Introduction
This guide is a walkthrough to get all the achievements in Divinity: Original Sin 2 with minimal spoilers. Read it as you play and don't jump too far ahead or some plot character names or location names will be revealed.

Divinity: Original Sin 2 has 97 achievements. Unlike in Divinity: Original Sin few are multiplayer ones. Luckily these are trivial to get and we will cover them in Part 0. Four of the achievements are difficulty based. Sadly you cannot get 100% Achievements on one playthrough as few are character or companion quests and you cannot have all companions with you on a single playthrough.

This guide was initially written while playing on Classic Edition, but I've included some Definitive Edition changes here too. For Divinity: Original Sin Enhanced Edition check out my achievement guide there
On companions and multiple playthroughs
So there are six Origin characters to choose from:

Beast (2 achievements, doable main/side)
Fane (3 achievements, doable main only)
Ifan ben-Mezd (2 achievements, one doable main and one as a side)
Lohse (2 achievements, doable main only)
The Red Prince (2 achievements, doable main/side)
Sebille (2 achievements, doable main/side)

Your party will be maximum of four people - you and up to three others. You can test and switch all the companions during the first act but after that you are bound with the ones you choose so there are going to be multiple playthroughs if you want to gain all achievements.

My playthrough suggestion is:
  • 1st playthrough on Tactician difficulty with a 4-person team with you being either Lohse or Ifan and your party consisting of Sebille, Beast and The Red Prince. You will really learn the game and its mechanics during this run. Mine was Lohse, Ifan, Sebille & The Red Prince. It took me 240 hours or 150 hours if reloads, writing this guide, alt tabbing and reading forums/wiki and trying out stuff is removed from the total.
  • 2nd playthrough doing a lone wolf solo/duo using the speedrun community tactics with Fane as a main and anyone else if you feel like it on the easiest difficulty. Mine was Fane & Beast and it took me 5 hours to beat the full game. Just play along while watching a D:OS 2 speedrun and mimic what they are doing. It was pretty easy, fun and very fast.
  • 3rd playthrough doing a lone wolf solo/duo Honour Mode run. Pick your main as the other character you didn't choose earlier from Lohse or Ifan and pick anyone else if you feel like it. Mine is Ifan/The Red Prince and I haven't beaten the Honour mode yet.

The suggestion is pretty much the 'optimal' playthrough amount to get 100% achievements in this game. If you are not a completionist and are fine leaving some character achievements or difficulty achievements away be my guest and pick an easier difficulty and your favorite companions - you're playing the game for fun you know :)
Part 0 - Multiplayer
Let's start with the two multiplayer achievements out of the way. First you need to start a Arena game and then host a game as a GM. You can quit immediately after the game has loaded when you've got your achievement.

  • Venture Forth
For this you need to click the Arena button in main menu. Click create and select an AI as your opponent. Select what ever heroes you want and start the match. Once loaded you should have your achievement.

  • Gather Your Party
Now the GM one. From Game Master menu click Play Campaign. Create your own and launch it. I didn't need anyone to join in - it popped right after the GM session started. If you're not lucky with that try creating your own campaign and launch that instead of the tutorial one.

Part I - The Ship
Character creation. You can create your character however you please or pick one of the origin characters. As we cannot get all companion achievements in one run refer to the companion selection part earlier. Do note that one of your characters needs to have a scholar tag whether it is your main character or a companion (Sebille, Fane and Red Prince have it). Keep that in mind. You need a scholar or you will miss an achievement tens ans tens of hours later.

Remember that you have to keep interacting with your companions - talk with them, respond to their comments and such for you to be able to do their companion achievements much much later in the game. Good thing they are so damn well written that talking with them is a pleasure.

Before starting the real run we will start off by getting two easy achievements out of the way with a custom made character. You can get these achievements during the first minute of actual game play. So if you created a character already just save and create a new one with WITS +3 and Persuasion 1 (nothing else matters).

  • Thief
(In Definitive Edition grab the ladder and skip the Tutorial area).Once you wake up walk and talk to the only person in room. Talk to her so that her back is turned towards the door and pedestal. Once you have her in place sneak behind her back and steal the book on the pedestal.

  • Convincing
After you have stolen the book just stop sneaking and talk to the person again so that she will turn towards the pedestal. Now wait. Once they accuse you of stealing just try to talk your way out of it by using the wits choice. Two achievements got - 93 to go!

You can continue now with your own character. Talk to the people and continue with the story. The next two achievements are story related and cannot be skipped. Continue reading this guide after you have exited the ship.

  • Reaper's Eye
    Story related - unmissable

  • Thunders of the Upper Deep
    Story related - unmissable

Achievement Tracker: 6/97
Part II - Fort Joy
While you're exploring this next area you will eventually notice a black cat who will start to follow you. Your ultimate goal is to get the next area and bring the cat with you. So keep a close eye on him as he can get killed by guards or disappear randomly. Notably the cat cannot climb ladders so if you go somewhere underground via a ladder the cat will follow you there but cannot get back up the same way. This turns out to be the best way to keep the cat safe - trap him in Fort Joy Dungeons via the hidden hatch near the big statue - you need some wits to notice the hidden button. Just resist the temptation to open the locked door while you're under there and the cat will be all right while you explore.

Enough of the cat. You will have to make a decision though. Do you want to spoil yourself on a plot event or be happily ignorant about it until later? If you don't mind some spoilers read the below paragraph and the description for Hammered achievement. If you want to keep the plot going on ignore the Hammered achievement now and come back later. It takes less than 10 minutes to speedrun the game to this point so you can very easily skip the Hammered for now, just saying.

If you decided to ignore the Hammered achievement for now ignore this paragraph. Otherwise go and create a new character with these specifications: Undead race (any), Knight preset, talent Lone Wolf, Telekinesis 1, Pyrokinetic 1, Polymorph 1, STR 18, skills Haste & Chameleon Cloak. Everything else is irrelevant. Your goal is to speedrun the Ship part and save your game before the Skull Marked door. Use Haste, open the door and run in to pick the two rightmost death fog barrels before they are destroyed. This might need a few tries but you can do it - you only need one barrel. If you just can't get it done, check out some videos or learn how to skip through walls. I don't recommend learning the wall skipping on your first playthrough. You will otherwise use it later and feel bad about it. Anyway once you have at least one death fog barrel play through everything again until you're right before the Fort Joy before the plot cut scene.

  • Hammered
(Optional at this point. Can be done later, on next playthrough even, if you want to avoid a big plot spoiler).Use death fog barrel on Alexander during the first cut scene right outside Fort Joy. You can use your Chameleon Cloak to sneak by and throw the barrel from inside the gates. Use ranged or firestorm grenade to break the death fog barrel. What happens will spoil you a plot event but you get an achievement for it. The real achievement actually is to beat Dallis in this combat but we can achieve that more easily by killing Alexander here instead. Reload save and continue your playthrough.

  • Criminal Intent
In Fort Joy save and try to pickpocket/steal something from the magisters (for example the two gate keepers). Fail on purpose and allow them to send you to prison. Reload your save.

Now you can explore Fort Joy for a while. Read the following achievements and keep an eye out for these ingredients as we will need them for crafting achievements: empty flask, oil barrel, sheet of paper and essence + object combo like water essence & herring. Check a crafting guide for full list for essence + object combinations. There's too many to list here. Also do not use all the skill books right away. We need to combine an elemental skill book with a non-elemental skill book for an achievement.

  • Took a Great Fall
There is a quest to retrieve an item which allows you to use teleport skill. Once you have acquired means of teleportation save and pick a fight with a monster or a npc. Once your terget is down to last it's last 10 hp use your teleportation skill to finish him off.

  • Gladiator
Around lvl 3 - once you feel like you have decent gear - go win an arena match at the Fort Joy underground arena. The entrance is quite close to Fort Joy kitchen.

  • Walk at Liberty
After you've won the Fort Joy arena go talk to the blacksmith. Save and she will get rid of your source collar. Note that if you walk freely In Fort Joy without a collar magisters will attack you on sight so it's better to reload the save for now.

  • Grenadier
Combine an empty flask with an oil barrel. There are plenty of other recipes but those ingredients seem to be the easiest to acquire. Consult a crafting guide for more recipes.

  • Arcane Author
Combine a sheet of paper, an essence and an essence related object. For example I had water essence and herring or life essence and antler options available from random drops. Consult a crafting guide what goes together with your essence.

  • Skillcrafter
Combine two different skill books. One skillbook needs to be elemental like aero, geo or hydro)and one skillbook needs to be non-elemental like huntsman or scoundrel. Once you combine two you get an achievement.

The next achievements can be read once you're inside Fort Joy Prison - it's the big building with silent monks.

  • Apprentice
If you haven't removed your source inhibator already and tried your source skill which is the requirement of this achievement once you beat the big fight in the playground you will get a Face Ripper. Use it on a corpse to acquire this achievement. For some reason the item counts as a source skill.

  • Cat's Out of the Bag!
Remember the cat we trapped in Fort Joy Dungeons? Once you've cleared the way to the boat (or to the playground sewer pipe) you can Waypoint Travel to Fort Joy Dungeons, use the lever near the door and escape Fort Joy. The cat should follow you and after some dialogue you will get this achievement.

  • The Great Escape
Once you escape Fort Joy you will get this achievement.

You can now resume exploring rest of Fort Joy or the next area. Note that you might've gotten Hero or Villain tags at this point depending on your play style. I didn't so I'm not listing them here.

Achievement Tracker: 17/97 (Excluding Hero & Villain achievements)
Part III - Hollow Marshes
You can now explore Hollow Marshes and read the following achievements. On the very Eastern edge of the map you will find something bound to earth. Come back to the guide once you are there but otherwise explore freely.

  • Masquerade
While exploring you will find Fane's Mask of Shapeshifting. It's on the Sourcerer near the Northern beach. Use it to change to each race once and you're done.

  • Runemaster
You most likely have found some runes and items with rune slots. Use the crafting panel and combine those and you will receive this achievement.

  • Spira, Spera
Plot-related - Once you find Shrine of Amadia.

  • A Good and Faithful Servant
Plot-related. Once you save a guy pinned by magisters.

  • Abound with Blessings
You will find a statue at the end of Braccus Rex's vault. Talk to it and go through a vision.

So you are now near the poor thing bound to earth? There's two achievements here and you need to make a save and choose one. Note: If you are cheeky you can get the rewards both for freeing and killing the thing. I'll let you figure how.

  • The Weird Sister
Explore the cave South of the dragon and kill a witch inside of it.

  • Dragonslayer
Kill the dragon. This can be quite hard fight depending on the difficulty you are on. If you want to cheese it read the spoiler tag. Move others away from the area and start the fight with someone who gets first initiative turn. Position yourself on higher ground and start lobbing 'Throw Explosive Trap' traps under the Dragon with your other guys outside combat. 20-30 should be enough. You get this skill combining huntsman & pyro skillbooks.

There's going to be one big fight in North East. If you are completionist you should clear everything and leave that area to last.

  • His Father's Son
You get this one after you clear the big fight in North East.

After you cleared the North East fight the plot makes you hop on board the big boat but it wont sail until you figure out how. Read the next achievements once you're onboard. There's one potentially missable one and two where you need to make a save to get both achievements. Do note that there is a pretty cool mirror below the deck. You can use it to reallocate any points/talents/whatnot on your character or any of your companions. Use the mirror to grant Pet Pal talent to at least one of your characters - you need Pet Pal in the future to get few achievements.

  • Scholar of Secret Arts
This achevement can be potentially missable. Welcome the prisoner in the captain's room to Seeker cause instead of fighting. Do not fight him or you will miss two achievements: this one and a later one.

  • Sails That Glance in the Sun
You get this once you get the vessel running

  • Swashbuckler
There's going to be a big battle after you are in the command of the vessel and there are two achievements awarded based on whether you killed all the enemies during the battle or not. The battle has a time limit. You get swashbuckler achievement if you manage to kill every single enemy on board. The two on the other vessel do not count so ignore them if you will.

  • Seascape
This achievement is awarded if you did not manage to kill all enemies before the plot npc did her thing. I somehow got both Swashbuckler and Seascape at the same time when I killed the last enemy on my last guy before time limit but if you did not just leave one enemy alive until time is up. Few people have mentioned that theirs were also buggy and they only got it via damaging enough either of the two plot npcs on the other vessel thus forcing an early retreat. Try that if you didn't get it via intended way.

  • Heaven's Gate
You get this achievement pretty much immediately once you finish your big battle

Achievement Tracker: 30/97 (Excluding Hero & Villain achievements)
Part IV - Driftwood I
Now the new area is pretty big and quite open and you can explore it in different order than my guide does so I have to spoil a minor plot detail so you won't actually miss a very easily missable achievement. Read the next part carefully and keep it in mind while exploring Driftwood.

The plot wants you to upgrade your characters' knowledge of source - more source points if you will - but counter-intuitively you actually don't want to do this because you will then lose a missable achievement. There will be six or less opportunities, depending on your playstyle, to learn more about source all around this new area via quests and npcs. Our plan is going to delay the completion of any quest that rewards you with source points until the very end when we have have pretty much explored the whole map and done all other main and side quests. Just keep in mind that you need to leave at least two opportunities for yourself to get the max source points achievement and deny/delay all others.

So just to rub it in do not upgrade your source points until you have explored all of the new area and keep at least two quests which reward source points ready to be completed. Otherwise you will miss an achievement or have to reload a save hours and hours ago!

Otherwise explore Driftwood as you please. You will start from a peninsula and all of the next four achievements are on that peninsula just South of the map marker named 'Gallows'. You will probably go to Driftwood very soon after these four so I will use the town as an middle point and write the guide with directional titles going clockwise around Driftwood starting with West of Driftwood. If you're not following the clockwise exploring just check this guide for your direction.

Peninsula

  • A Million of Waves
Plot related once you leave the boat.

  • Bridge Over Troubled Waters
You will meet a boy near a drawbridge - help him and you will get this achievement. Figure out a way to the other side. You can use your teleport/jumping skills to the nearby boat..

  • The Meistr
Once you find a lizard hanging around.

  • Proud Parent
You need Pet Pal talent for this. Once you find a chicken farm talk to one of the chickens to get a quest finding some lost eggs. Complete the quest and return their lost egg. Now go back where you found the egg (or somewhere else bit further) and return back to the chicken farm and you will get this achievement.

Driftwood

Be aware that there's a missable achievement in here so read Infiltration's details.

  • Kiss of the Spider Woman
Under the Driftwood Tavern you will find a woman who wants to kiss you. You know the drill. Save before and decide afterwards if it was worth it.

  • Andabata
Under the Driftwood Tavern you will find an arena. You need to fight a group and then the champion. The group fight can be tough as you're opted to fight blindfolded. I guess you can avoid the challenge but where's the fun in that. You get this achievement once you clear those two fights.

  • Infiltration
*Missable. Under Mordus' house you will find a hidden skull on the wall. Pick lock it. You need Thievery 5. Beware that you cannot pick lock the skull if you have entered the correct key so this achievement is missable.

  • Death in the Pot
Inside the tavern you will meet Garvan. There's few quests where he is involved. Either do them or not but serve him some void-tainted stew to get this achievement.

  • Spirit Animal
You can get this from many places but I'm listing it in here. You only need to acquire your see spirits skill and talk to a animal spirit. To get the skill you need to progress the main story a bit. When you have the skill return to the chicken farm next to Driftwood and talk to a chicken sprit there (I'm unsure if there's anything if you did not do the chicken quest line).

West of Driftwood

Once you explore Wrecker's Cave

  • Technical Eggstacy
Inside the cave downstairs you will find bloody egg sacs. Kill all of them. Some are hostile, some are not and you might need to hover your mouse over some to reveal them. So pay attention as some are pretty hidden in the walls. Once you kill all of them in the Reaper's Cove you get this achievement

North-West of Driftwood

This area is called Cloisterwood

  • Over to the Farther Coast
Talk to the dead ferryman and travel to the other island. Do note that you will be going through Deathfog which means all non-undead characters will die. I tried to travel with the Glowing Icon of Rebirth resurrection item but for some reason this method doesn't grant you an achievement so use an undead like Fane or buyable undead companion like Metamorph from Lady Vengeance to make the trip solo.

North of Driftwood

In the North you will find Gareth who has a dilemma. Save and reload to get these two achievements.

  • Furious Rebuke
Encourage Garth to kill and demand him to be vengeful against magisters.

  • Iniquity Forgiven
    Encourage Garth to show mercy.

Part IV - Driftwood II
East of Driftwood

Once you go over the river is you'll be in the vicinity of the Graveyard. You will find Tarquin here near the graveyard gates with a quest. Do it for a later achievement.

  • Existential Crisis
Near the Stonegarden waypoint you will find an overlook of sorts. There's a grave that you can dig up and you will chat philosophy with an interesting fellow. Answer the three questions right and you will get this achievement but be prepared for a very deadly consequences if you fail even one.Either use undead character, like Fane or hireable mercenary like Metamorph, to answer all questions correctly. Other way is to read three books 'The Essence of Existance Vol 1-3' which can be found in Ryker's house.

  • Cur of Low Degree
Once you'll find a dog called Andras - Kill it.

  • A Nest on High
You need pet pal for this. Near the Stonegarden waypoint is a stone tower. On top of the tower there is a bird and a dead dwarf. Don't touch the dwarf before talking to the bird. You need to succeed persuasion check to become the bird's master.

There's one achievement North-East of the Stonegarden waypoint:

  • The Good Doctor
You need a character with Scholar trait for this. Talk to the man outside his house who explains there's a wounded sourcerer in his basement. Approach the woman in question with your scholar tag character and once the fight starts you need to drop her health low enough to start a dialogue. Be sure not to kill her. If you want you can keep her knocked down and kill the other enemies for xp. After the dialogue starts you need to choose correct answers to heal her and to get this achievement. Ask swann to keep her steady, use delicate tweezers, slowly close up wound

Up in the very North there's a sawmill and one achievement to be had.

  • A Sheep in Wolf's Clothing
This one is bit tricky. You need to escort the elf woman away from the sawmill without fighting anyone. The initial fight doesn't count. There's basically two ways: either you sneak past everyone or kill everyone before talking to the elf woman. If you opt the sneaky way you should probably head straight east towards the wooden walkway and go behind the building and then teleport to the lower ground. If you choose to fight your way out do not talk to the elf woman after you have beaten her captor but instead go down and clear the path. Note that teleport stones or waypoints do not help you here.

If you have The Red Prince in your party, just skip the very North-East part of Driftwood (The Caravan). Don't bring The Red Prince there - just skip it.

Bloodmoon Island

Bloodmoon Island in the North-West should be your last stop. You will meet The Advocate there and can finally finish the original plan of getting the missable achievement. You can explore the island a little bit but I suggest you go up and meet Advocate straight on. Make a hard save before talking to the Advocate.

Our first step is getting the The Isle of Last Resort achievement. It means that you need to kill the Advocate and fail/deny all other source upgrade opportunities. Read the achievement description for more details/spoilers.

Once you got it reload your hard save before Advocate and this time help him with his quest and you will get the Lawyer Up achievement.

Lastly reload the save once more and fully upgrade your source points via your saved up opportunities and go do the ritual to meet up with your god. You will get the Expert, Master and Isle of the Gods achievement this way.

Now you can either reload the save once more and play as you like or continue with your maxed out source points.

Remember to pick up the second Anathema part from the Archive for Tarquin for a later achievement.

  • The Isle of Last Resort
For this achievement you need to deny or fail all source upgrade opportunities. You have failed enough when a war owl will track you down with a message from Malady: 'Vengeance. Now.'
Here are the list of potential source upgrade and how to fail them:
Mordus - In Wrecker's Cave you meet Mordus who is pretty insane. Just fight him and his allies to death.
Ryker - Ryker lives in a mansion in the Graveyard. He wants you to pick something precious for him and when you have retrieved said item refuse to give it to him. A fight will ensue.
Hannag - Hannag is in Cloisterwood. Just force attack her few times and kill her.
Almira - Almira can be found in Paradise Downs. She wants you to kill a voidvoken. Once done and returning to her you can refuse her advancements and she will attack you.
Jahan - Jahan is in Cloisterwood. Once you have killed the Advocate just flat out refuse Jahan's teachings.
Saheila - You can rescue Saheila from the Lumberyard. You can just flat out refuse her teachings and that's that.

  • Lawyer Up
You get this one from helping the Advocate.

  • Expert
You get this when you get your second source point.

  • Master
You get this when you get your third source point.

  • Isle of the Gods
After getting your third source point go see your god via the ritual.

  • What's in a Name?
Once you have beaten the Advocate you can talk to the Ancestor Tree. You need to awaken it by calling it by it's own name. You can find what the name is on the North-Eastern side of the Bloodmoon Island in a marker called 'The Archive'. Entrance to the Archive is hidden and you need wits of at least 22 to spot it.

You should now be pretty much done with Driftwood. Just in case you haven't got Hero and Villain tags yet, refer to their achievement descriptions and get them. Also you probably now have materials to craft a curse scroll if you don't have an undead in your party.

  • Curse Thine Enemies
You need to either use a curse skill as an undead or craft a curse scroll and use it against someone. To craft a curse scroll you need a Sheet of Paper, Void-tainted Fish (plenty in Driftwood town) and High or Alien Quality Tormented Soul. The achievement will pop once you've cursed someone.

Go to the Lady Vengeance to start your voyage towards your next goal. Before leaving just be sure you got the two Anathema parts for Tarquin (Graveyard, Bloodmoon Island) and check if you are happy about how many teleporter pyramids you have just in case the big dialog warning you get that says you cannot come back is actually true hehe. Also note that Driftwood is the last place with beehives that produce honey. Honey is used for crafting all sorts of charming scrolls, grenades and arrows so be sure to stock up your Lady Vengeance storage if you like that stuff.

Achievement Tracker 57/97 (including Hero & Villain achievements)
Part V - Nameless Isle
Welcome to the Nameless Isle! There's few quite interesting choices here to be made and perhaps first of your companion's achievements to be had, namely for Sebille and Fane. You can explore quite freely. Your goal achievement wise is to find and access all the seven altars of gods and pray on them before doing any other achievements. There are two NPCs on the map marked with red flags. Leave interacting with them until you have done the seven altars achievement.

Just explore and try to find all the 7 altars. Read the next two achievements and come back when you have pretty much explored the whole map. If you have Sebille with you do not get close to the Southern NPC or she will start talking to it.

  • Island Hopper
Plot related. You'll get it once you arrive to the isle.

  • The Seven Altars
Pray to all the seven altar of gods with one character. Most of them require some sort of sacrice - do them all. Your own race's god's altar does not require anything. Beware thought that gods see through Mask of the Shapeshifter so don't bother with it.

Once you have pretty much explored the whole map and have a good idea what is where it is time to progress with the plot. First if you have Sebille go and read and do the Sebille achievements down below. Otherwise we will continue by getting inside. Do a hard save now before taking a side with Alexander or Sallow man just so you can choose either later after getting Alexander's achievement.

There are two ways inside: Main puzzle door or back door. We are going to help Alexander first by doing his quest and making him progress inside and then we will find the back door. If you are not sure if you want to continue with siding with Alexander you can just rush the achievement inside and reload your earlier save.

If you have Fane with you check out Fane's achievement below before entering inside.

  • Serious Academic
Find the back door to the academy. It should be pretty obvious route but if you can't find it try hopping between the lava islands in the South-East corner of the map until the very end.

  • Chosen
You will need to persuade all your companions to let you be the one to ascend into divinity. If you have trouble with someone use the Lady Vengeance mirror to get your persuade and relevant attributes as high as possible temporarily and try again. I guess dismissing the harder to persuade companions might also work but did not need to do that myself so I can't vouch it works.

  • Back to School
Plot related. Once you are inside either through the main puzzle door or through the back door.

  • Favoured Son
Inside you will ultimately find Alexander as you helped with his quest. Persuade him to allow you to be divine and you will get this achievement. You can now reload your earlier save if you want to kill Alexander instead.

  • Get In The Ring
Plot related. You get this once you enter the Arena of the One

Sebille

If you have Sebille in your group you can do her achievements here. Note she must have trusted you with the scar song for the A Song Unsung achievement. Do not interract with Shadow Prince without making a hard save.

  • A Song Unsung
Once you kill the Master. You need to have earned Sebille's trust.

  • Stay Grounded
Kill Shadow Prince and give his heart to Mother Tree. Accept your destiny.

  • Liberator
Kill the Mother Tree. There are two ways of doing this - straight up using Sebille or with Shadow Princes help.

Note: this was really hard to obtain for me because the achievement trigger was bugged. If you are experiencing the same try doing it this way
    - Disband Sebille and go talk to Shadow Prince and accept his quest to use Deathfog to kill the Mother Tree.
    - Next plant the Deathfog barrel inside the Mother Tree (you need to fight here) and then go pick Sebille up again.
    - Head back up to Shadow Prince but leave Sebille far away from your party so she doesn't interrupt you talking to Shadow Prince.
    - Once Shadow Prince detonates the Deathfog you should get the achievement.

Fane

You can do one of Fane's achievements here. Save before heading inside through the main puzzle door.

  • The Promise
Once you complete the main puzzle and enter inside go talk to the being. You will get quite a revelation. Swear a pledge and you will get this achievement. Reload after getting the achievement. You will miss out a later achievement if you pinky promise here right now so don't do it. Unconfirmed by me: You can apparently get this if you've created your main character as an undead. Hireable NPC's do not help you here.

Achievement Tracker: 64/97 (Excluding character achievements, including Hero & Villain achievements)
Part VI - Arx
You can pretty much freely explore until you come into inside Arx proper. Most of the companion achievements are to be had in Arx. Since there are so many I moved them to their own part. Read them up in there while reading the below guide.

  • The Wreck of the Titan
    Plot related. Once you arrive.

Inside Arx in the very North of Arx is a Cathedral and in the North East you have a big mansion. Leave them to the last while exploring. There are few achievements in the North East mansion that require you to save and play until the end of game so that's why we're leaving it to the last. We want our decision making hard save as close to end of the game as possible. So have fun exploring Arx and farewell until the Cathedral.

  • Lair of the White Worm
You will get this achievement once you find the hidden basement inside Magister barracks.

  • Anathema
Hopefully you did Tarquin's quests in Driftwood as we can finally finish the quest chain. You will get this achievement once you have learned more about Dallis' mysterious companion (the guy whose name starts with Vre..). You can learn about him in the hidden basement under Magister's barracks. Once you return to Hall of Echoes and talk to Tarquin you will be rewarded with the Anathema, a powerful single use two-handed sword.

  • Toy Story
You get this achievement once you persuade the toyseller to give you his amulet. First you need to learn that he was the Path of Blood engineer though. There are multiple ways to learn this and you should be able to one of those by just exploring. Here's a spoiler if you are stuck: Speak with Arhu or read the toyseller report under the Magister's barracks

The Cathedral
The Cathedral will be our second to last place to explore. At the very North of the Cathedral is Path of Blood. Make a save before interacting with the statue.

Note: Do not fully complete the Path of Blood by descending into Lucian's crypt. We want to clear the way to there but no further. There's a dialog which warns for a point of no return. So get the waypoint, achievements and do the pipe and lever puzzles and then continue to the North East mansion. End of the game is looming and decision time is soon!
  • Cat Person
You need to find and save Arhu. Do this before you continue Path of Blood. These spoilers will help you if you can't find him:
    • (1)Lord Kemm has imprisoned Arhu under his mansion.
    • (2)You need to get the stolen painting from sewers to enter the hidden God King altar room. Do read the sign carefully: It's a puzzle.
    • (3)Responsibility - it's the painting in Lord Kemm's vault. Drop it on the altar.

  • Puritan's Walk
If you haven't stolen anything, murdered anyone (=landed a killing blow), done an evil quest or consumed any spirits for source points you can interact with the statue. Most likely you have done at least one of those and will be inpure and killed by the statue so don't touch it yet. Instead go to Hall of Echoes and recruit a companion from the lizard there and use that recruited new companion to talk to the statue and pass the test. As a reward you will get pretty damn cool permanent wings! ...Which you will lose immediately because you probably don't want to continue with the recruited guy and reload your save and do the secret path instead. Oh well it was fun while it lasted.

  • The Back Door
Don't talk to the statue sentinel. Instead, look for a hidden button near by and use the Toyseller's amulet on it.

The Black House
In the mansion we meet the Doctor guy. If you have Lohse in your group you can get invited in pretty easily. Otherwise you need to kill either Isbeil in the sewers or Lord Kemm in his secret vault to get invited in by the Doctor. Malady is waiting you by the gates. Note: If you want a bit of a challenge do not accept Malady's suggestion. Make a hard save anycase if you want to see what it was out later.

  • Doctor: Heal Thyself
This achievement is pretty straightforward. Refuse whatever the Doctor is offering you and start a fight and kill him. It's a hard fight and if you are having hard time with the fight you can make the fight easier by entering into his realm with Malady before challenging him. Entering his realm will on the other hand force you to make quite heavy decisions and is completely optional.

  • Deal with the Devil
This time we are going to accept whatever the Doctor is offering us. To get the second part of this achievement we need to play the game until the end.

End game
These all can be gained in the Lucian's crypt.

  • The Abysmal Sea
You get this achievement for completing the final fight. You only need to kill Braccus Rex. So focus on him with all your characters.

Make a save after the fight. There's four achievements to be chosen here. You get the difficulty achievements after you have chosen your destiny and before the end credits start rolling.

  • Risen
Accepting the ascension and choosing the first option.

  • One For All
Accepting the ascension and choosing the middle option.

  • Supreme Sacrifice
Accepting the ascension and choosing the last option.

  • Return of the King
Refusing to ascend. This is not available if you have a pact with the Doctor

  • Sourcerer/Godwoken/Divine/For Honour
You get these achievements based on the difficulty you played.

Now go and reload your save from the Doctor's house and this time make a deal with him and play through the game again.

  • Angel and Demon
Fight the final boss fight again and then you get to fight all your companions with Doctor on your side. Yay!

That should be it. You have now completed Divinity Original Sin 2 to it's fullest! Most likely the only achievements you are missing are some of the companion ones and probably the Honour mode difficulty.
Hero / Villain Tags
Depending on your playstyle you might either have one or the either. Here's how to get them by starting a new character and rushing to Fort Joy. You can do these in under 15 minutes.

  • Hero
Follow these steps exactly. Do not do anything besides following them.
    - First create a new character with the lowest difficulty. Any will do but you need to fight some low level fights so don't make a wimpy one. Pick at least Lone Wolf talent and 12 wits and 1 persuasion
    - Once in the boat open the door to Windego, go back to the four magisters in the beginning and attack them. Then just run towards the Sourceress and wait until Windego unleashes her source magic. This way we get extra 975xp for killing all the magisters in the beginning and get bit closer to lvl 2.
    - Kill the voidlings top deck and go back down to help your friends.
    - Once you have killed all voidlings below decks and your friends have escaped go help Magister Siwan. Hold the cloth tighter to the wound and continue holding.
    - On the beach skip past the voidlings and climb the ladder skipping Dallis
    - Go to Ifan & Elodi and shoo the bullies away via dialogue
    - Go talk to Griff and Amyro and start Imprisoned Elf quest. Agree to help Amyro.
    - Make Stingtail the Dreamer on the beach confess with the finesse option and give you the missing oranges
    - Go give the oranges back to Griff and deny to tell where you got them.
    - Once you deny telling Griff the second time who stole the oranges you will be awarded Hero tag

I did this twice and got it both times on different characters. Following this route will give you your Hero tag.

  • Villain
Follow these steps exactly. Do not do anything besides following them.
    - First create a new character with the lowest difficulty. Any will do but you need to fight some low level fights so don't make a wimpy one. Pick at least Lone Wolf talent and 12 wits and 1 persuasion
    - Once in the boat open the door to Windego, go back to the four magisters in the beginning and attack them. Then just run towards the Sourceress and wait until Windego unleashes her source magic. This way we get extra 975xp for killing all the magisters in the beginning and get bit closer to lvl 2.
    - Kill the voidlings top deck and escape the boat without helping anyone else.
    - Once on the beach skip past the voidlings and climb the ladder skipping Dallis
    - Avoid Ifan & Elodi for now but make your way toward the altar, inspect it and open the secret hatch
    - Go down the hatch and you should get enough xp to get to lvl 2. Lvl up.
    - Open the underground door with the semi-hidden lever and go kill Verdas, the decomposing elf. Remember to be nasty via dialogue.
    - Go back up and bully Ifan & Elodi via dialogue and kill them
    - Next up go to the Doctor and Dain and kill them both. Attacking them without talking is fine.
    - Inside the cave start a fight with Saheila and kill her and the two other npcs. Same here. No need to talk.
    - Go talk to Griff and Amyro and start the Imprisoned Elf quest
    - Make Stingtail the Dreamer on the beach confess with the finesse option and give you the missing oranges
    - Go back to Griff and rat on Stingtail
    - Amyro should now be free and if you kill him when he leaves the cage (or goes to Saheila cave) you should be 'awarded' with the Villain tag.

I did this twice and got it both times on different characters. Following this route will give you your Villain tag.
Companion Achievements in Arx
Most of the companion achievements are in Arx. Fane's and Ifan's are at the very end of the game. Others are throughout the city.

Red Prince
Red Prince's achievements can be unlocked inside the Lizard consulate. On Classic Edition, at the writing of this, you need Red Prince as your Main to get the 'A Dream Undone' done (seems to be bugged)
  • All-Father, Life-Shaper
The Lizard consulate is on the North-Eastern side of Arx. Make your way through the consulate and the Dream World until you meet Red Princess. Accept her offer to sleep with her and the achievement will pop.

  • A Dream Undone
Read the above achievement to learn where to find Red Princess. If Red Prince is your main you have the option to refuse her via dialogue and then proceed to kill her. If you are playing Definitive Edition and Red Prince is your companion you have dialogue options after you've slept with the Princess and if you are playing Classic Edition it's going to get bit tricky as we cannot refuse the Red Princess via dialogue or by attacking her. (Hopefully fixed in a later patch)

Beast
Under the Arx in the sewers you will meet Queen Justinia. Beast's achievements can be unlocked by either allying with or killing the queen. This decision is made when talking to the Queen the second time after a fight.
  • Regicide
Don't forgive Queen Justinia but instead start a fight. You don't need to even kill her as the achievement will pop immediately.

  • Royalty
Forgive Queen Justinia and let her go. I did not have this option when my main spoke to Queen Justinia so if that happens to you send only Beast to do the talking.

Lohse
In the Black House you meet up with the Doctor. You need to have Lohse as your main for the 'Demon's Embrace'.
  • An Apple a Day
Kill the Doctor. This is the same as the achievement 'Doctor: Heal Thyself'.

  • Demon's Embrace
Make a deal with the Doctor. This is also pretty much the same as 'Deal with the Devil' achievement.

Ifan
Within Lucian's crypt you get to decice whether you forgive or have vengeance on Lucian. You need to have Ifan as you main to do the 'Fool Me Once'
  • A Wolf Avenged
Start a fight with Lucian and kill him.

  • Fool Me Once
    Forgive Lucian.

Fane
Within Lucian's crypt you will get a decision to make. You need to have Fane as your main for both of these achievements.
  • Eternal Salvation
This is the first of the [FANE] options.

  • The Adventure Begins
This one is the 'share with everyone' option.

Thank you for the help, just like with the first game your guide helped me greatly. Now I just need to do an Ifan playthrough :stella_happy:

For easy "Villain" achievement just release Death Fog in Arx. I got it while having "Hero" tag.

Question about your 3rd playthrough suggestion : "3rd playthrough doing a lone wolf solo/duo Honour Mode run"

Is there a specific reason to run honour mode as a lone wolf? I don't seem to find any achievement requiring to do a solo run or with only 2 party members.

All that below said its appilcable if Red Prince is your companion. If you play yourself as a Red Prince main char I think you will always have a chance to agree to kill Sadha for Dream Undone achiev because Prince is you main and you get all the dialogues.

"What is the reason to not have Red Prince go to the Caravan in Act 2?"
The very important reason - if go to that caravan in Act 2 the Red Prince will have a night with Sadha and she would become pregnant and thus in Arx you wouldn't be able to persuade Prince to kill Sadha - he will become aggresive always no matter what because of the egg already made. Thus you would be able to only get All-Father achiev and not Dream Undone. If you however will ignore that caravan in Act 2 like the guide says Sadha wouldn't get pregnant by the time in Act4 and so no egg and so it'd be easy for Prince to agree with you that Sadha should be killed and the Dream Undone achiev also possible. that''s why.

i think you can get companion achievements without maining them in the DE version. except for fane sworn achievement.

What is the reason to not have Red Prince go to the Caravan in Act 2?

There is a way to get "Fool Me Once" without having Ifan as your main. Kill all your party members at the entrance (where Malady and the rest prays for you). Talk with Lucian as Ifan without resurrecting your members. Lucian will ask you to give up your source. You have to select "share source" with Ifan. Since your characters are dead they will not get a dialogue option.

Here's how I got the most frustrating ones (for me, at least):

Fool Me Once - Ifan has to be your main (or a friend in a co-op game), otherwise you don't get the dialogue option.

A Dream Undone - Someone mentioned this earlier, just get to Sadha in Act IV and leave Red Prince behind so he doesn't get in combat (unless you think you can kill him), and just attack and kill Sadha. Soon as she dies, you get the achievement. You can't kill her in Act II from what I can tell.

Demon's Embrace - Author's description is completely wrong. You have to NOT do the candle encounter with Lohse, and then REFUSE the Doctor's offer. If you accept and then share divinity with him in the end, it doesn't work. REFUSE him, and he'll start combat and channel like Malady does after Act I. On his third turn, he'll possess Lohse. Finish that dialogue and once she turns against the party, you get the achievement... and you also lose the game if she's your main in solo mode.

Yeah, that threw me off as well. I didn't heed the warning and went there anyway, some rather important plot developments happened, would've been weird to skip all that. Reached final chapter since then, we'll see how it ends.
