‘Hanged Man’ card.” After you examine the cloth in the visiting room, examine the tarot cards you picked up in the document room. Find the ‘Hanged Man’ card and ask Sophie what it means.

Tarot Reading: “Solve the ‘Tarot Reading’ puzzle.” On the tarot cloth, place the cards that match the words in the Dusty Notebook, with a future of "salvation" instead of "ruin." From left to right: The Emperor (Reversed) = tyranny, The Moon (Upright) = unease, Judgement (Upright) = salvation.

Naughty Kids: “Solve the ‘Security Box’ puzzle.” Enter Room 2305 (the door of which is ajar).
Check the poster on the wall four times to get the code for the security box: 5534.

The Second Ring: “Solve the ‘Ring Search’ puzzle.” Return to the Water Storage Tank after the scene with Keith. Go outside via the left exit in the underground passage, and check the drain. Climb up the middle elevator shaft back to the Kitchen, and get the Sieve from the counter. Go back down and outside, and put the Sieve on the drain. Then turn the valve in the bottom-right of the Water Storage Tank room. Get Keith's Ring from the drain outside, and return to the Control Room to give it to him.

Hide-and-Seek Champ: (Missable) “Successfully hide from your pursuer in the library.” After you unlock the library and explore a little, someone will come in and turn on all the lights. Avoid being caught by them.

Dice Box: “Solve the ‘Dice Box’ puzzle.” You should probably find all the pieces of the Torn Label first, but it’ll still work if you just enter 356142 into the Wooden Box.

Love Letter: “Solve the ‘Gift for the Doctor’ puzzle.” After reading the letters in the trash in the doctor's lounge, put the Headless Doll on Dennis Syniar's desk.
Building 3
The Man Who Walks His Hometown: “Solve the ‘Man Who Walks His Hometown’ puzzle.” In building 3, enter the staff's room and get the Sunnyside Map from the shelves. Enter the kitchen and read the diary. Go upstairs and enter Room 3202 (second from the right on the upper side), and a shadow will begin following you. Go inspect the drawings depicting each area in the order given: Oak Park (first floor kitchen), Sunnyside Village (third floor shower room), then Sunnyside Bay (first floor day room). You'll know it counted if you see the shadow looking at the drawing as you leave.

Watched Cell: “Solve the ‘Watched Cell’ puzzle.” Enter the taped-up room 3408 (which opens after you solve the map puzzle). If you've read the staff diary in the kitchen, tried to walk past the room, and read the notice in the hallway, you'll be able to interact with the chair. Push the chair into the top-left-most position and make it face upward. When it's positioned correctly, you'll get an option to sit in it. Select that. Afterward, go out and tear the notice off the wall.
Building 4
Devil in The Room: “Solve the ‘Devil in the room’ puzzle.” On the second floor, you'll hear a sound. Enter the ajar door and read the writing on the floor. Use the Spray Can to paint over the person-shaped red stains in this room (Room 4205), the second floor hallway, and the fourth floor shower room. Once they're all covered, the sound on second floor will stop and this achievement will pop.

Jailbreak: “Solve the ‘Jailbreak’ puzzle.” When you get trapped in room 4106, inspect the door and choose "Check the door handle." Then go into your items and use the Card.
End Game
Slow and Steady Wins the Race: (Missable) “Go along the road and get caught while playing tag.” During the chase minigame in the good endings, when you get to building 3, go straight down the stairs until you hit the bottom floor. Ed will catch you no matter what after you do this, earning you the achievement.

To the Heart of The Forest: (Missable) “Make it to the bottom of the forest without getting caught.” The game gives you an opportunity to save right before this minigame. Finish it in one shot without Ed catching you. Good luck.
No Particular Order
Clue: “View ‘Card’ and ‘Letter’ in your inventory.” This one is pretty straightforward: check your inventory and examine the Card and the Letter. You can do this at any time; the items are there from the beginning of the game.

Bad Places: “Inspect five ‘creepy’ places.” Occasionally, you’ll come across shadow-y spots (examples below). Examine 5 of them throughout the game to get this achievement.

























A Cornered Rat: “Get bad ending 1.“ Requirements: Don’t go after Sophie, don’t Give Keith his pistol.

Out of The Window: “Get bad ending 2.” Go after Sophie, don’t give Keith his pistol.

Deep Woods: “Get bad ending 3.” Go after Sophie, give Keith his pistol.

Real Good Man: “Get good ending 1.” Don’t go after Sophie, give Keith his pistol back.

Tiny Rat Journey: “Get good ending 2.” After getting the first good ending, play the game through a second time and do the same things to get the second good ending.

i wish there was save files for the endings i dont wanna play it all again :(

About the Bad Places; I missed them in two playthroughs so to say 'You'll find them.' is a bit uncool.
The spots are:
1. The chair next to the meeting room.
2. The table in the kitchen.
3. The graffitti in the shelf room with the moveable ladder.
4. The table with the rope where you leave the doll behind.
5. The room with the Lake doodle on the floor.

Make sure you either get a triggered cutscene or pressed the action button to get a message on that spot.

I didn't find any more than that and I went through every cell in the game.

@Flower there's a newspaper on the floor above after you've followed the guide that you have to read in order to get that achievement.

Can you help me out? I got all the achievement but "That’s What the Knife Is For" and I have no ideia why!
I thought it was because I was not doing it in the right sequence,so I followed the guide to the letter and still isnt popping up!

There's more than 5 in the game, you just need to examine 5 to get the achievement. They're all over the place, and you see more and more as you progress.
