Disable VSync (in-game option)
Go to Options → Video settings → VSync = OFF

Just that, easy enough!

This in-game option has been added in the 2013-04-04 update.
Disable vsync (NVIDIA Control Panel - not needed anymore)
Duke Nukem 3D Megaton Edition has vsync enabled by default, and has no in-game option to disable it. A game update on 2013-04-04 added an in-game setting for VSync, so you don't need to use the NVIDIA Control Panel anymore.

As consequence of VSync, there is severe mouse lag. There is a quick and solution for that, if you have a NVIDIA card.

  • Go to NVIDIA Control Panel, by right-clicking on the desktop or on the nvidia icon at system tray, or by finding it inside Windows Control Panel.
  • Go to 3D Settings → Manage 3D settings.
  • Go to the second tab: Program Settings.
  • Select a program to customize, most likely you will click on "Add" button and select "duke3d.exe" from the list.
  • Scroll down the settings list, find Vertical sync and select Off.
  • Enjoy!

for those that still want vsync, use rtss to cap the fps to 60 and the lag is greatly reduced (also happens to other usually older games that dont cap themselves properly)

I got a mouse problem a couple days ago, i thought my mouse was was starting to go through wear and tear, which aiming was quite annoying. That happened until i looked under my mouse and noticed a piece of hair sticking to the bottom of my mouse crossing the laser beam XD.

Good and important guide! no shit!
I found it out by myself today, but with a lot of luck. I always thought this issue depends to the FPS Lock. But I was wrong
