Simply put, this game is a mess. It is not all bad, but it held such promise that has completely gone to waste. This makes an otherwise mediocre/okay game an utter disappointment.
Anyhow, let's start with some positive points to get them out of the way, before we can discuss everything that has gone wrong:
- original settings: Spanish flu London
- original idea: a doctor becomes a vampire and is torn between saving people and becoming a bloodthirsty monster
- interesting story

And now, for everything else:
- it is not acceptable for a modern RPG to have autosaves and not free saves
- especially since the game is prone to random crashes that can set you back of half an hour
- the map is cramped, deserted, without personality and difficult to navigate
- enemies respawn
- the fundamental mechanism of the game makes no sense: if you bite a civilian they die, but if you bite an enemy during combat you barely scratch them; if you kill a single civilian you mess up the balance of the whole world, but killing hundreds of enemies on the streets doesn't count
- combat is pointless, as you barely get any exp out of it, and the enemies will respawn the second you walk around the corner
- the camera does whatever it wants, which never reflects what you want
- so much pointless talking; I'm a great fan of NPCs with background stories and personalities, but there is such a thing as too much chatting about random characters' daddy issues that have absolutely no relevance to the world or the story or my character
- the doctor's idea of helping the people is walking around the streets giving out potions for tiredness
- you're supposed to be a leading surgeon of internation reputation, and yet every drunkard on the street treats you as their errand boy; is your doctor really the right person to send attaching propaganda posters, collecting alcoholic bottles from a deposit and chasing down thiefs??
- no fast travel
- as soon as you become a vampire you get the ability to "teleport" to another position in sight, which reminds of the similar ability in Dishonored, but you cannot use it in a fight or to escape a fight; you can only use it in specific points of the map (a small bunch) when the game suggests it. This is obviously to cover up the fact that the world is extremely flat and empty. Stealth is not contemplated.
- you cannot jump or crouch, which feels ridiculous in a game that has an action-style combat system
- very repetitive enemies: dozens upon dozens of vampire-zombies and vampire-hunting grunts of basically only three classes
- every possible stereotype about vampires; I hoped that The Masquerade world had allowed us to move beyond the "fear of the cross" and "kill the vampire with a stake through the heart"
- there is no valid reason to classify it as "choices and consequences". You do not make any real choice, except to kill and drink the blood of a civilian or not; however, there are no consequences, except possibly missing out on a secondary mission the civilian was going to assign you.

To conclude, I expected so much more and cannot recommend it at all.
