Intel identified the issue to be specifically on 3rd (and newer) gen of Intel iGPUs with all Source Engine games some time ago, some of them might be already fixed (Some Valve titles).

Suggested short term workaround is to force lower DeviceID by using
-force_device_id 0x2400
Launch Option for Insurgency in Steam.
