This guide has not been updated since the last update, so its contents may not be all that accurate. I'll try to update it at some point, but for now, be warned :)
Opening
Note: The fun of playing Nepenthe is in the little details. This guide shows only the nessecary steps to reach the "good ending," but I highly recommend you explore the vast world and story hidden below the surface.

When the game starts, you are first presented with a choice of "Story Mode" or "Adventure Mode". This won't influence the story, only the gameplay. If you don't want to have to deal with difficult battles, Story Mode should be your choice. Otherwise, I'd recommend Adventure Mode.

Click your way through the first cutscene, then move down. When you reach the boat, you will gain control of it, and keep moving down. Lighting will occasionally flash, and if you keep moving down, you'll eventually get hit.

When you wake up, the green blob will be talking to you; type your name when prompted, and press enter. What you say to him won't effect anything (as far as I know). You will go to his house, then promptly get eaten. This is fine. When the cutscene is finished, go back to where you first met the blob, then go to the right. You will see the Trainer (the cute white blob guy) below you. DO NOT TALK TO HIM YET. Find the sign to your right saying "secret lair, do not enter," and enter. Go to the castle, and talk to the fire-being inside of it.
Battle
When you talk to the fire-being, you will enter battle. Instead of trying to defeat him, let him rant. Use "relax" to gain some hp if you're low, and avoid getting hit when he attacks. DO NOT ATTACK HIM. Eventually, he will give up, and come to a better understanding of his place in the universe.

Go back, and talk to the Trainer. Let him teach you, and follow his instructions in battle. When he offers a truce, AGREE TO IT. You don't wanna make him mad.

After the battle, accept his Diploma, then go to the lower left of the area. Talk to Mo, and enter the Ruins.
Ruins
Go to the left, until you find Granny. Talk to her, and offer to help. Then go to the second-leftmost building in the area (the tiny small one), and find the rope inside. Take it, along with anything else that looks useful (I recommend the teddy bear). Go back to Granny, and give her the rope. After collecting your small reward, she will run off. Don't worry, she'll be seen again.

Go in the leftmost building in the Granny area, then up the ladder on the right. Talk to the weird old man. Take some rolls while you're there; they fill HP.

If you interact with the graveyard, you'll find a Zombie, and be forced into a fight. If you don't want to kill it, wait out the fight until it gives up, but you won't get much EXP that way. If you kill it, future fights will be easer to deal with.

Keep on going to the left, and you'll find a village. Go into the one house you can enter, and read the sign at the back. When you try to exit, the Werewolf will be there. The fight is unavoidable, and you have to kill it (as far as I know). Once the werewolf is defeated, the town will open up to explore.
Town
Most of the events in the town are optional, so I won't cover them in this guide. I do recommend exploring town however, if you're interested in the plot. The shop on the far left has some useful goods for sale, but the owner will also give you a key. DO NOT USE IT. That will lead to a bad ending.

Go to the leftmost house, and steal the pickaxe. Many people find the battle that ensues a difficult one, and if that's the case... sorry. Defeat the Pig guy, then go outside.

Go left. Keep going. When you reach Mo, talk to him, and show the note.

Next, go back to the Ruins. Interact with the grey building, and break it open with the pickaxe. Enter, and go down the rope in the back.
Cellar
Climb down the rope, until you can walk left. Walk left. Keep on walking. After a while, you will reach a vine. Walk up the vine from below. Keep on going left once you reach the top, until you get attacked by a spider. It might be possible to avoid the spider, but I haven't been able to do it. Fight the spider, then climb up the next vine on the left. Keep on climbing until you reach the blue crystal room, and go outside. You have arrived at the Palace Grounds.
Castle Grounds
Everything inside the buildings in this area are optional, so will not be covered here. Again, they do contain some plot points and useful items, so I recommend exploring in your own time.

Walk left and talk to the green guard at the gates. Tell him you are human, then show the note. The battle this results in is unavoidable, as far as I know. Keep on going, until the first platform, and go up. Talk to the guard, and give him the diploma. He will walk away, and this is your chance to enter the castle.

Walk up with the guards, until stopped by the triangle-head guy. Fight him, and carry on. Eventually, you will reach the Magistrate - Granny. Tell her the truth, and ask with help saving the world. She will reluctantly agree, and send you with Mo to find Hado Ream.

Follow Mo closely, and if you get lost in the castle garden, just go up until you reach a wall, then search left and right until you find a space to exit. If you get really lost, talk to the guards, who will point you in the right direction.

After catching up with Mo, enter his ship, and head up until you find a dock.
City of Myway
When you dock, you will be greeted, then attacked by the Bridgeman. Fight him, and explore the city of Myway. This area is huge, so if you get lost, here's how to find Hado Ream's house:
  • Get to the dock you entered the area from. (if you're lost, go down, then walk right and left until you find it)
  • Walk to the colored path directly above the dock.
  • Walk straight up, past where the path ends, until you cross another path
  • Walk right, along the path, until you see a red well.
  • Enter the building above the well
Hado Ream's House
Walk up, until a hidden door appears. Walk through it. Eventually, a devil-looking-monster appears, and tests you. Keep on going, until you reach Hado Ream's room. Talk to him however you wish. He will fight you. This may be the hardest fight in the game, so don't worry if you die a few times. Eventually you will beat him, and walk out the just-appeared door on the top right of the room. Walk through, until you find yourself where you started. Talk to Mo, and breath a deep sigh. You beat the game!
