I'm also using the Application Audio Input plugin for OBS and
Windows Sound Settings -> Sound Control Panel -> Playback -> make sure your headphones are the default device. (Same goes for the 'Recording' tab, your mic should be default).
Warzone Settings -> Audio -> Set your voice chat to your default device or the headphones.
For obs I have the seperate tracks : Game Capture, Mic, Discord and Audio Ouput Capture. The problem here was I had Game Source instead of the Audio Output Capture. Game Source doesn't record in game/prox chat in mw2 (weird)
Remove the Game Source and replace it with Audio Output Capture. Go to properties of the Audio Output Capture and select your headphones. Under 'Audio Mixer' click on one of the cogwheels and select Advanced Audio Properties and make sure they are all seperated.
This fixed it for me, hope it helps you aswell!
